Principal Components of Thermal Stimulation while the Warm Needling: Diameter of the Acupuncture Needle and Distance from the Skin (온침 표준화를 위한 열자극 요소 연구: 침 두께 및 피부-뜸 거리를 중심으로)

  • Objectives : Warm needling is a combined treatment technique of acupuncture and moxibustion. In this study, we aimed to find out the components related with the thermal stimulation of the warm needling and to provide basic data for the guideline of the warm needling technique in the clinic. Methods : In this study, we measured thermal change of 3% agarose phantom embedding K-type thermocouples in depths of 0, 1, 2, 4, 8 and 16 mm. The warm needling was performed with acupuncture needles of various specifications (0.50×30, 0.50×40, 0.30×30, 0.30×40, 0.20×30 and 0.20×40 mm). A linear regression analysis was performed to find out the major component and quantify the effectiveness of the thermal stimulation during warm needling. Results : As a result of the measurement of temperature change, we could observe the thermal change pattern from the surface of the phantom to the 16mm deep part of the phantom. The thermal pattern was similar among the needles of different specifications. The regression analysis pointed the distance between the moxa cautery and the skin surface as the main component for the thermal stimulation of the warm needling. Conclusions : The authors suggest considering the distance between moxa cautery and the skin rather than the diameter of the acupuncture needle in accordance to the result of the study.

  • The posterior nasal bleeding sometimes develope a life threatening situation because of its limited access and the profuse vascular network of the bleeding area. There are various methods of hemostasis including packing, cautery, and arterial ligation those vary in effectiveness. But sometimes patients cannot tolerate these methods or show rebleeding sign. So, if all of these methods are not successful, we should consider the further treatment. The selective angiographic embolization has various advantages such as rapidness, repetition, good visualization, and being performed under local anesthesia, therefore it can provide useful way in patients with massive, intractable posterior nasal bleeding. We report a case of angiographic embolization for intractable posterior nasal bleeding patient and review the effectiveness of the this treatment.

Thoracoscopic Treatment of Spontaneous Pneumothorax (흉강경을 이용한 기흉의 치료)

  • The result of thoracotomy for recurrent and persistent pneumothorax is usually excellent. However the patients undergone thoracotomy suffer from postoperative chest pain and require long postoperative recovery period. Also the operative incision scar gives the patients cosmetic problems. Subpleural blebs are usually causes of pneumothorax. They can be reached through the thoracoscope without thoracotomy and can be ablated by electrical cautery through it. Six patients with recurrent and persistent pneumothorax were managed thoracoscopic-ally. Five patients were successful and one case was failed. The failed case was explored 21 days after thoracoscopy. Follow-up period was from one to eight months. Although the follow-up period was short, thoracoscopic management of recurrent and persistent pneumothorax is thought to be good for preventing thoracotomy.

Successful Surgical Treatments of Scleral Rupture with Hyphema in a Young Cat (어린 고양이에서 발생한 안구내출혈을 동반한 공막 파열의 수술적 치료)

  • A 3-month-old, female Persian cat was presented with sclera mass. The mass was detected after cat's squeals during the playing with a dog before 2 days. On ophthalmic examination, a $2{\times}5mm$ dark purple colored raised mass was observed 1 mm behind the limbus in the right eye. Anterior chamber was filled with blood and epithelial defect was detected at 2 o'clock in the right central cornea. After general anesthesia, resection of the protruded iris was performed with iris scissors and bipolar cautery. Ruptured sclera and conjunctiva were closed with simple interrupted and simple continuous suture, respectively. Also, irrigation and aspiration (I/A) of the anterior chamber was performed to remove blood clot with bimanual I/A handpiece in the right eye. Four months after the surgery, the right eye was recovered completely and menace response was positive in spite of the retinal lesions.

  • Objective : The aim of this article is to report the chinese published papers on the results using the direct moxibustion to treat warts.Methods : Searching chinese published papers, we classified the results involved in the direct moxibustion treatment about warts. And analysed remedy methods, periods, cure rate, etc.Results : According to chinese published papers, the direct moxibustion itself may be effective in patients with warts. In all paper, the direct moxibustion showed more than 90% cure rate. The recurrence rate was low.Conclusion : We introduce a variety of wart treatments with the direct moxibustion which is made in China and report meaningful results. We hope to this paper contributes to various studies on cauterization of direct moxibustion.

