Objective : Cerebrovascular disease (CVD) was the third most common cause of death in South Korea in 2014. Evidence from abroad suggests that comprehensive stroke centers play an important role in improving the mortality rate of stroke. However, surgical treatment for CVD is currently slightly neglected by national policy, and there is still regional imbalance in this regard. For this reason, we conducted a survey on the necessity of, and the requirements for, establishing regional comprehensive cerebrovascular surgery centers (CCVSCs). Methods : This investigation was performed using the questionnaire survey method. The questionnaire was consisted with two sections. The first concerned the respondent's opinion regarding the current status of demand and the regional imbalance of cerebrovascular surgery in South Korea. The second section asked about the requirements for establishing regional CCVSCs. We sent the questionnaire to 100 board members of the Korean Society of Cerebrovascular Surgeons. Results : Most experts agreed that cerebrovascular surgery patients were concentrated in large hospitals in the capital area, and 83.6% of respondents agreed that it was necessary to alleviate the regional imbalance of cerebrovascular surgery. With regards to personnel, over 90% of respondents answered that at least two neuro-vascular surgeons and two neuro-interventionists are necessary to establish a CCVSC. Regarding facilities, almost all respondents stated that each CCVSC would require a neuro-intensive care unit and hybrid operating room. The survey asked the respondents about 13 specific neurovascular surgical procedures and whether they were necessary for a regional CCVSC. In the questions about the necessity of cerebrovascular surgical equipment, all seven pieces of equipment were considered essential by all respondents. A further five pieces of equipment were considered necessary on site: computed tomographic angiography, magnetic resonance angiography, conventional angiography, surgical microscope, and surgical navigation. Our results may provide a basis for future policy regarding treatment of cerebrovascular disease, including surgery. Conclusion : Raising the comprehensiveness of treatment at a regional level would lower the national disease burden. Policies should be drafted regarding comprehensive treatment including surgery for cerebrovascular disease, and related support plans should be implemented.

Recently, accumulated clinical experience and advanced neuroradiological techniques have led to a better understanding of pediatric cerebrovascular disease (CVD), which was once considered rare. Approximately 10% of pediatric neurosurgical patients have CVD; therefore, it is no longer uncommon to pediatricians and pediatric neurosurgeons. Furthermore, children with CVD tend to recover better than adults after stroke because the immature brain is more plastic than the mature one. Therefore, active treatment often proves more rewarding in pediatric medicine than in adult medicine.
This study was conducted of investigate the antioxidant vitamins and serum lipids status of 23 healthy female subjects and 22 female patients suffering from cerebrovascular disease hospitalized at any of three of general hospitals in Taegu. The systolic blood pressure of patients suffering from cerebrovascular disease(151.4$\pm$30.2mmHg) was significantly higher than healthy controls(129.7$\pm$17.8mmHg). The frequency of drinking alcohol and smoking in patients was also higher. Analysis of patient and control dietary intakes revealed that all subjects were consuming the minimed recommended energy-nutrient levels. HDL-cholesterol(26.15 vs 37.74mg/dl), serum vitamin E(12.69 vs 17.74mg/l) and vitamin C(65.59 vs 152.50ug/dl) levels were significantly lower, but triglyceride(221.01 vs 87.30 mg/dl) and total-cholesterol / HDL-cholesterol levels(5.98 vs 4.89)were significantly higher in the patient group. According to the results of this study, it is expercted that refraining from smoking and drinking alcohol, cultivating healthy living habits which can decrease stress, and maintaining suitable levels of serum antioxidant vitamins can prevent the occurrence of cerebrovascular disease.
