제 3대구치를 이용한 자가치아이식술의 장기적 예후 관찰 (Long-term evaluation of autotransplanted third molars)

  • Purpose: The purpose of this study is to evaluate the long term clinical and radiographic outcome and stability after transplantation of third molar with complete root formation. Methods: The subjects were 31 teeth (male 17, female 14, aged 22-55, average 39.9 yr old) of 31 patients who visited the department of periodontics and passed more than two years after autotransplantation procedure and still under regular check up. Modified success criteria of Chamberlin and Goerig was applied to determine the success of autotransplantation. Results: Three out of 31 teeth failed and resulted 90.3% of success rate. When compared according to sex, 15 out of 17 teeth had succeeded in male, 13 out of 14 succeeded in female. When compared the success rate according to cause of extraction, tooth loss due to caries and root fracture had all succeeded but 3 out of 24 had failed in tooth loss due to periodontal disease. When compared according to donor teeth, 12 out of 14 maxillary third molars and 16 out of 17 mandibular third molars had succeeded. Conclusions: In long term evaluation over two years, if appropriate surgical procedure and proper case selection is made, autotransplantation of the third molar with complete root formation can be the alternative choice that substitutes prosthetic or implant treatment and it is a functionally acceptable procedure.

  • This is a clinical study on patients who had visitied the Emergency Room of Pusan National University Hospital and then been treated in the Department of Oral and Maxillofacial Surgery during recent 5 years, from 1992 to 1996. The results were as follow ; 1. The total number of patients was 2,680 and the ratio of male to female was 1.96:1, The highest monthly incidence was shown in September(12.1%) and October(10.5%) and the age distribution peaks was the third decade(24.3%), followed by the first(23.1%) and the fourth decade(17.2%). 2. Soft tissue injury group(29.1%) was the most prevalent, followed by tooth injury group(16.1%), facial bone injury group (16.0%), toothache group(11.2%), socket bleeding group(11.1%), infection group(9.8%) and TMJ dislocation group(5.9%). 3. The percentage of in-patients and out-patient were 21.6% and 78.4%, respectively. The frequent causes of admission were facial bone fracture(73.8%), infection(20.8%) and soft tissue injury(4.8%) in order. However, soft tissue injury was the most frequent cause in out-patient, followed by tooth injury(20.5%), toothache(14.3%), socket bleeding(14.2%) and TMJ dislocation(7.6%). 4. In the facial bone injury group, the mandibular fractures(70.6%) showed the highest incidence, followed by zygomatic bone and arch fractures(7.5%), maxillary bone fractures(4.0%) and nasal bone fractures(4.0%). 5. In the mandibular bone fracture, the most common location was symphysis(36.7%), followed by the mandibular angles(33.1%) and the condyles(21.8%). 6. The common causes of facial bone fractures were violence, fall and traffic accident in order. 7. The common causes of soft tissue injury were fall down, fight and traffic accident in order and the highest incidence was observed in infants before the age of 10 years(44.0%). 8. In the group of tooth injury, tooth luxation(38.5%) showed the highest incidence followed by tooth fracture(33.2%) and tooth loss(17.1%). The common causes of tooth injury were fall, fight and traffic accident in order. 9. In infected patients group, the ratio of in-patients to out-patients was 1 : 1.28, Buccal(24,7%) and infraorbital space abscess(23.3%) showed the highest incidence. 10. The pain caused by dental caries(39.0%) and pericoronitis(26.6%) showed high incidence in the toothache group. 11. The high incidence was observed during third(34.0%) and fourth (24.5%) decades in TMJ dislocation group. 12. In the group of socket bleeding patients, 92% was post-operative hemorrhage and 8% was accompanied with other systemic hemorrhagic diseases.

구치부 단일 임플란트의 생존율에 대한 후향적 연구 (A Retrospective Clinical Study of Survival Rate for a Single Implant in Posterior Teeth)

  • Purpose: Single implants, of which screw loosening has been observed frequently, presents problems such as fixtures fractures, marginal bone loss, and inflammation of the soft tissue around the implant. However, the single implant is more conservative, cost effective, and predictable compared to the 3 unit bridge with respect to the long-term outcome. This study evaluated the survival rate as well as future methods aimed at increasing the survival rate in single implants in posterior teeth. Methods: Among the implants placed in the Dankook University Dental Hospital department of Oral & Maxillofacial surgery from January 2001 to June 2008, 599 implants placed in the maxillar and mandibular posterior were evaluated retrospectively. Survival rates were investigated according to implant location, cause of tooth loss, gender, age, general disease, fixture diameter and length, surface texture, implant type and shape, presence of bone graft, surgery stage, surgeons, bone quality and opposite teeth. Results: Out of 599 single implants in posterior teeth, 580 implants survived and the survival rate was 96.8%. The difference in survival rate was statistically significant according to the implant location. The survival rate was low (84.2%) in implants exhibiting a wide diameter (${\geq}5.1mm$) and the surface treated by the acid etching group demonstrated a significantly lower survival rate (91.1%). One stage surgical procedure, which implemented a relatively better bone quality survival rate (100%), was higher than the two stage surgical procedure (96.1%). The survival rate of type IV bone quality (75%) was significantly lower than the other bone quality. Conclusion: Single posterior teeth implant treatments should use an improved surface finishing fixture as well as careful and safe procedures when performing implant surgery in the maxilla premolar and molar regions since bone quality is poor.

