An Analysis of Cancer Survival Narratives Using Computerized Text Analysis Program (컴퓨터 텍스트 분석프로그램을 적용한 암환자의 투병수기 분석)

  • Purpose: This study was done to explore experiences of persons living through the periods of cancer diagnosis, treatment, and self-care. Methods: With permission, texts of 29 cancer survival narratives (8 men and 21 women, winners in contests sponsored by two institutes), were analyzed using Kang's Korean-Computerized-Text-Analysis-Program where the commonly used Korean-Morphological-Analyzer and the 21st-century-Sejong-Modern-Korean-Corpora representing laymen's Korean-language-use are connected. Experiences were explored based on words included in 100 highly-used-morphemes. For interpretation, we used 'categorizing words by meaning', 'comparing use-rate by periods and to the 21st-century-Sejong-Modern-Korean-Corpora', and highly-used-morphemes that appeared only in a specific period. Results: The most highly-used-word-morpheme was first-person-pronouns followed by, diagnosis treatment-related- words, mind-expression-words, cancer, persons-in-meaningful-interaction, living and eating, information-related-verbs, emotion-expression- words, with 240 to 0.8 times for layman use-rate. 'Diagnosis-process', 'cancer-thought', 'things-to-come-after-diagnosis', 'physician husband', 'result-related-information', 'meaningful-things before diagnosis-period', and 'locus-of-cause' dominated the life of the diagnosis-period. 'Treatment', 'unreliable-body', 'husband people mother physician', 'treatment-related-uncertainty', 'hard-time', and 'waiting-time represented experiences in the treatment-period. Themes of living in the self-care-period were complex and included 'living-as-a-human', 'self-managing-of-diseased-body', 'positive-emotion', and 'connecting past present future'. Conclusion: The results show that the experience of living for persons with cancer is influenced by each period's own situational-characteristics. Experiences of the diagnosis and treatment-period are negative disease-oriented while that of the self-care period is positive present-oriented.

Building an Ontology-Based Diagnosis Process of Crohn's Disease Using the Differentiation Rule (감별 규칙을 이용한 온톨로지 기반 크론병 진단 프로세스 정의)

  • Crohn's disease, which is recently increasing in Korea, may be seen throughout the gastrointestinal tract and cause various symptoms. In particular, Crohn's disease is especially difficult to diagnose with several symptoms similar to other ulcerative colonic diseases. Thus, some studies are underway to distinguish two or more similar diseases. However, the previous studies have not described the procedural diagnosis process of it, which may lead to over-examination in the process. Therefore, we propose a diagnosis process of Crohn's disease based on the analysis of redundancy, sequential linkage and decision point in the diagnosis of Crohn's disease, so that it enables to identify ulcerative colonic diseases with symptoms similar to Crohn's disease. Finally, we can distinguish the colon diseases that have symptoms similar to Crohn's disease and help diagnose Crohn's disease effectively by defining the proposed process-oriented association as an ontology. Applying the proposed ontology to 5 cases showed that more accurate diagnosis was possible and in one case it could be diagnosed even with fewer tests.

  • The object of this study was to compare magnetic resonance imaging (MRI) findings and neurological signs in canine brain diseases. Brain diseases can cause severe neurological deficits and may be life-threatening. The antemortem diagnosis of the brain diseases is difficult for the clinician, since definitive diagnosis is based upon histopathological confirmation. Brain diseases are often associated with specific clinical signs, signalment, progression, and location. Accurate lesion localization through neurological examination and MRI findings is helpful for developing a differential diagnosis. A retrospective study was performed to compare the neurological examination of dogs with suspected brain disease to the MRI findings. Based on this study, neurological examination is a reliable way to localize most brain lesions. Postural reaction deficits do not provide sufficient information to localize lesions. Additionally, not all brain lesions present clinical signs and inflammatory lesions may cause no detectable abnormalities on MRI. Therefore, in clinical practice, a combination of neurological examination and MRI findings recommended for accurate brain lesion localization.

