Emergency In Situ Bypass during Middle Cerebral Artery Aneurysm Surgery : Middle Cerebral Artery-Superficial Temporal Artery Interposition Graft-Middle Cerebral Artery Anastomosis

  • Many reports have been published on complications related to middle cerebral artery (MCA) aneurysm surgical clipping procedures. We report an emergency intracranial in situ bypass surgery case which was performed as a rescue procedure after aneurysmal neck laceration during clipping of an MCA large aneurysm. In this case, we performed in situ M3-superficial temporal artery (STA) interposition graft-M3 bypass procedure. If a STA-MCA anastomosis is not available under MCA flow obstruction, we can consider an emergency in situ MCA-MCA bypass procedure with or without an STA interposition graft.

  • Simultaneous subarachnoid hemorrhage and infarction is a quite rare presentation in a patient with a spontaneous dissecting aneurysm of the anterior cerebral artery. Identifying relevant radiographic features and serial angiographic surveillance as well as mode of clinical manifestation, either hemorrhage or infarction, could sufficiently determine appropriate treatment. Enlargement of ruptured aneurysm and progressing arterial stenosis around the aneurysm indicates impending risk of subsequent stroke. In this setting, prompt treatment with stent-assisted endovascular embolization can be a reliable alternative to direct surgery. When multiple arterial dissections are coexistent, management strategy often became complicated. However, satisfactory clinical results can be obtained by acknowledging responsible arterial site with careful radiographic inspection and antiplatelet medication.

  • Cerebral aneurysm mostly occurs at a bifurcation of the circle of Willis. When the cerebral aneurysm is ruptured, a disease like subarachnoid hemorrhage and stroke is caused and this can be even deadly for patients. Generally it is known that causes of the intracranial aneurysm are a congenital deformity of the artery and pressure or shear stress from the blood flow. A blood flow pattern and the geometry of the blood vessel are important factors for the aneurysm formation. Research for several hemodynamic indices has been performed and these indices can be used for the prediction of aneurysm initiation and rupture. Therefore, the numerical analysis was performed for hemodynamic characteristics of the blood flow through the cerebral artery applying the various bifurcation angle and flow rate ratio. We analyze the flow characteristics using indices from the results of the numerical simulation. In addition, to investigate the flow pattern in the aneurysm according to the bifurcation angle and the flow rate ratio, we performed the numerical simulation on the supposition that the aneurysm occurs.

Coil Embolization for Distal Middle Cerebral Artery Aneurysm

  • Middle cerebral artery [MCA] aneurysms are a common source of subarachnoid hemorrhage [SAH]. But, ruptured distal MCA aneurysm is very rare, and their clinical and radiological features are poorly understood. Microsurgical repair remains the most common method used to treat distal MCA aneurysm, even though endovascular coiling has been favored recently. We report our experience of successful coiling for ruptured distal MCA aneurysm, In selected patients, coiling may be a good treatment option for ruptured distal MCA aneurysm.

The impact of collateral status on cerebral vasospasm and delayed cerebral ischemia in subarachnoid hemorrhage

  • Objective: Cerebral collateral circulation may affect subarachnoid hemorrhage (SAH) induced cerebral vasospasm and delayed cerebral ischemia. In this study our aim was to investigate the relationship between collateral status, vasospasm and delayed cerebral ischemia (DCI) in both aneurysmal and nonaneurysmal SAH. Methods: Patients diagnosed as SAH with and without aneurysm were included and their data investigated retrospectively. After the patients diagnosed as SAH according to cerebral computed tomography (CT)/magnetic resonance imaging (MRI), they underwent cerebral angiography to check for cerebral aneurysm. The diagnosis of DCI was made according to the neurological examination and control CT/MRI. All the patients had their control cerebral angiography on days 7 to 10 in order to assess vasospasm and also collateral circulation. The American Society of Interventional and Therapeutic Neuroradiology/Society of Interventional Radiology (ASITN/SIR) Collateral Flow Grading System was modified to measure collateral circulation. Results: A total of 59 patients data were analyzed. Patients with aneurysmal SAH had higher Fisher scores and DCI was more common. Although there was no statistically significant difference between the patients with and without DCI in terms of demographics and mortality, patients with DCI had worse collateral circulation and more severe vasospasm. These patients had higher Fisher scores and more cerebral aneurysm overall. Conclusions: According to our data, patients with higher Fisher scores, more severe vasospasm, and poor cerebral collateral circulation may experience DCI more frequently. Additionally aneurysmal SAH had higher Fisher scores and DCI was seen more common. To improve the clinical results for SAH patients, we believe that physicians should be aware of the DCI risk factors.

