Proliferative periostitis of the mandibular ramus and condyle: a case report

  • Proliferative periostitis is a rare form of osteomyelitis that is characterized by new bone formation with periosteal reaction common causes of proliferative periostitis are dental caries, periodontitis, cysts, and trauma. While proliferative periostitis typically presents as a localized lesion, in this study, we describe an extensive form of proliferative periostitis involving the whole mandibular ramus and condyle. Because the radiographic findings were similar to osteogenic sarcoma, an accurate differential diagnosis was important for proper treatment.

  • The aim of this study was to achieve healing of Peri-implantitis defects and hard tissue augmentation using a bovine-derived bone mineral on the defect site. Two patients were treated with the surgical approach. With a full muco-periosteal flap elevation, the implant surfaces were exposed and granulation tissue removed around the implant and between the threads. Each surface of the contaminated implant was prepared with the air-abrasive device(PerioFlow$^{(R)}$) for decontamination. Bovine-derived bone mineral(Bio-Oss collagen$^{(R)}$) was then used to fill the defects and muco-periosteal flaps sutured to achieve transmucosal healing. Radiographs and clinical photographs were taken before and after 6 months of healing and an estimate of bone fill was assessed. Within the limits of the present case report, a surgical approach in treatment of peri-implantitis defects using a collagen form of bovine bone mineral was visited. Although limited, the two cases showed the stability and biocompatibility of a bovine-derived bone mineral and effectiveness of air-abrasive device(PerioFlow$^{(R)}$) as a decontamination method.

  • The Buford complex is unusual variant of the glenohumeral joint. This complex is distinguished by a cord-like middle glenohumeral ligament that oriented directly form the superior labrum at the base of the biceps tendon and crosses the subscapularis tendon to insert on the humerus. There is no anterior-superior labral tissue present between this attachment and the mid-glenoid notch. This anatomical variation may lead the surgeon to confuse this complex with a sublabral hole, pathologic labral detachment, Bankart lesion or SLAP lesion. We report a case of Buford complex which was found incidentally during the operation of impingement syndrome with stiffness and treated with subacromial decompression only.

  • Amyloidosis is an all-inclusive disease of deposition of amyloid proteins in the extracellular spaces, which in localized or systemic form cause tissue damage and dysfunction. Herein, we report a case of small bowel involvement of systemic amyloidosis presenting with multiple polypoid wall thickening mimicking small bowel polyposis syndrome in an age 75 male. Interestingly, polypoid wall thickening and amyloidoma showed hypointensity on T2-weighted images. To our knowledge, there has been no literature describing MRI findings of poylpoid wall thickening and amyloidoma. Although the underlying mechanisms are unclear and need validation, hypointensity on T2-weighted images could be valuable in diagnosing small bowel involvement of amyloidosis in patients presenting with poylpoid wall thickening and amyloidoma.

  • Ossification of the yellow ligament (OYL) is a pathologic condition that causes spinal stenosis, which is a form of ectopic ossification. OYL causes compressive myelopathy and radiculopathy. Although the pathogenesis of OYL is still unclear, diffuse mechanical stresses and degenerative changes caused by extreme ranges of motion may be related to the development of OYL in young sportsmen. Here we report an interesting case of thoracic radiculopathy due to OYL in a 35-year-old male amateur judo player who was successfully treated with continuous thoracic patient controlled epidural analgesia and epidural adhesiolysis.

  • Objectives We report 1 treatment case with Euphorbiae Kansui Radix which is based on Sasang constitutional Medicine for Chronic Dizziness in Soyangin. Methos We treated Soyangin patient suffering form chronic dizziness. We prescribed Euphorbiae Kansui Radix for her physical symptoms. The improvement of her dizziness was evaluated by Visual Analogue Scale(VAS). Results After the Euphorbiae Kansui Radix was given, the patient's Dizziness was improved. And there was not any adverse effect. Conclusions This study shows that using Euphorbiae Kansui Radix can be effective treatment for Soyangin chronic dizziness patient.

Operative Treatment of the Bilateral Multiple Rays Brachymetatarsia using Callotasis (A Case Report) (가골 신연술을 이용한 양측성 다발열 단중족증의 치료 (1예 보고))

  • Among congenital brachymetatarsias fourth metatarsal bone shortening is most common form and unusually involves multiple metatarsal bones. We report a case of the patient with bilateral 1st and 4th multiple rays brachymetatarsia treated with callostasis. Twenty eight months after procedure, right side showed 1st metatarsal lengthening about 11.5 mm (29.23%) and 4th metatarsal 17.86 mm (35.87%). Twenty one months later, left side showed 1st metatarsal lengthening about 14.58 mm (36.8%) and 4th metatarsal 20.52 mm (43.01%). In healing index right side showed 1st metatarsal 3.4 month/cm, 4th metatarsal 2.5 month/cm and left side showed 1st metatarsal 4.2 month/cm, 4th metatarsal 2.3 month/cm. Final follow-up results presented excellent outcome in cosmetic problem, easy wear of shoes, and measurement of patient's self satisfaction.

A Case Report of the Korean Medical Treatment for Patulous Eustachian Tube Patient with Headache (두통을 동반한 개방성 이관의 한의학적 치료 증례보고)

  • This study was performed to report the case of Korean medical treatment for the Patulous Eustachian tube patient with headache. The patient was treated by acupuncture, herbal medicine, bee venom injection, needle-embedding therapy, chuna therapy, release of cranial base and sacro-occipital therapy. Korean medical treatment was administered during 3 weeks. The improvement of clinical symptoms was evaluated by VAS (visual analogue scale) and SF-36 (36-Item Short Form Healthy Survey Instrument). After treatment, most symptoms decreased, headache VAS score changed 10 to 6 and tinnitus VAS score changed 10 to 5. Also, most SF-36 scores increased. Our study suggested that Korean medical treatments are effective in the patient with Patulous Eustachian tube. And further studies are required to identify underlying mechanism of treatment.

  • The peripheral odontogenic fibroma (WHO type) is a relatively rare and characteristically benign and unencapsulated, exophytic gingival mass of fibrous connective. Odontogenic epithelium is found within the gingival mass, but usually appears to playa minor role when compared to the fibrous component. The connective tissue is ranged from markedly cellular to relatively acellular and well collagenized. but the connective tissue in this case report appears less cellular. Peripheral odontogenic fibroma must be differentiated histologically from peripheral ossifying fibroma, Peripheral cemental epithelial odontogenic tumor and Peripheral ameloblastoma. The author reports the following conclusions after clinical and radiological examination, excisional biopsy and reviewing literatures. 1. Peripheral odontogenic fibroma is rare lesion and frequently occurs in interdental papila as a form of fibroblastic connective tissue including odontogenic epithelium within the lesion. 2. Peripheral odontogenic fibroma must be differentially diagnosed with Peripheral ossifying fibroma by including less cellular connective tissue, odontogenic epithelium and dysplastic dentin 3. Treatment consists of surgical excision including removal of alveolar bone which is eroded under the lesion

  • Anoperineal tuberculosis is a rare extrapulmonary form of the disease and may present as abscess. We report a case of anoperineal tuberculous abscess, which showed low signal intensity on T1-weighted images, high signal intensity on T2-weighted images and diffusion restriction on diffusion weighted images.
