Tri-ramification of left external carotid artery associated with anatomical variation of its branches and aneurysm formation

  • Essential sources of arterial vascularisation in the head and neck region are the left and right common carotid arteries (CCA) and their branches. The left CCA (LCCA) originates from the arch of the aorta and the right CCA originates from the brachiocephalic trunk. In this case report, there was a bilateral higher division of CCA at the plane of the greater cornua of the hyoid bone, unilateral tri-ramification of the LCCA and the left external carotid artery (LECA), and the origin of the linguo-facial trunk and the pharyngo-occipital trunk from the LECA. An aneurysm formed in the distal part of LECA before its termination. In this case, we propose a novel categorization called the punnapatla classification for the anatomical variance branching forms of ECA. These kinds of variations are important to the surgeons, and anaesthetists, during the surgeries of the head and neck.

상안정맥을 이용한 해면정맥동루의 색전술 치험례 (Treatment of a Carotid-Cavernous Sinus Fistula via the Superior Ophthalmic Vein Approach: A Case Report)

  • Purpose: Arteriovenous fistulas that involve the cavernous sinus often produce ophthalmologic symptoms and signs. Transvenous endovascular access is the method of choice for a carotid-cavernous sinus fistula. The superior ophthalmic vein is a safe and reasonable alternative route for the transvenous embolization of carotid-cavernous sinus fistula. We report a case of the embolization of a carotid-cavernous sinus fistula using the superior ophthalmic vein approach. Methods: A 58 year old female had conjunctival congestion, periocular pain and diplopia with a 2 month duration. Diagnostic orbital CT, brain MRI and cerebral angiography revealed a carotid-cavernous sinus fistula. The fistula occlusion was treated by coil embolization using the superior ophthalmic vein approach. Results: The initial presenting symptoms, conjunctival congestion, periocular pain and diplopia, decreased after surgery. Coil embolization via the superior ophthalmic vein approach was difficult because of the venous tortuosity and friability. During the follow up period, the patient was in a good condition without complications. Conclusion: Surgical exposure of the superior ophthalmic vein provides direct venous access to the cavernous sinus as well as an effective and safe treatment approach. The cooperation of the plastic surgeon and interventionist is a factor in successful treatment.

Bilateral Superior Cerebellar Artery Infarction after Stent-Angioplasty for Internal Carotid Artery Stenosis

  • Spontaneous bilateral cerebellar infarction in the territory of the superior cerebellar arteries is extremely rare. Occasionally there have been reports of bilateral cerebellar infarction due to vertebrobasilar atherosclerotic occlusion or stenosis, whereas no report of bilateral cerebellar infarction due to complicated hemodynamic changes. In this report, we present a patient with bilateral cerebral infarctions related to stenoses of bilateral internal carotid arteries, in whom vertebrobasilar system was supplied by multiple collaterals from both posterior communicating arteries and right external carotid artery. We performed stent-angioplasty of bilateral internal cerebral arterial stenosis, and then acute infarction developed on bilateral superior cerebellar artery territories. The authors assumed that the infarction occurred due to hemodynamic change between internal carotid artery and external carotid artery after stent-angioplasty for stenosis of right internal carotid artery.

  • Objective : This study was carried to make out the connection between cerebral artery blood flow velocity and ischemic theory that presumed the cause of Bell's palsy. Method : We measured cerebral artery blood flow velocity each external carotid artery, internal carotid artery, common carotid artery, siphon, superficial temporal artery by TCD to 20 patients who diagnosed as facial nerve palsy from march 2001 to July 2001 and all objectives devided two groups as palsy side. A group is right side facial nerve palsy and B group is left facial nerve palsy. Results : 1. There is no effective change of blood flow in external carotid artery either A, B group. 2. There is no effective change of blood flow in internal carotid artery either A, B group. 3. There is no effective change of blood flow in common carotid artery either A, B group. 4. There is no effective change of blood flow in siphon artery either A, B group. 5. There is no effective change of blood flow in superficial temporal artery either A, B group.

  • Carotid cavernous sinus fistula(CCSF) is an abnormal communication at the base of the skull between the internal carotid artery and the cavernous sinus. Fistula is almost associated with extensive facial trauma as a result of direct or indirect forces. Most fistulas of traumatic origin develop as a result of fractures through the base of the skull, which cause the laceration of the internal carotid artery near the cavernous sinus. The signs and symptoms of CCSF are pulsating exophthalmosis, orbital headache, pain, orbital or frontal bruit, loss of visual acuity, diplopia and ophthalmoplegia. Angiography reveals a definite CCSF and a detachable balloon embolization is known to be the treatment of choice. Even though carotid cavernous sinus fistula is an uncommon complication after orthognathic surgery, several cases of CCSF due to congenital anomalies, pre-existing aneurysms and abnormally thickened maxillary posterior wall have been reported in the literature. We have experienced a case of CCSF after Le Fort I osteotomy for maxillary advancement in skeletal class III patient and the cause, pathogenesis, diagnosis and treatment of this case.

