A patient with stress induced cardiomyopathy that occurred after cessation of hormone replacement therapy for panhypopituitarism (범뇌하수체저하증의 호르몬 대체요법 중단 후 발생한 스트레스 심근병증)

  • Stress induced cardiomyopathy (SC) is characterized by transient left ventricular (LV) dysfunction in the absence of coronary artery disease. We report on a patient with panhypopituitarism who developed SC resulting from withdrawal of hormonal replacement therapy (HRT). A 52-year-old male visited our hospital for progressively worsening dyspnea. The patient had discontinued HRT 7 days ago, which had been administered for 18 months after transsphenoidal adenomectomy for pituitary macroadenoma. Initial electrocardiogram showed marked sinus bradycardia. Transthoracic echocardiography showed apical ballooning with an LV ejection fraction of 25%. No significant obstructive lesions were observed on coronary angiography. With a clinical diagnosis of SC associated with panhypopituitarism, HRT was restarted, including glucocorticoid and thyroxine, along with standard heart failure management. His LV function had normalized at 2-month follow-up. He remains asymptomatic and administration of beta-blocker and angiotensin converting enzyme inhibitor were discontinued He currently only requires HRT.

Relationships among Health Behavior, Wellness Condition, and Stage of Change in Health Behavior by Cardiocerebrovascular Risk in Male Office Workers (남성 사무직근로자의 심뇌혈관질환 위험요인에 따른 건강행위, 웰니스 상태 및 건강행위변화단계의 관계)

  • Purpose: The purpose of this study was to investigate the relationships among health behavior, wellness condition, and stage of change in health behavior by risk of cardiocerebrovascular diseases (CVD) in male office workers. Methods: A total of 205 male office workers participated in the 2017 National Health Examination at a manufacturing/ R&D business in Seongnam and completed self-reported questionnaires. Results: There were significant differences in health behavior scores by risk of CVD (F=4.78, p=.009) and statistically significant differences in no smoking (F=5.86, p=.003), exercise (F=5.49, p=.005), and health checkup (F=4.39, p=.014). There were statistically significant differences in health behavior (t=-4.14, p<.001) and wellness condition (t=-2.61, p=.010) by the stage of change in health behavior. Health behavior had a weak positive correlation with wellness condition (r=.36, p<.001). Logistic regression analysis showed that, when adjusted for age and employment period, the probability of becoming attention or risk group was 11% lower for quitting smoking (OR 0.89, 95% CI 0.81~0.97, p=.006) and 18% lower for regular exercise (OR 0.82, 95% CI 0.70~0.95, p=.009). Conclusion: The direction of health management at the workplace should be changed to promote the wellness of all workers, not the management of the disease, and a health promotion program should be continued to emphasize health behaviors such as smoking cessation and regular exercise.

  • Interferon alpha is an immunomodulator that is used as an antiviral agent to treat chronic active viral hepatitis C. However, interferon can induce or exacerbate sarcoidosis. We report a case of 42-year-old man with an exacerbation of pulmonary sarcoidosis after the cessation of interferon and ribavirin therapy for chronic hepatitis C. The patient's sarcoidosis improved spontaneously and he continues to be monitored regularly without steroid therapy.

  • Human erythropoietin(EPO) is a glycoprotein that enhances red blood cell production by stimulating proliferation and differentiation of erythroid progenitor cells in the bone marrow. Patients with chronic kidney disease(CKD) suffer from anemia caused by reduced production of EPO in the kidney. Recombinant human EPO protein has been used successfully for the treatment of anemia associated with CKD. Recently, attention has been paid to the development of side effect of EPO, pure red cell aplasia(PRCA), in some patients with CKD. PRCA is a rare disorder of erythropoiesis that leads to a severe anemia due to an almost complete cessation of red blood cell production. EPO-related PRCA is caused by the production of EPO-neutralizing antibodies(Abs) that eliminate the biological activity of EPO as well as endogenous EPO in patients undergoing therapy. Since 1988, almost 200 cases worldwide have been reported with Ab-positive PRCA after receiving EPO therapeutics. The underlying mechanisms of the breaking of immune tolerance to self-EPO have been investigated. Modification of formulation, organic compounds of container closures, and route of administration has been suggested for the possible mechanism of increased immunogenicity of EPO. A number of assays have been used to detect Abs specific to EPO. These assays are generally grouped into two major categories: binding Ab assays and neutralizing Ab assays(bioassays). There are several types of binding Ab assays, including radioimmunoprecipitation assay, enzyme-linked immunosorbent assay, and the BIAcore biosensor assay. In vitro cell-based bioassays have been utilized for the detection of neutralizing Abs. Finally, the recent experience with anti-EPO Abs may have considerable implications for the future development and approval of EPO preparations. Also, considering that millions of patients are being treated with EPO, clinicians need to be aware of signs and consequences of this rare but severe clinical case.

