Change in settlement conditions of mountain area in Chungcheong region over a 10 year period : categorization of mountain villages

  • This study analyzed settlement condition changes of Chungcheong region's mountain area during 10 years, from 2001 to 2012. We performed factor analysis and cluster analysis of data from the 'Census for Mountain Area' carried out by the Korea Forest Service in 2001 and 2012. Factors 1, 2, 3, and 4 represent 'the superiority of non-agricultural income', 'the industrialized mountainous area', 'residential conveniences and benefits', and 'the use of forest resources', respectively. Clusters 1, 2, 3, 4, and 5 are categorized as 'non-agricultural income', 'various mass social functions', 'production of forest products', 'industrialized mountainous region', and 'ordinary mountain region', respectively. We suggest that cluster 1 has potential for development and should be promoted as a possible tourist attraction by digging up geographically unique themes. Cluster 2 has great potential for development and needs planned management through the maintenance or expansion of existing infrastructure. Cluster 3 has potential for development with various high value added industries uncovered. Cluster 4 shows vitality as it holds plenty of more job opportunities than other regions. Cluster 5 is deteriorating as a mountainous region because of an aging population, and it urgently demands development. For a decade, 45 of the 60 regions belonged to a single category, which are now differentiated broadly into two types: Firstly, deterioration changes to potential development and Secondly, vitality is differentiated into potential development and deterioration.

포괄치위생관리 과정을 적용한 스케일링 수행 경험에 관한 질적 연구 : 근거이론적 접근 (Qualitative study on the scaling experience through the application of comprehensive dental hygiene care : A grounded theory approach)

  • Objectives: This study was performed in order to provide evidence-based data for the expected professional impact of dental hygienists, and to apply and disclose the comprehensive dental hygiene care process through an in-depth analysis of their scaling experience and investigation of the importance of an evidence-based scaling work performance. Methods: The data were collected from June 3, 2019 to October 3, 2019 by conducting in-depth individual interviews on 10 dental hygienists who are working in dental clinics and hospitals by region. The data were analyzed by using the grounded theory methodology, which is a field of qualitative research method. Results: Study results showed that the core category derived from the paradigm model and change process in this study was 'a process of becoming a mature professional outside practical work'. Conclusions: In this study, the participants were able to gain a sense of occupational accomplishment as dental hygienists by performing scaling based on the comprehensive dental hygiene care (CDHC) process, and to advance into professionals through continuous efforts and research in order to enhance their job competencies.

임상에서의 생존: 여성간호사에 대한 남성간호사의 적응 과정 (Clinical Survival: Male Nurses Adaptation Process for Female Nurses)

  • 본 연구의 목적은 남성간호사가 여성간호사에 대한 적응과정이 어떠한가를 남성의 관점에서 심도 있게 파악하여남성간호사의 여성간호사에 대한 적응의 의미를 찾는 것이다. 본 연구는 여성간호사에 대한 남성간호사의 적응은 어떠한가? 라는 연구 주제를 가지고 현상학적 방법을 통하여 남성간호사의 여성간호사에 대한 적응과정을 탐색하였다. 연구 참여자는 본 연구의 의도와 방법을 듣고 동의한 25개월 이상의 병원근무경력을 가진 남성간호사로 총 12명을 대상으로 하였다. 본 연구의 결과는 39개의 구성의미와 13개의 주제 묶음, 4개의 범주로 도출 되었다. 범주로는 부적응상황에 가로막힘, 대인관계로 동화되어 가기, 업무능력 향상시키기, 전담간호사로 피해가기로 나타났다. 본 연구 결과를 통해 남성간호사의 여성간호사에 대한 적응 과정을 이해하여 임상 현장에서 효과적인 간호인력 관리 방안 마련을 위한 기초자료로 활용될 수 있을 것이다.

인문사회의학(의료인문학)의 범주와 교육과정 - 한의학 교육 중심으로 - (The category and education curriculum of Medical humanities - focus on Korean Medicine Education -)

  • Objective : The definitions and categories of subjects related to humanities and social medicine are still controversial, and we tried to find the areas lacking compared to the standards of medical education Method : Humanities and social medicine-related subjects of the World Medical Education Standards, ASK2019, and KAS2021 were reviewed, and humanities and social medicine subjects of the College of Korean Medicine were searched. We compared subjects with the medical school learning outcomes and Korean Medicine Doctors' job competency. In order to increase validity, two authors independently classified them. In case of different opinions, we revised after sufficient discussion Results : Humanities and social medicine-related subjects in the Korean Medicine curriculum appear to be sufficient when compared with WFME and ASK2019. The humanities and social medicine-related subjects of Korean Medicine schools were not distributed evenly throughout. The areas to be supplemented in Korean Medicine education were 'a diverse understanding of life and death', 'prevention and response to patient safety incidents', 'effective communication with health-related organizations and groups', 'social responsibility and reflective practice' and 'communication between health and medical professions'. Conclusion : Humanities and social medicine-related subjects in Korean Medicine education are sufficient, but they are not evenly distributed, and the areas listed above need to be reinforced.

