경동맥 초음파진단과 관련된 임상변수에 대한 분석 (Analyzation of Correlation between Clinical Factors and Carotid Ultrasonography Diagnosis)

  • 동맥경화는 혈관이 좁아지면서 탄력을 잃어 혈액 순환 장애가 발생하는 질병으로, 최근 증가하는 심혈관 및 뇌혈관 질환의 잠재적 원인이 된다. 경동맥초음파 검사는 동맥경화 정도를 평가하여 심혈관 및 뇌혈관 질환의 예측인자로 사용되고 있다. 이에 본 논문은 경동맥초음파 검사에서 내중막 두께 증가와 여러 임상변수들 간 상관성을 알아보고자 하였다. 경동맥초음파검사 소견을 가지고 있는 환자를 대상으로 경미한 내중막 두께 비후, 경화반, 유의한 협착 3단계로 분류하였다. 경동맥의 동맥경화 정도와 신체적 특성(성별, 연령, 신체질량지수법, 혈압), 혈액검사(총콜레스테롤, 중성지방, 고밀도지단백질, 저밀도지방단백질, 크레아틴포스포키나제, 공복혈당)와 동맥경화도 검사에서 계측된 CAVI(Carotid Ankle Vascular Index : 혈관연령) 데이터를 수집하였다. 경동맥 내중막 두께는 고혈압, 고지혈증, BMI 등의 변수와 상관관계가 있었으며, 또한 CAVI가 증가될수록 심혈관계 질환의 위험요인과 상관관계가 있음을 확인하였다.

Impact of Multiple Cardiovascular Risk Factors on the Carotid Intima-media Thickness in Young Adults: The Kangwha Study

  • Objectives: Although risk factors for coronary artery disease are also associated with increased carotid intima-media thickness (IMT), there is little information available on the asymptomatic, young adult population. We examined the association between multiple cardiovascular risk factors and the common carotid IMT in 280 young Korean adults. Methods: The data used for this study was obtained from 280 subjects (130 men and 150 women) aged 25 years who participated in the Kangwha Study follow-up examination in 2005. We measured cardiovascular risk factors, including anthropometries, blood pressure, blood chemistry, carotid ultrasonography, and reviewed questionnaires on health behaviors. Risk factors were defined as values above the sex-specific 75th percentile of systolic blood pressure, body mass index, total cholesterol/ high-density lipoprotein cholesterol ratio, fasting blood glucose and smoking status. Results: The mean carotid IMT${\pm}$standard deviation observed was $0.683{\pm}0.079mm$ in men and $0.678{\pm}0.067mm$ in women (p=0.567) and the evidence of plaque was not observed in any individuals. Mean carotid IMT increased with an increasing number of risk factors(p for trend <0.001) and carotid IMT values were 0.665 mm, 0.674 mm, 0.686 mm, 0.702 mm, and 0.748 mm for 0, 1, 2, 3, and 4 to 5 risk factors, respectively. The odds ratio for having the top quartile carotid IMT in men with 3 or more risk factors versus 0-2 risk factors was 5.09 (95% CI, 2.05-12.64). Conclusions: Current findings indicate the need for prevention and control of cardiovascular risk factors in young adults and more focus on those with multiple cardiovascular risk factors.

  • In the present study, the characteristics of blood flow inside a carotid artery numerically investigated with shear rate specific blood viscosity. To simulate the blood flow with a patient-specific arterial geometry, the geometry of a carotid artery was constructed from 2D rain MRA data. The measured data of blood flow velocity at the common carotid artery were used as boundary conditions of the simulation. For the blood rheology data to be used in the simulation, the patient specific blood viscosity over the whole ranges of shear rate was obtained using $BioVisco^{TM}$. From the numerical results of the blood flow in the carotid artery, the increase of blood viscosity and the decrease of wall shear stress could be found in the carotid bifurcated region, more specifically at the post-plaque dilated region. These characteristics of blood viscosity and wall shear stress can be used more precisely and efficiently to predict the region vulnerable to plaque growht or thrombosis on top of the plaque.

