Objectives : The purpose of this study is to evaluate the computed tomographic (CT) images of the paranasal sinusitis(PNS). Materials and Methods : The author examined the extent and recurring patterns of the paranasal sinusitis and some important anatomic landmarks. The author analyzed PNS images retrospectively in 500 patients who visited Chonbuk National University Hospital between January 1996 and December 1997. Results : The most frequently affected sinus was maxillary sinus (82.9%), followed by anterior ethmoid sinus(67.9%), posterior ethmoid sinus(48.9%), frontal sinus(42.0%) and sphenoid sinus(41.4%). The characteristic features of CT images of the sinusitis were sinus opacification(22.4%), mucoperiosteal thickening(34.3%), and polyposis(2.0%). Sinonasal inflammatory diseases were categorized into 5 patterns according to Babber s classification. They were 1) infundibular(13.0%), 2) ostiomeatal unit(67.4%), 3) sphenoethmoidal recess (13.0%), 4) sinonasal polyposis (9.6%) and 5) unclassifiable patterns(18.0%). The incidences of contact between sinus and optic nerve were as follows ; the incidences of contact with posterior ethmoid sinus, sphenoid sinus. both posterior sinuses were 11.4%. 66.8%, 6.3%. respectively. The incidences of contact between sphenoid sinus and maxillary nerve, vidian nerve, internal carotid artery were 74.5%. 79.2%. 45.1%. respectively. The incidences of pneumatization of the posterior ethmoid sinus were as follows ; normal 70.6% and overriding type 29.4%. The incidences of sphenoid sinus pneumatization were as follows; normal 56.9% , rudimentary 12.5%, pterygoid recess 22.7%, anterior clinoid recess 2.7%, and both pterygoid and anterior clinoid recess type 5.2%. Conclusions : The inflammatory sinonasal diseases were classified into five patterns using the CT of PNS, which was proven to be an excellent imaging modality providing detailed information about mucosal abnormality, pathologic patterns, and the proximity of the important structures to the posterior paranasal sinuses. This result will aid in the interpretation of CT of PNS functionally and systemically.
Objectives : The purpose of this study was to compare cerebral biood flow between hypertensives and normotensives using transcranial doppler ultrasonography (TCD). Methods : I investigated cerebral blood flow of 72 hypertensives and 127 normotensives. To evaluate the cerebral blood flow, I measured the systolic peak velocity(Vs) and mean How velocity (Vm) of the middle cerebral artery(MCA), anterior cerebral artery(ACA), posterior cerebral artery(PCA), basilar artery(BA), and internal carotid artery(ICA) in the two groups using TCD. Result : 1. There was a decrease in the Vs and Vm of all examined vessels of hypertensives in comparison with normotensives. There was a significant difference in the Vs of ACA and Vm of ACA, PCA, ICA. 2. In males, there was a decrease in the Vs of ACA, PCA, ICA and Vm of MCA, ACA, PCA, ICA of hypertensives in comparison with normotensives. However, there was no significant difference in the Vs or Vm of all examined vessels. 3. In females, there was a decrease in the Vs and Vm of all examined vessels of hypertensives in comparison with normotensives. There was a significant difference in the Vs of MCA, ACA and BA and Vm of ACA, PCA and BA. 4. In 30-49 year-olds, there was a decrease in the Vs and Vm of all examined vessels of hypertensives in comparison with normotensives. There was a significant difference in the Vs of ACA and Vm of ACA. 5. In 50-69 year-olds, there was a decrease in the Vs of ACA, PCA, BA, ICA and Vm of all examined vessels of hypertensives in comparison with normotensives. However, there was no significant difference in the Vs or Vm of all examined vessels. 6. In 70-89 vests old. there was a decrease in the Vs. Vm of PCA, BA, ICA of hypertensives in comparison with normotensives. But, there was no significant difference in the Vs, Vm of all examined vessels. Conclusions : There was a significant difference in the cerebral blood now velocity between hypertensives and normotcnsives. These results suggest that blood pressure has influence on cerebral blood flow.
