Purpose : Skip the repetitive HRCT axial scan in order to reduce the exposure of patients during chest HRCT scan, Helical Scan Data into a reconstructed image, and exposure of the patient change and visually evaluate the usefulness of the HRCT images. Materials and method : Patients were enrolled in the survey are 50 people who underwent chest CT scans of patients who presented to the hospital from January 2015 to March 2015. 50 people surveyed 22 people men and 28 people women people showed an average distribution of 30 to 80 years age was 48 years. 50 patients to Somatom Sensation 64 ch (Siemens) model with 120 kVp tube voltage to a reference mAs tube current to mAs (Care dose, Siemens) as a whole, including the lungs and the chest CT scan was performed. Scan upon each patient CARE dose 4D (Automatic exposure control, Siemens Medical Solution Erlangen, Germany) was to maintain the proper radiation dose scan every cross-section through a device that automatically adjusts the tube current of. CT scan is the rotation time of the Tube slice collimation, slice width 0.6 mm, pitch factor was made under the terms of 1.4. CT scan obtained after the raw data (raw data) to the upper surface of the axial images and coronal images for each slice thickness 1 mm, 5 mm intervals in the high spatial frequency calculation method (hight spatial resolution algorithm, B60 sharp) was the use of the lung window center -500 HU, windows were reconstructed into images in the interval -1000 HU to see. Result : 1. The pathogenesis of the primary spontaneous pneumothorax is the rupture of subpleural bleb or bullae and subsequent sudden collapse of the affected lung. Mostly, the bullae or blebs are present bilaterally, but detecting the number, size, and location of the causating foci by plain chest film is quite difficult . We have performed chest CT scans for detecting the bullous lesions in 33 cases of primary spontaneous pneumothorax, and compared the results with surgical findings.1. Seventy-four blebs were identified in the chest CT scan, and 100 blebs or bullae were detected surgically [ Sensitivity was 0.74 ].2. Diagnosis rate was 80% [40/50] at right upper lobe, 75.7% [28/37] at left upper lobe, 55.6% at right lower lobe, and 25.0% at left lower lobe, respectively.3. Blebs or bullae smaller than 1 cm of its diameter were detected by 57.1% [24/42] of sensitivity, and in the cases of size larger than 1 cm, it revealed 86.2% [50/58] of sensitivity respectively.4. Of the 45 cases, 7 cases were false negative [15.6%], most of these were ruptured or small size [< 0.5 cm]. 5. One case was false positive, which was irregular adhesion at the apex of the lung.6. We could detect blebs or bullae with preoperative CT scans in 84.4% [38/45] of total patients. In conclusion, chest CT scan is a very advantageous diagnostic tool for proper management and preventing recurrence of primary spontaneous pneumothorax patient.
CT scan is reported to have a high risk of cancer due to a relatively high dose among medical radiological examinations. In particular, exposure to radiation to the breast, which is sensitive to radiation, is inevitable during a chest CT scan for female patient. In this study, the dose reduction effect of wearing a compression band during chest CT scans in women was evaluated, and the lifetime attributable risk due to the effective dose exposed during the CT scan was estimated. As a result, when the compression band was used, the effective tube current decreased as the outer perimeter of the chest became smaller, and it was analyzed that the CT dose index and effective dose were also reduced. In addition, the lifetime attributable risk by chest CT scan was found to reduce the cancer risk by 3.2 per 100,000 for all cancers, 0.2 per 100,000 for solid cancer, and 0.8 per 100,000 for breast cancer, based on women in their 30s when using a compression band. It is judged that the risk of cancer can be reduced through the use of appropriate scan parameters and dose optimization measures such as compression bands for future CT examinations.
