일 지역 대학생의 흡연 실태와 흡연에 영향을 미치는 요인에 관한 연구 (A Study on the Factors Related to Smoking and Smoking Conditions among College Students in Some Area)

  • 본 연구는 일 지역 대학생들의 흡연 실태와 흡연에 영향을 미치는 요인을 확인하고자 수행되었다. 자료는 H군의 일개 대학에서 2015년 10월 26일부터 10월 30일까지 181명으로부터 설문지와 측정을 통하여 수집하였다. 수집한 자료는 SPSS/WIN 18.0 통계프로그램을 이용하여 $X^2-test$, t-test, Spearman rho Correlation과 Logistic Regression으로 분석하였다. 흡연유무와 체중, 신장, 흡연을 하지 않을 자신감, 우울은 양(+)의 상관관계를 보였고, 흡연유무와 흡연태도는 음(-)의 상관관계를 보였다. 체중, 신장, 흡연태도, 흡연하지 않을 자신감, 우울, 음주유무 중에서 대상자의 흡연유무에 영향을 미치는 요인을 검정하기 위해 로지스틱 회귀분석 한 결과 회귀모형은 유의한 것으로 나타났으며($X^2=72.854$, p<.001), 모형의 설명력은 36.4%, Negelkerke의 결정계수($R^2$)도 62.0%의 설명력을 보여 주고 있다. 흡연에 영향을 미치는 요인으로 흡연에 대한 태도가 부정적인 경우(OR 0.717, 95% CI 0.543-0.945), 흡연을 하지 않을 자신감이 높은 경우(OR 1.281, 95% CI 1.117-1.470)로 확인되었다. 따라서 일 지역 대학생들에게 적합한 금연 프로그램을 개발하기 위해서는 흡연에 대한 부정적인 태도를 증가시키고 금연을 할 수 있다는 자신감을 향상시킬 필요가 있다고 생각된다.

담배에 부착된 담뱃갑 경고 그림이 금연효과에 미치는 영향 (Effect of Cigarette Warning Sign Attached to Cigarettes on Smoking Cessation Effects)

  • 본 연구는 국내에서 판매되고 있는 담뱃갑 흡연 '담뱃갑경고그림'이 흡연자의 금연효과에 미치는 영향을 조사하여 향후 금연 및 건강관련 보건자료의 기초로 사용할 목적으로 시행된 서술적 조사연구이다. 이를 위해 인터넷 네이버 웹상에서 20세 이상 성인 남녀 500명을 대상으로 네이버 폼 설문지를 이용해 블러그와 카페 등을 이용하여 2018년 10월1부터 10일간 조사하여 응답이 미흡한 설문지를 제외하고 500부를 본 연구에 사용하였다. 분석된 데이터는 SPSS WIN20.0을 이용한 요인 분석, T-검정 및 상관관계를 분석하였다. 주요 연구 결과는 첫째, 인체해 유해한 담뱃갑 경고 그림이 금연효과에 가장 큰 영향을 미친 것으로 나타났다. 둘째, 금연효과는 담뱃갑 경고그림에 대한 설문 전 의식과 가장 높은 상관관계를 보였다. 이는 '담뱃갑 경고그림'이 흡연자에게는 가장 민감한 부분으로 받아들여지고 있음을 나타낸다. 본 논문의 결과를 토대로 담뱃갑 경고 그림의 다양화와 경고그림의 정기적인 교체 등은 금연 본능을 억제시키는 기전으로 작동함에 따라 향후 금연예방의 실천적 방안을 제안 할 수 있는 유용한 기초자료를 제공할 것으로 기대된다. 따라서 흡연경고 그림은 언론이나 매체를 통해 널리 홍보하여 국민들에게 금연을 실행하는데 긍정적인 영향을 미치며, 나아가 국민들의 건강 증진을 위한 보건정책의 기초자료로 활용될 것으로 사료된다.