Etiology and clinical characteristics of fever of unknown origin in children: a 15-year experience in a single center

  • Purpose: Fever is one of the most common symptoms in children. In previous studies, infectious disease was the most common cause of pediatric fever of unknown origin (FUO). The aim of this study is to investigate the etiology, clinical characteristics and prognosis of pediatric FUO in 21 century with more diagnostics available and to analyze the factors for certain disease categories. Methods: Among the children under 18 years old who were hospitalized at Samsung Medical Center from January 2000 to December 2014, the patients who met the criteria including fever of ${\geq}38.0^{\circ}C$ for longer than ${\geq}14days$ and failure to reach a diagnosis after one week of investigations were included. Results: Total 100 patients were identified. Confirmed diagnosis was achieved in 57 patients (57%). Among them, infectious diseases (n=19, 19%) were most common, followed by connective tissue diseases (n=15, 15%), necrotizing lymphadenitis (n=8, 8%), and malignancies (n=7, 7%). Children with fever duration over 28 days had a trend for higher frequency of connective tissue diseases (28.3%) except undiagnosed etiology. The symptoms such as arthritis, lymph node enlargement and only fever without other symptoms were significantly related with connective tissue diseases, necrotizing lymphadenitis and undiagnosed respectively (P<0.001). Ninety-two patients have become afebrile at discharge and 1 patient died (1%). Conclusion: Almost half of our patients were left without diagnosis. Although it has been known that infectious disease was most common cause of pediatric FUO in the past, undiagnosed portion of FUO have now increased due to development of diagnostic techniques for infectious diseases.

Traumatic Tracheobronchial Injury: Delayed Diagnosis and Treatment Outcome

  • Background: Most traumatic tracheobronchial injuries are fatal and result in death. Some milder cases are not life threatening and are often missed at the initial presentation. Tracheobronchial rupture is difficult to diagnose in the evaluation of severe multiple trauma patients. We reviewed the traumatic tracheobronchial injuries at Konyang University and Eulji University Hospital and analyzed the clinical results. Materials and Methods: From January 2001 to December 2011, 23 consecutive cases of traumatic tracheobronchial injury after blunt trauma were reviewed retrospectively. We divided them into two groups by the time to diagnosis: group I was defined as the patients who were diagnosed within 48 hours from trauma and group II was the patients who diagnosed 48 hours after trauma. We compared the clinical parameters of the two groups. Results: There was no difference in the age and gender between the two groups. The most common cause was traffic accidents (56.5%). The Injury Severity Score (ISS) was 19.6 in group I and 27.5 in group II (p=0.06), respectively. Although the difference in the ISS was not statistically significant, group II tended toward more severe injuries than group I. Computed tomography was performed in 22 cases and tracheobronchial injury was diagnosed in 5 in group I and 6 in group II, respectively (p=0.09). Eighteen patients underwent surgical treatment and all four cases of lung resection were exclusively performed in group II (p=0.03). There were two mortality cases, and the cause of death was shock and sepsis. Conclusion: We believe that close clinical observation with suspicion and rigorous bronchoscopic evaluation are necessary to perform diagnosis earlier and preserve lung parenchyma in tracheobronchial injuries from blunt trauma.

Mortality Determinants in Colorectal Cancer Patients at Different Grades: a Prospective, Cohort Study in Iran

  • Background: Colorectal cancer (CRC) is an important cause of mortality and morbidity in many communities worldwide. This population based study was conducted to assess determinants of colorectal mortality in Iranian patients. Materials and Methods: A cohort of 1,127 cases of confirmed colorectal cancer registered in a population based registry covering 10 referral hospital in Tehran, Iran, were followed for five years. Information about tumor characteristics, smoking status and family history were collected at base line and survival status were followed every six months by contacting patient or next of kin (if patients died during the follow-up). The cause of death for each case was validated by verbal autopsy and referring to patient medical records at the time of death. The data were analyzed by Stata software using univariate and multivariate analysis (Cox regression). In building the model a p value of less than 5% was considered as significant. Results: The age at diagnosis was $53.5{\pm}14$ years. Sixty one percent were male. Colorectal mortality among the patients was 96.9 person-years among men and 83 person-years among women. Seventy five percent of patients lived for 2.72 years, 50% for 5.83, and 25% for 13 years after the diagnosis of colorectal cancer. The age at diagnosis was significantly different between men and women (p<0.03). Higher tumor grade predicted higher death rate; the adjusted hazard ratios were 1.79 (95%CI, 0.88-3.61), 2.16 (95%CI, 1.07-4.37), and 3.1 (95%CI, 1.51-6.34) for grades II, III, and IV respectively when they were compared with grade I as reference. Ethnicity, marital status, family history of cancer, and smoking were related to survival with different degrees of magnitude. Conclusions: Among many factors related to survival among the colorectal patients, tumor grade and smoking showed the highest magnitudes of association.

