Case Report of Breast Reconstruction with SIEA Flap (얕은 하복벽동맥 천공지 피판을 이용한 유방 재건술 - 2예 보고 -)

  • Purpose: Breast reconstruction with abdominal flap has many advantages. However, it might cause abdominal complications such as bulging or hernia. SIEA (Superficial inferior epigastric artery) flap is the most advanced form of abdominal flap which has no adverse effect on abdominal fascia. We report 2 cases of breast reconstruction with SIEA flap with reference review. Methods: From Jun 2006 to Jan 2009, 110 patients underwent breast reconstruction with free abdominal flap. We tried to find the SIEA in every cases and adopted SIEP flap if the diameter was larger than 1mm and it had visible pulsation. 2 patients underwent breast reconstruction with SIEA flap. After design of abdominal flap SIEA was investigated with portable Doppler. SIEA was usually found 4-5 cm lateral to the midline. Flap was elevated with SIEA and SIEV to their maximal length at hiatus. Results: There were no complications, such as infection, hematoma, and necrosis of flap. In both cases, flaps survived completely with excellent vascularity and breast reconstruction was successful. In one case, there was skin necrosis of mastectomy flap, and it was healed by conservative management. Conclusion: With the SIEA flap, donor site morbidity can be minimized along with reduction of operation time. If there is reliable SIEA, SIEA flap would be the preferred skills. However, the limitation of the SIEA flap is difficulty in identifying the SIEA. The reason for less availability of the flap in Korea might include high prevalence of the Caesarian section scar and relatively higher level of the lower margin of the flap.

Unusual Benign Neoplasms of the Larynx (양성 종괴로 나타나는 후두 질환)

  • Background and Objectives : Benign neoplasms of the larynx are rare, and papillomas account for approximately 90% of these neoplasms. Other benign neoplasms of the larynx are very rare and form a hetergenous group. We present clinical manifestations of unusual benign neoplasms based on our experiences and review of literatures. Materials and Method : We reviewed retrospectively the clinical records of 14 patients with benign neoplasms of the larynx, excluding papillomas, that were examined in our department during 11-year period from 1995 to 2006. Results : The presenting symptom was most commonly progressive dysphonia. Pathologic diagnosis revealed 5 cases of hemangioma, 3 granular cell tumor, 2 amyloidosis, 2 laryngocele, 1 schwannoma, 1 chondroma. Subsites of the neoplasms were 5 in true vocal cord, 3 in arytenoids, 2 in false vocal cord, 2 in supraglottis, and others were subglottis ; aryepiglottic fold. Treatment was surgical, by a external approach in 1 case of chondroma, and by laryngoscopic approach in other cases. In laryngoscopic approach, carbon dioxide laser was used in 10 cases. Postoperative course was satisfactory. Recurrence was encountered in I case of amyloidosis and revision operation was done 3 times. No recurrence was encountered in other cases. Conclusion : Uncommon benign neoplasms of the larynx require high index of suspicion and histological confirmation. Complete excision with an attempt to maintain normal structures generally results in cure.

  • In the average adult with a normal immune state, Epstein-Barr virus pneumonia is very rare, especially in the form of interstitial lung disease. According to recent studies, the Epstein-Barr virus is also associated with lymphocytic interstitial pneumonia, AIDS and Langerhans cell histiocytosis, but not with sarcoidosis. BOOP is caused by lung injury due to an infection or drug intoxication, and is related to connective tissue disease or bone marrow transplantation, but is sometimes idiopathic. We experienced a patient with symptoms and signs of interstitial lung disease, with confirmed BOOP and EBV ingection from an open lung biopsy and serologic examination, respectively Herein, this case is reported, with a review of the literature.

  • Research methodology on Traditional Medicine in East Asia refers to logical thinking system, empirical positivism system and methodology of developing these knowledge systems. Logical thinking system of abstract concepts such as analogy or abduction and positivism system of reasonable explanation such as the five elements and their characteristic theory have been used in various ways empirically or in the form of humanities and knowledge system was developed through parallel structure of empirical positivism and exegetical studies. After the 16th century, evidence was required along with the tradition of putting emphasis on rationality, logicality and empirical positivism and characteristics of medical humanities can be found in emphasizing on medical ethics. Data that can be considered as structural review paper or meta analysis from original data of research on Traditional East Asian Medicine should be evaluated as historical evidence which is equivalent to specialist opinion, descriptive disease research, single case report or case series. Historical evidence based medicine is a research method using Historical evidence to selectively support data that are faithful to traditional theory with higher possibility to be used in future traditional east Asian medicine that links between traditional knowledge and scientific research methodology. Moreover, historical evidence based medicine tries to re-evaluate the value of traditional knowledge and ultimately, guides the direction of development of traditional medicine through scientific rationality based on history and culture.

A REPORT OF CALCITYING ODONTOGENIC CYST (석회화 치원성 낭종의 치험례)

  • The calcifying odontogenic cyst was identified as a pathological entity by Gorlin & his associates in 1962. This lesion is one of the rarest and most disputable cysts in the oral region. The calcifying odontogenic cyst has variable clinical and radiological features. We review the previous literatures and report 2 cases of calcifying odontogenic cyst at Department of Oral and Maxillofacial Surgery, Kyung-Hee University. The 1st case was as follows. The patient vas 22 year old female. The past dental history revealed extraction of prolonged retained #73 tooth about 15days ago. She complained a painful swelling on the lower anterior teeth area. There were chin and vestibular swelling on the lower anterior teeth area, tenderness and missing of #33 tooth. The radiograph revealed well-demarcated unilocular radiolucency containing radiopaque calcific flecks around impacted #33 tooth. The clinical diagnosis was COC, so surgical enucleation was done. There was no recurrence and COC was confirmed by pathologist. The second case was as follows. The patient was 72 year old male. The past history revealed inactive tuberculosis, bronchial asthma and denture construction. The chief complaint was rapidly growing mass on the lower left anterior edentulous area. The clinical findings were chin swelling protruding mass with surface ulceration, fluctuation and a few bloody fluid in aspiration. The radiograph revealed well-demarcated radiolucency mimiking the residual cyst. The biopsy result was COC. The surgical excision was done, but the lesion was recurred 10 months later. The treatment was surgical excision with aggressive peripheral bone grinding and FTSG form groin area. There was no problem during the postoperative period.