The Comparison of Carotid Artery Intima-media Thickness and Plaque Characteristics between Patients with ST-elevation Myocardial Infarction and Coronary Artery Chronic Total Occlusion

  • ST-segment elevation myocardial infarction (STEMI) and chronic total occlusion (CTO) of coronary artery are well-known atherosclerotic vascular diseases. However, the difference of intima-media thickness and plaque characteristics of carotid arteries between STEMI and CTO patients were not directly compared in previous studies. Medical records of a total of 158 (101 STEMI, 57 CTO) patients, who underwent carotid artery ultrasonography, were selected for the analysis. The baseline characteristics, ultrasonography findings, and clinical outcomes of the two groups were compared. The prevalence of hypertension, diabetes mellitus, and dyslipidemia was significantly higher in CTO patients. Carotid intima-media thickness ($0.97{\pm}0.13$ vs. $0.78{\pm}0.17cm$, P < 0.0001) and number of plaques ($2.2{\pm}1.0$ vs. $1.7{\pm}1.2$, P < 0.0001) were greater in CTO than STEMI patients. Multiple (${\geq}3$) or echogenic plaques were more frequently observed in CTO patients. During the median follow-up duration of 27 months, major adverse cardiovascular events occurred in 31% of CTO and 14% of STEMI patients (P = 0.008). We found that, compared with STEMI, CTO patients have higher burden of carotid artery atherosclerosis associated with more comorbid diseases and poor clinical outcomes.

머리의 전방 이동이 경동맥과 추골동맥 혈류량의 변화에 미치는 영향: 사례연구 (Effect of Blood Flow on Carotid and Vertebral Artery During Forward Shift on Head: Case Study)

  • Background: The purpose of this study was to identify the effects of the blood flow on carotid and vertebral artery during the forward shift on head. Methods: A 20-year female healthy subject participated in this study. This study was set up the forward shift on head at the 3cm and 6cm in a shoulder midline (acromion). Measurement method were using duplex ultrasound with colour doppler imaging for the blood flow on carotid and vertebral artery. Results: Carotid artery was increased the blood flow and vertebral artery was decreased the blood flow during forward shift on head. Conclusions: These findings suggest that carotid and vertebral artery changes to the blood flow during forward shift on head. Therefore, we should be consider that hemodynamic factor when apply to the therapeutic exercise for patients of forward head posture.

  • This study was done to identify correlates of carotid artery ultrasonography's measurement and Inyoung pulse in college students. We measured the amplitude of Inyoung pulse, Chongu pulse, ratio of Inyoung to Chongu and ratio of Chongu to Inyoung on 30 college students. Also, We measured the Distance, Diameter), RI(resistivity index), S/D(systolic, diastolic ratio), PI(pulsatility index), PSV(peak systolic velocity), EDV(End diastolic velocity), Vmean using carotid artery ultrasonography. The data was analyzed by Pearson's correlation coefficient using SAS program. The results were as follow. Results showed a positive correlation between Inyoung pulse and diameter by carotid artery. It showed a positive correlation between Inyoung pulse and S/D. Also, It showed a positive correlation between Inyoung pulse and PSV. As a result, the strength of Inyoung pulse related with the diameter of carotid artery and blood flow velocity.

무맥증 수술치험 2례 (Pulseless Disease: Report of Two Cases)

  • A patient was 29 year old house wife who was admitted to the Yonsei University Medical Center on 6 th of Nov., in 1970, with chief complaints of complete loss of bilateral visual acuity, generalized weakness, frequent palpitations, claudication of masseter muscles and intermittent fainting. These symptoms were developed 5 years prior to admission. Physical finding on admission revealed complete loss of bilateral visual acuity, absence of both radial and carotid pulse, but there was good femoral and popliteal pulse. She couldn't open her mouth as she desired and had weakness of mastication. Radiological findings of plane chest PA were not significant. Aortogram(Cineangiogram) showed non-visualization of both carotid and subclavian arteries. It showed only innominate and interal mammary artery preoperatively. The operative findings were as follows: There were complete obliterative changes in both common carotid and subclavian arteries, and periarteritis in the innominate artery. Tube Dacron Prosthesis Bypass with V-arm between innominate artery and both common carotid arteries was performed after thromboendarterectomy. Histopathological finding of the thromboend arterectomy specimen was compatible with pulseless disease, which showed marked fibrous thickening of intima and a diffuse inflammatory cell infiltration of the whole layers. Her postopererative course was uneventful. Follow up aortogram(Cineangiogram) was taken on 11th postoperative day, which revealed both common carotid arteries patent. Her preoperative Subjective symtoms disappeared remarkably, such as her visual acuity improved much, fainting and vertigo disappeared completely and she can go to bath room without difficulty and help. Another patient was 34 year old house wife who was admitted on August, 1964 with chief complaints of frequent fainting and progresive visual loss. She was operated only thromboendarterectomy of both common carotid arteries. Postoperative course was smooth and subjective symptoms were disappeared.

Histomorphological changes in the common carotid artery of the male rat in induced hypogonadism

  • The role of androgens in the development of cardiovascular diseases remains controversial. The current study therefore sought to determine the changes in the histomorphology of the common carotid artery of the male rat in orchidectomy-induced hypogonadism. Twenty-two Rattus norvegicus male rats aged 2 months were used. The rats were randomly assigned into baseline (n=4), experimental (n=9), and control (n=9) groups. Hypogonadism was surgically induced in the experimental group by bilateral orchiectomy under local anesthesia. At experiment weeks 3, 6, and 9, three rats from each group (experimental and control) were euthanized, their common carotid artery harvested, and routine processing was done for paraffin embedding, sectioning, and staining. The photomicrographs were taken using a digital photomicroscope for morphometric analysis. Orchidectomy resulted in the development of vascular fibrosis, with a significant increase in collagen fiber density and decrease in smooth muscle and elastic fiber density. Moreover, there was development of intimal hyperplasia, with fragmentation of medial elastic lamellae in the common carotid artery of the castrated rats. Orchidectomy induces adverse changes in structure of the common carotid artery of the male rat. These changes may impair vascular function, therefore constituting a possible structural basis for the higher incidences of cardiovascular diseases observed in hypogonadism.
