Surgical treatment of postpneumonectomy empyema associated with bronchopleural fistula; A report of Three cases (기관지늑막루를 동반한 농흉의 외과적 치료-3 치험례-)

  • Persistent bronchopleural fistula still presents a troublesome therapeutic challenge and demands an aggressive approach when conventional measures fail. Empyema associated bronchopleural fistula developed after resectional surgery and their primary diseases were lung abscess in one case, bronchiectasis in two cases. Three cases of postpneumonectomy empyema associated bronchopleural fistula were treated surgically with a pedicled intercostal muscle grafting and concomittent thoracoplasty. After the procedure, patients had no recurrent symptoms or signs of bronchopleural fistula and discharged from hospital with improved condition.

  • Cardiac surgery in neonates with congenital heart disease has progressed dramatically in the past three decades. However, low-birth-weight neonates with congenital heart disease continue to challenge the intellectual and technical skills of healthcare professionals. We present a case of a low-birth-weight neonate with pulmonary atresia and a ventricular septal defect, in whom palliation was achieved with a right ventricular outflow tract stent using a hybrid procedure.

  • Patients with concomitant surgical diseases of the heart and lungs are a therapeutic challenge to cardiothoracic surgeons. A 59-year-old woman underwent right middle lobectomy for lung cancer and redo double valve replacement with tricuspid annuloplasty simultaneously. Concomitant operation is a safe procedure and might allow prompt correction of both conditions, thereby sparing the patient a second major thoracic procedure with its attendant risks.

  • Light weight superstructure is beneficial for bridges in remote areas and in emergency erection. In such weight sensitive applications, combination of fibre reinforced plastics (FRP) as material and box-girders as a structural system have great scope. This combination offers various options to tailor structure and its elements but this flexibility poses greater challenge in optimum design. In this paper a procedure is derived for a generalised optimum design of FRP box-girder bridges, using genetic algorithms (GA). The formulation of the optimum design problem in the form of objective function and constraints is presented. Size, configuration and topology optimization are done simultaneously. A few optimum design studies are carried out to check the performance of the developed procedure and to get trends in the optimum design which will be helpful to the new designers.

  • Traumatic dental injury can result in avulsion of anterior teeth. In young patients, it is a challenge to the dental professional because after replantation, late complications such as ankylosis require tooth extraction. Although prosthetic and orthodontic treatment, and implant placement have been described as the options for intervention, autogenous tooth transplantation could be an effective procedure in growing patients if there is a suitable donor tooth available. This case presents the treatment of a patient who suffered a traumatic injury at 9 years old with avulsion of tooth 21, which had been replanted, and intrusion of tooth 11. Both teeth ankylosed; thus they were removed and autotransplantation of premolars was carried out. After transplantation, the tooth underwent root canal treatment because of pulpal necrosis. Orthodontic treatment began 3 months after transplantation and during 7 years' follow-up the aesthetics and function were maintained without signs of resorption.

Arthroscopic supraspinatus advancement for retracted rotator cuff tears: a technical note

  • Irreparable rotator cuffs with retracted torn ends remain a significant challenge for most shoulder surgeons. Since repairs are preferable to reconstruction or replacement whenever possible, studies for anatomical reductions with minimal tension and secure fixation are important. In this study, the authors introduce an arthroscopic supraspinatus advancement (ASSA) procedure for retracted rotator cuff tears that could not be adequately reduced to the original footprint. Using modified long, narrow, curved Cobb elevators, procedures can be performed through lateral portals without any additional skin incision. Following meticulous stepwise three-compartment elevation procedures based on the supraspinatus insertion anatomy, the supraspinatus muscle could be safely elevated from the fossa and sufficiently advanced laterally. The authors suggest that ASSA could be a useful procedure for management of challenging retracted rotator cuff tears by maximizing lateral excursions that could convert irreparable tears to reparable tears in select patients.