  • The street system in multi-family housing is an important design element to control the circulation of vehicles and pedestrians, and to form the spatial organization of the projects. In particular, in the case of high-rise apartments, the street patterns have an influence on the safety, amenity, and convenience of facilities. Until now, however, the design of street patterns in most high-rise multi-family housing projects has diminished the quality of residential environments because priority of design consideration has primarily been given to planning road ways and parking areas. Nevertheless, the design competitions introduced for the first time in the 1980s have played a significant role in developing fresh ideas and concepts in housing design. Because street patterns have been respectably modified as a result of the generalizing of design competitions, it is important to review the ideas that have been proposed in competitions over the recent past. In this context, this study classified the types of street patterns and analyzed their design characteristics for the proposals of housing design competitions since the 1990s. As a result of this analysis, the road patterns are classified into 4 types: 1) serial type, 2) isolated type, 3) mixed type, and 4) not a road pattern due to the system of connection. In addition, these were further divided into 8 types: 1) loop, 2) ring, 3) penetration, 4) tree, 5) direct entry, 6) ring and penetration, 7) loop and penetration, 8) not a road pattern due to appearance and the collector.

A Case of Successful Pregnancy with Electroejaculation and In Vitro Fertilization (전기사정과 시험관아기시술에 의한 성공적인 임신 1례)

  • Although anejaculation is a relatively uncommon occurrence in the general population, over 12,000 new cases are reported annually. Anejaculation may result from spinal cord injury, retroperitoneal lymph node dissection, diabetes mellitus, transverse myelitis, multiple sclerosis, or psychogenic disorders. At least 30% of men with this problem are or will be married and many will seek help to remedy their infertile state. The evolution of technique and instrumentation over the last 30 years has made electroejaculation an accessible and acceptable form of therapy. Recent successes in inducing ejaculation by means of rectal probe electrostimulation or vibratory stimulation combined with assisted reproductive techniques, such as zygote intrafallopian transfer (ZIFT), gamete intrafallopian transfer (GIFT), and in vitro fertilization (IVF), have provided these men means of producing their own biologic offspring. We have experienced a successful pregnancy with electroejaculation and in vitro fertilization in a infertile patient whose husband had an ejaculatory disturbance due to a spinal cord injury. So we report this case with a brief review of literatures.

  • Abdominal pregnancy is a very rare form of ectopic pregnancy following IVF-ET procedure. Because the symptoms of abdominal pregnancy is not typical, it is difficult to diagnose early and the mortality rate is higher than other ectopic pregnancies. Liver, spleen, omentum and diaphragm are main implantation sites of abdominal pregnancy. We reported an unusual first case of an etopic pregnancy on uterosacral ligament following in vitro fertilization and embryo transfer with brief review of literature.

  • Malaria, caused by any of four species of protozoan parasites of the genus Plasmodium, is charaterized by high fever, anemia and splenomegaly. Although malaria is a cause of significant morbidity and mortality worldwide, in Korea indigenous malaria has been believed to be eradicated by 1984. However, since the case report of native malaria in 1993, reported cases have been increased annually, reaching more than 300 cases in 1996. We experienced a 2 years-old male with fever, severe anemia and splenomegaly who resided in Inchon city. He had the history of travelling to the area (Yunchon) near western Demilitarized Zone for 1 month this summer. After more than 2 weeks without special attention, he was presented with pallor, anemia and splenomegaly. He was diagnosed to have malaria by Plasmodium vivax with the help of peripheral blood smears which showed various forms of malaria, i.e., ring form, trophozoites, shizonts and gametocytes. He was treated successfully with hydroxychloroquine and primaquine. We report this case with brief review of related literature.

Analysis of a clinical dentistry course to study the need for integrated curriculum : in the case of dental hygiene department in Korea and Japan (임상치과 영역 교과목 분석을 통한 통합교육과정에 대한 고찰 : 한국, 일본 일부 치위생학과 사례를 중심으로)

  • The purpose of this study is to analyze the clinical dentistry curriculum in Korea and Japan in order to review the application of integrated curriculum in the field. We collected the clinical dentistry syllabi for 2015-16 of the department of dental hygiene of Korea G University and Japan N University. Using the data from Korea, items that were duplicated and integrated were classified by the subjects covered in clinical dentistry. In addition, through case studies conducted in Korea and Japan, we analyzed and compared the credits, class hours, teaching methods, and evaluation methods, by subject. In Korea the total class hours in clinical dentistry, including clinical practice, was 1,095, and in Japan it was 1,104. In Korea, the syllabus covered by each department consisted of about 200 hours centering on theoretical lectures. In Japan, the integrated form of clinical dentistry was taught, and the instructions included all the contents of clinical dental study and also problem-based learning for approximately 100 hours. To strengthen the competence of dental hygienists as professionals, who are actively involved in dental practice, it is necessary to integrate the education contents of segmented clinical dentistry from the perspective of dental hygiene. It is also important to apply teaching methods that can help students improve their ability to solve problems.
