Sacral Meningeal Cyst Detected during Caudal Epidural Block (미추 경막외차단 도중 발견된 천추 수막낭 -증례 보고-)

  • Sacral meningeal cyst is usually asymtomatic, but may be responsible for sciatic pain syndromes and other clinical symptoms. Sacral meningeal cyst might be suspected when definite explanation for the clinical symptom, such as herniation of the intervertebral disc or spinal stenosis is not found. Plain films and CT may suggest the presence of sacral meningeal cyst, but MR is the current imaging study of choice. Evaluation of the correlation between the symptom and the cyst is as important as detection of it. We have experienced a case of sacral meningeal cyst detected during caudal epidural block. The patient complained of low back pain radiating to thigh. Plain films and lumbar spine CT showed no remarkable finding except disc bulging. During caudal epidural needle insertion, there was leakage of clear CSF, and intrasacral cystic shadow was visualized by dye injection. MR confirmed sacral meningeal cyst.

Postoperative Analgesia by Caudal Nalbuphine HCL (Nalbuphine 천골강내 주입에 의한 술후 제통효과)

  • Epidural injection of narcotics for postoperative pain relief has been well reported. Caudal nalbuphine was assessed as a postoperative analgesic in a randomized double blind study of 80 patients after perianal surgery. Caudal block was carried out with 1.5% lidocaine 25 ml (Group 1) in 20 patients, and mixed with nalbuphine 3 mg (Group 2) in 20 patients, nalbuphine 5 mg (Group 3) in 20 patients, and nalbuphine 10 mg (Group 4) in 20 patients. Pain relief was evaluated by the subsquent need for systemic analgesics (Pethidine). In group 4, the use of systemic analgesics was significantly reduced for the first 24 hours postoperatively. Urinary retention was not correlated with nalbuphin dose.

Studies on the Fish Kills by Histopathological Characteristics in Gills and Caudal Fins (아가미 혈종과 지느러미 표피탈락 현상을 이용한 어류 폐사원인 연구)

  • Histopathological changes of gills and caudal fins isolated from fishes, Cyprinus carpio, Carassius auratus, and Hernibarbus labeo, which were killed by oxygen deficiency or toxic chemicals, were studied. The toxic chemicals were HCl, NaOH, chloroform, benzene, heavy metals(Cu, Cr, Zn, Pb, Hg), and o-dichlorobenzene. The exposure level was enough to kill the fishes within 30 minutes. Oxygen deficient water was prepared by aeration of nitrogen gas and the oxygen concentration was less than l ppm. Cryocutting was used for the rapid preparation of tissue slides and the tissues were stained by hematoxylin/eosin. In the fishes killed by hazardous chemicals, congestion and/or hyperplasia of secondary lamella and erosion of fin were found as the major histopathological changes. Whereas, these characteristics were not observed in gills or caudal fins of fishes killed by oxygen deficiency. These different bioindications appeared in the fishes killed by toxic substances or natural causes, can be used for the rapid identification of the causes of fish kills.

Exposing Zebrafish to Silver Nanoparticles during Caudal Fin Regeneration Disrupts Caudal Fin Growth and p53 Signaling

  • Zebrafish were exposed to commercial silver nanoparticles (${\sim}$10-20 nm) at 0.4 and 4 ppm during cadual fin regeneration. The silver was in the $Ag^+$ ionic form. Fin regeneration was slow in the group exposed to the lower concentration. The cadual fin, gill, and muscle were assayed after 48 hours and subjected to histological analysis. In all tissues sampled, fish exposed to nanoparticles exhibited infiltration, large mitochondria with empty matrices, and accumulation of nano-sized silver in blood vessels. The results suggested mitochondrial damage and induction of inflammation. Microarray analysis of RNA from young zebrafish (52 hours post-fertilization) that were exposed to nanometer-sized silver particles, showed alteration in expression of the p53 gene pathway related to apoptosis. Gene expression changes in the nanoparticle-treated zebrafish led to phenotypic changes, reflecting increased apoptosis.

Experimental study on vortex induced vibration of risers with fairing considering wake interference

  • Vortex Induced Vibration (VIV) is a typical flow-structure interference phenomenon which causes an unsteady flow pattern due to vortex shedding at or near the structure's natural frequency leading to resonant vibrations. VIV may cause premature fatigue failure of marine risers and pipelines. A test model was carried out to investigate the role of a stationary fairing by varying the caudal horn angle to suppress riser VIV taking into account the effect of wake interference. The test results show significant reduction of VIV for risers disposed in tandem and side-by-side. In general, fairing with a caudal horn of $45^{\circ}$ and $60^{\circ}$ are efficient in quelling VIV in risers. The results also reveal fairing can reduce the drag load of risers arranged side-by-side. For the tandem configuration, a fairing can reduce the drag load of an upstream riser, but will enlarge the drag force of the downstream riser.