Saccular Aneurysm at the Anterior Communicating Artery Complex Associated with an Accessory Middle Cerebral Artery : Report of Two Cases and Review of the Literature

  • Accessory middle cerebral artery (MCA) is an infrequent vascular anomaly of the brain. Cerebral aneurysms associated with this anomalous artery are also very rare. To our knowledge, there have only been ten previous reports of an aneurysm associated with accessory MCA. The authors present two patients with accessory MCA-related aneurysms. A 38-year-old male and a 59-year-old female both presented with sudden-onset severe headache. In both patients, computed tomography (CT) scan revealed subarachnoid hemorrhage. A subsequent angiogram demonstrated an accessory MCA arising from the anterior cerebral artery (ACA) and a saccular aneurysm at the anterior communicating artery (ACoA) complex associated with an accessory MCA. Surgical clipping allowed for complete exclusion of the aneurysm from the arterial circulation. Based on our review of the ten cases of aneurysms associated with accessory MCA documented in the literature, we suggest that accessory MCA-related aneurysms can be classified according to whether the accessory MCA originates from the proximal A1 segment or from the ACoA complex. We also emphasize the importance of precise interpretation of preoperative angiograms and intraoperative precaution in determining the presence of this anomalous artery prior to temporary clip placement.

  • Intracranial aneurysms in the neonate are very rare and their clinicopathological findings remain unclear. We report a 26-day-old premature neonate who underwent microsurgical clipping on the ruptured middle cerebral artery bifurcation aneurysm successfully with a review of relevant literature.

뇌 동맥류 진단에 있어 디지털 감산 혈관조영술과 다중검출기 전산화단층촬영 혈관조영술의 유용성 비교 (The Comparison of Usefulness between MDCT Angiography and DSA in the Diagnosis of Intracranial Aneurysm)

  • 본 연구는 뇌동맥류로 진단받은 환자 중 디지털 감산 혈관조영술(DSA)과 다중검출기 전산화단층촬영 혈관조영술(MDCT Angiography)의 영상을 비교하여 MDCT 혈관조영술에 대한 뇌동맥류의 선별검사로써 그 유영성을 평가하고자 하였다. 이를 위해 대학병원에서 DSA을 시행한 후 뇌동맥류로 진단 받은 환자 중 MDCT 혈관조영술을 함께 시행한 194명의 환자를 대상으로 DSA과 MDCT 혈관조영 장치의 영상에서 뇌동맥류의 유무를 분석하였고, 동맥류의 길이를 경부의 크기로 나누어 1보다 크면 경부가 좁은 협경 동맥류, 1보다 작으면 경부가 넓은 광경 동맥류로 분류하여 협경, 혹은 광경 동맥류인지 비교 분석하였다. 본 연구 결과 DSA과 비교한 MDCT 혈관조영술의 뇌동맥류에 대한 민감도는 97.4%로 분석되었고, 협경 혹은 광경 동맥류의 일치도는 90.2%, MDCT 혈관조영술에서 뇌동맥류를 발견하지 못한 경우가 2.5%로 경부의 평균 크기는 2.4 mm였다. 이를 통해 뇌동맥류 선별검사를 위하여 비침습적인 MDCT 혈관조영술은 침습적인 DSA에 비하여 높은 민감도를 보여 매우 유용한 검사로 기대된다.

  • A 58-year-old woman was admitted to our department due to headache. Brain computed tomography [CT] indicated subarachnoid hemorrhage [SAH], and emergency angiography showed dissecting aneurysm involving the left A1 segment We performed trapping of dissecting A1 aneurysm and the postoperative course was uneventful. We present a case of dissecting aneurysm in the anterior cerebral artery with spontaneous SAH which was treated by early surgery and resulted in clinically good prognosis.
