  • One of the major problems of cancer chemotheraphy is the development of drug resistance in tumors, resulting in reduced responsiveness to subsequent treatments. The folate antagonists are being used to treat such diverse illnesses as cancer, leukemia, psoriasis, rheumatoid arthritis, etc. Previous studies have established that resistance to antifolates may occur in mammalian tumor cells by one or more of five mechanisms ; (a) an increase in the levels of the target enzyme, generally as a consequence of gene amplification ; (b) an alteration in the target enzyme, leading to an enzyme with a decreased binding affinity for the drug ; (c) a decrease in the uptake of the drug into the cells ; (d) increased extrusion of drugs out of cells ; (e) impaired ability to polyglutamylate the parent drug which is capable of being intracellularly metabolized to longer chain length.

High Grade Surface Osteosarcoma - Case Report - (고등급 표면(High Grade Surface) 골육종 - 증례 보고 -)

  • High grade surface osteosarcoma is the most rare subtype of osteosarcoma arising on the surface of bone, accounting for less than 1% of the total number of osteosarcomas. Only a few case reports and studies have been reported in the world. In Korea, only one case out of 127 osteosarcomas has been described up to now, but there was no information about the patient, clinicopathologic features and treatment. We experienced a case of high grade surface osteosarcoma in the subtrochanteric area of a 66-year-old female and treated her with neoadjuvant chemotheraphy, wide resection and limb salvage operation with tumor prosthesis and adjuvant chemotheraphy. This tumor is identical to conventional high grade intramedullary osteosarcoma in histology, treatment and prognosis. So, this tumor should be differentiated from other surface osteosarcomas such as parosteal osteosarcoma and periosteal osteosarcoma.

  • Osteosarcoma is the most common primary bone tumor except for multiple myeloma. Hematogenous spread of osteosarcoma presents the t1susal route of dissemination. Lung metastasis is the most common, however the lymph node involvement is quite rare. In addition, according to Mirra, radiologically detectable involvement of lymph node is an extremely rare manifestation of osteosarcoma. The authors had experienced a patient with an osteosarcoma of the right distal femur which spread to the right inguinal lymph node and lung. After preoperative chemotheraphy, a limb salvage operation was performed around the lesion of right distal femur and the right inguinal lymph nodes were dissected. A adjuvant postoperative chemotheraphy was performed. After the treatment, there was no recurrence of osteosarcoma or any other metastatic evidence for two years and 8 months.

Characterization of Soluble Type Human Folate Receptor on Folate Binding and Transport (Soluble type의 human folate receptor의 folate 결합과 수송에 관한 특성)

  • We have cloned a soluble type human folate receptor(hFR type${\gamma}$) from human thymus cDNA library using the PCR amplification technique. To examine whether hFR type${\gamma}$ has a folate transport activity, CHO cells were transfected with the pcDNAhFR${\gamma}$ expression plasmid, and the stable cell line CHO/hFR${\gamma}$ expressing a high level of the hFR type${\gamma}$ was identified by northern and western blot analysis. The CHO/hFR${\gamma}$ cells produced a [$H^3$]folic acid binding protein in the culture medium. However, we couldn't detect any cell surface [$H^3$] folic acid binding and transport activities. The growth of the CHO/hFR${\gamma}$ cells was more rapidly inhibited than the wild type CHO cells in the low concentration folic acid media. These observations indicate that although soluble type human folate receptor can bind [$H^3$]folate, it does not involve in folate transport.

  • Approximately 9.8% of neurogenic tumors of the mediastinum extend into the spinal column so that the composite neoplastic mass was dumbbell shaped. We experienced three patients confirmed by Dumbbell shaped mediastinal tumor radiologically and tried surgical resection by posterolateral thoracotomy only corpectomy and costotransversectomy was performed simultaneously in three patients and interbody fusion in two. In one patient the diagnosis was liposarcoma and in 1 neuroblastoma and in 1 neurilemmoma. then followed by radiation theraphy in case I and radiation and chemotheraphy in case II. All three cases showed satisfactory results clinically and radiographically.

  • The fibromatoses are defined as a group of non encapsulated, non metastasizing, fibrous tumors that have a tendency for infiltrative growth and local recurrence following surgical excision and can be divided into fascial and musculoaponeurotic fibromatoses. Infantile fibromatosis is childhood counterpart of musculoaponeurotic fibromatosis and is more common in the head and neck, the shoulder and upper arm and the thigh. Treatment includes excision, radiation, steroid and chemotheraphy, etc. Treatment of choice is complete excision but postoperative deformity and sequelae should be considered. Recently, we have experienced a case of infantile fibromatosis originating from infratemporal fossa and report this case with a review of literatures.