Ultrastructure of Vascular Meristems in the Rhizome of Botrychium ternatum (고사리삼 지하경의 유관속 분열조직 미세구조)

  • To elucidate the origin of secondary growth in the rhizome of B. ternaturn, the developmental changes of vascular cambium was observed in ultrastructural features. The vascular cambium was gradually differentiated from procambium as in seed plants, but the cambial activity did not persist very long so that the cambial cells became a dormant state like fossil cryptogams. Dense cytoplasm of procambial cells became progressively sparse during the growth, and the tiny vesicles were fused to form numerous small vacuoles and then a few large vacuoles. These gradual changes and the occurrence of storage materials which was associated with the developmental stages might support the progressive differentiation of the cambial cells. In addition, the cessation of cambial activity could be indicated by the facts that late vascular cambial cells accumulate large lipid bodies and show very small peripheral cytoplasm and unlikely thickened cell wall, compared to other meristematic cells. Therefore. the vascular cambium showed the characteristics of both seed plants and fossil cryptogams from the view point of cambial ontogeny and activity.tivity.

Effects of Cardiac Rehabilitation Teaching Program on Knowledge Level and Compliance of Health Behavior for Patients with Myocardial Infarction (심장재활 교육프로그램이 심근 경색증 환자의 질병관련 지식과 건강행위 이행에 미치는 효과)

  • The purpose of this study was to investigate the effects of cardiac rehabilitation teaching program on knowledge level and compliance of health behavior for the patients with myocardial infarction. Method: The subjects were 47 patients 23 were assigned to the experimental group and 24 were for the control. The cardiac rehabilitation teaching program is a individualized teaching program which was delivered to the experimental group during hospitalization period by present researcher. Data were collected through questionnaire surveys for knowledge level and compliance of health behavior from September 15, 1999 to December 31, 2000. The collected data was analyzed by using the SAS program. Results: 1. With regard to the knowledge scores 1) The total knowledge level in the experimental group was significantly higher than in the control group. 2) As to the knowledge domains, nature of disease, risk factors, diet, medication, exercise, and daily activities were significantly higher in score in the experimental group than in the control group. 2. With regard to the compliance of health behavior 1) The average compliance with good health behavior was significantly higher in the experimental group than in the control group. 2) As to the health behavior domains smoking cessation, diet, stress management, regular exercise, and other measures for lifestyle modification were significantly higher in score in the experimental group than in the control group. 3. The pre-treatment knowledge score was positively correlated to the post-treatment knowledge score and post- treatment knowledge score was positively correlated to the post-treatment compliance of health behaviors. Conclusion: The above findings indicate that the cardiac rehabilitation teaching program for the experimental group was effective in increasing level of knowledge and improvement of compliance with good health behavior of patients with myocardial infarction.

Moderate Effects of Gender on the Knowledge and Attitude toward Smoking and Self-concept among High School Students (성별에 따른 고등학생의 흡연에 대한 지식, 태도 및 자아개념)

  • Objectives: This study aims to investigate the attitude toward smoking and knowledge on smoking, self-efficacy, and self-esteem among high school students, and examine the moderate effects of gender on the differences in those variables between smoking and non-smoking students. As the result of investigation, it would suggest useful information for those who want to provide interventions of smoking prevention and smoking cessation to adolescents. Methods: Participants were 1,117 high school students (511 males, 584 females), whose mean age was 16.38 (SD=.87). To identify whether participants were smoking or not, bogus-pipeline was used. The data was collected with questionnaire include Smoking Attitude Inventory, General Self-Efficacy Scale, and Rosenberg's Self-Esteem Scale. A Pearson chi-square test and 2-way MANOVAs were performed with SPSS 11.5 for Windows. Results: With bogus-pipeline, one hundred thirty five (26.0%) of male students and 125(21.4%) of female students reported as they were smoking, while 115(22.2%) of male students and 78(13.3%) of female students reported as they were smoking at unidentified survey which was performed one week before bogus-pipeline. Results revealed that students who smoked had more positive attitude toward smoking and more knowledge on smoking than those who didn't smoke. Difference in knowledge on smoking between smoking and non-smoking students only existed in female students. Female students who smoked had lower self-esteem than female students who didn't smoke, but this difference didn't exist in male students. Conclusion: With the results of this study, it is considered smoking rates of adolescents surveyed with unidentified self-report questionnaires as underestimated. Health practitioners might consider female smokers' attitude toward and knowledge on smoking and self-esteem, especially for high school students. Despite of limitation of this study, it might help those who want to prevent initiation of smoking and give up smoking in adolescence.