경동맥 죽상경화반의 고해상도 자기공명영상 (High-Resolusion Magnetic Resonance Imaging of Carotid Atherosclerotic Plaque)

  • A thromboembolic stroke is believed to be precipitated by a rupture of vulnerable atheromatous plaques. Until recently the assessment of a further risk of stroke in high-risk patients in whom atherosclerosis has presented with a transient ischaemic attack (TIA), has been confined to a quantitative assessment of the luminal patency of the internal carotid artery. These traditional stratification parameters are no longer believed to be the most accurate predictors of a thrombo-embolism. This is because the process of vessel wall remodeling can maintain a luminal patency, and consequently, quite large friable plaques may remain unidentified. Accordingly, there is a need for an improved risk assessment. The fibrous cap of a vulnerable plaque is thinner, and an intraplaque hemorrhage and inflammation can occur during the development of atherosclerotic plaque. Several imaging methods for identifying vulnerable plaques have been developed. Recently, high resolution magnetic resonance (MR) imaging has emerged as an accurate non-invasive tool that can characterize the carotid plaque components in vivo. A High resolution carotid magnetic resonance is capable of distinguishing an intact, thick fibrous cap from a thin and ruptured cap in carotid plaque. In addition, a plaque MR can identify the active inflammation and detect a hemorrhage. High resolution carotid MR imaging is a valuable noninvasive method for quantifying the plaque components and identifying vulnerable plaque.

경동맥 경화판의 고해상도 자기공명영상 (High-resolution MR Imaging of Carotid Atherosclerotic Plaques)

  • 고해상도 경동맥 자기공명영상(MRI)을 이용하여 경화판 특성을 파악할 수 있다. MRI는 경화판의 활동성 염증이나 경화판내 출혈을 비침습적으로 진단할 수 있는 능력이 있다. 3T MRI는 1.5T MRI에 비해 신호 대 잡음비와 대조도 대 잡음비가 높다. 동맥벽의 면적이나 표준화된 동맥벽 면적을 MRI로 측정하면 약물 치료 후 반응을 평가할 수 있다. 결론적으로 고행상도 MRI는 일과성 허혈이나 뇌졸중을 발생하기 쉬운 경화판의 진단과 치료 후 평가에 유용하다.

진행된 경부 전이암에서 경동맥 절제를 위한 술전검사와 절제술의 의의 (Preoperative Evaluation and Significance of Carotid Resection in Advanced Cervical Metastatic Cancer)

  • Objectives: We studied what is the role of elective carotid artery resection in the management of advanced cervical metastatic cancer. Materials and Methods: 5 patients with elective carotid artery resection in advanced metastatic cervical cancer were reviewed retrospectively. The patients underwent complete neuroradiologic evaluation, including CT/MRI. angiography, duplex doppler, balloon occlusion test with EEG, and brain SPECT for determination of compatible collateral circulation after carotid artery resection. Results: Perioperative complication were appeared in 2 patients those were middle cerebral arterial infarction and mediastinal bleeding. Postoperative mortality rate was 20%. 4 patients recurred within 1 year. Conclusion: Preoperative collateral study rarely provide whether resection carotid artery or not. Elective carotid artery resection cannot provide locoregional control of tumor and don't promote survival.

좌총경동맥루 1례 보고 (Aneurysm of the Left Common Carotid Artery: Report of A Case)

  • True aneurysm of the carotid artery is relatively rare in comparison with the total number of aneurysms of the arterial system. The threat of rupture and embolization from mural thrombi are indications for treatment even though no symptoms may be present. Resection of the aneurysm and restoration of arterial continuity is the treatment of choice. A case of aneurysm of the Lt. common carotid artery of a 21 year-old male patient is presented. The confirmatory diagnosis was made by left carotid angiogram, and the aneurysmectomy & reconstruction with woven dacron vascular prosthesis was done, while cerebral circulation was maintained by internal shunt.

초음파를 이용한 경동맥 내-중막 두께와 대사증후군 진단지표의 연관성 (Relationship Between Carotid Intima-Media Thickness Using Ultrasonography and Diagnostic Indices of Metabolic Syndrome)