한국 해기사의 흡연 및 간접흡연 실태와 흡연에 대한 지식, 태도 및 금연의도 (A Study on the Smoke and Secondhand Smoke status, Knowledge about Smoking, Attitude for Smoking behavior and Smoking Cessation Intention of Seafarers)

  • 육지와 떨어진 제약적 환경 때문에 선박에서 근무하는 해기사는 사무직근로자보다 보건소와 같은 금연지원체계에 대한 접근성이 떨어질 수 있다. 본 연구는 한국 해기사의 흡연 및 간접흡연 실태를 파악하고 관련요인을 조사하기 위해 총 155명의 대상자에게 홉연 및 간접흡연 경험, 흡연에 대한 지식, 태도 및 금연의도와 선박 내 흡연시설에 대해 설문하였다. 조사 결과, 대상자의 흡연율은 32.3%였고, 간접흡연율은 86.5%로 높은 수준이었다. 선박 내 금연구역이 있는 경우에도 대부분의 흡연행위가 휴게실과 같은 해기사들의 생활 공간에서 주로 발생한다고 답하였다. 또한 선박 내 지정흡연실이 없거나 환풍시설이 미비한 경우도 40%정도에 이르는 것으로 조사되었다. 한편, 대상자 중 금연교육을 받은 사람은 21.9%에 불과하였다. 흡연행위에 대한 태도는 34.58점(총 60점)으로 부정적이었고, 흡연에 대한 지식은 16.12점(총 20점)으로 중간보다 높았다. 흡연자 10명 중 9명은 금연을 시도한 경험이 있었으나 금연 의도는 낮은 것으로 나타났다. 제한된 공간에서 근무와 생활을 병행하는 장기간 체류하는 해기사들의 흡연율을 감소시키기 위해서는, 해기사가 활용 가능한 방법으로 주기적인 금연교육을 통해 동기를 부여하고 금연의도를 높일 필요가 있다. 또한 정기적으로 흡연 실태조사를 조사하여 간접흡연의 피해를 주기적으로 파악하는 한편, 선박 내 흡연관련시설 등 환경적 요인에 대한 법규를 재정비하는 등 제도적 정비가 필요하다.

울산광역시 시민에서 금연 광고가 금연 의향 및 유지에 미치는 영향 (The Effectiveness of Anti-Smoking Advertisement on Smoking Cessation Intention in Citizens of Ulsan Metropolitan City)

  • Objectives: In this study, we evaluated the effectiveness of the typical anti-smoking advertisement on smoking cessation intention in citizens of Ulsan Metropolitan City. Methods: A total of 700 citizens (600 adults and 100 high school student) participated in face-to-face interviews survey using paper questionnaire. Three anti-smoking advertisements were used in this study; chronic obstructive pulmonary disease (COPD) patient video advertisement, tobacco hazard information advertisement, and COPD patient voice advertisement. Each participant randomly evaluated only one of the three non-smoking anti-smoking advertisements. Participants were asked whether they had seen or heard anti-smoking advertisements before and asked whether they understood the advertisement well. They also assessed the effectiveness of non-smokers to maintain smoking cessation and how effective it would be to help smokers quit. Results: Among the three anti-smoking advertisements, 54.8% of participants said that they watched COPD patient video advertisement before. More than 95% of participants said they can understood anti-smoking advertisements. Among the three anti-smoking advertisements, tobacco hazard information advertisement was evaluated to be most effective to maintain non-smokers' smoking cessation (234, 92.5%). Tobacco hazard information advertisement was also evaluated to be most effective to make smokers quit smoking (216, 84.7%). Conclusion: Anti-smoking advertisements have a positive effect on non-smoker's willingness to keep smoking and smokers' willingness to quit smoking. In future studies, it would be meaningful to look at the long-term effects of smoking cessation or to evaluate the effectiveness of the more various anti-smoking advertisements.

노년기 운전중단 결정 인식과 태도에 관한 연구 (Older Drivers' Perception and Attitudes Towards Driving Cessation)

