Lateral Third Infraclavicular Implantation of the Vagal Nerve Stimulation Generator Through Axillary Incision

  • Objective : This report describes the clinical study of the surgical method of lateral third infraclavicular implantation of vagal nerve stimulation (VNS) generator through the axillary wrinkle incision. Methods : In a retrospective study, the data for 20 patients with medically intractable epilepsy treated by this approach were examined. The mean age was 31.4 years (range : 14-50), and the mean follow-up period was 12.15 months (range : 4-21 months). The male to female ratio was 2.3 : 1. The subcutaneous pocket for the generator was located in the lateral third infraclavicular area through the axillary wrinkle. Our method was a modification of the standard VNS generator implantation in the mid-infraclavicular pocket through anterior axillary incision. Results : There were the excellent or good cosmetic satisfaction in 95% of the cases and fair in 5%. The generator was located outside the lung field in 15%, periphery of the lung field in 45%, and crossed over the lung field in 40%. Discomfort from shoulder motion occurred transiently in 35% of cases. Other complications were minimal. Conclusion : These results demonstrate that the lateral third infraclavicular apporach will offers cosmetic benefits and reduction of obscuration of the lung field without serious complications. Thus, this technique provides an attractive alternative among the surgical techniques for the vagal nerve stimulation.

An Experimental Study on the Characteristics of Moxa Combustion in the Moxa Materials (재료(材料)에 따른 뜸의 연소(燃燒) 특성(特性)에 대한 연구(硏究))

  • In order to understand the stimulation quality of Artemisine Vulgaris Folium(Bong), Mori Ramulus(Sangi) and Persicae Ramulus(Dogi) combustion, and get the basic data for the development of electric moxibustion apparatus, the average temperature, peak temperature, average gradient temperature, maximum gradient temperature and combustion time of heating period on the three moxa materials were measured. 1. The average combustion temperature was high in order of Dogi, Sangi and Bong, and these were acknowledged to have a significant difference each other in the average temperature. 2. The peak combustion temperature among those was high in order of Dogi, Sangi and Bong, and Bong was acknowledged to have significant differences with Dogi and Sangi. Sangi and Dogi however were not acknowledged to have difference each other. 3. The average gradient temperature among those was high in order of Dogi, Sangi and Bong, and Bong was proved to have significant differences with Dogi and Sangi. Sangi and Dogi however were not proved to have difference each other. 4. The maximum temperature among those was high in order of Dogi, Sangi and Bong, bong was acknowledged to have signigicant differences with Dogi and Sangi, but Sangi and Dogi were not proved to have difference each other. 5. The combustion time was short in order of Dogi, Sangi and Bong. Bong was acknowledged to have significant differences with Dogi and Sangi, but Sangi and Dogi were not. In order to understand well the characteristics of combustion, it is required to have a quantitative interpretation of combustion calory, and, in the future, we expect it is required to have a consistent study for the clinical effectiveness and the mutual relationship according to the combustion characteristics.

NaCl Content in Korean Storage Foods (우리나라 저장식품중의 NaCl 함량)

  • In order to help food habits of Korean, major foods of side dishes in Korean foods were analyzed by Mohr's Silver Nitrate Method. The kinds of foods were Kanchang (Sa)Sauce) Kochuchaag(Redpepperbean paste), Doeuchang(Soy bean Paste) and Kimchies, and selected into 3 areas in Korea; Seoul (city area), Kyung Puk Province (Fishery area) and Chun-Puk Province (Farming area). The results are as follows; Average NaCl content of Kan-chang is 26.04g/dl ranged from 24.27 to 28.22g/dl and among them Farming area is the highest content. In Ko-chu-chang, average content is 11.36g% ranged from 10.77 to 11.60g%. Doen-chang has an average 15.89% ranged from 13.28 to 18.54g% and in Kimckies they were resulted 3.36g%.

  • Though he is a experienced expert, it takes much time and considerable effort to review this items in detail since the widespread terms to plan distribution system is considered. Therefore it needs software system for planning of distribution system to solve much time and human resoureces due to manipulate many data and calculate sophiscated technology. The object this study raises its availability by assisting distribution works such as load forecast, substation planning in ChangWon city based on CADPAD software package.

Percutaneous Balloon Compression of Trigeminal Ganglion for the Treatment of Idiopathic Trigeminal Neuralgia : Experience in 50 Patients

  • Objective : We assessed the surgical results of percutaneous balloon compression in 50 patients with idiopathic trigeminal neuralgia. Methods : Fifty patients with follow-up period of more than 12 months were retrospectively analyzed. The mean follow-up period was 42 months (range, 12-82). The mean age was 65.8 years (range, 27-83). Seventeen patients (34%) had other previous surgical procedures. The balloon was inflated by injecting radio-contrast media under brief general anesthesia according to Mullan’s technique. The mean inflating time was 88 seconds (range, 60-120). The whole procedure took about 20 minutes. Results : We reported excellent and good results in 70% of the cases, poor in 6% as annoying dysesthesia, recurrence in 16%, and 8% failure due to technical deficiencies. Forty-six patients (92%) were initially relieved of their pain. There were permanent motor weakness of the masseter muscle in 4% of patients and transitory diplopia in 8%. Neither anesthesia dolorosa nor keratitis occurred. Almost all patients (92%) were discharged postoperatively within two days. Conclusion : These results indicate that balloon compression would be an effective method with acceptable morbidity, technically, It can be performed rapidly and simply in the treatment of idiopathic trigeminal neuralgia.

  • Since bridge decks have many cyclic loadings by such as moving loads in a whole life, effects of cyclic loading as well as static loading should be considered. Particularly, in a view of serviceability and durability, structural performance under cyclic service loadings is very important. In this paper, experimental studies on flexural behavior of RC beams with loop joints under cyclic loadings were conducted. Then the structural behavior for serviceability and strength were discussed.