This study aims for examines the actual physical symptom and stress in caregivers of patients with cerebrovascular disease. The data were collected by a survey conducted from August to September, 1997 which included 65 caregivers of cerebrovascular disease patients in 4 hospitals located in Seoul. The caregiver's stress was measured by Choi (1992)'s instrument and the actual physical symptoms were investigated. The data were analyzed using ANOVA, Scheffe test, Pearson correlation coefficient and stepwise multiple regression. The results were as follows : 1. The mean number of caregiver's physical symptom was 3.5. There were significantly higher number of physical symptom in women, those of who have a religious affiliation, those of who perceive their own health status perceived as bad, and those of who perceived their patients disease condition as serious than in their counterparts. Also, the number of caregiver's physical symptom was significantly higher in caregivers whose patients have a paralysis sypmtom and the disease onset as accident than in caregivers whose patients have no paralysis symptom and the disease onset as spontaneous. 2. The average of caregiver's stress was 57.9. The caregiver's stress was the highest in between the ages of 50 and 59. There were also significantly higher level of stress in women, those of who perceived the disease condition of their patients as serious than in their counterparts. 3. The most common caregiver's physical symptom was fatigue(87.7%). This was followed by insomnia(58.5%) and muscle pain(47.7%). 4. Caregiver's physical sypmtom was positively correlated with caregiver's stress and negatively correlated with patient's activity of daily life. 5. The most important vairable affecting the caregiver's physical symtom was patient's activity of daily life which accounted for 12.7% of the total variance in stepwise multiple regression analysis. The most important vairable affecting the caregiver's stress was the patient disease condition perceived by the caregiver that accounted for 12.1% of the total variance.
Objectives : The purpose of this study is to evaluate the effects of Joganiknoe-tang on diplopia and ptosis from cerebrovascular disease. Methods : We recruited 2 patients who have diplopia and ptosis because of cerebrovascular disease. In addition, the patients had been administrated by Joganiknoe-tang. Diplopia and ptosis of patients had been estimated with VAS (Visual Analogue Scale) and difference of both eye's interpalpebral fissure. We measured VAS (Visual Analogue Scale) and difference of both eye's interpalpebral fissure for every five days Results : After the treatments, symptoms were decreased and almost disappeared. In both cases, diplopia patients complained decreased from VAS 10 to VAS 1, and difference of both eye's interpalpebral fissure disappeared Conclusions : This study suggests that Joganiknoe-tang is significantly effective on diplopia and ptosis because of cerebrovascular disease.
Objective : To evaluate the prevalence and risk factors of clopidogrel resistance, and association between thromboembolic complications and clopidogrel resistance in patient with stent-assisted angioplasty for atherosclerotic cerebrovascular disease. Methods : Between September 2006 and June 2008, clopidogrel resistance test was performed on 41 patients who underwent stent-assisted angioplasty for atherosclerotic cerebrovascular disease. It was performed before drug administration and about 12 hours after drug administration (loading dose : 300 mg, maintain dose : 75 mg). Two patients were excluded, and 41 patients were included (mean : $67.59{\pm}7.10$ years, age range : 41-79). Among 41 patients, 18 patients had intracranial lesions, and 23 had extracranial lesions. We evaluated the prevalence, risk factors and complications related to clopidogrel resistance. Results : Twenty-one patients (51.2%) showed clopidogrel resistance [intracranial : 10 patients (55.6%), extracranial : 11 patients (47.8%)] and no clopidogrel resistance was seen in 20 patients. Hypercholesterolemia was an indepedent risk factor of clopidogrel resistance. Stent-assisted angioplasty was technically successful in all patients, but acute in-stent thrombosis occurred in 5 patients with intracranial lesions (4 patients with clopidogrel resistance and 1 without clopidogrel resistance). Acute thrombi were completely lysed after intra-arterial infusion of abciximab. Conclusion : There was relatively high prevalence of clopidogrel resistance in patients with atherosclerotic cerebrovascular disease. Hypercholesterolemia was an independent predictive factor of clopidogrel resistance. Acute in-stent thrombosis was more frequently seen in the clopidogrel resistant group. Therefore, clopidogrel resistance test should be performed to avoid thromboembolic complications related to stent-assisted angioplasty for atherosclerotic cerebrovascular disease, especially patients with hypercholeterolemia and intracranial lesion.
