Studies of interictal epileptiform discharges are essential for improving the diagnosis, classification, and management of epilepsy. In this case series we sought to identify the clinical and neurophysiological significance of bifid spikes, whose pattern bears a strong resemblance to the cardiac M pattern. We hypothesize that, analogous to the cardiac M pattern, the cerebral M pattern is generated by a conduction defect associated with asynchronous spatiotemporal averaging of electrical signals in the cortex, resulting in the signals reaching the scalp with different latencies. Unlike the cardiac M pattern, the pathology underlying the cerebral M pattern is unknown, although congenital CNS anomalies may be a culprit.
A 6000-year-old male with carcinoma of the prostate and cerebral infarction underwent a Tc-99m MDP bone scintigraphy for the evaluation of skeletal metastases. Bone scintigraphy (Fig. 1) showed multiple areas of increased uptake of Tc-99m MDP in the skull, spine, and ribs representing skeletal metastases. Two different patterns of uptake occurred in the skull region (Fig. 1A-C); one represents bony metastasis and the ether represents cerebral infarction. The shape, size, location, intensity, and border of the increased uptake differed between the two lesions. An oval-shaped pattern with smaller size, greater intensity and more sharply defined border in the frontal region was consistent with bony metastasis. A rectangular-shaped pattern with larger size, lesser intensity and relatively indistinct border in the temporo-parieto-occipital region was consistent with cerebral infarction. Increased uptake of bone-seeking radiotracers in cerebral infarction has been reported previously.$^{1-4)}$ A suggested mechanism by which bone-seeking radiotracers accumulate in the necrotizing cerebral tissue is an alteration of the blood-brain barrier induced during cerebral infarction, which results in entry of the radiotracers into the extracellular space of the brain.$^{4)}$ Brain CT (Fig. 2) performed 7 days before and one month after the bone scintigraphy revealed lesions on the right temporo-parieto-occipital region consistent with acute hemorrhagic and chronic cerebral infarction, respectively.
Progressive supranuclear palsy (PSP) is a parkinson-plus syndrome characterized clinically by supranuclear ephthalmoplegia, pseudobulbar palsy, axial rigidity, bradykinesia, postural instability and dementia. Presence of dementia and lack of cortical histopathology suggest the derangement of cortical function by pathological changes in subcortical structures in PSP, which is supported by the pattern of behavioral changes and measurement of brain metabolism using positron emission tomography. This study was done to examine whether there are specific changes of regional cerebral perfusion in PSP and whether there is a correlation between severity of motor abnormality and degree of changes in cerebral perfusion. We measured regional cerebral perfusion indices in 5 cortical and 2 subcortical areas in 6 patients with a clinical diagnosis of PSP and 6 healthy age and sex matched controls using $^{99m}Tc$-HMPAO SPECT. Compared with age and sex matched controls, only superior frontal regional perfusion index was significantly decreased in PSP (p<0.05). There was no correlation between the severity of the motor abnormality and any of the regional cerebral perfusion indices (p>0.05). We affirm the previous reports that perfusion in superior frontal cortex is decreased in PSP. Based on our results that there was no correlation between severity of motor abnormality and cerebral perfusion in the superior frontal cortex, nonmotoric symptoms including dementia needs to be looked at whether there is a correlation with the perfusion abnormality in superior frontal cortex.
