Percutaneous Insertion of the Distal Catheter during Ventriculo-Atrial Shunts. A Simple and Reliable Method

  • Although the ventriculo-peritoneal shunt is the treatment of choice when hydrocephalus should be corrected, the ventriculo-atrial shunt still holds strong alternative when the peritonium is precluded due to the several reasons. During the ventriculo-atrial shunt operation, it is not always easy to dissect and find the corresponding venous structures. In this technical note, the author describes a simple method of percutaneous insertion for placement of the atrial end.

소아의 중심정맥로 설치술 (Central Venous Access in Children)

  • To evaluate the effectiveness of central venous catheters(CVCs) in children, 320 CVCs placed in 255 neonate and children over a 10-year period were analyzed retrospectively. CVC was placed by one pediatric surgeon for a total of 6, 116 patients days. Catheters were placed preoperatively for TPN or chemo in 223 cases. CVC was solely for TPN in 57 cases and for chemotherapy in 40. Local anesthesia was utilized in 71 cases, and the general anesthesia was administered in the remainder of the patients. The subclavian vein was catheterized(SCV) in 202 cases(82 infants and neonates), tunneled external jugular venotomy(EJV) was utilized in 38, tunneled internal jugular venotomy(UV) in 2, the facial venotomy(FV) was used in 3, and the umbilical vein was catheterized UVC) with vein transposition in 74 infants. In neonates, 72 UVCs were placed during laparotomy. SCV was increased with ages, from 3 kg of minimal body weight. The average catheter-periods over-all were 19.1 days, SCV 17 days, EJV 40, IJV 60 and UVC 14. Technical complications were; arterial puncture(6), puncture failure(5) and abnormal location(12) in SCV; insertion failure(3) in EJV; abnormal location in the portal vein(4) and the liver parenchyma(2) cystic fluid accumulations in UVC. Twelve migrations(3.8 %) out of position occurred; SCV(2), EJV(1) and UVC(9). There were 4 cases(1.2 %) of catheter obstruction and 11(3.4 %) of catheter infection(3 SCV, 2 EJV and 6 UVC). Rescue procedures were utilized with some success. There was one mortality(0.3 %) due to deep sedation in a 1.06 kg baby during placement of an EJV. The surgeon's experience, proper catheter selection and following safety rules are the most important factors for successful CVCs.

소아에서 복막투석도관 삽입시 복강경을 이용한 방법과 기존의 수술법에 따른 초기 합병증 발생의 차이 (Comparison of Early Complications after Peritoneal Dialysis Catheter Implantation by Laparoscopic Surgery and Conventional Surgery in Children)

  • 목적 : 복강경을 이용한 복막투석도관 삽입술은 직접 복강 내를 들여다보며 시술할 수 있어 위치 선정이 쉽고, 조직 손상이 적으며 합병증이 적다는 장점이 있으나, 소아에서의 경험은 많지 않다. 본 연구에서는 소아에서 복강경을 이용한 복막투석도관 삽입술의 유용성을 평가하고자 한다. 방법 : 2002년부터 2006년까지 삼성서울 병원에서 31명의 환아를 대상으로 복강경을 이용하여 복막투석도관 삽입술을 시행한 21례와 고식적 수술 방법으로 투석도관을 삽입한 16례의 의무기록을 바탕으로 복막투석관 삽입 후 첫 2개월간 투석도관의 누출, 폐쇄, 삽입 부위의 감염, 위치 이동 등 복막투석관 삽입과 관련된 합병증에 대해 후향적 분석을 시행하였다. 결과 : 고식적 수술을 받은 군(16례)에서 1례(6.3%)에서 수술 직후 심한 출혈로 재수술을 받았고, 복막액의 누출은 2례(12.5%) 있었으며, 보존적인 치료로 호전되었다. 삽입 부위 감염은 없었으나, 2례(12.5%)에서 복막염이 발생하였다. 투석도관의 이동으로 인한 기능부전이 3례 있었고 이중 2례에서 재수술을 받았으며, 1례는 보존적인 치료로 호전되었다. 복강경 수술을 받은 군(21례)에서 2례(9.5%)에서 투석액의 누출이 있었고, 보존적인 치료로 호전되었다. 삽입 부위의 감염은 없었고, 복막염이 2례(9.5%)에서 있었다. 1례에서 투석도관의 막힘이 있었고, 이 경우는 기존의 고식적 수술로 투석관 삽입 후 투석관 이동으로 재 삽입 후 불응성 복막염으로 투석관을 제거한 뒤 복강경 수술을 받은 증례였다. 투석도관의 이동은 없었다. 두 군의 합병증의 발생빈도는 통계적으로 유의한 차이는 없었다. 결론 : 본 연구에서 소아에서 복강경적 투석도관 삽입술을 시행한 경우, 1세 미만의 소아에서도 시술이 가능하였으며, 기존의 수술법에 비해 합병증이 증가하지 않았다. 또한 절개범위를 더 넓히지 않으면서도 장간막 절제 및 투석도관의 고정이 가능하여, 이를 통해 추후 투석도관의 이동이나 폐쇄의 위험요소를 줄일 수 있다는 장점이 있었다. 따라서 소아에서 복막투석도관 수술 시 복강경적 방법을 이용하는 것이 효율적인 복막 투석을 위해 유용하다고 생각된다.