Caudal and epidural blocks in infants and small children: historical perspective and ultrasound-guided approaches

  • In infants and small children, ultrasound (US) guidance provides ample anatomical information to perform neuraxial blocks. We can measure the distance from the skin to the epidural space in the US image and can refer to it during needle insertion. We may also visualize the needle or a catheter during real-time US-guided epidural catheterization. In cases where direct needle or catheter visualization is difficult, US allows predicting successful puncture and catheterization using surrogate markers, such as dura mater displacement, epidural space widening due to drug injection, or mass movement of the drug within the caudal space. Although many experienced anesthesiologists still prefer to use conventional techniques, prospective randomized controlled trials using US guidance are providing increasing evidence of its advantages. The use of US-guided regional block will gradually become widespread in infants and children.

Overview of Secondary Neurulation

  • Secondary neurulation is a morphological process described since the second half of the 19th century; it accounts for the formation of the caudal spinal cord in mammals including humans. A similar process takes place in birds. This form of neurulation is caused by the growth of the tail bud region, the most caudal axial region of the embryo. Experimental work in different animal species leads to questioning dogmas widely disseminated in the medical literature. Thus, it is clearly established that the tail bud is not a mass of undifferentiated pluripotent cells but is made up of a juxtaposition of territories whose fate is different. The lumens of the two tubes generated by the two modes of neurulation are continuous. There seem to be multiple cavities in the human embryo, but discrepancies exist according to the authors. Finally, the tissues that generate the secondary neural tube are initially located in the most superficial layer of the embryo. These cells must undergo internalization to generate the secondary neurectoderm. A defect in internalization could lead to an open neural tube defect that contradicts the dogma that a secondary neurulation defect is closed by definition.

Osteological Development of Korean Striped Bitterling, Acheilognathus yamatsutae (Cyprinidae) (줄납자루, Acheilognathus yamatsutae (잉어과)의 골격 발달)

  • Osteolgical development of Korean striped bitterling, Acheilognathus yamatsutae (Cyprinidae), were investigated using cartilage and bone staining. The parasphenoid, dentary, pterygoid, ceratohyal, branchial arch and parts of the caudal skeleton were formed first as cartilaginous elements at $6.82{\pm}0.08mm$ total length (TL). Formation of frontal, parietal, and maxillary were notable at $7.76{\pm}0.09mm$ TL, and the proximal radial, scapula,and coracoid in the pectoral girdle were developed at this stage. At the same time, the neural and hemal spine were present in the caudal vertebra. The opercle and branchiostegals were observed at $9.68{\pm}0,14mm$, and the posttemporal was formed at $12.9{\pm}40.64mm$ TL, respectively. Ossification in the parasphenoid, pharyngeal bone, dentary, premaxillary, maxillary, and opercle began at about $9.68{\pm}0.14mm$ TL. The cleithrum, supracleithrum, urostyle, and caudal fin were calcified at this stage. The vertebral column was formed and ossified at $11.52{\pm}0.13mm$ TL, and the frontal, preopercle, subopercle, and hyomandibular were ossified at $15.30{\pm}0.68mm$ TL. This fish was late in developing the skeletal formation and ossification as compared with other fishes.

Morphological Characteristics of Three Species, Genus Chaenogobius(Gobiidae), from Korea (한국산 꾹저구속(망둑어과) 3종에 대한 형태적 특징)

  • The morphological characteristics between male and female of three species, Chaenogobius urotaenia, C. sp. MR, C. sp. BW, collected from June 1996 to Aug. 1999, Korea was studied. Of 13 characters to be studied, 12 characters with exception of caudal fin ray length were significantly different among three species and these three species were distinguished each other by the results of the multiple comparison test (REGWQ test). Interspecific differences in female was distinct in body depth, head length, caudal peduncle length, caudal peduncle depth, snout length, pectoral fin ray length; In particular, C. sp. MR was shorter in upper jaw length, anal fin ray length and eye diameter, and to be smaller in head depth and head width than other two species. Chaenogobius urotaenia was longer in pelvic fin ray length. In case of male, there was not characters to distinguish each one among three species; Chaenogobius urotaenia was longer in head length, eye diameter and pelvic fin ray length than other two species, C. sp. MR to be longer in caudal peduncle length, but shorter in snout length and anal fin ray length, and to be lower in head depth. C. sp. BW was higher in body depth and caudal peduncle depth, and to be longer in upper jaw length and pectoral fin ray length.
