Radical Oncological Surgery and Adjuvan Therapy in Non-Small Cell Lung Cancer Patients over 70 years of Age

  • Background: The incidence of lung cancer increases with age. Approximately 50% of non-small cell lung cancer (NSCLC) patients are over 70 years old. Because of the increasing elderly population, treatment approaches in this age group continue to be studied similar to groups of young people. Materials and Methods: In the current study, 26 patients who underwent radical surgery and adjuvan chemoradiation at Ataturk Chest Diseases and Chest Surgery Training and Research Hospital were evaluated retrospectively. Results: Of 21 patients (81%) were male and the average age was 74.4. Lobectomy was performed in 18 cases, pneumonectomy in 3, sleeve lobectomy in 3 and bilobectomy in 2. There was no perioperative or early period mortality. Overall survival was 24.5 months. Conclusions: From our study, lung cancer surgery and adjuvant therapy can be performed safely with low morbidity in the elderly.

Rate of Transformation and Normal Range about Cardiac Size and Cardiothoracic Ratio According to Patient Position and Age at Chest Radiography of Korean Adult Man (한국인 성인 남성의 흉부 방사선영상에서 자세와 연령에 따른 심장 크기 및 심흉비의 정상 범위와 변환율)

  • Purpose of this study is present the normal range of cardiac size and cardiothoracic ratio according to patient position(chest PA and AP) and age of Korean adult male on digital chest X - ray, And to propose a mutually compatible conversion rate. 1,024 males were eligible for this study, among 1,300 normal chest patients who underwent chest PA and low-dose CT examinations on the same day at the 'S' Hospital Health Examination Center in Seoul From January to December 2014. CS and CTR were measured by Danzer (1919). The mean difference between CS and CTR was statistically significant (p<0.01) in Chest PA (CS 135.48 mm, CTR 43.99%) and Chest AP image (CS 155.96 mm, CTR 51.75%). There was no statistically significant difference between left and right heart in chest PA and AP images (p>0.05). CS showed statistically significant difference between Chest PA (p>0. 05) and Chest AP (p<0.05). The thorax size and CTR were statistically significant (p<0.01) in both age and chest PA and AP. Result of this study, On Chest AP image CS was magnified 15%, CTR was magnified 17% compare with Chest PA image. CS and CTR were about 10% difference by changing posture at all ages.

Value of Porous Titanium Alloy Plates for Chest Wall Reconstruction after Resection of Chest Wall Tumors

  • Objective: To explore the value of porous titanium alloy plates for chest wall reconstruction after resection of chest wall tumors. Materials and Methods: A total of 8 patients with chest wall tumors admitted in our hospital from Jan. 2006 to Jan. 2009 were selected and underwent tumor resection, then chest wall repair and reconstruction with porous titanium alloy plates for massive chest wall defects. Results: All patients completed surgery successfully with tumor resection-induced chest wall defects being $6.5{\times}7cm{\sim}12{\times}15.5$ cm in size. Two weeks after chest wall reconstruction, only 1 patient had subcutaneous fluidify which healed itself after pressure bandaging following fluid drainage. Postoperative pathological reports showed 2 patients with costicartilage tumors, 1 with squamous cell carcinoma of lung, 1 with lung adeno-carcinoma, 1 with malignant lymphoma of chest wall, 2 with chest wall metastasis of breast cancers and 1 with chest wall neurofibrosarcoma. All patients had more than 2~5 years of follow-up, during which time 1 patient with breast cancer had surgical treatment due to local recurrence after 7 months and none had chest wall reconstruction associated complications. The mean survival time of patients with malignant tumors was ($37.3{\pm}5.67$) months. Conclusions: Porous titanium alloy plates are safe and effective in the chest wall reconstruction after resection of chest tumors.