Accuracy and Safety of Bedside External Ventricular Drain Placement at Two Different Cranial Sites : Kocher's Point versus Forehead

  • Objective : External ventricular drain (EVD) is commonly performed with a freehand technique using surface anatomical landmarks at two different cranial sites, Kocher's point and the forehead. The aim of this study was to evaluate and compare the accuracy and safety of these percutaneous ventriculostomies. Methods : A retrospectively review of medical records and head computed tomography scans were examined in 227 patients who underwent 250 freehand pass ventriculostomy catheter placements using two different methods at two institutions, between 2003 and 2009. Eighty-one patients underwent 101 ventriculostomies using Kocher's point (group 1), whereas 146 patients underwent 149 forehead ventriculostomies (group 2). Results : In group 1, the catheter tip was optimally placed in either the ipsilateral frontal horn or the third ventricle, through the foramen of Monro (grade 1) in 82 (81.1%) procedures, in the contralateral lateral ventricle (grade 2) in 4 (3.9%), and into eloquent structures or non-target cerebrospinal space (grade 3) in 15 (14.8%). Intracerebral hemorrhage (ICH) >1 mL developed in 5 (5.0%) procedures. Significantly higher incidences of optimal catheter placements were observed in group 2. ICH>1 mL developed in 11 (7.4 %) procedures in group 2, showing no significant difference between groups. In addition, the mean interval from the EVD to ventriculoperitoneal shunt was shorter in group 2 than in group 1, and the incidence of EVD-related infection was decreased in group 2. Conclusion : Accurate and safe ventriculostomies were achieved using both cranial sites, Kocher's point and the forehead. However, the forehead ventriculostomies provided more accurate ventricular punctures.

  • 중심정맥카테터 삽입 후 생길 수 있는 드문 합병증 중에 하나인 흉수 발생은 카테터와 관련된 혈관벽의 손상을 흉수 발생의 기전으로 설명하고 있다. 저자들은 우측 쇄골하정맥으로 이어지는 이상혈관 내로 카테터가 위치한 후 혈관 파열로 인하여 발생한 흉수 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • It is important to treat cancer-related pain in cancer patients to ensure the life quality of the patient, as well as to improve their life span. It has been estimated that at least 5% of cancer patients have pain refractory to medical treatment. Therefore, the need for epidural or intrathecal analgesia with opioids and local anesthetics is indicated if systemic treatment has failed. Intrathecal catheter placement and implantation of the injection port for administration of opioids and local anesthetics may improve pain relief in patients who are unresponsive to epidural routes. Although intrathecal implantation has several complications, similar infection rates have been reported between intrathecal and epidural administration. In addition, intrathecal administration showed better outcomes, including improved pain control, lowered daily doses, and an improvement in the level of drowsiness experienced when compared to epidural administration. We report here a case in which a terminal cancer patient was treated using an intrathecal catheter and subcutaneous port. The patient had cancer-related pain that could not be controlled by epidural opioid administration. Based on the results presented here, we suggest that intrathecal implantation is a feasible long term pain management method for intractable cancer pain patients.

