Comparison analysis of rescuer's neck muscle tone and stiffness according to cardiopulmonary resuscitation skill-level

  • The purpose of this study was to identify differences in the muscle tone and stiffness of neck muscles according to levels of cardiopulmonary resuscitation (CPR) skill. The subjects were 30 female students in their 20s who were divided into a skilled group (n=15) and an unskilled group (n=15). According to the results, the skilled group showed statistically significant decreases in the stiffness of the lower cervical muscle and the muscle tone of the upper trapezius on the above hand side of hand grips after CPR (p<.05). The unskilled group exhibited a statistically significant difference in both the muscle tone and stiffness of the upper trapezius muscle on both sides after CPR (p<.05). However, no statistically significant differences were found between the two groups. In addition, the non-skilled group showed statistically significant lower values than the skilled group in the mean compression rate, total number of compressions, accuracy of chest compression, and accuracy of chest relaxation (p<.05). This study confirmed that CPR has a myophysiological effect on the neck muscles of those who perform CPR. However, it found that no differences result from individual CPR skill levels that are an important element for the accuracy of CPR.

  • Congenital posterolateral diaphragmatic hernia [Bochdalek hernia] is the result of a congenital diaphragmatic defect in the posterior costal part of the diaphragm in the region of the tenth and eleventh ribs. There is usually free communication between the thoracic and abdominal cavities. The defect is most commonly found on the left [90%], but may occurs on the right, where the liver often prevents detection. The male to female ratio is 2:1. Owing to the negative intrathoracic pressure, herniation of abdominal contents through the defects occurs, with resultant collapse of the lung. Shifting of mediastinum to the opposite side and compression of the opposite lung occurs. Most often these hernias are manifestated by acute respiratory distress in the newborn. A second, but less well recognized, group of patient with Bochdalek hernia survive beyond the neonatal period, usually present at a later time with "failure of thrive, intermittent vomiting, or progressive respiratory difficulty. " The diagnosis can often be made on clinical ground from the presence of respiratory distress, absence of breath sounds on the chest presence of bowel sounds over the chest . Roentgenogram of the chest confirm the diagnosis. Obstruction and strangulation have been reported but are rare. Treatment consists of early reliable identification of these congenital diaphragmatic hernia with high risk and surgical repairment. and postoperative pharmacological management with extracorporeal membranous oxygenation [=ECMO] support in the period of intensive care. On the surgical approach, for defects on left side, an abdominal incision is preferred, because of the high incidence of malrotation and obstructing duodenal bands. In the neonate, the operative mortality may be appreciable, but, later repair almost always is successful. During the period from 1972 to 1982, 4 cases of congenital Bochdalek hernia were experienced at the Kyung-Hee University Hospital.

Middle Lobe Syndrome (중엽증후군)

  • "Middle lobe syndrome" which was described y Graham and associates at first is always caused by ex- ternal bronchial compression by Iymph nodes. Although the patients may not present any symptom, the most common presenting symptoms were cough, dyspnea, fever, hemoptysis, and chest pain. Diagnostic procedures includ chest X-ray bronchoscopy, brochography, chest CT, and the principal finding is the contracted middle lobe which is usually airless. We experienced fifteen cases of middle lobe syndrome from April 1990 to May 1995. Eleven patients were treated surgically. The surgical candidates for middle lobe syndrome are suspicious malignancy, fixed bronchiectasis, bronchostenosis, intractable to medical treatment, recurrent infection. Operations were right middle lobectomy (8), right middle and lower bilobectomy (2), right upper and middle bilobectomy (1). Postoperative histological findings were tuberculosis in six, chronic inflammation in three, malignancy in one, and focal hemorrhage in one. There were two cases of postoperat ve complications which were postoperative atelectasis and hepatopathy.patopathy.