An Overview of Bootstrapping Method Applicable to Survey Researches in Rehabilitation Science

  • Background: Parametric statistical procedures are typically conducted under the condition in which a sample distribution is statistically identical with its population. In reality, investigators use inferential statistics to estimate parameters based on the sample drawn because population distributions are unknown. The uncertainty of limited data from the sample such as lack of sample size may be a challenge in most rehabilitation studies. Objects: The purpose of this study is to review the bootstrapping method to overcome shortcomings of limited sample size in rehabilitation studies. Methods: Articles were reviewed. Results: Bootstrapping method is a statistical procedure that permits the iterative re-sampling with replacement from a sample when the population distribution is unknown. This statistical procedure is to enhance the representativeness of the population being studied and to determine estimates of the parameters when sample size are too limited to generalize the study outcome to target population. The bootstrapping method would overcome limitations such as type II error resulting from small sample sizes. An application on a typical data of a study represented how to deal with challenges of estimating a parameter from small sample size and enhance the uncertainty with optimal confidence intervals and levels. Conclusion: Bootstrapping method may be an effective statistical procedure reducing the standard error of population parameters under the condition requiring both acceptable confidence intervals and confidence level (i.e., p=.05).

  • This study has the purpose to investigate other countries' arbitrator systems to compare with the current KCAB's system and to find their merits and demerits so that we can make up for the demerits of KCAB's arbitrator system and to make the most use of its merits. The most important factor of arbitral procedure is the arbitrator. If we compare clauses related with the arbitrator to KCAB's arbitration rules, expand the merits of it and apply the merits of other arbitrating organs, KCAB's arbitration can avoid criticism which it has got so far while it was handling the international cases. Also, we may need to grow up the role of the executive office in the range of respecting the self-government of the concerned party for the rapid proceeding of arbitral procedure. According to the foreign countries' international arbitration rules, they go with the process that they firstly give the concurrence period to the concerned parties, especially related with the arbitral procedure such as selection of arbitrator or filling the vacancy of the arbitrator, and as for the concerned party who doesn't fulfill within that period, the arbitrating organ or the other one corresponding to the pertinent arbitrating organ in case of the ad-hoc arbitration rules fulfill instead.

A Study on the 'Emergency Relief' System of International Centre for Dispute Resolution (국제분쟁해결센터(ICDR)의 '긴급구제'제도('emergency relief' system)에 관한 연구)

  • This article examines the requirements of Article 37 of the ICDR International Arbitration Rules and issues that could arise if a party petitions a U.S. Federal Court to enforce an emergency arbitrator's Article 37 decision to grant pre-arbitration provisional relief. On May 1, 2006, ICDR introduced a new procedure for the granting of emergency arbitral relief under its ICDR Rules. The procedure enables a party to apply for emergency interim relief before the appointment of an arbitrator or tribunal to adjudicate the merits of the dispute. Instead, the application for emergency relief is considered by an emergency arbitrator appointed by the ICDR. In short, the ICDR has quickly appointed emergency arbitrator and resolved a challenge to an appointment within 36 hours. In addition, the emergency decisions have been issued within just a couple of weeks. In particular, we looked at what would happen after Article 37 emergency relief is granted. Based on my examination of U.S. cases on the enforceability of interim awards and orders, We conclude that U.S. courts would enforce Article 37 interim measures, whether they are characterized by the emergency arbitrator as an interim order or award. Where the situation warrants, arbitration executives should embrace and use emergency relief procedure of ICDR Rules.

  • Increasing user demand for the more bandwidths in wireless communications has motivated a new challenge to utilize the benefits of wireless ATM technology This paper focuses on how to provide IP services to a mobile user over wireless ATM networks. By enhancing the NHRP standard, several shortcomings are improved that are resulted from the naive combinations of Mobile IP over ATM and IP over wireless ATM. This paper presents a mobile location management algorithm that is referred to as the Mobile NHRP scheme. The mobile tracking procedure and the mobile locating procedure are explicitly defined in the proposed scheme. On the locating procedure, the source station may obtain the current location of the destination from NHSs(Next Hop Servers). Therefore the optimized connection can be setup directly. The total costs per move and per setup over ATM/IP overlaid networks are estimated.