Purpose: The purpose of this study was to find out the relationship between health concern, knowledge of cardio-cerebrovascular disease, and health-promoting behaviors among the elderly in the community. Method: The subjects of the study were 142 senior citizens aged 65 years or older in Cheongju-si, Chungcheong-do. Data were collected using structured questionnaires that included questions on the general characteristics, health concerns, knowledge of cardio-cerebrovascular disease, and health-promoting behaviors. The collected data were analyzed by the statistical program SPSS 20.0. The frequencies, percentages, averages, and standard deviations of the collected data were analyzed using a t-test, ANOVA, Mann Whitney U test, and Pearson's correlation coefficient, basing on the purpose of the study. The Scheffe test was carried out as a posteriori test. Result: The elderly were found to have a high health concern, but their knowledge of cardio-cerebrovascular disease was low, and their health-promoting behaviors were above the intermediate level. There was a significant positive correlation between each variable, which means that the higher the health concern was, the more knowledge an individual had of cardio-cerebrovascular disease, and the more health-promoting behaviors one had. Conclusion: This suggests that the elderly in the community should be encouraged to engage in the actions that promote their health by raising their level of health concern and awareness of diseases through health education, basing on their individual characteristics.
Objectives: The objective of study was to calculate the municipal level environmental burden of disease (EBD) due to heat wave. Methods: The data used were Korea National Health Insurance 2011 claim data and 2011 death registry. Heatwave related diseases included hypertensive heart diseases, ischemic heart diseases, cerebrovascular disease, and heat related illness. According to the method that WHO proposed, the study computed population-attributable fraction with relative risk which come from previous study and proportion of exposure which the study calculated with historical meteorology data. Results: The Average of 251 municipal EBD was 2.11 per thousand persons. The value of years lost due to disability was 11 times higher than that of years of life lost. On average EBD of county and southern geographical areas tended to be higher than those of District or city areas. The relationship between municipal deprivation index (composite deprivation index) and EBD showed the positive association, which means that the worse deprived municipal is, the higher EBD takes. Conclusions: Climate change is getting one of the major risk factors of cardio-cerebrovascular disease, which is the second leading cause of death. The study results suggested the urgent policy planning and reaction of climate change adaptation.
Purpose: The purpose of this study was to examine the differences in requirement and activity related to workers' health promotion according to business type and enterprise size. Methods: This study is a secondary analysis of data extracted from a survey on the Enterprise Health Promotion Index. For the present analysis, 7,784 companies were selected. Results: Requirements for lifestyle management and activities to prevent musculoskeletal disorders were the highest. The requirement and activity of musculoskeletal disorders and cerebrovascular disease were high in the manufacturing industry. Meanwhile, in the non-manufacturing industry, the requirement and activity of job stress management were high. Small-sized companies had a high requirement to prevent musculoskeletal disorders and cerebrovascular disease. Conclusion: Government support should be strengthened in workers' lifestyle management. Additionally, workers' health promotion in small-sized companies needs to focus on preventing musculoskeletal disorders and cerebrovascular disease.

The association between apolipoprotein E (apo E) gene polymorphism and ischemic cerebrovascular disease (ICVD) has been controversial. These controversies may be due to inaccurate classification of patients and ethnic differences. We investigated the association between apo E genotypes and ICVD patients by case-control study in a Korean population. The association between apo E polymorphism and ICVD was examined in 121 patients with ICVD and 132 controls without ICVD. The E3/E4 phenotype was more frequent in control subjects (23.8%) than in patients (13.0%) (p<0.05). The E2/E3 phenotype was more frequent in patients (14.8%) than in control subjects (10.8%), but the difference was not statistically significant (p>0.05). These results suggest that the E4 allele may be a protective factor against early vascular morbidity, and the E2 allele may be a risk factor for cerebrovascular morbidity.