목적: 파킨슨병 환자의 뇌혈류는 일반적으로 정상 또는 미만성의 혈류 감소를 보이는 것으로 보고하고 있다. 하지만 임상 상황에서 파킨슨 환자의 뇌관류 SPECT 결과를 판독할 때 다양한 뇌혈류 양상을 보이는 것을 발견할 수 있다. 이에 본 연구에서는 파킨슨병 환자의 뇌혈류 양상을 뇌관류 SPECT와 SPM 프로그램을 이용하여 나이, 성별, 신경심리 검사에 따라 어떤 차이가 있는지 보았고, 뇌혈류 양상을 유사한 형태끼리 나누어 보았다. 대상 및 방법: 치매가 동반되지 않은 파킨슨병으로 진단된 환자 219명(평균 $62.9{\pm}6.9$세, 남자 70명, 여자 140명)과 대조군으로 질병에 대한 과거력이 없는, 55명(평균 $61.4{\pm}9.2$세, 남자 15명, 여자 40명)을 대상으로 $^{99m}Tc$-HMPAO 뇌관류 SPECT를 실시하였고 SPM을 이용하여 환자군 정상 대조군을 비교하였다. 결과: 첫 번째로 전체 대조군에 비해 전체 환자군에서 좌측 하전두이랑, 좌측 뇌섬엽, 자측 가로측두이랑, 좌측 하두정소엽, 좌측 상두정소엽, 우측 쐐기전소엽, 우측 미상핵꼬리에 혈류 감소를 보였다. 두 번째로 성별에 따른 환자군과 정상춘의 비교에서 전반적으로 남, 여 환자 모두 좌반구의 혈류 감소가 우측에 비해 넓은 영역을 보였고, 남자 환자에 비해 여자 환자에서 좀 더 넓은 영역의 혈류 감소를 보였다. 세 번째로 나이에 따른 환자군과 정상군의 비교에서 50세 이하 환자군 에서는 주로 양쪽 후두엽, 두정엽과 좌측 측두엽에 정상인에 비해서 혈류 감소를 보였고 나이가 증가함에 따라 후두엽, 두정엽의 뇌혈류는 정상으로 나타났으며 양쪽 전두엽, 측두엽, 변연엽의 혈류 감소가 주로 관찰되었다. 네 번째로 뇌혈류가 저하되어 있는 영역을 기준으로 환자를 나누었을 때, 전두엽에 주로 감소되어 있는 환자 45명(20.6%) 측두엽에 주로 감소되어 있는 환자 38명(17.4%), 두정엽 39명(17.9%). 후두엽 40명(18.3%), 미만성으로 감소되어 있는 환자 14명(6.4%), 뇌혈류가 정상인 환자 32명(14.7%), 위의 형태로 분류하기 어려운 경우 10명(4.6%)으로 분류할 수 있었다. 마지막으로 신경심리검사에서 유사한 결과를 보이는 환자들끼리 분류하여 뇌혈류를 보았을 때 신경심리검사와 뇌혈류 양상사이에서 시공간 기능 항목 외에 특별한 상관관계는 발견할 수 없었고, 다시 연령별로 환자를 분류하여 신경심리검사결과와 뇌혈류 양상의 관계를 비교하여 보았을 때 일정 부분에서 유사한 양상을 관찰할 수 있었다. 결론: 파킨슨병 환자의 뇌혈류는 성별과 나이에 따라 다양한 국소적 혈류 저하의 소견을 보였고, 뇌혈류 양상에 따라 환자를 분류할 수 있었다.
The effect of herbal medicine on glutamate mediated neurotoxicity was studied in mouse neurons in primary culture. Immature cerebral cortex neurons (ED14) were maintained for up to 2 weeks in vitro, and we investigated the expression pattern of neuron differentiation and cytotoxicity of cell death, including LDH activity. Neuronal maturation initiated on day 7 and the susceptibility to glutamate-induced cell death was highly sensitive on Day 11 (Fig. 1). Thus, the exposure of the neurons to glutamate caused a dose$(0.1mM{\sim}1mM)$ and time$(4h{\sim}24h)$-dependent neurotoxicity(Fig. 4). Glutamate-induced neurodegeneration was prevented by Shipchondaebotang(SD), Yollyounggobondan(YG), Yugmijihwangwon(YJ) and the death of neurons exposed to glutamate was blocked by the NMDA receptor antagonist MK-801 (Fig. 5).
8 patients of delayed CO sequelae were evaluated using Brain CT and $^{99m}Tc-HMPAO$ SPECT. The results were as follows; 1) CT findings of delayed CO sequleae were bilateral low density lesion in globus pallidus (l pt.), diffuse low density in white matter with bilateral low density in white matter (l pt.), diffuse low density in white matter with bilateral low density in globus pallidus (l pt.), diffuse low density in white matter with cortical atrophy (l pt.), bilateral low density in globus pallidus and diffuse low density in white matter with cortical atrophy (l pt.) and normal in 3 pts. 2) $^{99m}Tc-HMPAO$ Brain SPECT findings of delayed CO sequelae were decreased regional cerebral blood flow (rCBF) in frontal (1 among 8 pts.), frontal and basal ganglia (3 among 8 pts.), and diffuse patch decreased rCBF pattern (4 among 8 pts.) 3) $^{99m}Tc-HMPAO$ Brain SPECT study was well correlated with neurologic symptoms and signs in delayed CO sequelae. Our results may suggest that reduced cerebral blood flow contributes to the development of delayed CO sequelae.
Heat shock proteins (HSPs) are highly conserved cellular proteins that contribute to adaptive responses of organisms to a variety of stressors. In response to stressors, cellular levels of HSPs are increased and play critical roles in protein stability, folding and molecular trafficking. The mRNA expression pattern of two well-known heat shock protein transcripts, HSP70 and HSP90 were studied in two tissues of nerve ganglia, cerebral ganglion and pleuropedal ganglion of Pacific abalone (Haliotis discus hannai). It was observed that both HSP70 and HSP90 transcripts were upregulated under heat stress in both ganglion tissues. Expression level of HSP70 was found higher than HSP90 in both ganglia whereas cerebral ganglion showed higher expression than pleuropedal ganglion. The HSP70 and HSP90 showed higher expression at Day-1 after exposed to heat stress, later decreased at Day-3 and Day-7 onwards. The present result suggested that HSP70 and HSP90 synthesize in nerve ganglion tissues and may provide efficient protection from stress.
