전산화단층촬영장치를 이용한 뇌동맥류의 호발부위 분석 (The Analysis of Cerebral Aneurysm's Prone position Using Computed Tomography Equipment)

  • 본 연구에서 2008년 10월 1일부터 2010년 9월 30일까지 경북지역 S대학 병원을 내원하여 전산화단층 혈관촬영을 시행한 환자 중 뇌동맥류가 발견된 249명을 대상으로 분석한 결과는 다음과 같다. 연구 대상자 총 249명 중 여자가 159명(63.9%)으로 남자 90명(36.1%) 보다 높은 분포를 보였다. 뇌동맥류의 혈관별 분포에서는 후교통동맥이 34.9%로 가장 많았으며, 다음으로 내경동맥이 21.7%, 중대뇌동맥이 15.7%, 전교통동맥이 14.5%, 후대뇌동맥과 뇌저동맥이 각각 3.6%, 전대뇌동맥과 추골동맥이 각각 2.4%, 후하소뇌동맥 1.2% 순이었다. 성별에 따른 뇌동맥류의 분포에 대해 분석한 결과 남자의 경우 후대뇌동맥과 뇌저동맥에 질환이 있는 경우가 한명도 없었으며, 여자의 경우에는 후하소뇌동맥에 질환이 있는 경우는 한명도 없었다. 성별에 따른 뇌동맥류의 혈관별 분포에서는 통계적으로 유의성이 있는 것으로 나타났다(p<0.05). 연령에 따른 뇌동맥류의 혈관별 분포에서는 통계적으로 유의성이 없는 것으로 나타났으며(p>0.05), 차이가 나는 집단을 파악하기 위해 사후 분석을 적용한 결과, 61~70세의 경우가 가장 높았고(4.21), 30세 이하(2.0)가 가장 낮은 것으로 파악되었다. 계절에 따른 뇌동맥류의 혈관별 분포에서는 통계적으로 유의성이 있는 것으로 나타났으며(p<0.05), 차이가 나는 집단을 파악하기 위해 사후 분석을 적용한 결과, 가을이 가장 높았고(4.55) 봄(2.50)이 가장 낮은 것으로 파악되었다.

백서의 피부 및 뇌혈류에 미치는 고려홍삼 사포닌 및 비사포닌의 영향 (Effects of Crude Saponin and Saponin-free Fraction of Korea Red Ginseng on the Skin and Cerebral Blood Flow in the Rats)

  • 본 연구에서는 혈류량의 변화를 실시간으로 측정할 수 있는 레이져 도플러 혈류량측정장치를 이용하여 피부혈류 및 뇌혈류량에 미치는 고려홍삼의 사포닌과 비사포닌의 효능을 연구하고자 고려홍삼의 정맥내 복강내 및 구강내로 주입하고 혈류의 변화를 관찰하여 다음과 같은 결론을 얻었다. 고려 홍삼의 사포닌 성분은 피부혈류량에는 영향을 주지 않으나 뇌혈류량을 증가시키는 효능이 있다. 고려 홍삼의 비사포닌 성분은 피부혈류량 및 뇌혈류량에 거의 영향을 주지 않았다. 고려홍삼사포닌에 의한 뇌혈류량의 증가는 복강내 및 경구로 투여할 경우는 관찰할 수 있으나 정맥내로 직접 투여할 경우는 관찰되지 않았다. 이상의 결과를 종합해 볼 때 고려홍삼의 사포닌 성분은 뇌혈류량을 증가시키는 혈류개선작용이 있는 것으로 사료된다.