The Effect of the Chest Physiotherapy in Brain Injury Patients (뇌손상 환자에게 적용한 흉부물리요법의 비교연구)

  • The purpose of this study was to compare chest percussion with chest vibration in brain injury patients. 30 patients in SICU of one general hospital in T city were randomely divided by 3 groups and the arterial oxygen tension(PaO2) and the retained secretion were measured. The chest vibration was done among the first group, the chest vibration by hand after the chest percussion was done among the second group and the mechanical chest vibration after the chest percussion was done among the third group. The data was analyzed using SPSS 7.0(5% significance) and the results are below. 1) It was adopted the hypothesis that the amount of retained secretion of endotracheal suction after stopping the actions among the first, second and the third groups is different from each other.(F=41.62, p=0.00) 2) It was rejected the hypothesis that the arterial oxygen tension(PaO2)s are different from each other among the first, second and third group.(F=1.22, p=0.31) The amount of the retained secretion after chest percussion, chest vibration by hand or mechanical chest vibration was significantly different from each other. Therefore, chest physiotherapy could be regarded as the effective nursing intervention for the unconscieus patients who have the inappropriate airway cleaness and it was more effective to be together than to be alone.

Research on the Amount To Be Removed at the Neckline According to the Upper Chest Type (흉부상부형태에 따른 네크라인의 줄임분량에 관한 연구)

  • This research is focused on how to divide the upper chest type and to find out the amount to be removed at the neckline according to the different upper chest type and different neckline depth. The result of the research is as followed. 1. In the result of the factor analysis, the upper chest could be explained by six factors such as the shape factor of the upper chest, shoulder curve factor, shoulder sloping factor, space factor from shoulder to waist, upper chest slope factor, and the bust size factor. 2. The shape of the upper chest could be divided by three groups. The first group is characterized as lagging and bent backward shoulder, slight slope of upper chest, and small bust. The second group is comparatively curved at the upper chest and big bust. The third group is rising and bent forward shoulder, slight curved upper chest. 3. The amount to be removed at the different neckline depth and in these three types of upper chest had a difference due to the differences of the upper chest shape containing the shoulder shape.

Comparisons of Image Quality and Entrance Surface Doses according to Care Dose 4D + Care kV in Chest CT (Chest CT에서 Care Dose 4D+Care kV에 따른 화질과 입사표면선량 비교)

  • This study compared DLP values along with phantom entrance surface doses and the image quality of chest CT scans made using a Care Dose 4D+Care kV System, scans that are made using only the Care Dose 4D function, and scans that are made with changes made by applying 80 kVp, 100 kVp, 120 kVp, and 140 kVp to the Care Dose 4D and tube voltage to search for methods to maintain the highest image quality with minimal patient doses. It was shown that DLP values decreased 6.727% when scans were taken with Chest Care Dose 4D + Care kV semi 100 and 6.481% when scans were taken with Chest Care Dose 4D + Care kV. With Chest Non as a standard, skin surface doses decreased 16.519% when scans were taken with Chest Care Dose 4D + Care kV semi 100 and 15.705% when scans were taken with Chest Care Dose 4D + Care kV. With comparisons of image quality, when comparisons were made with Chest Non, comparisons made of SNR values and CNR values in all scanning conditions including Care Dose 4D + Care kV showed that there were no significant differences at P>0.05. Imaging using Chest Care Dose 4D + Care kV in chest CT showed that exposure doses decreased similarly to result values gained from the best conditions through manual adjustments of kV and mAS, and there were no significant differences in image SNR and CNR. If the Chest Care Dose 4D + Care kV function is used, image quality is maintained and patient exposure to radiation can be reduced.

Traumatic Funnel Chest due to Blunt Trauma -A Case Report- (둔상에 의한 외상형 누두흉 -1례 보고-)

  • This 51 years old male patient was admitted to the department of thoracic and cardiovascular surgery via OPD because of anterior chest pain. 7days before admission, He got the chest trauma after traffic accidents,the lateral chest roentgenogram showed complete transverse sternal fracture. He also complained of mild dyspnea. We also noticed that he had depressed anterior chest wall. It looks like funnel chest. The operative findings revealed dislocated & callus formations at the both 4th and 5th costochondral junction and transverse fracture of sternal body between 4th and 5th costochondral junction, the upper end of sternal fracture was situated below the lower end of sternal fracture. The two ends of sternal fracture were situated at the same level and reapproximated the two ends by two-interrupted wire sutures. The patient is well on the road to recovery after the operation.