A Comparison of Quality of SimPad based on Field Focus Type CPR and Video Self-lnstruction CPR (SimPad를 이용한 현장중심형 심폐소생술과 영상자가학습 심폐소생술의 질 비교)

  • This study was designed to find effective CPR education methods by comparing and analyzing the quality of Field-Focus Type (FFT) CPR using SimPad and the quality of Video Self-lnstruction(VSI) CPR for the learners. The data collection were conducted from November 28, 2018 to December 5, 2018, on 64 first graders who completed CPR lectures at universities located in G metropolitan city, and analyzed using the SPSS ver. 23.0. The results of the study, Field Focus Type (FFT) CPR showed higher quality of Video Self-lnstruction (VSI) CPR in areas such as CPR total score, chest compressions score, chest compressions dept, chest compressions rate, chest compression velocity, total ventilation score, and Total number of ventilation than that of the subjects. Therefore, using Field Focus Type CPR (FFT) education law was found to improve the quality of cardiopulmonary resuscitation compared to the existing Video Self-lnstruction CPR (VSI) education method, and it is believed that it can be used as a basic data for cardiopulmonary resuscitation education in the future.

May-Thurner Syndrome Treated with Endovascular Wall Stent - Report of two cases - (May-Thurner 증후군의 혈관 내 스텐트를 이용한 치료 - 치험 2예 -)

  • Deep vein thrombosis (DVT) is a common disease. However, May-Thurner syndrome, which is the cause of DVT, is an uncommon processes in which there is an impaired venous return due to compression of the left iliac vein by the overlying right common artery. This condition results in a left iliofemoral deep thrombosis and severe leg edema. It is, therefore, called iliac compression syndrome. Catheter-directed thrombolytic therapy of acute extensive iliofemoral DVT and balloon angioplasty with venous stenting are recommended. Two cases with history of left leg swelling are diagnosed as May-Thurner syndrome, which was demonstrated by venography. We successfully treated the patients with thrombolysis, balloon angioplasty, and stent insertion at the site of common iliac vein compression. Therefore, we report the cases with overall review of the literature.

Cervico-Mediastinal Lipoma with Horner's Syndrome -A case report- (호너 증후군이 유발된 경부-종격동 지방종 -1예 보고-)

  • Lipoma is a circumscribed mesenchymal tumor originating from the adipose tissue. The lesion is usually small and asymptomatic. The most common location is in the neck region, however, lipoma can be found in the mediastinum in rare occasions. Although lipoma reach to the large proportions in the mediastinum, it rarely compresses the neurovascular structure. We present a case of a 58-year-old man, in which a hourglass-type cervicomediastinal lipoma produced Horner's syndrome with voice change. The man presented a swelling at the right side of his neck, ptosis and anhidrosis on the right side of his face, and right chest discomfort. After the removal of the mass, all the symptoms which had been provoked by compression, as well as Horner's syndrome and hoarseness, nearly disappeared.

Surgical Treatment of Benign Mediastinal Tumor (양성(良性) 종격동(縱隔洞) 종진(腫疹)의 외과적(外科的) 치료(治療))

  • This is a report on the cases of benign mediastinal tumors in the Department of Thoracic and Cardiovascular Surgery, Chonnam University Hospital during the period from 1961 to 1975. Age distribution was from 18 to 62 years old with the highest incidence in the 3rd decade, and sex ratio of male to female was 7 : 8. The tumor were classified as follows; 6 cases of teratoma 5 cases of neurogenic tumor one case of pericardial cyst one case of cystic hygroma one case of dermoid cyst one case of bronchogenic cyst. The symptomatic patients were 10 cases (66.7%) and asymptomatic patients were 5 cases (33.3%), who were found incidentally by routine chest n-ray. The symptoms occurred by compression to the adjacent nerve system in 7 cases, by perforation into the lung with infection in one case of teratoma and by infection of bronchogenic cyst in one case and of teratoma in one case. Complications were Pancoast's syndrome including Horner's syndrome 2 cases, middle lobe syndrome 2 cases, intercostal neuralgia 1 case and bronchitis 1 case. All tumors were surgically resectable with good recovery. In all 10 cases of symptomatic patients, their symptoms disappeared dramatically after operation.