TCD를 이용한 정상군과 고지혈증군의 혈류측정에 관한 비교연구 (Cerebral Blood Flow as Measured by TCD in Hyperlipidemic Group)

  • The purpose of this study was to compare cerebral blood flow between hyperlipidemic patient group and normal healthy aldult group using transcranial doppler ultrasonography(TCD). I investigated cerebral blood flow of 63 hyperlipidemic patients and 42 healthy adults. To evaluate the cerebral blood flow, I measured the systolic peak velocity(Vs) and mean flow velocity(Vm) of the milddle cerebral artery(MCA), anterior cerebral artery(ACA), posterior cerebral artery(PCA), basilar artery(BA), internal carotid artery(ICA) in the two groups using TCD. In normal healthy adults, subjects showed a decerease in Vs and Vm with advancing in age. There was a significant difference in the Vm of ICA. There was no significant differences in the Vm of ACA and ICA. In normal healthy adults, females showed high velocities of all examined vessel. There was a significant difference in the Vs of ICA. There was a significant differences in the Vm of MCA and ICA. Normal healthy adults higher than hyperlipidemic patients in the Vs of MCA, ACA, PCA. There was no significant difference in the Vs of all exmined vessels. Hyperlipidemic patientsincrease higher than normal healthy adults in the Vm of ACA. There was a significant difference in the Vs of BA. In the patients aged under 50's, normal healthy adults higher than hyperlipidemic patients in the Vs and Vm except BA. But there was no significant difference in all exmined vessels. In the patients aged over 50's, hyperlipidemic patients higher than normal healthy adults in all examined vessel expect PCA. There was a significant difference in the Vs of BA and ICA. And hyperlipidemic patients higher than that normal healthy adults in all exmined vessels. There was a significant difference in BA and ICA. In this study, visible differences in blood flow between hyperlipidemic patient group and normal healthy aldult group were shown. However, these results do not come up to the previous values that were reported and known worldwide.

Rheogram으로 본 실혈시 신장혈액유통 변화 (Rheographic View of the Change in the Renal Blood Flow in Acute Hemorrhage)

  • Studies were undertaken on the changes in the renal blood flow by relating them with the alterations in the amplitudes in the rheogram of rabbits. The changing pattern of the electrical conductivity was recorded by means of the needle electrodes inserted into the kidney and the surrounding aluminium foil which was grounded. The Impedance Rheograph manufactured by the Narco Company was used. The small artifact which persisted after ligation of the renal vessels was subtracted from the value obtained in each pulsatile wave in the rheogram. The animals were nembutalized intravenously, 30 mg/kg. A plastic canule was inserted into the carotid artery and the arterial blood pressure was monitored continuously with the pressure transducer connected to the physiograph. Stepwise bleedings were performed on the animal. The first bleeding was between 13 to 18 ml in the amount, and it was folowed by consecutive hemorrhages, 5 or 10 ml each time. The total amount of bleeding was summed as much as 1.5-2% of the body weight. Two minutes fter each bleeding th arterial blood pressure, ECG and the rheogram were taken. That was the necessary time to obtain the stabilized picture of each parameter. After closing the bleeding process, the shed blood was retransfused into the animal and the response in the renal blood flow was observed as well as the arterial blood pressure. Particularly the presence or absence of the autoregulatory mechanism in the situation of the hemorrhage was also studied. The results obtained were as follows: 1. In 7 cases out of 22, that was about one third of the total number of experiments, the autoregulatory mechanism of the renal blood flow persisted even in acute hemorrhage, and the decreases in the renal blood flow were less than 10% of the control values even when the arterial blood pressure dropped to 66-87% of the original value obtained before the bleeding. 2. Because of the stepwise bleeding the exact blood pressure at which the renal blood flow reduced as much as one third of the control value could not be obtained. However, the results revealed that the approximate pressure, expressed as percentage of the control value, was 50-60% in 3 cases, 61-70% in 4 cases and 71-80% in 8 cases. In one case the decrease in the renal blood flow exceeded one third of the control value before the pressure dropped to 80% of the control. 3. In 19 cases the decreases in the renal blood flow exceeded one half of the control values by hemorrhage. Then the arterial blood pressure revealed less than 40% of the control value in 6-cases. In 2 cases the pressure was 51-60% of the control pressure. In 5 cases the range of bleed pressure was 61-70%, and in e remaining 6 cases the pressure ranged from 71 to 80% of e control value. 4. Out of 15 cases of retransfusion after definite decreases in the renal blood flow loller·ing the hemorrhage, 9 cases restored their renal blood flow. On the contrary 6 cases showed low values even when the shed blood was retransfused. 5. Theories concerning the mechanism of the autoregulation of the renal blood flow were reviewed for the purpose of explanation of the results obtained. However, there are much to be done before greater satisfaction