The water extract of Dohongsamul-Tang(DHSMT) has been traditionally used to stroke and brain injuries in Oriental Medicine. The present study was designed to investigate the effects of DHSMT on the gene expression profile of cerebral infarction by cDNA microarray in photothrombotic ischemia mouse model. Photothrombotic ischemia was induced in stereotactically held male BALB/c mice using rose bengal and cold light. MRI was performed 24 hours after inducing photothrombosis using 1.5 T MRI and 47 mm surface coil to obtain T2-weighted, and contrast-enhanced images. After MRI test, animal was sacrificed and the brain sections were stained for hematoxylin and eosin and immunohistochemistry. MRI and histological analysis revealed that lesion of thrombotic ischemia was well induced in the cortex with the evidence of biological courses of infarction. The target area of thrombotic infarction was 1 mm anterior to bregma and 3 mm lateral to midline with 2 mm in diameter, which were decreased by administration of DHSMT. To assess gene expression pattern of cerebral infarction, mRNA was isolated and reacted with microarray chip(Agilant's DNA Microarray 44K). Scatter and MA plot analysis were performed to clustering of each functional genes. M value [M=log2(R/G), A={log2(R ${\times}$ G)}/2] was between -0.5 and +0.5 with 40% difference. After pretreatment with DHSMT, the expression levels of mRNA of many genes involved in various signaling pathway such as apoptosis, cell cycle, cell proliferation, response to oxidative stress, immune response, angiogenesis, and inflammatory cytokine were markedly inhibited in photothrombotic ischemia lesion compared to the control group. These results suggest that DHSMT prevent ischemic death of brain on photothrombotic ischemia model of mice through modulation of gene expression at the transcriptional level.
목적: 저자 들은 주 우울증 환자들의 뇌 혈류 SPECT을 정상 대조군과 비교하여, 기존에 보고된 주 우울증 환자들의 소견과 어떤 차이가 있는 지 알아보고자 하였다. 대상과 방법: DSM-IV criteria로 주 우울증인 환자들 중에서 4주 이상 모든 정신과적 약물을 중지시킨 12명(남: 7, 녀: 5, 나이 범위: $19{\sim}52$세, 평균 나이: $29.3{\pm}9.9$세)과 자발적으로 지원한 정상 대조군 14명(남: 8, 녀: 6, 나이 범위: $19{\sim}53$세, 평균 나이: $31.4{\pm}9.2$세)을 대상으로 하였다. Tc-99m ECD 주사 후 SPECT 영상을 얻은 후, 주 우울증 환자군과 정상 대조군에서의 국소 뇌 혈류 패턴을 비교하였다. 결과: 정상인 대조군에 비해서, 주 우울증 환자들은 우측 후두엽의 설상 회와 방추상 회, 좌측 후두엽의 설상 회와 설전부, 그리고 좌측 측두엽의 상측두 회의 국소 뇌 혈류가 의미있게 증가하였다. 또한 우측 뇌교, 좌측 전두엽의 내측 전두회, 좌측 변연계의 대상 회, 우측 전두엽의 대상 회, 우측 변연계의 대상 회에서는 대조군에 비해 주 우울증 환자에서 국소 뇌 혈류가 의미있게 감소하였다. 결론: 주 우울증 환자군의 Tc-99m ECD뇌 혈류 SPECT 소견은 기존의 보고와 큰 차이가 없었으며, 우측 뇌교와 대상 회 그리고 좌측 내측 전두 회에서 특정적인 국소 뇌 혈류 감소 소견이 있었다.
본 연구에서는 $^{99m}Tc$-HMPAO SPECT를 이용한 AD의 진단에서 허혈성 뇌병변의 유무와 대조군의 선정방법이 진단성적에 미치는 영향을 조사하기 위하여 지역주민을 대상으로 진단된 PAD군 11례와 연령을 맞춘 대조군 12례에서 SPECT를 시행하여 국소 혈류비를 분석하였다. MRI 소견을 조사한 결과 PAD군의 27%(3/11)와 대조군의 25%(3/12)에 유의한 뇌허혈 또는 경색 병변이 관찰되었다. 뇌병변의 유무와 관계없이 두군을 비교한 결과 측두엽이나 두정엽에 평균혈류비의 유의한 차이가 없었으며 두군을 신뢰성있게 감별할 만한 특징적인 혈류양상을 발견하지 못하였다. 그러나 뇌허혈병변이 있는 자를 대조군에서 제외시켰을 때 두군의 측두엽 및 두정엽 혈류비의 차이가 증가하였다. 또 별도로 설정한 건강한 대조군과 비교한 결과 같은 부위의 혈류비에 현저한 차이가 관찰되었다. 이상의 결과로 뇌혈류 SPECT에 의한 AD의 진단성적은 연구대상의 선정에 의해 영향을 받으며 특히 허혈성 뇌병변의 유무는 연구결과에 많은 영향을 미친다고 생각되었다. 향후 허혈성 뇌병변에 의한 영향을 해결하기 위해 SPECT 영상을 방사선학적 검사와 함께 평가하는 방법에 대한 연구가 필요하다고 사료되었다.