  • This study was performed to investigate the survival rate of single implant used in maxillary anterior region during follow up periods. 231 patients whose single missing tooth in maxillary region had been replaced with 237 implant at the periodontal dept. of Yonsei University Hospital between February 1993 and December 2004. The following results are compiled from 231 patients who received single implant surgery. 1. The major cause of single tooth loss in maxillary anterior region is trauma, followed by periodontal disease and congenital missing. 2. The total survival rate of single implant placed in maxillary anterior region is 94.5%. 3. The survival rate of single implant placed in type II and type IV was 100% and in type III was 92.7%. As for the bone quantity, the survival rate in type A(100%) was most, followed by type B(97.3%) and type C, D (93.5%). 4. The survival rate of implant placement combined with advanced surgery was 94.4%. The results showed that the placement of single implant is considered as a reliable treatment option for a single missing tooth in maxillary anterior region except in certain extreme conditions especially with poor bone quality and quantity.

Effect of Toothbrushing Application for Kids on Dental Plaque Removal and Interest in Toothbrushing of Preschool Children

  • Background: Dental caries in preschool children can cause early loss of teeth, reduced masticatory function, malocclusion, and speech disorders, making oral care for preschool children essential. However, because children have a poor ability to perform oral hygiene by themselves, educational tools that help establish correct oral health behaviors are required. This study evaluated the effect of toothbrushing application for kids on dental plaque removal and toothbrushing interest in preschool children. Methods: Seven 5-year-old children enrolled in a daycare center participated in the evaluation, and the same children participated in the experiment twice a week. The Pokémon Smile application (App) was used as an auxiliary application for tooth brushing, and the degree of dental plaque removal on the tooth surface was evaluated by quantitative light-induced fluorescence-digital imaging. After the experiment, children's toothbrushing preferences were investigated through interviews. Results: The levels of AREA R30 and AREA R70 measured after the children's toothbrushing as usual decreased compared to those before toothbrushing; however, the change was not significant. The levels of AREA R30 and AREA R70 measured after using the Pokémon Smile App were significantly reduced compared to those before toothbrushing. Children's interest in brushing their teeth increased by 28.59% after using the Pokémon Smile App. Conclusion: The toothbrushing application for kids effectively removes dental plaque by helping preschool children brush their teeth. It also increased preschool children's interest in tooth brushing. Therefore, an oral health education application would be useful for children who need to develop correct oral care methods and habits.

  • 광범위한 치아의 부식 및 마모는 구강에서 심각한 기능 및 심미의 상실을 야기한다. 이를 회복하기 위해서는 환자의 교합관계에 대한 면밀한 분석이 필요하다. 특히 수직 고경과 중심위의 재현가능성, 전방유도 및 교합평면의 적절성 등을 고려하여 치료계획을 수립하여야 한다. 또한, 마모가 심한 환자의 경우 보철재료의 선택 또한 중요한 고려사항이다. 본 증례에서는 치아의 전반적인 마모와 부식이 있고 있는 환자에서 교정치료를 통하여 전방유도를 부여하고 수직고경의 변화없이 단일 구조 지르코니아를 이용한 완전 구강 회복술로 안정적인 결과를 얻었기에 보고하고자 한다.

  • pH varies in the oral environment and on tooth surfaces. The surface of a plaque covered resinous restoration has a low pH. In human dental plaque a number of acids are produced by bacterial metabolism. The predominant acids are acetic, propionic and lactic acids. These acids induced a surface swelling of restorative resin. The swelling cause formation of pores inside the resin matrix from which organic substances can be released, resulting in a mass loss. The purpose of this study was to qualitatively and quantitatively analyse the leached monomers of dental composites after storage in acetate buffer solution as a function of time by means of high performance liquid chromatography(HPLC) / mass spectrometer and further determine whether pH and time has any influence on the degradation behavior of composite restoration.(omitted)

  • Permanent canine are very important teeth in point of function as well as esthetic. Therefore impaction of canines is a frequently encountered clinical problems. The incidence of impaction ranges between 1-3%. The cause of canine impaction can be result of ectopic position of tooth germ, defect of dental follicle, early loss or delayed exfoliation primary canine, space problem, supernumarary teeth, cyst, odontoma, Impaction frequently involves further complication such as root resorption of adjacent teeth, cyst formation and migration of the neighboring teeth. Various treatment modalities include extraction of primary canine, surgical opening and orthodontic traction, autotransplantation, surgical extraction of impacted canine.

  • Ectopic eruption of the permanent first molar is defined as the eruption of the tooth in an abnormal position or orientation. It may causes distal root resorption and premature exfoliation of the adjacent primary second molar and uncontrolled space loss is followed. Prolonged partial impaction of the permanent molar may also cause undetected caries or abscess formation of the neighbor teeth. The purpose of this paper is to provide a brief review regarding the etiology, classification, and different management techniques for correcting ectopic eruption of permanent first molar.