Disappearance rate of injected $D_2O$ from the arterial blood as well as the effect of histamine on the rate were studied in rabbits. The concentrations of $D_2O$ in the serial arterial samples obtained through a Polyethylene tubing inserted into the carotid artery were assayed by the freezing point elevation method of Reaser. At zero time 3 ml of isotonic $D_2O$ in normal saline was injected into the jugular vein and at the same time serial sampling of arterial blood started. The serial sampling interval was either 7.7 sec or 12.3 sec. In the histamine treated animals histamine diphosphate (0,5 mg of histamine base) was injected intravenously 30 minutes prior to the zero time. The following results were obtained. 1. $D_2O$ concentration in arterial plasma water, x, was empirically obtained by the sum of 2 exponential terms of time, $x=Ae^{-k1t}+Be^{-k2t},$ and its theoretical basis was sought. The first term of the right member of the equation was regarded to be attributable to the compartment P which possessed instantaneous exchange of water with plasma. The second term was postulated to represent the poorly exchangeable compartment. 2. The constant A of the equation was evaluated as 4,37% and 14.3% in the control and histamine treated groups, respectively. B was 1.19% in the control and 0.849% in histamine treated animals. 3. The disappearance rates determined were; $k_1=0.0519{\pm}0.0221\;sec^{-1}\;K_2=0.00454{\pm}0.00247\;sec^{-1}$ in the control group. $k_1=0.1137{\pm}0.0290\;sec^{-1}\;K_2=0.00499{\pm}0.00204\;sec^{-1}$ in the histamine group. 4. In the histamine treated animals the disappearance rate of the first term was larger than that of the control animals, suggesting an enlarged size of the rapidly exchangeable compartment with regard to the plasma water. On the other hand the constant B was decreased by histamine administration, suggesting a distribution of $D_2O$ in an enlarged volume. This view was also made clear by comparing the apparent asymptotes to which the concentration curves of $D_2O$ approached in respective groups. The asymptotes in the histamine treated group showed lower values.
Objectives : This study was investigated the effects of the root of Polygala tenuifolia (POL) on learning and memory impairment induced by chronic cerebral hypoperfusion in rats. Methods : Chronic cerebral hypoperfusion was produced by permanent bilateral common carotid artery occlusion (pBCAO). POL was administered orally once a day (130 mg/kg of water-extract) for 28 days starting at 4 weeks after the pBCAO. The acquisition of learning and the retention of memory were tested on 9th week after the pBCAO using the Morris water maze. In addition, effects of POL on $A{\beta}$ generation and expressions of APP and BACE1 were observed in the hippocampus of rats. Results : POL significantly prolonged the swimming time spent in target quadrant and significantly reduced the swimming time spent in the quadrant far from the target. POL significantly increased the percentage of swim in the targer quadrant in the retention test, while POL was not effective on the escape latencies in the acquisition training trials. POL significantly reduced the levels of $A{\beta}_{(1-40)}$ and $A{\beta}_{(1-42)}$ in the cerebral cortex and the level of $A{\beta}_{(1-42)}$ in the hippocampus produced by chronic cerebral hypoperfusion. POL also significantly attenuated the up-regulation of APP and BACE1 expression in the hippocampus produced by chronic cerebral hypoperfusion. Conclusions : The results show that POL alleviated memory deficit and up-regulation of $A{\beta}$ and BACE1 expressions in the hippocampus. This result suggests that POL may exert ameliorating effect on memory deficit through inhibition of ${\beta}$-secretase activity and $A{\beta}$ generation.

Objective : To analyze trends in the incidence and treatment of diseases associated with ischemic stroke, namely, cerebral infarction (CI), cerebral arterial stenosis (CASTN), and moyamoya disease (MMD), based on Korean National Health Insurance Service (NHIS) data from 2008 to 2016. Methods : Data was extracted from the national health-claim database provided by the NHIS for 2008-2016 using International Classification of Diseases codes. The crude and age-standardized incidences of each disease (CI, CASTN without a history of CI, and MMD) were calculated; additional analyses were conducted according to age and sex. Trends in the number of patients undergoing treatment according to treatment method were analyzed for each disease using the Korean Classification of Diseases procedure codes. Results : In 2016, the total number of adults with newly diagnosed CI was 83939, reflecting a 9.4% decrease from that in 2008. The age-standardized incidence of CI in adults was 153.2 per 100000 person-years in 2016, reflecting a 37.2% decrease from that in 2008, while that of CASTN was 167.3 per 100000 person-years in 2016, reflecting a 73.3% increase from that in 2008. Among treated cases, the number of patients who underwent intra-arterial (IA) treatment, including IA fibrinolysis and mechanical thrombectomy, showed the most prominent increase, increasing at an annual rate of 25.8%. For CASTN, the number of cases treated with carotid artery stenting or balloon angioplasty (CAS) showed the most prominent increase, increasing at a rate of 69.8% over the 9-year period. For MMD, the total number of patients with newly diagnosed MMD and that with adult MMD demonstrated significantly increasing trends, while the number of pediatric patients with newly diagnosed MMD declined by 18.0% over the 9-year period. The age-standardized incidences of pediatric and adult MMD in 2016 were 2.4 and 3.4 per 100000 person-years, respectively. Conclusion : Although the incidence of CI showed a declining trend over a 9-year period, the number and proportion of patients treated for CI increased. Meanwhile, the incidence of CASTN and the number of patients treated for CASTN have demonstrated increasing trends since 2008. On the other hand, the number of patients diagnosed with pediatric MMD decreased, despite no significant change in the incidence. In contrast, the number of patients and the incidence of adult MMD increased. These trends reflect changes in the population structure, gains in the accessibility of imaging examinations, and the development of endovascular techniques.
