신생아 패혈증의 원인 병원체에 대한 조사 (Causative Organisms of Neonatal Sepsis)

  • 전국의 신생아 수련 병원에 설문지를 보내어 각 병원의 1997년 1월부터 12월까지 출생한 신생아 중 패혈증으로 진단되고 혈액 배양 검사에서 균이 검출된 경우, 병록지를 통해 후향적 조사를 실시하였고, 37개 병원에서 참여해 34.9%의 응답율을 나타내었으며, 결과는 다음과 같다. 1) 신생아 패혈증의 발생 빈도는 평균 0.73%(0~2.95%)였고, 남녀 비는 1.15:1이었다. 2) 신생아 패혈증의 원인균은 candida류를 포함하여 64종의 균이 검출되었다. 가장 흔한 원인균은 S. aureus였고, S. epidermidis, CONS의 순으로 많았다. 3) 조기 신생아 패혈증의 가장 흔한 원인균은 S. aureus였으며 CONS와 S. epidermidis가 그 다음으로 많았다. 후기 신생아 패혈증도 S. aureus가 가장 많이 검출되었고. S. epidermidis, CONS의 순으로 많았다. 4) 신생아 패혈증에 관련된 산모의 위험인자는 조기 파수가 104명(21.8%)으로 가장 많았고, 그 외에 난산, 태아의 빈맥, 융모양막염, 산모감염의 순이었다. 5) 신생아 패혈증과 연관된 국소 감염의 분포는 73명(26.1%)의 환아에서 폐렴이 동반되어 가장 많은 빈도를 보였고, 요로 감염, 뇌막염, 관절염의 순으로 많았다. 6) 신생아의 병원내 감염의 가장 흔한 원인균으로는 S. spidermidis가 가장 많았고, S. aureus, Entero-bacter의 순이었다. 7) 도관과 관련된 신생아 패혈증의 원인균은, S. aureus가 가장 많았고, S. epidermidis, Enterobacter의 순으로 많았다. Candida는 8례에서 발견되었다. 8) 그람 양생균의 항생제에 대한 반응검사에서 Penicillin이나 oxacillin에 대해 S. aureus, S. epidermidis, CONS 모두 30% 이하의 감수성을 보였다. Vacomycin에 대해서는 S. aureus, CONS, Enterococcus 각각 1례에서 내성을 나타내었다. 9) 그람 음성균의 항생제에 대한 반응검사에서 Gentamicin에 대해서 60% 정도의 감수성을 나타내었고, amikacin에 대해서는 80% 이상의 감수성을 보였다. Piperacillin과 aztreonam에 대해서, Pseudomonas가 100%의 감수성, ticarcillin에 대해서는 Klebsiella가 100%의 감수성을 나타내었다.

돼지위축성비염과 가금티푸스 병원균을 저해하는 Bacillus amyloliquefaciens TK3의 분리 및 특성 조사 (Isolation and characterization of Bacillus amyloliquefaciens TK3 inhibiting causative bacteria of atrophic rhinitis and fowl typhoid)

  • Bordetella bronchiseptica에 의한 돼지위축성비염과 Salmonella gallinarum에 의한 가금티푸스 방지 방법을 찾기 위해서 두 세균병원체에 항균활성을 갖는 세균 균주를 축산농가의 여러 시료로부터 분리하였다. 분리균주 372개 중 TK3 균주가 두 병원체에 대해 가장 우수한 항균활성을 나타내었으며, 16S rRNA 염기서열 분석 결과 Bacillus amyloliquefaciens로 동정되었다. B. amyloliquefaciens TK3는 siderophore, rhamnolipid와 antimicrobial peptide 같은 항균물질을 분비하여 두 병원체의 생장을 저해할 수 있었다. TK3 균주는 Chrome azurol S 한천 평판에서 siderophore 생성반지름이 14일 배양 후 0.53 cm이었으며, King's B 배지에서 1.06 mmol/ml의 siderophore를 생성하였다. 또한 82.4 mg/L의 rhamnolipid를 분비하였으며, LB배지에 $30{\mu}l/ml$ 농도의 추출된 antimicrobial peptide 첨가 시 두 병원체의 생장을 완전히 저해하였다.

