TONGUE INJURIES BY SELF MUTILATION IN LESCH-NYHAN SYNDROME PATIENT: A CASE REPORT (Lesch-Nyhan 증후군 환아의 자해에 의한 혀의 외상)

  • Lesch-Nyhan syndrome is a rare X-linked recessively inherited disorder, caused by complete absence or decrease in activity of hypoxanthine guanine phosphoribosyl transferase(HPRT), an enzyme involved in purine metabolism. This enzyme deficiency gives rise to nephropathy symptoms, such as hyperuricosuria and hyperuricemia by excessive uric acid production and neuropathy symptoms, such as mental retardation, choreoathetosis and self mutilation behavior. Patients with Lesch-Nyhan syndrome have tendency to bite their lip, tongue and finger. In severe cases, partial or even total amputation of tongue or finger occur. Self-inflicted bites are often complicated by secondary infection to the injured site as well as pain. Furthermore tissue loss by biting results in esthetic problems. The dental management of self mutilation includes treatment with appliances such as soft mouth guard or lip bumper, extraction of all the teeth, and orthognathic surgery. We report a case of a 13 year-old boy with Lesch-Nyhan syndrome, who severely injured himself on his tongue. At first, conservative treatment using soft mouth guard was considered, but it could not prevent trauma on his tongue. Therefore, extraction of the lower anterior and posterior teeth was carried out.

A Study on Women's Health Status for Setting up Women's Health Nursing Center and Developing Health Program (일부지역 여성건강간호센터 설립 및 여성건강관리 프로그램을 위한 기초조사)

  • The purpose of this paper was to identify the performance of health promoting lifestyles and health perception over 18 years old women living Wonju city so that the results will be based for setting up women's health nursing center. The subjects were 1080 women selected by stratified and purposive sampling. The data were collected by self reporting questionnaire and interview from May to June, 1998. Data were analyzed by SPSS win program. The results were as follows : 1. The range of age was 18-84 years, The proportion according to women's lifecycle was premarital group 20.0%, delivery and rearing group 49.9%, over middle aged-elderly group 29.8%. 2. The mean menarchial age was 15.2 and menopausal age was 48. Mean frequenices of pregnancy is 2.4 and artificical abortion rate is 36.4%. Primary cause of abortion was unwanted babies 42.8%. The practice rate of family planning was 79.4% and permanent sterilization rate was 37.6%. 3. Fatigue was predominated problem in target population. Depression and headache was predominated in premarital group, headache and nervous felling in delivery and rearing group, arthritis and loss of memory in over middle aged-elderly group. 4. Only 13.7% of the target population make some efforts for their health in compare to 85.9% have attention for their health. Perception of unhealthy rate was 9.1% in premarital group, 24.8% in delivery and rearing group, 30.1% in middle aged-elderly group. 5. The average score of the HPLP(Health Promoting Lifestyle Profile) was 2.41. The variable with the highest degree of performance was interpersonal relationship, whereas the one with the lowest degree was the professional health maintenance. The significant difference was found in HPLP according to age, residential area, marital status, educational level, income level. 6. Majority(95.1%) of the target population agreed on necessity for women's health nursing center. Proper location area was presented to women's center and public health center. The priority for health education program was proper diet, family health, stress management, and exercise. In conclusion, we should prepare the education program for women's health according to women's lifecycle, because health perception, HPLP, and education program needed was differentiated in women's lifecycle. Also we suggest that women's health nursing center based community was needed for proper management of women's health.

Knowledge about Colorectal Cancer in Northern Iran: a Population-Based Telephone Survey