노인종합복지관 급식업무 종사자들의 위생지식 및 수행 평가 (Evaluation of Foodservice Workers' Food Safety Knowledge and Practices at Senior Welfare Centers)

  • The purpose of this study was to evaluate food safety practices and knowledge of foodservice workers at senior welfare centers. A self-administrated questionnaire was administered to 1200 foodservice workers at senior welfare centers in Seoul, Incheon and Gyeonggi province. Two hundred five responses were returned, with 181 usable for analysis. Statistical analyses were conducted using SPSS for Windows(ver.14.0). Volunteers of respondents comprised 64.1% while chefs and employees comprised 20.9%. Food safety practices were assessed using a 5-point scale(1: strongly disagree - 5: strongly agree), and the average score of food safety practices was 4.27. For specific practices, 'personal hygiene (4.46)' scored the highest, followed by 'cleaning and sanitizing(4.43)', 'separate handing(4.40)', 'cooking process(4.11)', and 'receiving and storing(4.05)'. There were significant differences in the practice average score by age(p<0.01) and experience of food safety education(p<0.01). The total score for food safety knowledge was 14.43 out of 20 points. In the category of food knowledge, 'equipment and facility(80.5%)' had the highest correct answer rate, followed by 'personnel hygiene(75.9%)', 'cooking process(70.4%)', and 'cleaning sanitizing(65.8%)'. There were significant differences in the knowledge total score by job type(p<0.05), age(p<0.05), working experience(p<0.05), chef certification (p<0.01), and frequency of food safety education(p<0.01). The knowledge and practice scores were significantly correlated(p<0.01). This study suggests that food safety education for foodservice workers should be conducted continuously and repetitively, and the development of proper education materials is needed to improve the effectiveness of food safety education for chefs, employees and volunteers at senior welfare centers.

Factors Affecting Dental Utilization and Dental Expenses in the Economically Active Population: Based on the 2010~2014 Korea Health Panel Data

  • Background: The health of the economically active population contributes to increased corporate productivity by reducing the productivity loss caused by disease and increasing job efficiency, which in turn is a national benefit. Since the economically active population is a concept encompassing workers and a source of economic development for a country, that population's health should be treated with importance not only from a personal standpoint but also at a national level. Methods: In this study, data of 11,007 adults aged 20 years and older who participate in economic activities were analyzed in the five-year Korea Health Panel Study from 2010 to 2014 including the number of dental visits and dental medical expenses. Results: Factors related to "gender," "education level," "age," "duty category," "income level," "employment type," "national health insurance," and "chronic disease status" of the economically active population are affected in relation to the number of visits and dental medical expenses. The number of dental visits increased with higher education levels (p<0.001), and the number of visits to the dentist increased with older age (p <0.001). Dental medical expenses were 91,806 Korean won (KRW) more for "white-collar workers" than for "blue-collar workers" (p<0.03), and 127,674 KRW more for "regular workers" than for "atypical workers" (p<0.02). Conclusion: When it is necessary to improve policies to enhance the efficiency of the distribution of health and medical resources in the overall balance of the dental health sector, we should try to identify various factors of oral health disorder due to income inequality among the classes according to the country's employment type in order to find ways to reduce the health gap among the social classes.

Association between lone work and self-rated health status: using the 5th Korean Working Conditions Survey

  • Background: Lone workers are generally defined as individuals who work alone without supervision, including self-employed people. While lone workers are considered a vulnerable group in some countries, there is a lack of research on their health status in domestic studies. Globally, the number of lone workers has been increasing, and this trend has been further accelerated since the coronavirus disease 2019 (COVID-19) pandemic with the rise of remote work. Methods: The study analyzed data from 44,281 participants, excluding unpaid family workers, soldiers, and those with missing data. Lone workers were defined as individuals who reported having no colleagues with the same job at their current workplace. Self-rated health status was categorized as "good" or "poor." Results: This study found a statistically significant higher number of lone workers among women compare to men. The largest occupational category for lone workers was service and sales workers, followed by agriculture and fisheries workers. A majority of non-lone workers reported working 40 hours or less per week, while the majority of lone workers reported working 53 hours or more per week. In addition, lone workers had significantly poorer health status evaluations compared to non-lone workers (odds ratio: 1.297; 95% confidence interval: 1.165-1.444). Conclusions: Further research is needed to investigate the causal relationship between lone work and health, using data collected after the COVID-19 pandemic.