극소저출생체중아에서 신생아 패혈증의 원인균 및 항생제 감수성의 변화 (Changes in Pathogens and Antibiotic Sensitivities in very Low Birth Weight Infants with Neonatal Sepsis)

  • 목 적 : 신생아 패혈증의 원인균 및 항생제 감수성의 변화를 조사하여 극소 저출생 체중아 및 미숙아의 이환율 및 사망률 증가와 관련된 패혈증에 대하여 가능한 신속하고 적절한 치료 방침을 세우는데 도움을 주고자 하였다. 방 법 : 2000년 1월부터 2006년 6월까지 을지대학병원 신생아집중치료실에 입원한 출생체중 1,500 g 미만의 극소 저출생 체중아를 대상으로 의무기록을 후향적으로 분석하여 패혈증의 원인균과 항생제 감수성에 대해 조사하여 시기별 분포를 비교하였다. 결 과 : 대상 환아 164명 중 균이 증명된 환아는 19명(11.6%)이었다. 균이 증명된 19명의 환아에서 총 26례의 패혈증 발생을 보였으며, 후기 지발형 패혈증의 발생빈도가 제일 높았다. K. pneumoniae가 가장 흔한 원인균이였으며, 그 뒤로 Streptococcus spp., CNS, Enterobacter spp.의 순으로 배양되었고, 대부분이 경험적 항균제에 내성을 나타내었다. 결 론 : 과거 연구나 타 의료기관에서 증명된 흔한 원인균과 항생제 감수성 결과에 의존하여 광범위 경험적 항생제를 사용하기보다는 주기적으로 원인균 및 항균제 감수성의 변화를 파악하고 적절한 항생제를 사용하는 것이 신생아 패혈증의 치료의 효과뿐만 아니라, 다제내성균의 출현을 막을 수 있을 것으로 사료된다.

  • The protozoan parasite Giardia lamblia has been implicated as the causative agents of many outbreaks of waterborne intestinal illness. Accurate evaluation of Giardia lamblia removal in water treatment process requires a reliable method for measuring the concentrations of these pathogens in water. The relative recovery of Giardia cysts was assessed for seeded samples of distilled water. Cysts preparation was done by encystment in vitro. Membrane filtration was evaulated with cellulose acetate, polycarbonate, polypropylene, polyethersulfone, nylon membranes. Elution conditions were varied to improve cyst recovery.

Factors Influencing Ventilator-Associated Pneumonia in Cancer Patients

  • Background: With increasing survival periods and diversification of treatment methods, treatment of critically ill cancer patients has become an important factor influencing patient prognosis. Patients with cancer are at high risk of infections and subsequent complications. This study investigated the incidence and factors contributing to the development of ventilator-associated pneumonia (VAP). Materials and Methods: This retrospective study investigated the incidence of VAP and factors leading to infection in patients admitted to the intensive care unit (ICU) of a cancer center from January 1, 2012 to December 31, 2013. Results: The incidence of VAP was 2.13 cases per 1,000 days of intubation, and 13 of 288 patients (4.5%) developed VAP. Lung cancer was the most common cancer associated with VAP (N=7, 53.9%), and longer hospital stays and intubation were associated with increased VAP incidence. In the group using a "ventilator bundle," the incidence was 1.14 cases per 1,000 days compared to 2.89 cases per 1,000 days without its use; however, this difference was not statistically significant (p=0.158). Age (${\geq}65$, OR=5.56, 95% confidence interval [CI]=1.29-23.95), surgery (OR=3.78, 95%CI=1.05-13.78), and tracheotomy (OR=4.46, 95%CI=1.00-19.85) were significant VAP risk factors. The most common causative organisms were methicillin-resistant Staphylococcus aureus (MRSA) and Pseudomonas aeruginosa (N=4, 30.8% each), followed by Acinetobacter baumannii and Candida albicans (N=2, 15.4% each). Conclusions: The incidence of pneumonia among critically ill cancer patients is highest in those with lung cancer, but lower than among non-cancer patients. The length of hospital stay and time on mechanical ventilation are important risk factors for development of VAP. Although not statistically significant, "ventilator bundle" care is an effective intervention that delays or reduces incidence of VAP. Major risk factors for VAP include age (${\geq}65$ years), surgery, and tracheostomy, while fungi, gram-negative bacteria, and multidrug-resistant organisms were identified as the major causative pathogens of VAP in this study.