  • 1970년대 이후 기대수명과 자동차 수의 증가로 '노인운전자'라는 신(新) 사회현상이 생겼다. 노인운전자에 대한 정책 개발은 '개인의 이동권'과 '공공의 안전' 사이의 균형을 찾는 과정으로, 정책에 직접적으로 영향을 받게 될 노인이 운전중단에 대해 어떤 태도를 갖고 준비를 하는지에 대한 이해가 선결적이다. 본 연구는 노년기 운전중단 결정에 대한 이해를 증진시키고자 전국 17개 시도에서 실시된 만 65세 이상 노인을 모집단으로 비례층화할당 표본 추출을 활용한 설문조사 데이터를 사용하였다(표본 크기: 2,076명). 운전을 그만두는 것을 생각해본 적이 있는지와 운전중단 시점을 스스로 정할지 아니면 타인이 조언해주는 것이 좋은지에 대한 선호를 종속변수로 일련의 로지스틱 회귀분석을 수행하였다. 분석결과 38.1%가 운전을 해본 적이 있으며, 21.2%는 면허가 있고 지난 한 달 동안 운전을 한 적이 있다고 응답하였다. 운전은 성별에 따라 큰 차이를 나타내어 남성노인은 43%가 운전을 하고 있는 반면에, 여성노인은 5.1%에 불과하였다. 노인운전자의 약 2/3인 62.8%가 운전을 그만두는 것을 생각해본 적 없다고 응답하였으며, 역시 약 2/3인 66.0%가 운전을 그만두는 시점을 스스로 판단하는 것을 선호하였다. 여성이 남성에 비해, 도시지역 거주자가 농어촌지역 거주자에 비해 운전중단을 생각해본 확률이 2배 정도 높았다. 나이가 한 살 증가할 때마다 운전중단을 생각해본 확률이 5%씩 증가하였고, 만성질환으로 일상생활 어려움이 클수록 운전중단을 생각해보는 경향이 있었다. 반면, 운전중단 시점을 스스로 판단하는 것에 통계적으로 유의미한 영향을 미치는 사회인구학적, 건강 요인은 없었다. 본 연구결과는 현재 시행 중인 연령에 따른 면허 갱신주기 단축 정책에 더불어 노인운전자의 자발적 운전중단에 동기를 부여할 수 있는 정책적 개입이 보완적 효과를 가져올 수 있음을 시사한다. 아울러, 노년기의 운전중단은 사전 예방적으로 접근하여 건강한 전기 노년기부터 미리 계획하고 준비할 수 있는 교육이 필요하며, 이때 농어촌지역 거주자와 남성 노인운전자에 보다 큰 관심이 요구된다.

금연학교 입원프로그램 전과 후의 흡연자의 흡연에 대한 인식변화 (Changes in Smokers' Perception about smoking before and after Participation in a Hospitalized Smoking Cessation Program)

  • Purpose: This study was done to analyze the changes in smokers' subjectivity about smoking before and after participation in a 5-day hospitalized smoking cessation program. Design: Q-methodology, which provides a scientific method on subjectivity, was applied. Findings: Nine types of subjectivity changes about smoking were identified; Type I, Nicotine $craving{\to}Confident$ of self control for smoking; Type II, Nicotine $craving{\to}Cognition$ of the link between habituation and smoking, Type III, Nicotine craving ~ Nicotine craving, Type IV, smoking as a social $behavior{\to}Cognition$ of the link between habituation and smoking, Type V, smoking as a social $behavior{\to}Motive$ to turn away from being addicted to smoking to being healthy, Type VI, Habitual $smoking{\to}Confident$ of self control for smoking, Type VII, Habitual $smoking{\to}Cognition$ of the link between habituation and smoking, Type VIII, Habitual $smoking{\to}Motive$ to turn away from being addicted to smoking to being healthy and Type IX, Denial of the negative effects of $smoking{\to}Confident$ of self control for smoking Conclusion: The types of changes in smokers' subjectivity before and after the smoking cessation program in this study suggest a better tailored intervention for individual smokers. The types of changes on smokers' subjectivity provide insight about which factors of the interventions for smoking cessation are especially valued for a particular group of smokers.

An integrated Approach to Worksite Tobacco Use Prevention and Oral Cancer Screening Among Factory Workers in Mumbai, india

  • Background: Tobacco control and cessation interventions are among the most cost effective medical interventions but health systems in low resource countries lack the infrastructure to promote prevention and cessation among tobacco users. Workplace settings have the potential to provide opportunities and access for tobacco prevention interventions. Methods: This is a single group study evaluating tobacco use prevention and cessation through a structured three stage intervention program for tobacco users comprising education on harmful effects of tobacco, oral cancer screening and behavior therapy for tobacco cessation at the worksite. Results: All the 739 workers who were invited participated in tobacco awareness program and were screened for oral pre cancer lesions. 291 (39.4%) workers were found to be users of tobacco in some form. Education, gender and alcohol use (p<0.0001) were some of the factors associated with tobacco user status. The prevalence of clinical oral precancer lesions among tobacco users was 21.6%. Alcohol consumption (p<0.001), the type of tobacco consumed (p<0.018), personal medical history of chronic diseases (p<0.007) and combined use of alcohol and tobacco (p<0.001) were some factors found to be associated with presence of oral pre cancer lesions. Conclusion: An integrated approach for worksite based tobacco use prevention with oral cancer screening program showed good acceptance and participation and was effective in addressing the problem of tobacco consumption among the factory workers.

