두경부에 발생한 모기질종(Pilomatricoma) 4예 (4 Cases of Head and Neck Philomatricoma)

  • Pilomatricoma is a benign neoplasm of the hair follicle origin which most common occur in the head and neck, particularly in the cheek and preauricular region. This neoplasm is superficially located and stony, hard consistency on palpation and sometimes shows reddish or bluish discoloration on the overlying skin. The diagnosis is confirmed by histopathologic examination. Radiologic evaluation is little diagnostic and fine needle aspiration biopsy often misdiagnose because both tissues from basaloid cell component and shadow cell component should be obtained for correct diagnosis. And Otolaryngologist should consider the possibility of philomatricoma when childhood or early adulthood persion visits with hard, supferficial mass on the parotid or cheek area. The treatment of choice is surgical excision with clear resection margin. In many instances, overlying skin excision is needed. We report 4 cases of philomatricoma arising from preauricular, cheek and posterior neck.

깨물근하 농양 (Submasseteric Abscess)

  • Purpose: The masserteric space is an important tissue compartment of the face, but a disease in it is difficult to diagnose and treat. The submasseteric abscess is located between the masseter muscle and mandibular ramus with different appearances such as sepsis, infection, or tumor. Especially the common misdiagnosis of submasseteric abscess is acute or chronic parotitis. The purpose of this report is to pay special attention to the possible diagnosis of submasseteric abscess for the symptoms of unilateral cheek swelling and tenderness that accompany marked trismus. Methods: A 11-year-old boy came to our hospital because of facial swelling, tenderness, and trismus in a history of left cheek swelling and toothache. We diagnosed his case as submasseteric abscess by CT scan and surgical intervention was performed. Under general anesthesia, the abscess was opened by the intraoral incision and considerably massive pus was drained. Results: Swelling, tenderness, and trismus became to subside during postoperative 10 days and general condition and vital signs became stable. After 6 months, CT scan showed that both masseteric muscles were symmetric and there was no periosteal reaction of the mandible. Conclusion: In conclusion, submasseteric abscess is a rare infection with the symptoms of cheek tenderness and marked trismus. A detailed medical history and clinical examination of a patient as well as computed tomography(CT) are important tools in the accurate diagnosis and efficient treatment of the submasseteric abscess. Adequate drainage, removal of cause, and antibiotic infusion are the management of choice.

미용외과적 측면에서 본 안면부 종양 제거후 재건술에 대한 임상적 고찰 (Aesthetic Reconstruction of Facial Skin after Resection of Facial Tumor)

  • Skin replacement in large cheek defects after excision of benign or malignant tumor on the face is a challenging task. The physical characteristics of cheek skin are matched best by adjacent skin. Various methods of reconstructing of the facial surface such as forehead flap, distant flap, or a full thickness or split thickness skin graft have replaced adjacent tissue for coverage in many cases. We have reviewed ten cases of aesthetic reconstruction of the face after resection of the facial skin tumor within the last 5 years. The first group of 3 patients were reconstructed with split thickness skin graft from the scalp or lower abdomen. The second group of patients were reconstructed with cheek flap. The third group of 3 patients were reconstructed with cervicofacial flap. The last 2 patients were reconstructed with nasolabial flap & island falp respectively. The advantages from our experience with various method of coverage are its hidden donor area & good color match with the facial skin & increased success rate.

Method to prevent cheek depression using an island sternocleidomastoid muscle flap with the middle pedicle as a feeding vessel in immediate reconstruction of the facial nerve with the sural nerve following resection of a parotid gland tumor

  • Many surgeons have demonstrated the validity of sternocleidomastoid muscle flaps for the reconstruction of head and neck tumors. We present a case in which we used an island sternocleidomastoid muscle flap to reconstruct a cheek depression after excision of a malignant parotid tumor. A 44-year-old woman presented with a right malignant parotid tumor. We performed total resection of the parotid gland and facial nerve with the sural nerve and reconstructed the facial nerve and cheek depression with an island sternocleidomastoid muscle flap. The sternal head of the right sternocleidomastoid muscle was cut at the cranial and caudal segments to elevate it as an island flap. We used the superior thyroid artery as the sole pedicle for the island muscle flap. At 1 year and 3 months after the operation, the mimic muscles had gradually recovered and progressed without complications such as Frey syndrome, cervical motor dysfunction, or concave deformation of the neck and cheeks.