  • Background: Colorectal cancer (CRC) is one of the most common malignancies of the gastrointestinal tract. The aim of this study was to assess the general knowledge of CRC in individuals living in Rasht, Iran, using a population-based cross-sectional telephone survey. Materials and Methods: A total of 1557 participants between 18 and 80 years of age were interviewed using random sampling from the telephone directory. Knowledge of risk factors, symptoms, diagnosis, and prevention of CRC was assessed using a validated questionnaire. Results: The mean knowledge level of the 1,557 respondents (average age 46 y) was $13.5{\pm}4.29$ (maximum possible score = 26), and 46.4% (722/1,557) of the subjects achieved grades lower than the mean score. The mean scores for knowledge of symptoms and risk factors were $3.97{\pm}1.83$ (range: 0-7) and $5.17{\pm}1.65$ (range: 0-9), respectively. Older age, higher education, and employment were significantly associated with better scores for recognition of risk factors and warning symptoms. The majority of subjects correctly identified weight loss (70.2%; 1,093/1,557) and rectal bleeding (63.3%; 986/1,557) as symptoms of CRC, and that smoking (85.9%; 1,337/1,557) and a low-fiber diet (73.4%; 1,143/1,557) were risk factors. Approximately half of the subjects noted increasing age, genetic background and fried food as other risk factors. A considerable number (54.8%; 853/1,557) identified colonoscopy as a screening method for detecting CRC in asymptomatic patients. However, a third of the subjects in the target group for screening (${\geq}50y$) were not interested in undergoing screening, primarily due to a lack of symptoms. Conclusions: Our results suggest that the knowledge of CRC is poor among the public, and therefore greater attempts should be made to increase awareness. Public education emphasizing the risk factors and symptoms of CRC, as well as the importance of regular screening regardless of the presence of symptoms, may help to reduce CRC morbidity and mortality.

Risk Factors for Ketosis in Dairy Cows and Associations with Some Blood Metabolite Concentrations

  • Ketosis has become a very common and important metabolic disorder that causes substantial economic loss in modern dairy herds. We determined the risk factors for ketosis and associations with some blood metabolite concentrations in dairy cows. Blood from 475 Holstein cows on four dairy farms was collected weekly until 4 weeks after calving to measure blood ${\beta}$-hydroxybutyrate (BHBA) concentrations using electronic handheld meters. Cows were grouped based on the BHBA concentration into two groups: a ketosis group (${\geq}1.2mmol/L$, n = 150) and a non-ketosis group (< 1.2 mmol/L, n = 325). Peripartum health status (dystocia, retained placenta, and metritis), cow parity, and calving season were recorded to identify the risk factors for ketosis. Serum albumin, total cholesterol (TCH), and haptoglobin concentrations were compared between sub-groups of cows selected from the ketosis (n = 92) and non-ketosis (n = 50) groups 1 week postpartum ($7.5{\pm}0.2days$). The farm influenced the incidence of ketosis (P < 0.05). Cows calved during summer tended to have a higher risk (odds ratio [OR]: 1.61, P < 0.1) of ketosis than cows calved during spring. Cows with parities of two (OR: 1.95, P < 0.05) and three or higher (OR: 2.55, P < 0.01) were at higher risk than primiparous cows. Cows with metritis had a higher risk (OR: 7.02, P < 0.0001) of ketosis than cows without metritis. Serum albumin and TCH concentrations were lower (P < 0.01) in the ketosis group than in the non-ketosis group, whereas haptoglobin concentration was higher (P < 0.05) in the ketosis group than in the non-ketosis group. In conclusion, our results indicate that farm, summer calving, parity greater than one, and postpartum disease (metritis) were risk factors for ketosis. In addition, lower serum albumin and TCH concentrations and higher haptoglobin concentration were also associated with the incidence of ketosis in dairy cows.

Quality Characteristics of Low-Fat Sausage Containing Curcumin Extract during Cold Storage (울금 추출물 함유 저지방 소시지의 냉장저장 중 품질 특성)

  • The effects of curcumin extract on the physicochemical, microbial and sensory properties of low-fat sausages during refrigerated storage were studied. Sausage products were produced with three different formulations including 0%, 2.5%, and 5.0% curcumin extract. Low-fat sausages made with the addition of curcumin extract had lower (p<0.05) crude fat content, pH and TBARS values than the control sample. The addition of curcumin extract did not affect the water holding capacity, cooking loss, shear force, meat color, texture profile, and total bacterial count in low-fat sausages duringstorage (p>0.05). With regard to sensory evaluation, 2.5% curcumin extract added to low-fat sausages resulted in a high overall acceptability (p<0.05). In conclusion, low-fat sausages with added 2.5% curcumin extract had a higher acceptability and lipid oxidative stability during storage than products without curcumin extract.