Effect of Boron on the Development of Adventitious Roots in Sunflower Seedlings (해바라기 유식물의 부정근 발달에 미치는 붕소의 효과)

  • Three-day-old sunfower(Helianthus annuus L.) seedlings were do-rooted and incubated in the nutrient solutions that contained either deficient or sufficient boron supply, and exposure to various pH and temperatures. In the absence of boron, no adventitious roots were formed in the majority of the seedlings. Boron caused the development of numerous adventitious roots in the lower part of the hypocotyl. The low pH damage was ameliorated by the simultaneous application of calcium, implying the involvement of calcium-requiring process in overcoming proton toxicity. Borate showed a strong ameliorative effect with $Ca^{2+}$. These results suggest that the primary target of proton toxicity may be linked to a disturbance of the stability in the pectic polysaccharide network, where calcium plays a key role in plant roots. Maximum temperature for the growth of adventitious roots was $25^{\circ}C$. Although cessation of growth is the most apparent symptom of boron deficiency, exogenous ascorbate improved adventitious root growth in plants in the absence of boron. From the results it is suggested that the inhibition of adventitious root resulting from boron deficiency, low pH and temperature damage may be a consequence of disrupted ascorbate metabolism.

A Critical Evaluation of DNA Adducts as Biological Markers for Human Exposure to Polycyclic Aromatic Compounds

  • The causative role of polycyclic aromatic hydrocarbons (PAH) in human carcinogenesis is undisputed. Measurements of PAH-DNA adduct levels in easily accessible white blood cells therefore represent useful early endpoints in exposure intervention of chemoprevention studies. The successful applicability of DNA adducts as early endpoints depends on several criteria:i.adduct levels in easily accessible surrogate tissues should reflect adduct levels in target-tissues, ii. toxicokinetics and the temporal relevance should be properly defined.iii. sources of inter- and intra-individual variability must be known and controllable, and finally iv. adduct analyses must have advantages as compared to other markers of PAH-exposure. In general, higher DNA adduct levels or a higher proportion of subjects with detectable DNA adduct levels were found in exposed individuals as compared with non-exposed subjects, but saturation may occur at high exposures. Furthermore, DNA adduct levels varied according to changes in exposure, for example smoking cessation resulted in lower DNA adduct levels and adduct levels paralleled seasonal variations of air-pollution. Intra-individual variation during continuous exposure was low over a short period of time (weeks), but varied significantly when longer time periods (months) were investigated. Inter-individual variation is currently only partly explained by genetic polymorphisms in genes involved in PAH-metabolism and deserves further investigation. DNA adduct measurement may have three advantages over traditional exposure assessment: i. they can smooth the extreme variability in exposure which is typical for environmental toxicants and may integrate exposure over a longer period of time. Therefore, DNA adduct assessment may reduce the monitoring effort. ii. Biological monitoring of DNA adducts accounts for all exposure routes. iii. DNA adducts may account for inter-individual differences in uptake, elimination, distribution, metabolism and repair amongst exposed individuals. In conclusion, there is now a sufficiently large scientific basis to justify the application of DNA adduct measurement as biomarkers in exposure assessment and intervention studies. Their use in risk-assessment, however, requires further investigation.

A Case Report of Recurrent Aphthous Stomatitis resulted from Cessation of smoking in Stroke Patient (중풍환자에서 금연 후 발생한 재발성 아프타성 구내염에 대한 증례보고)

  • Recurrent Aphthous Stomatitis(RAS) is inflammatory ulcerative condition of the oral mucosa. The lesions of RAS are self-limited and persist for 1 to 2 weeks, resolved with or without scarring and recurred after periods of remission. It is known that nicotine may protect oral mucosa from aphthous ulcers by keratinization of the oral mucosa. After quitting smoking, a stroke patient who suffered from RAS, used to relapse into RAS unless he didn't gargle with nicotine extract water. We diagnosed his case as heat in the stomach and treated with Chungwie-San(Qingwei-San). After medication with Chungwie-San, RAS was subsided and he stopped smoking during admission period.