Isolation and identification of insect pathogenic fungus from silkworms with suspected white muscardine disease

  • The value of silkworms as functional health food materials has increased, as has the interest in its disease control for stable production, and in the economic value of entomopathogenic microorganisms. In this study, we isolated and identified disease-causing fungi from white muscardine silkworms, and confirmed whether this strain could produce white muscardine silkworms. For the analysis of the cause of white muscardine disease in the infected silkworms, the fungi and prokaryotes causing the disease were identified, isolated, and identified using metagenome analysis. Metagenomic analysis detected a large amount of the fungus Metarhizium rileyi in silkworms, and a large amount of the bacterium Enterococcus mundtii, which was presumed to be the causative agent of the disease. For accurate identification of the fungi, these were purified by culture medium, and sequencing and phylogenetic tree analyses were performed using an internal transcribed spacer. As a result, M. rileyi, Cladosporium cladosporioides, and C. tenuissimum were identified. In general, M. rileyi is known to form green conidia, but in this study, white-yellow conidia were formed, indicating that the exact causative agent of the fungal disease cannot be estimated by diagnosing the symptoms. Thus, a diagnostic method is necessary for the continuously collection of required pathogens, and identifying their morphological and genetic characteristics.

A revaluation of algal diseases in Korean Pyropia (Porphyra) sea farms and their economic impact

  • As with land crops, cultivated algae are affected by various diseases ranging from large outbreaks of a disease to chronic epiphytes, which may downgrade the value of the final product. The recent development of intensive and dense mariculture practices has enabled some new diseases to spread much faster than before. A new disease is reported almost every year, and the impact of diseases is expected to increase with environmental change, such as global warming. We observed the incidence of diseases in two Pyropia sea farms in Korea from 2011 to 2014, and estimated the economic loss caused by each disease. Serious damage is caused by the oomycete pathogens, Pythium porphyrae and Olpidiopsis spp., which decreased the productivity of the Pyropia sea farms. In Seocheon sea farms, an outbreak of Olpidiopsis spp. disease resulted in approximately US $1.6 million in loss, representing approximately 24.5% of total sales during the 2012-2013 season. The damage caused by green-spot disease was almost as serious as oomycete diseases. An outbreak of green-spot disease in the Seocheon sea farms resulted in approximately US $1.1 million in loss, representing 10.7% of total sales in the 2013-2014 season in this area. However, the causative agent of green-spot disease is still not confirmed. "Diatom felt" is regarded as a minor nuisance that does not cause serious damage in Pyropia; however, our case study showed that the economic loss caused by "diatom felt" might be as serious as that of oomycete diseases. Bacteria and cyanobacteria are indigenous members of epiphytic microbial community on Pyropia blades, but can become opportunistic pathogens under suitable environmental conditions, especially when Pyropia suffers from other diseases. A regular acid wash of the Pyropia cultivation nets is the most common treatment for all of the above mentioned diseases, and represents approximately 30% of the total cost in Pyropia sea farming. However, the acid wash is ineffective for some diseases, especially for Olpidiopsis and bacterial diseases.

Clinical and laboratory profiles of hospitalized children with acute respiratory virus infection