암성 통증 관리에 사용된 부가적 진통제로서의 Propofol -증례 보고- (Propofol as an Adjuvant in the Treatment of Cancer-Related Pain -A case report-)

  • Cancer is a devastating disease, and the treatment of related pain is an extremely challenging task. Providing adequate analgesia while avoiding unnecessary drug effects often requires a polypharmacologic approach in cancer pain management. A 36-year old woman with breast cancer metastatic to the axial skeleton and bilateral hip joints was admitted to hemato-oncology service with complaints of intractable abdominal and hip pain. Despite rapidly increasing doses of intravenous morphine up to 350 mg per day; transdermal fentanyl; midazolam; ketorolac; lorazepam; dexamethasone, the patient continued to describe her pain as 10 of 10, refusing all surgical/diagnostic interventions not directly related to pain control. She did, however, consent to lumbar epidural catheter placement. The patient was sedated with titrating doses of propofol to assist with positioning. Even though the procedure was not successful due to significant thoracolumbar scoliosis, the patient admitted feeling better than she has in months during attempted placement. After continuous infusion of propofol was initiated at subhypnotic dose, the patient's analgesic demand was drastically reduced and described her pain as "1 to 3" of "10". Approximately 96 hours after the propofol infusion was started, the patient expired comfortably. There had been no change in her medical regimen during fecal 48 hours. In the case described, propofol was extremely advantageous as an adjuvant in the management of cancer related pain.

요부 경막외마취 후 경막외강으로 투여한 Dexamethasone의 효과 (The Effect of Epidural Dexamethasone after Lumbar Epidural Anesthesia)

  • Background: Epidural steroid injection(ESI) is often adminstered to treat lower back pain, but its effect iveness on postepidural backache have not yet been determined. Methods: Sixty patients scheduled for cesarean section under epidural anesthesia were randomly assigned to receive, 1 ml of normal saline(Group 1), or 5 mg of dexamethasone (Group 2), epidurally via indwelling catheter after delivery. Results: The incidences of postepidural backache occured significantly more often in Group 1(56.7%) than in Group 2(26.7%)(P<0.05). There was a significant association between postepidural backache and multiple attempts at epidural needle placement. Conclusion: Epidural dexamethasone has a preventive effect on postepidural backache.

경막외 카테터 거치후 발생한 척추 경막외.경막하 농양 -증례 보고- (Spinal Epidural and Subdural Abscess following Epidural Catheterization -A case report-)

  • The placement of epidural catheter may cause complications such as epidural hematoma, epidural abscess and neural damage. Among the above complications, epidural abscess is a rare but serious complication. This report pertains to a diabetic metlitus patient who developed spinal epidural and subdural abscess after continuous epidural catheterization for management of pain caused by reflex sympathetic dystrophy. The patient experienced urinary incontinence, as a neurologic sign, 8 days after epidural catherization. In was considered that the poor prognosis was due to a combination effects of a delayed visit to the hospital for treatment, rapid progression of abscess and uncontrolled blood sugar level. We therefore recommend aseptic technique and proper control of blood sugar level to prevent infection during and after epidural catheterization for diabetic patients. Early diagnosis of epidural abscess following surgical procedure must be required to avoid sequelae.

경막외 마취중 사고로 발생한 경막하 차단 (Accidental Subdural Block Developed during Epidural Anesthesia)

  • Subdural block is a rare but well recognized complication of epidural anesthesia. The placement of local anesthetics into the subdural space can lead to potentially life-threatening conditions. A healthy 46-year-old women underwent total abdominal hysterectomy under continuous lumber epidural anesthesia. The technical procedure for continuous epidural catheterization went smoothly without a single problem. However, signs of high epidural block such as apnea, cranial nerve paralysis and pupil dilatation developed gradually, about 20 minutes after the epidural injection of 2% lidocaine 20 ml through the epidural catheter. Such extensive segmental block can only be explained as the result of injection into subdural space even if it was not confirmed radiologically.