Background: Blunt chest trauma accounts for 90% of all chest traumas in Europe and the United States and this causes 20% of all trauma-related deaths. The major cause of morbidity and mortality after blunt chest trauma is undetected injuries. For this reason, chest computerized tomography has gained popularity for the evaluation of trauma, but it is expensive and it exposes patients to radiation. This study identified the clinical features associated with the diagnosic information obtained on a CT chest scan, as compared with a standard chest X-ray, for patients who sustained blunt trauma to the chest. This study also evaluated the role of a routine computed tomographic (CT) scan for these patients. The patients who had chest computed tomography done after the initial chest x-ray were analyzed separately for the presence of occult injuries. Material and Method: We studied 100 consecutive patients from November 2006 to July 2007: 74 patients after motor vehicle crashes and 26 patients after a fall from a height >2m. Simultaneous with the initial clinical evaluation, an anteroposterior chest radiograph and a helical chest CT scan were obtained for all the patients. The data extracted from the medical record included the vital signs, the interventions and the type and severity of injury (RTS). Result: Among the 100 cases, 79 patients showed at least more than one pathologic sign on their chest radiograph, and 21 patients had a normal chest radiograph. For 17 of the patients who had a normal chest X ray, the CT scan showed multiple injuries, which were pneumothorax, hemothorax, lung contusion, sternal fracture etc. This represents that a CT scan is statistically superior to a chest radiograph to diagnose the pathologic signs. But on the other hand, as for treatment, only 31 patients were diagnosed by CT scan and they were treated with chest tube insertion ect. 42 patients needed ony conservative management without invasive thoracosurgical treatment such as chest tube insertion or open thoracotomy. 27 patients were treated based on the diagnosis made by the chest radiograph and physical examination. Conclusion: Chest computerized tomography was significantly more effective than routine chest X-ray for detecting lung contusion, pneumothorax and mediastinal hematoma, as well as fractured ribs, scapula and, sternum. Although the occult findings increased, the number of patients who needed treatment was small. Therefore, we suggest making selective use of a CT scan to avoid its overuse in ERs.
Low-dose chest CT, which is used as a lung screening test, also includes information on coronary artery calcification within the scan range. The purpose of this study was to investigate the usefulness of determining coronary artery calcification using Low-dose chest CT. Those who underwent low-dose chest CT and coronary artery calcification score CT on the same day were eligible. Coronary artery calcium score CT results were divided into 4 groups (Low: 1〈CACS〈10, Mild: 10〈CACS〈100, Moderate: 100〈CACS〈400, High: 400〈CACS) by referring to the Coronary artery calcium score categories and risks. After selecting 30 people each group, five radiotechnologists with more than 15 years of experience in coronary artery calcium measurement retrospectively analyzed the presence or absence of coronary artery calcification in low-dose chest CT images. The results of the five observers' uniform interpretation of the low-dose chest CT image were consistent with the coronary artery calcium score CT results in Low group: 56%, Mild group: 96.6%, Moderate group: 100%, and High group: 100%. appeared. In the Low group, all 5 observers observed calcification in 17 out of 30 cases, and in 7 cases all 5 observers decided that calcification could not be identified. Coronary artery calcification could be observed in 100% of asymptomatic adults with a calcium score of 15 or higher in low-dose chest CT scans. The minimum calcium score that can be identified is 1, and it was found that even very small calcifications can be identified when the subject's body size is small or the scan is performed at a time when heart movement is minimal.
In chest and abdomen CT scans, the radiation exposure doses by scattering lines were measured at the eyeball and thyroid. Radiation exposure was investigated by using shielding devices. The chest and abdomen CT scan protocols used in the real examination were applied to measure and compare radiation doses before and after the use of shielding devices at the eyeball and the thyroid. The radiaton doses were measured with OSLD dosimeters. Barium, tungsten sheets, goggles and neck shields were used to protect the scattered X-ray. The chest CT scans showed respectively 3.01 mSv and 6.21 mSv at the eyeball and the thyroid by the scattered X-ray. The abdomen CT scans showed 0.55 mSv and 3.22 mSv for the eyeball and the thyroid respectively. Barium and tungsten sheets had 11% to 13% protection rates at the eyeball and the thyroid for chest CT scan, and 34% to 49% reduction in radiation dose for the abdomen CT scan. Because of the significant radiation dose, which causes cataracts and thyroid cancer by the repeated and continuous radiation exposure, for the chest and the abdomen CT scans, it is required to use shielding devices to reduce radiation dose for examinations.
The purpose of this study is to provide basic clinical data by evaluating images, measuring absorbed dose and effective dose by using high resolution CT and low dose CT by using anthropomorphic chest phantom and glass dosimeter. Tissue dose was measured by inserting a glass dosimeter into the anthropomorphic chest phantom. A 64-slice CT system (SOMATOM Sensation 64, Siemens AG, Forchheim, Germany) and CARE Dose 4D were used, and the parameters of the high resolution CT were 120 kVp, Eff. Scan parameters of mAs 104, scan time 7.93 s, slice 1.0 mm (Acq. 64 × 0.6 mm), convolution kernel (B60f sharp) were used, and low dose CT was 120 kVp, Eff. mAs 15, scan time 7.41 s, slice 3.0 mm (Acq. 64 × 0.6 mm), scan of convolution kernel B50f medium sharp. CTDIvol was measured at 8.01 mGy for high resolution CT and 1.18 mGy for low dose CT. Low dose CT scans showed 85.49% less absorbed dose than high resolution CT scans.