  • In the Oriental Western Consideration of Habitual Abortion, the result of this study is summarized as follows: 1. Habitual abortion means repetition of Tatae(abortion within 3 months), Sosan(abortion after 3 months), and Bansan(abortion after 5 months), as it agrees with Hoaltae, Sutatae and Nuing-Nuta in oriental medical science. 2. Frequency in Occurrence of habitual abortion is about 0.2-0.4%> in the whole pregnancy and the Ratio of Risk increases according to frequency and age increase. 3. Generally, the cause of habitual abortion is due to the cause of the mother. Therefore, diagnosis and treatment of before-childbirth are needed, and 'measures taken to prevent abortion' is in use to improve the condition of health of before-childbirth in oriental medical science. 4. The 50-60% of early abortion is due to the heterochromosome and the ratio of risk of habirual abortion is hightest in heterochromosome. 5. The causes of habitual abortion are summarized as vital energe and blood weakness. impairment of Chong and Ren, aflection by exopathogen, fever caused by blood deficiency. weakness of the spleen and the stomach, excess of seven emotion. excess of a sexual desire and injury of a contusion and also the treatments are summarized as invigorating qi (vital energy) and enriching the blood, reducing fever and enriching the blood, reinforcing the spleen, tonifying the Chong and Ren, the practice of a sceticism, psychological peace in oriental medical science. 6. The approch of modem oriental medical science is based on diagnosis and treatment based on 'over all analysis of symptoms and signs' of traditional oriental medical science. it goes abreast with diagnosis of western medical science, and it can be expected mare inclusive effect of treatment because 'Acupuncture for prevent abortion', 'Acupuncture with the needle warmed by burning moxa', and so on being developed except medicinal therapy.

Pathophysiology and Treatment of Gout Arthritis; including Gout Arthritis of Hip Joint: A Literature Review

  • Gout is triggered by the accumulation of uric acid in the body, leading to hyperuricemia. Genetic, metabolic, and environmental factors can influence this condition. Excessive uric acid buildup results in the formation of monosodium urate (MSU) crystals, which precipitate in specific areas of the body, including the joints, where they can cause symptoms of gout. While the acute and chronic symptoms of gout have been well-documented, diagnosis of gout affecting the hip joint poses significant challenges. The global incidence of gout, the most prevalent form of inflammatory arthritis, is on the rise. Evaluation of the clinical signs, laboratory results, and imaging results is generally required for diagnosis of gout in cases where MSU crystals have not been detected. Hyperuricemia is considered a primary cause of arthritis symptoms, and comprehensive guidelines for treatment are available. Therefore, the choice of medication is straightforward, and moderate effectiveness of treatment has been demonstrated. Gout is a chronic disease, requiring lifelong uric acid-lowering medications, thus application of a treatment strategy based on the target blood uric acid concentration is necessary. Consequently, cases of gout will likely be observed more frequently by hip surgeons in clinical scenarios in the future. The objective of this review is to provide an overview of the pathophysiology of gout and subsequently examine recent advances in diagnostic methods and therapeutic agents based on an understanding of its underlying mechanisms. In addition, literature on gout-related issues affecting the hip joint, providing a useful reference for hip surgeons is examined.

Acute upper limb ischemia in a patient with newly diagnosed paroxysmal atrial fibrillation

  • Acute limb ischemia (ALI) due to an embolism is associated with high mortality rate and poor prognosis, and early diagnosis with prompt revascularization is required to reduce the risk of limb amputation or even death. The etiologies of ALI are diverse, and it includes an embolism from the heart and thrombotic occlusion of the atherosclerotic native vessels, stents, or grafts. An uncommon cause of ALI is acute arterial thromboembolism, and atrial fibrillation (AF) is the single most important risk factors for systemic thromboembolism. It is important to correctly identify the source of ALI for secondary prevention, as it depends on the underlying cause. Percutaneous transluminal angioplasty (PTA) has been proven to be a safe and effective treatment for focal atherosclerotic and thrombotic occlusive diseases of the aorta and its major extremity branches. Herein, we report on a 77-year-old female patient with acute upper limb ischemia, treated by PTA using a catheter-guided thrombectomy. He was newly diagnosed with paroxysmal AF (PAF) while evaluation the cause of his acute arterial thromboembolism. We recommend that cardiologists always consider PAF as a possible diagnosis even in patients without any history of AF under ALI because it is possible to develop thromboembolism in clinical practice.

Cause Diagnosis and Reduction Measures of Foaming in the Treated Wastewater Outlet of D Wastewater Treatment Plant (하수처리수 방류구의 거품발생 원인진단 및 저감방안: 국내 하수처리장 사례를 중심으로)

  • This study was conducted to suggest the cause analysis and mitigation measures of foaming generated in the effluent of wastewater treatment plant. The foam generated in the outlet connected with the tidal river system was identified as structural problems. And the main cause of foaming was air entrainment by an impinging jet and the internal accumulation by the diffusion barrier. In consideration of these conditions, it present the effective ways such as micro-screen and submerged outlet, to mitigate the foaming generated in the water channel and outlet end.