  • The purpose of this study was to investigate the chest physical therapy in the hospital of seoul, kyungido and to activate the chest physical therapy. Dept. of physical therapy in 15 hospitals, 65 physical therapists disabled and analysis of interview. The results are as follows: 1) $86.7\%$ of respondents were not chest physical therapist in the hospital. $79.4\%$ of respondents were a lack of 5 patients per month. 2) The chief diagnosis of chest physical therapy was respiretory muscles weakness by reason of spinal cord injury and muscle diseases. Many physical therapists was obtain the chest physical therapy information from university or collage. 3) Management of chest Physical therapy Patient was to be alike physical therapist and doctor A problem of chest physical therapy was a lack of chest physical therapy information, cognition of a physician. The results of this study suggested that chest physical therapy need to organization of the treatment and magnification of the treatment area.

Comparison of New Infant Chest Compression Methods: Simulation Study on Randomization Using Manikin

  • In this paper, we propose a the effects of new chest compressions in the implementation of chest compressions during infant cardiopulmonary resuscitation, and to provide basic data for high-quality CPR. On March 12, 2019, the research target used the SPSS 22.0 Version as an experimental study using randomized cross-design of 30 emergency medical services students who completed BLS Health Care-provider. The study also showed significant differences in chest depth and average rate of pressure($34.61{\pm}1.29$, $39.40{\pm}1.08$, <0.001, $105.46{\pm}4.23$, $107{\pm}3.84$, <0.001) depending on chest compressions. There was also a significant difference in the convenience and degree of pain of chest compressions(<0.001). In addition, new chest compressions appeared close to vertical and showed statistically significant differences(p<.001). Based on the results of this study, we can see that the accuracy of the new chest compressions during infant cardiopulmonary resuscitation is increased, and the depth of chest compressions is improved, improving the quality index of chest compressions. However, it will be necessary to further study the use of the new chest compressions to identify the potential for clinical use.

The Effect of Chest Physiotherapy on the Amount of Tracheal Secretion and $PaO_2$ (흉부물리요법이 기관 분비물량과 동맥혈 산소분압에 미치는 영향)

  • The purpose of this research was to find out the effect of chest physiotherapy on the amount of tracheal secretion and $PaO_2$. After changing position of the neurosurgical patients who had tracheostomy cannula, experimental treatments were applied as bellows, manual chest percussion on groupI, chest percussion and manual chest vibration on groupII, chest percussion and mechanical chest vibration on groupIII were done. After these trials, we have analyzed the efficacy of each procedures comparing the group differences in the quantity of tracheal secretion and $PaO_2$. Target samples were sixty patients aged between 20 to 60 who have tracheostomy state and decreased consciousness status that were admitted in NICU of a university hospital from June 1 to August 31, 1999. They assigned randomly into three experimental groups. To compare the effect of each interventions, tracheal secretion quantify was measured and $PaO_2$ was analyzed via arterial blood gas analyzer. The data were analyzed by ANCOVA of 5% significance level using SPSS P/C program. The results were as bellows. 1) The first hypothesis 'There is a difference In the quantify of the secretion among GroupI, GroupII and GroupIII' was accepted.(F=29.27, p=0.00) 2) The second hypothesis 'There is a difference in $PaO_2$ among GroupI, GroupII and GroupIII' was rejected.(F=1.71, p=0.19) From this study results, positional change and manual chest vibration including chest percussion were the most effective treatment to get maximum amount of tracheal secretion and it was confirmed that mechanical chest vibration also made much better effect than sole chest percussion method. Therefore, we concluded that the mechanical or manual chest vibration with chest percussion is more effective respiratory care method than the sole chest percussion.