To prevent recurrence of spontaneous pneumothorax, 23 patients were operated through median sternotomy for simultaneous resection of bilateral bullae, And 27 patients with spontaneous pneumothorax were treated with unilateral thoracotomy, We studied the number, duration and sites of recurrence including findings of CT scan, as well as comparing the both operated group. The incidence of spontaneous pneumothorax was 88% in patients with the ages between 16 to 35 Forty one patients[82%] were operated with the indication of recurrent pne-umpthorax. The number of pneumothorax attack was 2.34 per patient with recurrent pneumothorax. The 87.8% of recurrence was occured within 6 months from last attack. Ips-ilateral recurrnet pneumothorax was 56.1% and contallateral involve was 43.9%. The bilaterality of visible bullae was 90.9% in the findings of chest CT scan and 91.3% in the operative finding. The sensitivity and accuracy for bulla detection with chest CT were 92.6%, respectively. Exclude one case of complicated median sternotomy infection, the postoperative hospital stay was shorter in median sternotomy approached group[P<0.05]. In conclusion, the bullous lesions of the lung have tendency of bilaterality so that median sternotomy for simultaneous resection of bilateral bullae should be considered in patients with contralateral visible bullae with chest CT.

Purpose: The regional emergency medical centers manage the patients with major blunt trauma according to the process appropriate to each hospital rather than standardized protocol of the major trauma centers. The primary purpose of this study is to evaluate the effectiveness and influence on prognosis of additional cervical-thoracic-lumbar-spine computed tomography (CTL-spine CT) scan in diagnosis of spinal injury from the victim of major blunt trauma with impaired consciousness. Methods: The study included patients visited the urban emergency medical center with major blunt trauma who were over 18 years of age from January 2013 to December 2016. Data were collected from retrospective review of medical records. Sensitivity, specificity, positive predictive value, and negative predictive value were measured for evaluation of the performance of diagnostic methods. Results: One hundred patients with Glasgow coma scale ${\leq}13$ underwent additional CTL-spine CT scan. Mechanism of injury was in the following order: driver, pedestrian traffic accident, fall and passenger accident. Thirty-one patients were diagnosed of spinal injury, six of them underwent surgical management. The sensitivity of chest, abdomen and pelvis CT (CAP CT) was 72%, specificity 97%, false positive rate 3%, false negative rate 28% and diagnostic accuracy 87%. Eleven patients were not diagnosed of spinal injury with CAP CT and C-spine lateral view, but all of them were diagnosed of stable fractures. Conclusions: C-spine CT scan be actively considered in the initial examination process. When CAP CT scan is performed in major blunt trauma patients with impaired consciousness, CTL-spine CT scan or simple spinal radiography has no significant effect on the prognosis of the patient and can be performed if necessary.
Background : the prediction on changes in the lung function after lung surgery would be an important indicator in terms of the operability and postoperative complications. In order to predict the postoperative FEV1 - the commonly used method for measuring changes in lung function- a comparison between the quantitative CT and the perfusion lung scan was made and proved its usefulness. Material and Method : The subjects included 22 patients who received perfusion lung scan and quantitative CT preoperatively and with whom the follow-up of PFT were possibles out of the pool of patients who underwent right lobectomy or right pneumonectomy between June of 1997 and December of 1999. The FEV1 and FVC were calibrated by performing the PFT on each patient and then the predicted FEV1 and FVC were calculated after performing perfusion lung scan and quantitative CT postoperatively. The FEV1 and FVC were calibrated by performing the PFT after 1 week and after 3 momths following the surgery. Results : There was a significant mutual scan and the actual postoperative FEV1 and FVC at 1 week and 3 months. The predicted FEV1 and FVC(pneumonectomy group : r=0.962 and r=0.938 lobectomy group ; r=0.921 and r=913) using quantitative CT at 1 week postoperatively showed a higher mutual relationship than that predicted by perfusion lung scan(pneumonectomy group : r=0.927 and r=0.890 lobectomy group : r=0.910 and r=0.905) The result was likewise at 3 months postoperatively(CT -pneumonectomy group : r=0.799 and r=0.882 lobectomy group : r=0.934 and r=0.932) Conclusion ; In comparison to perfusion lung scan quantitative CT is more accurate in predicting lung function postoperatively and is cost-effective as well. Therefore it can be concluded that the quantitative CT is an effective method of replacing the perfusion lung scan in predicting lung function post-operatively. However it is noted that further comparative analysis using more data and follow-up studies of the patients is required.