  • Fourth branchial pouch anomaly is an extremely rare condition involving the pharyngeal apparatus with only about 30 cases having been reported in the last 20 years. It almost invariably occurs on the left side and typically presents with repeated episodes of neck swelling, recurrent thyroiditis, or repeated bouts of retropharyngeal abscess. This is a presentation of 2 patient with pyriform sinus fistula, presumably of fourth branchial origin, which, were successfully closed with 10ole trichloroacetic acid chemical cautery. Details of the cases along with the summary of embryology and fourth branchial anomaly are given.

A Comparison of Surgical Methods of Inducing Femoral Head Osteonecrosis in Rats (랫드에서 대퇴골머리 골괴사 유발 외과적 방법의 비교)

  • Osteonecrosis of the femoral head is an idiopathic and progressive disease. It was reported that several animal models have been used for the research of osteonecrosis. However, no standardized animal model for the study of osteonecrosis has been developed to date. This study was conducted to compare the degree of osteonecrosis of three surgically induced osteonecrosis models in rats. Twenty Sprague-Dawley rats (24 weeks old, male) were divided into three experimental groups and a control group, five heads each. Three groups were surgically induced into osteonecrosis; the ligamentum teres were cut and the periosteum of the femoral neck was stripped (Group S), the steel wire was ligated to the neck of the femoral head (Group W), and the femoral neck was tied up with a wire in the same way as in the W group, and burned by attaching the electrode tip to the wire and then the wire was removed (Group B). After two weeks, rats were sacrificed and the femoral head and neck were collected. Histological findings were evaluated with H/E stains, Safranin-O and TUNEL for osteonecrotic lesions in the bones and cartilages of the femoral head. Osteonecrosis was induced successfully in all groups (Group S, W and B) in two weeks, a short period of time. Significant necrotic changes of the cartilage were detected only in Group B. In the modified cautery model in particular, the method of removing the wire after cautery was completed in the experimental model of osteonecrosis more efficiently than any other method.

  • Segmental resection or wedge resection of the lung and direct cutting across of bronchi frequently results in postoperative airleaks or bronchopleural fistula in some cases. Many methods have been devised to handle air leak problems by oversuturing the raw edges, the application of pleural onlay flaps, the use of cautery or tissue adhesives, but these still has not solved the problem of air leaking from raw surfaces of the lung with only partial successful we have tried the use of histoacryl to closure the raw surface of the resected lung and compared it’s results with of the closure with chromic catgut sutures in cats from May 1989 to Jan. 1990 at the department of the thoracic and cardiovascular surgery, Yonsei University, College of Medicine. Ninety lobe of the lungs were used in this study and forty nine of which have been divided segmentally and closed with histoacryl on the raw surfaces. Forty one of which have been divided segmentally and closed with sutures using chromic catgut for control. There were air leakages in twelve cases among 49 cases with applying histoacryl and airleak were in two cases among 12 cases with reapply histoacryl, But there were air leak in two cases among 41 cases with sutures using chromic catgut and airleak were in one case among with resutures using chromic catgut.

Case Study on Treating Acne Scar Using Hani-maehwa Laser (하니매화레이저를 이용한 여드름 흉터 치료 1례)

  • Objective : The aim of this study is to report the effect of Hani-maehwa laser(fractional mode) treatment on acne scar. Methods : The patient with atrophic facial acne scars was treated with 5 sessions of laser therapy at 4-week intervals. The therapeutic response to treatment was assessed at 3 months after last laser session. The treatment effect was evaluated by standardized photography, anti aging, skin brightness and flatness. That was measured by micro fluorescence measurement camera(ECO SKIN, Cuvitz Inc.) in 16(left zygoma) and 26(right zygoma) spot. And side effects were also checked. Results : Atrophic facial acne scars have improved markedly. Anti aging, skin brightness and flatness were improved. Especially anti aging and flatness were noticeably improved. Adverse effects were not reported.Conclusions : Korean medicine cautery method applies to high level laser(CO2 Hani-maehwa laser). These are different designs but same principle. It can be considered that laser therapy is effective method for treating acne scars.