  • We report a case of pilomatrixoma presenting in the right cheek of a 6-year-old girl. In the panoramic view, a small, ovoid-shaped, and nonhomogenous calcified mass was superimposed on the right mandibular angle, The mass was located on the skin overlying the right mandibular ramus area in the skull P-A view. The tumor had a strongly reflective pattern with acoustic shadowing in sonographic view. We also illustrate how these lesions can be effectively diagnosed.

Specific Absorption Rate Values of Handsets in Cheek Position at 835 MHz as a Function of Scaled Specific Anthropomorphic Mannequin Models

  • A specific anthropomorphic mannequin (SAM) model was used to investigate the relation between local specific absorption rate (SAR) and head size. The model was scaled to 80 to 100% sized models at intervals of 5%. We assumed that the shell of the SAM model has the same properties as the head-equivalent tissue. Five handsets with a monopole antenna operating at 835 MHz were placed in the approximate cheek position against the scaled SAM models. The handsets had different antenna lengths, antenna positions, body sizes, and external materials. SAR distributions in the scaled SAM models were computed using the finite-difference time-domain method. We found that a larger head causes a distinct increase in the spatial peak 1-voxel SAR, while head size did not significantly change the peak 1-g averaged-SAR and 10-g averaged-SAR values for the same power level delivered to the antenna.

  • One specimen of the percophid fish, Pteropsaron evolans Jordan and Snyder and two specimens of the labrid fish, Xyrichtys verrens (Jordan and Evermann) were newly collected from Jeju Island of Korea. P. evolans is characterized by having one pair of spines at snout, cheek without scales, and elongated first dorsal fin in male. X. verrens is easily distinguished by having tip of pectoral fin black, many rows of scales on cheek, and an elongated pelvic fin. We describe as new to Korean fish fauna and propose new Korean names, "Sil-nun-tung-i" for the former and "Jang-mi-ok-du-nol-rae-gi" for the latter.

  • Main disadvantages of conventional pectorails major myocutaneous flap is bulkness of muscular pedicle. It makes difficult to use this flap in a case of supraomohyoid neck dissection. Pectoralis major myocutaneous island flap is a modification to overcome this shortcoming. And bilobular design of skin portion of this flap could be used for reconstruction of a through and through defect. We report a case of reconstruction of full-thickness defect of cheek with bilobular pectoralis major myocutaneous island flap and compare it with conventional pectoralis myocutaneous flap.

당뇨병 환자에서의 구강점막각화도 및 세포상에 대한 박리세포학적 연구 (Study on Exfoliative Cytology of Keratinization and Morphology of Oral Mucosal Epithelium in Adult Diabetic Patients)

  • This study was undertaken to study the changes of epithelium of oral mucosa caused by diabetic disease in terms of keratinization and morphology of epithelial cells of oral mucasa, and to diagnose the oral diseases caused by systemic origns in terms of oral exfoliative cytology as a diagnostic tool. The author has studied the changes of epithelial cells on the cheek mucosa and upper antirior gingiva of 20 adult diabetic patients by Oral Exfoliative Cytology. And 50 healthy adults were selected as control group. The cytologic smears were stained by Papanicolaou method. The results were as follows : 1. In diabetic patient's cheek mucosa, Yellow staining cells were reveald as 11.8%, which was higher than the control group(4.2%). 2. In diabetic patients' upper anterior gingiva, Yellow staining cells were reaveale as 12.4%, which was lower than the control group(68.2%) 3. The changes of nucleus and cytoplasmic changes were not significantly different in diabetic patients and control group.

Multiple foreign bodies causing an orocutaneous fistula of the cheek

  • Foreign bodies impacted in the maxillofacial region are often a diagnostic challenge. They can be a source of chronic inflammatory reactions and infections leading to the formation of an orocutaneous fistula. Such orocutaneous fistulas cause significant morbidity in most patients, eventually requiring surgery. Recently, we encountered a very rare case of an orocutaneous fistula caused by multiple foreign bodies in the cheek. Precise removal of the foreign bodies was required, and a double-sided anterolateral thigh free flap was used to reconstruct the defect. Surgeons should be aware of the complications of multiple foreign bodies and should be able to diagnose these on careful clinical examination.