내인성물질의 수송계를 이용한 혈액-뇌관문에의 약물송달V-약물의 혈액-뇌관문 투과성에 대한 염기성 아민 및 모노카르본산 수송계의 역할- (Drug Delivery into the Blood-Brain Barrier by Endogenous Substances-A Role of Amine and Monocarboxylic Acid Carrier Systems for the Drug Transport-)

  • The contribution of endogenous transport systems to the blood-brain barrier (BBB) transport of basic and acidic drugs was studied by using a carotid injection technique in rats and an isolated bovine cerebrovascular disease state were compared between the normotensive rats (WKY) and stroke-prone spontaneously hypertensive rats (SHRSP) which have been well established as an animal model with pathogenic similarities to humans. Basic drugs such as eperisone, thiamine and scopolamine inhibited, in a concentration dependent manner the in vivo uptake of $[{^3}H]choline$ through BBB, whereas amino acids and acidic drugs such as salicylic acid and valproic acid did not inhibit the uptake. The uptake of $[^3H]choline$ by B-CAP increased with time and showed a remarkable temperature dependency. The uptake of $[^3H]choline$ by B-CAP showed the very similar inhibitory effects as observed in the in vivo brain uptake, and was competitively inhibited by a basic drug, eperisone. The in vivo BBB uptakes of $[^3H]acetic$ acid and $[^{14}C]salicylic$ acid were dependent on pH of the injectate and the concentration of drugs. Several acidic drugs such such as salicylic acid, benzoic acid and valproic acid inhibited the in vivo uptake of $[^3H]acetic$ acid, whereas amino acid, choline and a basic drug such as eperisone did not inhibit the uptake. The uptake of acetic acid by B-CAP was competitively inhibited by salicylic acid. The permeability surface area product (PS) through BBB for $[^3H]choline$ in SHRSP was significantly lower than that in WKY. The concentration of choline in the brain dialysate in SHRSP was about half of that in WKY, while no significant difference was observed in the plasma concentration of choline between SHRSP and WKY. No significant difference was observed in the transport of monocarboxylic acids, glucose and neutral amino acid through BBB between SHRSP and WKY. From these results, it was concluded that BBB transport system of choline contributes to the transport of basic drugs through BBB, that acidic drugs can be transported via a moncarboxylic acid BBB transport system and that the specific dysfuntion of the BBB choline transport in SHRSP was ascribed to the reduction of the maximum velocity of choline concentration in the brain interstitial fluids.

Higenamine의 합성 및 가토의 심혈관계에 미치는 영향 : 베타-아드레날린성 효능 약물 (Synthesis of Higenamine and its Cardiovascular Effects in Rabbit: Evidence for ${\beta}-Adrenoceptor$ agonist)

  • 최근에 미나라아제비과 (Ranunculaceae)에 속하는 부자(Aconiti tuber)로부터 강심작용을 나타내는 성분을 분리하여 Higenamine이라 명명하였고 그 작용기전을 밝히려는 시도가 활발히 진행되고 있다. 그러나 생약제로 부터 얻을 수 있는 Higenamine은 극히 미량이고 그 과정 또한 복잡하다. 따라서 본 연구에서는 유효성분을 대량 얻기 위한 방법으로서 전합성(total Synthesis)을 시도하였으며 IR, UV, NMR 및 elemental 분석등을 통하여 합성된 물질을 확인하였으며 가토에 대하여 in vivo에서 혈압, 심박동수, 호흡 및 말초저항에 미치는 영향과 아울러 in vitro에서 심근수축 증강작용(positive inotropic action) 및 심박속도 증강작용(positivechronotropic action)을 관찰 분석하여 다음과 같은 결론을 얻었다. 1) Higenamine은 $1-10\;{\mu}g/kg/min$ 정맥주사시 수축기 및 이완기혈압 모두를 용량 의존적으로 하강시켰고 후자가 전자보다 더욱 현저하였으며 호흡은 촉진되었고 말초 혈류량은 증가되었으나 심박동수는 영향이 없었다. 2) 혈압에 대한 Higenamine의 작용은 propranolol 전처치에 의하여 억제되었다. 3) Higenamine의 catechol핵과 커다란 아미노기는 베타 수용체기 대하여 전자는 활성을 후자는 친화력과 관계있을 것으로 추정하였다. 이상의 결과 Higenamine은 adrenergic ${\beta}$-수용체에 작용하여 그 작용을 발휘하는 것으로 사료 되었다.