병원의 비정규직 고용정책과 경영전략 (Employment Policy & Strategy of Irregular Workers in Hospital)

  • The objectives of this study were to inspect how the law effected on hospital employment system and which policy and strategy are needed to cope with the present situation as the law regarding irregular workers law has enacted for a year. To grasp the changes of employment style in hospital, 56 hospitals among 311 general hospitals were questioned in this study. And employment policy and strategy were developed by referencing examples of developed nations and other industries. The survey showed that the wage and welfare level of irregular workers in hospitals was improved compared to that in the other industries. But there were still much discrimination between regular and irregular workers. The policy direction of government is, first of all, to follow the principle of equal treatment to equal value of labour. The first feasible policy is to enforce social security. The second one is an employment promotion policy which reduces or exempts hospitals employing irregular workers from tax and insurance fee. The third one is to extend employment contract period to 3 years. and finally there are policies to permit more dispatched jobs and to expand the social insurance coverage. The strategies to solve the problem of irregular workers are as follows; 1) performance wage system, 2) guaranteeing employment by unlimited contract, 3) creating new category of workers, 4) the wage system of management by object, 5) the method of job classification. This study has a meaning in the point that it was studied on hospital which is special industrial part and analyzed the changes after enacting irregular workers law and presented management strategy for countermeaure program. In this study, it was expected to contribute to decisions-making in hospital management, especially when using human resources.

개인 노출량 조사를 통한 한국인의 극저주파 자기장 노출 수준 (Estimation of ELF-MF Exposure Levels in the Korean Population through 24-Hour Personal Exposure)

  • Objectives: The purpose of this study was to estimate the exposure level to extremely low frequency-magnetic fields (ELF-MF) among a selected Korean population using 24-hour personal exposure measurement. Methods: Participants were randomly selected for the measurement of MF exposure under the assumption that the subjects are representative of the overall Korean population. Levels of personal exposure to MF were measured according to the subject's daily activities. Results: The 24-hour time-weighted average (TWA) of 250 participants was $1.56{\pm}4.56$ mG (GM, GSD: 0.79, 2.46 mG). Personal exposure levels for females were higher than for males. The highest personal exposure level was shown in the age group between 20-60 years old. Personal exposure levels according to job category were higher for the non-occupational group than for the occupational group. Conclusions: Our results showed MF exposure exceeding 2 mG per day among 11.3% of the Korean population, indicating a somewhat higher percentage compared to the EMF RAPID Program's results for the U.S population.

장기요양시설 인간중심 치매케어 온라인 교육 프로그램 개발을 위한 델파이 조사연구 (A Delphi Study for Developing a Person-centered Dementia Care Online Education Program in Long-term Care Facilities)

  • Purpose: There has been a growing recognition that person-centered care enhances the quality of life of nursing home residents with dementia. This study was conducted to develop a person-centered dementia care online education program for direct care staff in long-term care facilities. Methods: Delphi method with expert group was used to validate contents. We developed 61 draft items based on literature review. Twenty experts participated in consecutive three round surveys including 5-point Likert scale questions and open-ended questions. Based on experts' opinions, the content validity ratio for content validity and the coefficient of variation for stability were calculated. Results: Three-round Delphi surveys and additional feedback from the expert panel established a consensus of core contents: 1) dementia (7 categories), 2) person-centered care (6 categories), 3) communication (8 categories), and 4) behavioral and psychological symptoms of dementia (6 categories). Specific sub-categories in each category were differentiated according to the job qualifications (65 sub-categories for registered nurses, 64 sub-categories for nursing aids, and 41 sub-categories for personal care workers). Conclusion: This delphi study identified person-centered dementia education curricula, in which the person-centered approach should be a key policy priority in Korean long-term care system. Now it is urgently needed to develop education programs utilizing online platforms that enable efficient and continuous learning for long-term care staff, which can contribute to behavior changes in the person-centered dementia care approach and improvement of care quality in long-term care facilities.