The present study was aimed to explore the neuroprotective role of imatinib in global ischemia-reperfusion-induced cerebral injury along with possible mechanisms. Global ischemia was induced in mice by bilateral carotid artery occlusion for 20 min, which was followed by reperfusion for 24 h by restoring the blood flow to the brain. The extent of cerebral injury was assessed after 24 h of global ischemia by measuring the locomotor activity (actophotometer test), motor coordination (inclined beam walking test), neurological severity score, learning and memory (object recognition test) and cerebral infarction (triphenyl tetrazolium chloride stain). Ischemia-reperfusion injury produced significant cerebral infarction, impaired the behavioral parameters and decreased the expression of connexin 43 and phosphorylated signal transducer and activator of transcription 3 (p-STAT3) in the brain. A single dose administration of imatinib (20 and 40 mg/kg) attenuated ischemia-reperfusion-induced behavioral deficits and the extent of cerebral infarction along with the restoration of connexin 43 and p-STAT3 levels. However, administration of AG490, a selective Janus-activated kinase 2 (JAK2)/STAT3 inhibitor, abolished the neuroprotective actions of imatinib and decreased the expression of connexin 43 and p-STAT3. It is concluded that imatinib has the potential of attenuating global ischemia-reperfusion-induced cerebral injury, which may be possibly attributed to activation of JAK2/STAT3 signaling pathway along with the increase in the expression of connexin 43.
Aconiti tuber butanol fraction, which is isolated from the chloroform insoluble and water soluble extract of Aconitum volubile, has been recently known to have a potent positive inotropic effect in the isolated cardiac muscle preparations of various animals. The positive inotropic mechanism of Aconiti tuber butanol fraction, in relation with the external calcium, was studied using the isolated cat papillary muscle. The positive inotropic effect was dependent on the calcium concentration in the nutrient medium, and a synergistic relation could be demonstrated between Aconiti tuber butanol fraction and the external calcium. The inotropic effect of $10^{-4}g/ml$ of Aconiti tuber butanol fraction was equivalent to that of 0.06mM of calcium in the medium. After the treatment with a calcium influx inhibitor, Verapamil$(2{\pm}10^{-7}-10^{-6}M)$, the contractile force of the papillary muscle was markedly inhibited. In these preparations, Aconiti tuber butanol fraction restored the decreased contractility in a dose-dependent manner. It was suggested that the positive inotropic effect of Aconiti tuber butanol fraction might be related with the stimulating action on the calcium influx through the slow inward calcium channels in the cardiac cell membrane. In contrast with digitalis cardiac glycoside, Aconiti tuber butanol fraction infused intravenously into the anesthetized rabbit decreased the systemic arterial blood pressure and increased the carotid blood flow, but produced no prominent changes in the heart rate.