흑혈류영상 획득 시 신호소실을 활용한 고속스핀에코기법의 유용성 (Effectiveness of a fast spin echo technique using the signal void in acquisition of black blood images)

  • 본 연구는 흑혈류영상 획득에서 고속스핀에코기법의 신호소실 현상을 이용하여 검사시간을 최소화시켜 환자의 불편을 줄이고 진단적 가치를 유지하고자 하였다. 연구방법은 흑혈류영상을 획득한 환자 32명을 대상으로, 기존의 이중역전회복기법과 새로운 기법인 고속스핀에코기법을 각각 적용한 후, 기법에 따른 혈류의 신호소거 차이를 알아보기 위해 T1, T2 강조영상에서 내경동맥 내강의 SNR과 CNR을 비교 평가하였다. 연구결과, SNR은 고속스핀에코기법이 이중역전회복기법 보다 T1, T2 강조영상에서 11.49%(4.73), 13.66%(6.03) 낮았으며, CNR은 고속스핀에코기법이 T1 강조영상에서 8.69%(15.04) 높았고, T2 강조영상에서 7.55% (13.17) 낮았으나, 통계적 수준에서 두 기법간의 SNR과 CNR의 차이는 유의하지 않았다. 그러므로 흑혈류영상 획득 시 고속스핀에코기법을 이용한다면 진단적 가치는 유지하면서 검사시간을 최소화하여 환자의 불편을 줄일 수 있다고 판단된다.

노령 흰쥐의 뇌허혈 손상시 양격산화탕(凉膈散火湯)이 뇌해마의 c-Fos 및 c-Jun 발현에 미치는 영향 (Effect of Yanggyuksanhwa-tang on c-Fos and c-Jun Expression in Ischemic Damaged Hippocampus of Aged BCAO Rats)

  • This study investigated the effect of Yanggyuksanhwa-tang on cerebral ischemia of the rats. Considering age-related impact on cerebral ischemia, aged rats (18 months old) were used for this study. Ischemic damage was induced by the transient occlusion of bilateral common carotid arteries(BCAO) under the hypotension. Yanggyuksanhwa-tang was administered twice orally. Then changes of immunohistochemical expression of c-fos and c-jun in ischemic damaged hippocampus were observed. The BCAO in aged rats led significant increase of c-fos expression in CA1 and DG of hippocampus. While the treatment of Yanggyuksanhwa-tang significantly attenuated the increase of c-fos expression in CA1 hippocampus following BCAO ischemia. Depending on changes of the normalized optical density(NOD) of immunohistochemical c-fos expression, the treatment of Yanggyuksanhwa-tang significantly attenuated the increase of NOD in CA1 and DG of hippocampus. And there was not changes in CA2 and CA3 hippocampus with respect to the control BCAO group. The BCAO in aged rats led significant increase of c-jun expression in CA1 hippocampus. While the treatment of Yanggyuksanhwa-tang significantly attenuated the increase of c-jun expression in CA1 hippocampus following BCAO ischemia. Depending on changes of the NOD of immunohistochemical c-jun expression, the treatment of Yanggyuksanhwa-tang significantly attenuated the increase of NOD in CA1 hippocampus. And there was not changes in CA2, CA3 and DG of hippocampus with respect to the control BCAO group.