  • Purpose: Despite the availability of molecular methods, identification of the causative virus in children with acute respiratory infections (ARIs) has proven difficult as the same viruses are often detected in asymptomatic children. Methods: Multiplex reverse transcription polymerase chain reaction assays were performed to detect 15 common respiratory viruses in children under 15 years of age who were hospitalized with ARI between January 2013 and December 2015. Viral epidemiology and clinical profiles of single virus infections were evaluated. Results: Of 3,505 patients, viruses were identified in 2,424 (69.1%), with the assay revealing a single virus in 1,747 cases (49.8%). While major pathogens in single virus-positive cases differed according to age, human rhinovirus (hRV) was common in patients of all ages. Respiratory syncytial virus (RSV), influenza virus (IF), and human metapneumovirus (hMPV) were found to be seasonal pathogens, appearing from fall through winter and spring, whereas hRV and adenovirus (AdV) were detected in every season. Patients with ARIs caused by RSV and hRV were frequently afebrile and more commonly had wheezing compared with patients with other viral ARIs. Neutrophil-dominant inflammation was observed in ARIs caused by IF, AdV, and hRV, whereas lymphocyte-dominant inflammation was observed with RSV A, parainfluenza virus, and hMPV. Monocytosis was common with RSV and AdV, whereas eosinophilia was observed with hRV. Conclusion: In combination with viral identification, recognition of virus-specific clinical and laboratory patterns will expand our understanding of the epidemiology of viral ARIs and help us to establish more efficient therapeutic and preventive strategies.

남원지역 도축돈에 대한 돼지호흡기 복합감염증에 관한 연구 (Study on porcine respiratory disease complex from slaughtered pigs in Namwon, Korea)

  • Porcine respiratory disease complex (PRDC) continues to be a significant economic problem to the swine industry. In order to elucidate the etiology of PRDC including porcine circovirus type 2 (PCV2), porcine reproductive and respiratory disease syndrome virus (PRRSV), swine influenza virus (SIV), Mycoplasma hyopneumoniae (MH), Pasteurella multocida (PM) and Actinobacillus pleuropneumoniae (APP) in Namwon, the 455 lung samples were randomly collected from slaughtered pigs, examined gross lesions indicative of respiratory disease of lung and classified the lung lesion according to the severity of lung lesions. Two hundred pigs lung tissues with pneumonic lesions were examined for pathogen by PCR. As a result, the numbers of pneumonic lesions were 357 (78.5%), mean pneumonic score ($mean{\pm}SD$) was $2.03{\pm}0.90$ and the highest gross lesion according to stages was 1 (11~20%). In detection of pathogens, PCV2, PRRSV, SIV, MH, APP and PM were positive in 76.5%, 5.0%, 6.0%, 9.0%, 4.5% and 6.0%, respectively and PCV2-MH was the most detected causative pathogens of PRDC in co-infection. In the serological test for PRRSV, PCV2, MH, APP2, APP5, HP and PM, showed high antibody positive rates 93% or more.

국내 시판용 Frozen Yogurt의 병원성 미생물 검출 및 미생물학적 품질 평가에 관한 연구 (Detection of E. coli O157:H7 and Listeria monocytogenes, and Appraisal for Microbiological Qualities in the Commercial Frozen Yogurt Products in Korea)

  • Recently the high outbreaks of intestinal disease caused by the consumption of frozen dairy foods containing pathogenic bacteria has generated considerable interest in the causative agent such as Listeria monocytogenes and E. coli O157:H7. This study was carried out to detect the above pathogens and compare the microbiological qualities of three commercial forzen yogurt products. The main results obtained were as follows. L. monocytogenes coliforms and E. coli O157:H7 were not detected in the total of seven frozen yogurt samples. For microbiological qualities the viable lactic counts of products manufactured by FA company were about 2.9$\times$108 -1.6$\times$109cfu/ml 1.7$\times$106 cfu/ml for FB's and 1.2$\times$106 cfu/ml for FC's The PH values of FA's FB's and FC's products was in the range of pH 4.1~5.3 and the values of FA's were 4.1~4.6 compared by the pH 5.2~5.3 of FB's and FC's products. During refrigeration of the test samples the survival rates of L. monocytogenes spiked into thawed frozen yogurt sample(FA's FB,s and FC's) were 0.55% 15.61% and 16.89% respectively. On the other hand E. coli O157:H7 and L. monocytogenes were 12.4% and 25.0% for FA's 10.8% and 20.8% for FB's and 10.26% and 22.7% for FC's under 37$^{\circ}C$ storage, As the results described above each frozen yogurt products were different in microbiological qualities. The survival rates of pathogens spiked into the samples increased with the pH of the products. This indicates that the pH or any other factors pre-sumable supressed the growth of E. coli O157:H7 and L. monocytogenes in frozen yogurt products.