365 nm LED laser treatment on beagle for gingival whitening without gum dermabrasion (잇몸 박피술 없는 치은 미백을 위한 비글에서 365 nm LED 레이저 치료)

  • Purpose: Gingival whitening is one of dental treatment purposes which is close to treating aesthetic disorders. Initial gingival whitening treatment was done by dermabrasion using a high power Diode Laser. However, this treatment method cannot be free from any infection or pain after the treatment. Therefore, we have decided to progress gingival whitening treatment using a low power LED laser. Materials and Methods: The laser was irradiated on pork meat then the safety of output power, temperature change and skin denaturalization was measured. Bison 365 nm LED laser was irradiated on oral mucosal pigment of a 15 - 20 kg beagle for 15 min for 1 - 2 weeks, one or two times each. Any pigment loss was checked through Hematoxyline-Eosin staining. Results: The melanin pigments at the area of 365 nm LED Laser irradiation were decreased. Conclusion: The 365 nm LED Laser proposed in this study is considered to compensate the bleaching effect achieved by either using Diode laser or surgical methods.

Poly(arylene ether ketone) block copolymer prepared through sulfonation process for polymer electrolyte membrane fuel cell (술폰화 공정을 통해 제조한 고분자 전해질형 연료전지용 폴리(아릴렌 이서 케톤) 블록 코폴리머)

  • In this study, a sulfonated poly(arylene ether ketone) block copolymer was prepared from hydrophilic oligomer and hydrophobic oligomer. The structure of the prepared membrane was characterized by $^1H$-NMR, FT-IR and GPC. The $M_w$(weight-average molecular weights) of the polymer was $209,700g\;mol^{-1}$ and the molecular weight distribution($M_w/M_n$) of 1.25 was obtained. The prepared membrane showed excellent thermal stability with gradual weight loss up to $200^{\circ}C$. The proton conductivity of SPAEK block copolymer reached the maximum of $9.0mS\;cm^{-1}$ at $90^{\circ}C$ under 100% relative humidity (RH). From the observed results, it is necessary to do more aggressive attempt to study the possibility of application as an ion-conductive composite electrolyte.

Discordant Congenital Cytomegalovirus Infection in Twins (쌍생아에서 선천 거대세포바이러스 감염 증례)

  • Cytomegalovirus (CMV) infection is one of the most common congenital infections. The first case of discordant congenital CMV infection in twins occurred in Korea. A 31-year-old woman became pregnant with twins (dichorionic-diamniotic). An elective caesarean section was performed at 37 weeks. The first baby was male, weighing 2,410 g with an Apgar score of 8/9. The second baby was female, weighing 1,380 g with an Apgar score of 5/8. She had experienced intrauterine growth retardation, and presented with microcephaly, micrognathia, and joint stiffness. During the work-up for discordant twins, the second baby's serum test was positive for CMV immunoglobulin M. Her urine, blood, and cerebrospinal fluid (CSF) were CMV polymerase chain reaction positive. The first baby's CMV tests were negative. Ophthalmologic exam and audiometry performed on the second baby showed CMV retinitis and bilateral sensorineural hearing loss. She was treated with intravenous ganciclovir. Currently, she is bed-ridden and has significant developmental delay. Although the causes of discordant congenital CMV infection in twins are unclear, this case shows that discordant congenital CMV infection should be considered in twins with significant differences in intrauterine growth or clinical symptoms after birth.

A Clinical Study on $^{125}I\;T_3$ Resin Uptake Rate and Serum Thyroxin ($T_4$) in Hyperthyroidism (갑상선기능항진증(甲狀腺機能亢進症)에 있어서 $T_3RU$$T_4$에 관(關)한 임상적(臨床的) 연구(硏究))

