소아 급성 간부전의 임상적 의의 (Acute liver failure in children)

  • Acute liver failure (ALF) is a very rare but devastating illness in children. Specific treatment to recovery is often not available, and the underlying cause of the liver failure is often unknown and diverse especially in children. Liver transplantation has increased the chance of survival; however it needs an optimal timing to reach the best result which is not familiar to pediatrician. This article discusses the current knowledge of the epidemiology, backgrounds and factors to be considered before establishing the treatment of ALF in children.

만성 신부전 환자의 심장수술 - 개심술 4례 보고 - (Open Heart Surgery in Patients with Chronic Renal Failure)

  • The leading cause of death in patients with chronic renal failure is cardiovascular diseases. The problems relevant to cardiac surgery in these patients are occurring more frequently with a growing number of patients at risk. Among these, important risk factors related to uremic patients undergone open heart surgery are fluid and electrolytes imbalance, coagulopathy, increased susceptibility to infection. Since 1968 when Lansing and colleagues reported the first successful aortic valve replacement in patients with chronic renal failure and infective endocarditis, there have been increasing reports of the cardiopulmonary bypass surgery in chronic renal failure patients with acceptable perioperative morbidity and mortality From Jan. 1988 to Nov. 1989 we have experienced four uremic patients necessitating open heart surgery ; one needing a coronary artery bypass graft and the other 3 needed cardiac valve replacement. Based on our observations we would like to suggest followings 1]Intraoperative ultrahemofiltration during C-P bypass thought to be an excellent means for the control of hyperkalemia and fluid balance. 2] The immediate postoperative application of peritoneal dialysis instead of hemodialysis is beneficial in controlling fluid and electrolyte imbalance. 3]The cause of one early postoperative death was not associated to renal failure, rather it was the result of an accidental rupture in the right ventricular wall.

RCM 기반 설비 고장 예측시스템 (RCM Based Failure-Prediction System for Equipment)

  • 발전소를 비롯한 각종 플랜트 설비는 많은 기계설비들로 구성되어 고장이 발생할 경우, 고장이 발생된 설비와 시간을 명확하게 파악하기가 쉽지 않다, 또한 고장발생시 정비비용 증가와 함께 설비의 이용률이 감소하여 막대한 경제적 손실이 발생한다. 발전소는 각종 계측센서를 설치하여 설비를 상시 감시하고 있으며, 경고발생 이전 지시치의 변화를 관찰하여 설비의 이상상태를 자동 감지하는 신호기반 고장 조기경보 시스템도 설치된 발전소도 있다. 그러나 고장경보 시스템은 이상신호 발생을 인지하여 설비의 이상상태 여부는 알려주지만 고장원인과 중요도에 대한 정보는 제공하지 않는다. 본 연구에서는 고장분석 기능을 가지고 있는 RCM(Reliability Centered Maintenance) 분석 시스템과 고장경보시스템을 연계하여 설비의 이상신호로부터 고장원인과 정비방법에 대한 정보를 제공하는 고장예측시스템을 개발하고자 한다.

  • 본 연구에서 동부산S/S #3M.Tr 및 부민S/S #1,2M.Tr의 중성선에 설치 사용 중인 NGR 과열원인을 분석하고 해소방안을 제시하여 안정적 전력공급에 이바지하고자한다. 전력계통에 사용 중인 변압기 중성점 접지리액터는 1선지락 고장시 변압기에 유입되는 지락고장전류의 크기를 제한하여 변압기 고장을 감소시키는 역할을 하며 가공절연전선의 용융단선고장방지 일단접지변압기 폭발가능성방지 차단기 차단용량감소 등에 기여하는 설비로 고장발생시 전력계통운용에 막대한 지장을 초래할 수 있다. 따라서 전력계통에 설치 운전 중인 동부산S/S #3M.Tr 중성점 접지리액터 TOP&BOTTOM BRACE부의 온도 상승원인분석과 부민S/S 중성점 접지리액터에 흐르는 과전류로 인한 과열원인 분석을 하였다.