Clinical Ewperiences of Rib Fixation Using Judet's Strut (Judet's strut를 사용한 늑골 고정술의 임상적 고찰)

  • For the purpose of evaluation of clinical characteristics in multiple rib fracture due to accident , 24 cases treated by surgical rib fixation using Judet`s strut for multiple rib fracture and flail chestduring the period from June 1993 to October 1994 were reviewed. There were 17 males and 7 females.They ranged in age from 19 years old to 56 years old. The causes of rib fracture were traffic accident in 18 cases, fall down in 3 cases, compression in 2 cases stab wound in 1 case. The number of rib fracture were five in 7 cases, six in 5 cases, four in 5 cases, three in 3 cases. Associated intrathoracic injuries were hemopneumothorax in 12 cases, hemothorax in 10 cases, lung laceration or hemorrhagic contusion in 7 cases. Associated extrathoracic injuries were abdominal injuries in 21 cases, orthopedic problem in 7 cases, head trauma in 4 cases. The most common fractured site was posterolateral portion of the ribs. The causes for operation were flail chest, severe rib displacement and pain, hemothorax or hemopneumothorax with continuous air leakage and stab wound. There were 6 postoperative complications ; one with hydrothorax, two with fibrothorax, two with wound infection and one case of death due to multiful organ failure. Postoperatively, all patients became comfortable and complained less painful. Twenty patients restored spontaneous breathing without ventilator support, three patients were ventilated during a day and one patient expired after 2 days. There were no morbidity and mortality related to operation.

A Case of Acute Respiratory Failure Presenting Lobar Consolidation (대엽성 폐경화로 나타난 급성 호흡부전 1예)

  • Pulmonary embolism is one of the moot common acute pulmonary disease in the adult general hospital population However, the disease is still frequently unsuspected and underdiagnosed due to the nonspecificity of both clinical findings and laboratory tests. The chest radiography in a patient suspected acute pulmonary embolism do not provide adequate information to establish or exclude the diagnosis of pulmonary embolism. Even in the case of infarction, there is no pathognomonic clues on the chest film. Rarely infarction presents unusual roentgenologic manifestation such as lobar consolidation, coin lesion, multinodular opacity, or massive pleural effusion Especially, lobar consolidation in pulmonary embolism might mislead into the diagnosis of pneumonia. We experienced a case of pulmonary embolism presenting lobar consolidation in a 62 years old woman, originated from deep vein thrombosis. She took a compression stocking and underwent anticoagulant therapy with excellent outcome.

A Retained Polyethylene Catheter Fragment in Superior Vena Cava - A case report- (상대정맥내 폴리에틸렌관 잔존물 치험 1례)

  • This represents a case report of the retained polyethylene catheter fragment in superior vena cava. A 39 year old male was admitted to this Korea University Hospital a short time after compression wound on abdomen with heavy cement material in emergency room, a polyethylene catheter was introduced into the right subclavian vein through a needle. But when the polyethylene catheter was attempted to withdraw the catheter was severed by the beveled tip of the needle. Later that day, chest X-ray disclosed the presence of the fragment extending from right subclavian vein to the superior vena cava. {Fig. 1 and Fig. 2]. Local exploration by way of an infraclavicular incision was unsuccessful in locating the catheter fragment. Another attempt was then made remove the catheter by means a biotome, which is originally a device for the biopsy of the myocardium, introduced through the right great saphenous vein. This procedure, though well tolerated by the patient, was in vain. After 11 days later, during that time he was taken a laparotomy with drain, another operation for removal of retained catheter fragment was performed through median sternotomy. After exposure of the right subclavian vein, innominate vein, and superior vena cava, an incision 1 cm in |length was made directly over the palpated catheter. The catheter immediately was picked upward and removed. The length of the catheter was approximately 8 cm. [Fig 3 ] There was no evidence of thromboembolism from the catheter or other complications. The patient made an uneventful recovery, and was discharged asymptomatic on the 9th postoperative day.