Treatment of MFH(Malignant fibrous histiocytoma) in Extremity (사지에 발생한 악성섬유조직구종의 치료 경험)

  • Purpose: Malignant fibrous histiocytoma(MFH) is the most common soft tissue sarcoma in adult. As to this date, tissue development, treatment and prognosis of the tumor has not been definitely clarified, however, it has been reported that wide surgical resection of the tumor along with the radiotheraphy and chemotheraphy is needed for treatment. In MFH with high recurrence rate, the reconstruction method and points to be considered for reconstruction in recurrent case were studied in 10 patients who were treated in our hospital. Methods: From August of 1991 to August 2007, location of tumor, initial mass size, 1st recurred period, lymph node metastasis, recurrence rate, treatment modality, complication, reconstruction in recurrent defect, and follow up period was studied in 10 patients who underwent reconstruction at our Plastic surgery department following wide excision. Results: The average age was 62.8(46 - 73) years old, average follow up period was 7.7(1 - 17) years. Various reconstructions has been performed for recurrent cases and postoperative chemotheraphy and radiotheraphy was done. As for reconstruction in recurrent cases, After wide excision, local flap was performed in 6 cases, and free flap in 2 cases. After radiotherapy, osteoradionecrosis was occurred in 4 cases. Recurrence rate was 1 - 5(2.6) times and reconstruction due to recurrence was 7 out of 10 cases(70%). Conclusion: The treatment modality of MFH is not yet defined. Due to it's high recurrence rate, radiotherapy and chemotherapy is commonly combined with surgery. Even still, additional excision and reconstruction may be required. Therefore, possibility of re-operation must be considered when performing every excision and reconstruction; in case a recurrence or osteoradionecrosis occurs. Free flap coverage should be left as the last resort, according to the principle of reconstruction. Nevertheless, if the defect is large or osteoradionecrosis is present, it will benefit greatly to the patient's quality of life.

Effects of Laughter Therapy on Depression, Quality of Life, Resilience and Immune Responses in Breast Cancer Survivors (웃음요법이 유방암 생존자의 우울, 삶의 질, 극복력, 면역반응에 미치는 효과)

  • Purpose: In this study, the effects of laughter therapy on levels of depression, quality of life, resilience and immune responses in breast cancer survivors were examined. Methods: A quasi-experimental nonequivalent control group, pretest-posttest design was used. Participants (n=37) included breast cancer survivors who finished chemotheraphy and radiation therapy: 16 in the experiment group and 21 in the control group. Data were collected from August to November 2009. The experimental group participated in laughter therapy eight times, twice a week for 60 min per session. Questionnaires were used to me-asure pretest and posttest levels of depression, quality of life and resilience. A blood test was used to analyze changes in Total T cell, T helper, T suppressor, Th/Ts ratio, Total B cell, T cell/B cell ratio and NK cell for immune responses. Results: The results showed that laughter therapy was effective in increasing the quality of life and resilience in breast cancer survivors. but depression and immune responses did not differ significantly between the groups. Conclusion: The results of the study indicate that laughter therapy may be an effective nursing intervention to improve quality of life and resilience in breast cancer survivors.

Surgical Treatment of Primary Pulmonary Leiomyosarcoma; Two Cases Report (원발성 폐평활근육종의 외과적 치료;2례 보고)

  • The incidence of pulmonary leiomyosarcoma is very rare as a primary lung tumor. Usually, pulmonary leiomyosarcoma arise from the smooth muscle present in the bronchi or blood vessles. We had experienced two cases of primary pulmonary leiomyosarcoma. The first case was 28-year old male patient who had been in good health until admission, when he experienced an episode of dyspnea and sudden hemoptysis. The chest X-ray film revealed a large round tumor mass in left lower lobe measuring 6.5x9.5x5.3cm in dimension. On physical examination,the patient was friction rub and rales on the left lower chest and postoperative course was smooth and non-eventful. Emergency left lower lobectomy was performed due to repeated hemoptysis. Chemotheraphy was done postoperatively as an adjuvant therapy.The second case was 52-year-old man who had been well prior to admission, when recently he noticed a abrupt growing tendency of old pulmonary coin lesion in right lower lobe on routine physical examination. Since 1968, small round mass was gradually enlarged very slowly, during recent one year interval, the tumor mass was enlarged abruptly as twice in size on chest X-ray. Bronchoscopic examination revealed no specipic findings. Right lower lobectomy was performed and pathologic examination was answered as primary leiomyosarcoma without lymph node metastasis. Postoperative course was smooth, except local wound infection.

Synthesis of Chlorogenic Acid and p-Coumaroyl Shikimates from Glucose Using Engineered Escherichia coli

  • Chlorogenic acid and hydroxylcinnamoyl shikimates are major dietary phenolics as well as antioxidants, with recently discovered biological, activities including protection against chemotheraphy side effects and prevention of cardiovascular disease and cancer. Certain fruits and vegetables produce these compounds, although a microbial system can also be utilized for synthesis of chlorogenic acid and hydroxylcinnamoyl shikimates. In this study, we engineered Escherichia coli to produce chlorogenic acid and p-coumaroyl shikimates from glucose. For the synthesis of chlorogenic acid, two E. coli strains were used; one strain for the synthesis of caffeic acid from glucose and the other strain for the synthesis of chlorogenic acid from caffeic acid and quinic acid. The final yield of chlorogenic acid using this approach was approximately 78 mg/l. To synthesize p-coumaroyl shikimates, wild-type E. coli as well as several mutants were tested. Mutant E. coli carrying deletions in three genes (tyrR, pheA, and aroL) produced 236 mg/l of p-coumaroyl shikimates.