인공심방판막실패에 대한 임상적 고찰 (Reoperation for prosthetic valve failure -clinical analysis of 15 cases-)

  • Despite the multivariate improvements in tissue treatment, material, and design of prosthetic heart valves in recent years, numerous complications that may lead to valve dysfunction remain a constant threat after valve replacement. Most common indications for prosthetic valve failure are primary valve failure, infective endocarditis, paravalvular leakage, and thromboembolism. From 1977 to 1986, 15 patients underwent reoperation for prosthetic valve failure in 278 cases of valve surgery. The etiology of prosthetic valve failure were primary valve failure in 12 patients [80 %], infective endocarditis in 2 patients [13.3 %], and a paravalvular leakage [6.7 %]. The average durations of implantation were 45.5 months; 53.9 months in primary valve failure, 16 months in infective endocarditis, and 4 months in paravalvular leakage. The rate of valve failure was high under age of 30 [11/15]. Calcifications and collagen disruption of prosthesis were main cause of primary valve failure in macro- & micropathology. Prosthesis used in reoperation were 5 tissue valves and 10 mechanical valves. Operative mortality were 13.3 % [2/15], due to intractable endocarditis and ventricular arrhythmia.

  • Conical crown-retained denture (CCRD) has been used as a very effective treatment method in cases with few remaining teeth with heterogeneous prognosis. However, in spite of many advantages of CCRD, high technical failure rate was a problem to be considered. Incorrect path of insertion and excessive retention were thought to be the main cause of technical failure and to result from laboratory procedure with a coping misfit and/or a coping transfer error. In order to prevent this error, secure anchoring of inner coping and re-examination and milling of convergence angle were recommended on the master model from pick-up impression.

  • This paper investigates the failure of a car with a 2.5-liter CRDi engine of the Hyundai Company. The failure is caused by intermittent poor acceleration while driving. To analyze the cause, we investigated the air intake volume, the fuel injection, and the air-fuel ratio, which were determined to be normal. The brake switch signal error was discovered while analyzing the function that limits the output of the engine. While investigating the cause, we discovered the corrosion of the pins on the connector of the brake switch. We determined that it was generated by soapy water flowing in the solar film. Therefore, the cause of the failure was the brake switch signal errors. Additionally, we determined that ECM was the normal fail-safe mode that implemented the override device for safety during normal acceleration. Based on these results, further solar film experiments must be conducted to fully elucidate the causes.

커넥팅로드 소단부 파단의 해석 (Failure Analysis of Connecting Rod at Small End)

  • Failure of connecting rod in automotive engine may cause catastrophic situation. The corner radius at small end has an effect on stress raising. To investigate the stress distribution in connecting rod during operation, the finite element analysis was used by giving possible maximum tension and compression. Excessive sizing after forging connecting rod may result in the tensile residual stress which lower the fatigue life and cause premature failures. It was shown that when the sizing amount is too large, the location of high tensile residual stress coincide with that of high stress amplitude during operation through the elastic-plastic finite element analysis. The endurance limit moves down due to the surface finish and decarburization, which combines with the movement of resultant stress points to dangerous range. It was concluded that the precise control of sizing and enough corner radius are necessary to a reliability of connecting rod.

배전용 변압기의 단락시험 불량원인 및 그 대책에 관한 연구 (A Study on the Cause and Countermeasures of the Short-Circuit Test Failures of the Distribution Transformer)

  • This study aims to research and analyze the cause and countermeasures of the short-circuit test failures of the distribution transformer, which captures failure share at the highest level when carrying out its performance test. For this purpose, the research was done on the basis of 77 failure cases out of 998 tests in total performed by the Korea Electrotechnology Research Institute(KERI) from 2004 to 2010. Based on the research, the paper also includes analysis of the causes of the short-circuit test failures in its early stage of transformer development and proposes its countermeasures accordingly.

승모판막질환에 대한 재수술: 21례 보고 (Reoperation for Mitral valvular disease - Clinical analysis of 21 cases -)

  • After first successful mitral valvotomy by Harken and Bailey in 1948, improvement of surgical technique and cardiac device may last rapidly for several years, but there after many patients deteriorate because of various causes, so incidence of reoperation for cardiac valvular disease has increased time by time. This paper is concerned with 21 patients in whom a second operation has been carried out from Jan. 1963 to Aug. 1984 at the department of Thoracic and Cardiovascular department, National Medical Center. Of 21 patients, 7 were male and 14 were female, and ages ranged from 14 to 37 years The second operation are classified into groups of secondary closed mitral commissurotomy [3 cases], open commissurotomy following closed mitral commissurotomy [1 case], Valve replacement following closed mitral commissurotomy [14 cases] or bioprosthetic valve replacement [3 cases]. Main cause of reoperation was restenosis or steno insufficiency, and that of bioprosthetic valve failure was bacterial endocarditis [1 case], fibrous tissue overgrowth on the Xenograft [1 case] and technical failure [1 case]. Early operative mortality was absent, but during follow-up, 4 patients died, so late mortality was 19.0%, and main cause of death was congestive heart failure.