  • Health education aims at behavior change rather than just delivering health knowledge to people. In Korea health education activities in public sector began in 1960 and they were included in the primary prevention program in communities. This article reviewed current health education programs in healthy living practice programs provided by local public health centers in Korea and drew implications for the future role of health education in community setting. Health education has been a core function of the National Health Promotion programs in the nation since the enactment of the National Health Promotion Law in 1995. The National Health Promotion programs are funded by the National Health Promotion Fund which are drawn from tobacco tax. The National Health Promotion programs include healthy living practice programs (smoking prevention and cessation programs, moderate alcohol use programs, physical activity promotion programs, and nutrition programs), chronic disease prevention programs, oral health programs and public hygiene programs. Methods of the National Health Promotion programs include health education, health counseling, health class, health information management, survey and research. Smoking prevention and cessation programs include smoking cessation clinic, smoking cessation education, non-smoking environment program, and non-smoking campaign. Moderate alcohol use programs include alcohol use education, moderate alcohol use campaign, alcohol use counseling, and alcohol free environment programs. Physical activity promotion programs include obesity control, targeted exercise program, and exercise civic group programs. Nutrition programs include nutrition management, obesity management, nutrition education, breakfast eating program, and nutrition counseling and treatment programs. The health education programs in community are not efficient today because there are many overlapping contents and short term goals. Community health education programs needs to be more comprehensive. Workforce development is another big issue at the moment because the National credential program will begin in 2009. Variety of community health education programs should be developed and funded by the national health promotion fund.

Patterns of Use, Cessation Behavior and Socio-Demographic Factors Associated with Smoking in Saudi Arabia: a Cross-Sectional Multi-Step Study

  • Smoking is accountable for the fatality of a substantial number of persons and increases the likelihood of cancer and cardiovascular diseases. Although data have shown high prevalence rates of cigarette smoking in Saudi Arabia, relatively little is known about the broader scope. The objectives of this study were to investigate socio-demographic factors, patterns of use and cessation behavior associated with smoking in Saudi Arabia (KSA). The study utilized a cross-sectional, multi-step design of sampling. Residents (N=1,497; aged 15 years and older) were recruited from seven administrative areas in Southwest Saudi Arabia. A pretested questionnaire was utilized to obtain data on participant cigarette smoking, including their daily use, age, education, income, marital status and employment status. The current study is the first of its kind to gather data cessation behavior of Saudi subjects. With the exception of 1.5% females, all the respondents were male. The majority of the respondents were married, had a university level of education, were employed, and were younger than 34 years old. The same trends were also observed among smokers' samples. The current prevalence of cigarette smoking was 49.2% and 65.7% of smokers had smoking at less than 18 years of age. The mean daily use amongst smokers was 7.98 cigarettes (SD=4.587). More than 50% of the study sample had tried at least once to quit smoking. However, 42% of the smokers participating had never. On the other hand, about 25% of the respondents were willing to consider quitting smoking in the future. Modeling of cigarette smoking suggested that the most significant independent predictors of smoking behavior were geographic area, gender, marital status, education, job and age. Considerable variation in smoking prevalence was noted related with participant sociodemographics. Findings recommend the necessity for control and intervention programs in Saudi community.

Perception and Practices of Physicians in Addressing the Smokeless Tobacco Epidemic: Findings from Two States in India

  • Background: Smokeless tobacco use in South Asia is believed to be a significant contributor to morbidity and mortality. In India, only a few studies involving health educational intervention by health care providers have demonstrated reduction in smokeless tobacco usage. In the present study we assessed the cessation efforts towards smokeless tobacco by physicians in two high tobacco prevalence states of India. The study also identified opportunities and barriers for integration of tobacco cessation services in routine practices of physicians. Materials and Methods: This mixed method study involved qualitative (phase I) and quantitative research study (phase II). In phase I, 59 in-depth interviews with physicians were conducted. In phase II, a quantitative study conducted among 238 physicians. An inductive approach was followed to analyze qualitative data using ATLAS. Ti software. The Chi-square test was employed to test the association between different variables of interest using SPSS version 17. Results: The majority of physicians related only respiratory problems and cancer with smokeless tobacco. Other major health effects like cardio-vascular problems, oral diseases, and effects on reproductive and neonatal health were recognized only by a few physicians. The age-group of 10-19 years was identified as most vulnerable to smokeless tobacco use. Less than one-third of physicians reported recording smokeless tobacco history of all patients. Findings indicated that less than half of physicians provided information on harmful health effects of smokeless tobacco with regard to specific diseases. Conclusions: The study revealed a low level of knowledge of physicians about harmful effects of tobacco and their suboptimal engagement in tobacco control practices. The study indicates the need of capacity building initiatives to equip physicians with skills in tobacco cessation.