Calcitonin Gene Related Peptide에 의한 치수미세순환 조절 (REGULATION OF PULPAL MICROCIRCULATION BY CALCITONIN GENE-RELATED PEPTIDE)

  • 본 연구에서는 감각성 neuropeptide인 CCRP의 치수혈류 조절에 관해 교감신경과의 유기적 관계를 연구함으로써 CGRP의 치수혈류 조절기전을 밝히고자 하였다. 열두 마리의 전신마취된 고양이에서 실험하였으며 CGRP를 혈관을 통해 전신적 또는 국소적으로 투여하였다. 견치에서 치수혈류의 변화를 측정하고 paired t-test로 통계분석 하였으며 $95\%$ 수준에서 유의성을 검증하였다. CGRP $(0.31{\mu}g/kg)$를 전신정맥으로 주사시, 전신혈압에 현저한 영향을 나타내면서 치수혈류는 평균 $68.85\%$의 일차적인 증가와 감소를 보였고 이차적으로 다시 평균 $161.8\%$ 증가하였다가 감소하였다. CCRP를 저용량 $(0.03{\mu}g/kg)$으로 국소적으로 투여시, 치수혈류는 평균 $2.92\%$의 미약한 증가를 나타내었다. 교감신경을 전기자극시 (10Hz, 4V, 1.5ms), 전신혈압은 영향을 받지 않으면서 치수혈류가 유의하게 평균 $57.88\%$ 감소하였다. 교감신경 자극으로 치수혈류가 저하되어 있는 동안 주입한 CCRP는 저하된 치수혈류를 유의하게 회복시켰다. CGRP의 이 치수혈류 증가 효과는 $CGRP_{8-37}$에 의해 효과적으로 차단되었다.

Cerebral Arterial Stenosis in Patients with Spontaneous Intracerebral Hemorrhage

  • Objective : Spontaneous intracerebral hemorrhage (ICH) and ischemic stroke share common vascular risk factors such as aging and hypertension. Previous studies suggested that the rate of recurrent ICH and ischemic stroke might be similar after ICH. Presence of cerebral arterial stenosis is a potential risk factor for future ischemic stroke. This study investigated the prevalence and factors associated with cerebral arterial stenosis in Korean patients with spontaneous ICH. Methods : A total of 167 patients with spontaneous ICH were enrolled. Intracranial arterial stenosis (ICAS) and extracranial arterial stenosis (ECAS) were assessed by computed tomography angiography. Presence of ICAS was defined if patients had arterial stenosis in at least one intracranial artery. ECAS was assessed in the extracranial carotid artery. More than 50% luminal stenosis was defined as presence of stenosis. Prevalence and factors associated with presence of ICAS and cerebral arterial stenosis (presence of ICAS and/or ECAS) were investigated by multivariable logistic regression analysis. Results : Thirty-two (19.2%) patients had ICAS, 7.2% had ECAS, and 39 (23.4%) patients had any cerebral arterial stenosis. Frequency of ICAS and ECAS did not differ among ganglionic ICH, lobar ICH, and brainstem ICH. Age was higher in patients with ICAS ($67.6{\pm}11.8$ vs. $58.9{\pm}13.6years$ p=0.004) and cerebral arterial stenosis ($67.9{\pm}11.6$ vs. $59.3{\pm}13.5years$, p<0.001) compared to those without stenosis. Patients with ICAS were older, more frequently had diabetes, had a higher serum glucose level, and had a lower hemoglobin level than those without ICAS. Patients with cerebral arterial stenosis were older, had diabetes and lower hemoglobin level, which was consistent with findings in patients with ICAS. However, patients with cerebral arterial stenosis showed higher prevalence of hypertension and decreased kidney function compared to those without cerebral arterial stenosis. Multivariable logistic regression analyses showed that aging and presence of diabetes independently predicted the presence of ICAS, and aging, diabetes, and hypertension were independently associated with presence of cerebral arterial stenosis. Conclusion : 19.2% of patients with spontaneous ICH had ICAS, but the prevalence of ECAS was relatively lower (7.2%) compared with ICAS. Aging and diabetes were independent factors for the presence of ICAS, whereas aging, hypertension, and diabetes were factors for the cerebral arterial stenosis.