  • Hyperthyroidism may be defined as those clinical conditions which result from an increase in the circulating levels of one or both thyroid hormones. Hyperthyroidism in broad sense could be classified with toxic diffuse goiter, toxic adenomatous goiter, and toxic multinodular goiter on the basis of the circulating thyroid hormone levels. For this study, the subject included 94 cases with hyperthyroidism were presented in 77 with toxic diffuse goiter, 8 with toxic adenomatous goiter, and 9 with toxic multinodular goiter on the levels of $^{125}IT_3$ resin uptake rate and serum thyroxine ($T_4$). The observed results were as follows: 1) In the cases of hyperthyroidism including toxic diffuse goiter, toxic adenomatous goiter, and toxic multinodular goiter, 20.21% of the patients were male and 79.79% female. The majority of the patients were in 2nd to 4th decades of their lives. 2) There were objective signs clearly manifested in hyperthyroidism including toxic diffuse goiter and toxic adenomatous goiter which were rare in the multinodular goiter. The clinical signs in toxic diffuse and toxic adenomatous goiter included wide pulse pressure, tachycardia, systolic murmur, exophthalmos, tremor and warm skin etc. (Table 3.) 3) The most freauent complaints of the patients with hyperthyroidism were palpitation, weight loss, increased appetite, perspiration, heat intolerance, nervousness, exertional dyspnea, and menstrual disturbance etc. (Table 4.) There was no clear difference in the incidence of symptoms between toxic diffuse goiter and toxic adenomatous goiter, but there was clear difference between toxic multinodular goiter. 4) Considering of results of $^{125}IT_3$ resin uptake rate and serum $T_4$ level in toxic diffuse goiter, toxic adenomatous goiter and toxic multinodular goiter, $^{125}I\;T_3$ resin uptake rate was $49.15{\pm}9.94%$ (mean) and serum $T_4\;21.29{\pm}7.04ug/dl$ (mean) in toxic diffuse goiter. In toxic multinodular goiter, $^{125}I\;T_3$ resin uptake rate was $32.47{\pm}6.74%$ (mean) and serum $T_4$ level $11.03{\pm}5.0ug/dl$, and then there was clear difference in the results of $^{125}I\;T_3$ resin uptake rate and serum $T_4$ between toxic diffuse goiter and toxic multinodular goiter. The levels of $^{125}I\;T_3$ resin uptake rate and serum $T_4$ in toxic adenomatous goiter were $40.32{\pm}13.08%$ (mean), $15.47{\pm}8.25ug/dl$ (mean) respectively, so there was no clear difference between toxic diffuse goiter and toxic adenomatous goiter. 5) There was no significant differnece in length and width performed with thyroid scanning in toxic diffuse goiter, toxic adenomatous goiter, and toxic multinodular goiter.

Polyvinylchloride Plasticized with Acetylated Monoglycerides Derived from Plant Oil (아세틸화 모노글리세라이드계 가소제 합성 및 PVC 가소성능에 관한 연구)

  • To replace phthalate plasticizer for PVC, acetylated monoglyceride (AMG) plasticizers were prepared from plant oil and their plasticization effects were also investigated. Transesterification of coconut oil by glycerol followed by acetylation with acetic anhydride gave AMG-CoCo (Coco : Coconut Oil). In addition, AMG-GMO (GMO : Glycerol monooleate) and AMG-GMO-Epoxy were synthesized by acetylation and epoxidation with glycerol monooleate. It was found that the thermal stability of AMG plasticizers increased in the following order: AMG-GMO-Epoxy > AMG-GMO > AMG-CoCo and all three plasticizers were thermally more stable than those of common petroleum-based plasticizer DOP (Dioctyl phthalate). The tensile strain values of the PVC containing AMG compounds were ca. 770~810%, while tensile strength values were ca. 19~22 MPa, which were higher than those of PVC containing DOP. DMA (Dynamic Mechanical Analysis) results showed that the miscibility of AMG-GMO-Epoxy in PVC was excellent and the $T_g$ of PVC containing AMG-GMO-Epoxy at 50 phr decreased down to $24^{\circ}C$. Finally, the leaching experiment result showed that the weight loss values of PVC containing AMG-GMO and AMG-GMO-Epoxy at 50 phr were as low as 2 and 1%, respectively, indicating that they have high water migration resistance. The above findings suggested that AMG-GMO-Epoxy could be one of plant oil-based PVC plasticizers to replace DOP.