By measuring changes in blood lactate and plasma enzyme (CPK, GOT, GPT) with electrical stimulation applied at two duty cycles, this study is intended to look into which type of duty cycle may have more effects on blood lactate and plasma enzyme constituents through animal experiment so as to determine any duty cycle appropriate for electrical treatment. In this study, electrical stimulation was applied to total 20 Korean house rabbits (weight: 3~3.5 kg) by means of an electrical therapeutic apparatus called TS6000 (made in Netherlands) at duty cycle of 50% and 20% respectively for 30 minutes. Here, 5 cc of blood was collected from their carotid artery before stimulation and in 30 minutes after stimulation respectively to carry out biochemical experiment and analysis. As determined through the above experiment, blood lactate rate was increased to 333.07% at 50% duty cycle after experiment and 185.71% at 20% duty cycle after experiment respectively. In both cases, blood lactate rate was significantly increased to higher level after electrical stimulation than before. Moreover, the rate of change in the average of blood lactate rate at both duty cycles also showed significant differences. CPK rate was boosted to 301.82% at 50% duty cycle after experiment and 321.35% at 20% duty cycle after experiment respectively. In both cases, CPK rate was remarkably boosted to higher level after stimulation than before (p<.05). However, there was not any significant difference in the rate of change in average CPK at both duty cycles (p<.05). GOT rate was significantly boosted up to 38.97% at 50% duty cycle after experiment (p<.05), while it was slightly increased to 1.68% at 20% duty cycle after experiment without any significant difference. Rather, GPT rate dropped slightly at both duty cycles after experiment, but there was not any significant difference. Although blood lactate and GOT were relatively less generated at 20% duty cycle after electrical stimulation than at 50% duty cycle, the change of duty cycle didn't have any significant influence on CPK rate. In this regard, this study failed to come any consistent conclusion about the association between change of duty cycle and muscle fatigue. Therefore, it is advisable that follow-up studies seek various ways to a little more effectively apply electrical stimulation to laboratory animals by avoiding their muscle fatigue. GOT rate was significantly boosted up to 38.97% at 50% duty cycle after experiment (p<.05), while it was slightly increased to 1.68% at 20% duty cycle after experiment without any significant difference. Rather, GPT rate dropped slightly at both duty cycles after experiment, but there was not any significant difference. Although blood lactate and GOT were relatively less generated at 20% duty cycle after electrical stimulation than at 50% duty cycle, the change of duty cycle didn't have any significant influence on CPK rate. The purpose of this study is to evalute the diagnostic value of the brain scanning and compare the diagnostic accuracy between the scan and carotid angiography. 109 cases which are proved by specific method to each disease, are analized to evalute the diagnostic value of the brain scanning. The 70 cases among the proven 109 case are performed both the scanning and the arteriography and analized to compare the accuracy between the scanning and the arteriography. The results are as follows; 1. The diagnostic accuracy of the brain scanning in the diseases of the central nervous system is 64.2%. 2. The diagnostic accuracy of the brain scanning in the brain tumor is 88%, especially brain abscess. glioma, glioblastoma multiforme, menirgioma and metastic tumor show high positive rate. 3. The diagnostic accuracy in the disease of the brain vessels is 54 %. 4. The comparison of the diagnostic value between the scanning and the arteriography is as follows; 1) The diagnostic value in all diseases of the central nervous system is nearly equal. 2) The diagnostic accuracy in the intracranial tumor is slightly higher in the brain scanning (90.9%) than in the arteriography (81.8%). 3) The diagnostic accuracy in the disease of the brain vessel is higher in the arteriography (77.3%) than in the scanning (54.5%). 5) The diagnostic value when combining the scanning and the arteriography, is 83% in the all central nervous system-lesions, 97% in the cranial tumor and 81.8% in the disease of the central nervous system-vessel. The brain scanning is simple and safe procedure, and moreover has excellent diagnostic value in the diagnosis of the central nervous system lesion.
The pectoralis major myocutaneous flap represents a major contribution to head and neck cancer reconstruction. Its advantages are improved viability, one-stage reconstruction, and carotid protection. The oropharyngeal defect especially tonsillar area reveals valley shaped one with loss of a wide mucosal area. Using pectoralis major myocutaneous flap to this defect is sometimes difficult due to its natural figure of bulkiness. This article reviews our experience with patients undergoing 14 pectoralis major myocutaneous flap in carcinoma of the tonsillar area. Complications and their incidences were I total loss, 3 marginal loss, 2 minor seperation of suture, I wound infection and 2 hematoma. Most of the complications did not require a second procedure for reconstruction. Bulkiness of the flap and gravity force to the upper suture line were thought to be causes of the complications. Modification of the flap design with bilobular figure was useful to reduce its bulkiness at the folding area. More stable suture around hard palate was needed to overcome seperation of the suture.

일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