  • 본 연구의 목적은 대사증후군(MetS) 진단지표와 경동맥 내-중막 두께(carotid IMT)의 관련성을 이해하기 위하여 수행하였다. 연구대상자는 동맥경화와 기타 심혈관계 질환이 없는 남성 근로자 315명으로 하였다. 이 연구는 한국산업안전보건공단 산업안전보건연구원의 생명윤리위원회의 승인을 받은 후, 연구대상자의 동의를 받고 수행하였다. 인체계측과 혈청생화학 항목에 대한 검사는 체성분분석기기와 생화학자동분석 기기를 이용하여 측정하였고, MetS의 진단은 NCEP-ATP III 기준에 의하여 판단하였다. Carotid IMT는 고해상도 B-mode 초음파기기를 이용하여 경동맥 분지점과 근위부의 내-중막 두께를 측정하였다. Carotid IMT는 $0.739{\pm}0.137\;mm$이었고 연령증가와 더불어 유의하게 증가되었다. 또한 수축기 및 이완기혈압, 중성지방 및 공복혈당도 연령증가에 따라 유의한 증가를 보였다. Carotid IMT는 BMI(r = 0.170, p = 0.004), 수축기(r = 0.148, p = 0.011)와 이완기혈압(r = 0.123, p = 0.036) 및 HDL-cholesterol(r = -0.164, p = 0.005)과 통계적으로 유의한 상관성을 보였다. MetS 진단지표와 carotid IMT에 대한 다중 로지스틱 회귀분석을 실시한 결과, carotid IMT는 혈압(교차비 = 4.220, p < 0.01)과 MetS(교차비 = 1.301, p < 0.05)과 유의한 관련성을 보였다. 이상의 결과는 혈압과 대사증후군이 죽상동맥경화증의 중요한 위험인자로 작용함을 보여주는 결과라 판단된다.

혈압 수준과 경동맥 내중막 두께 및 동맥경화반의 관련성 (Association of Blood Pressure Levels with Carotid Intima-Media Thickness and Plaques)

  • Objectives : The aim of this study was to investigate the association of blood pressure levels with the common carotid artery intima-media thickness (CCA-IMT) and carotid plaques. Methods : Data were obtained from 2,635 subjects, aged 50 years and over, who participated in the Community Health Survey (a population-based, cross-sectional study) in Dong-gu, Gwangju city between 2007 and 2008. Participants were categorized into three groups according to blood pressure levels; normotensives (<120/80 mmHg), prehypertensives (120-139/80-89 mmHg), and hypertensives ($\geq$140/90 mmHg). Prehypertensives were further categorized as low prehypertensives (120-129/80-84 mmHg) and high prehypertensives (130-139/85-89 mmHg). Carotid intima-media thickness and plaques were evaluated with a high-resolution B-mode ultrasound. Statistical analyses were performed using chi-square test, ANOVA, and multiple logistic regression. Results : Prehypertensives had significantly greater maximal CCA-IMT values than normotensives, with a multivariate adjusted odds ratio of 1.78 (95% CI=1.36-2.32) for abnormal CCA-IMT (maximal CCA-IMT$\geq$1.0 mm), and 1.45 (95% CI=1.19-1.77) for carotid plaques. The multivariate adjusted odds ratio of low prehypertensives was 1.64 (95% CI=1.21-2.21) for abnormal CCA-IMT, and 1.30 (95% CI=1.04-1.63) for carotid plaques compared with normotensives. Subject with hypertension had higher frequency of abnormal CCA-IMT (odds ratio, 2.18; 95% CI=1.49-3.18), and carotid plaques (odds ratio, 1.98; 95% CI=1.46-2.67) compared with normotensives after adjustment for other cardiovascular risk factors. Conclusions : Our results indicate that there is a significant increase in the prevalence of carotid atherosclerosis in subjects with prehypertension (even in low prehypertensives) compared with normotensive subjects. Further studies are required to confirm the benefits and role of carotid ultrasonography in persons with prehypertension.

경동맥 절제술 후 재건에 사용된 복재정맥의 혈관구경 불일치를 극복하기 위한 Titanium Hemostatic Clip 봉합방법 (Titanium Hemostatic Clip Tailoring Method to Overcome Vessel Caliber Discrepancy in Interposition Saphenous Vein Graft for Carotid Artery Resection)

  • Objectives: To completely excise a malignant tumor which invades carotid artery walls, the resection and reconstruction of the carotid artery is essentially required. In most of the cases, interposition graft using a saphenous vein has been performed, however the vessel caliber discrepancy between a common carotid artery and the saphenous vein can result in a problem on surgical technique. We have introduced and evaluated a new titanium hemoclip tailing method to overcome vessel caliber discrepancy in interposition saphenous vein graft for carotid artery resection in the treatment of head and neck cancers. Method: After carotid artery resection, the calibers of the proximal common carotid artery and the vein were compared, and an orifice of the common carotid artery was gradually reduced to a little larger than or the same size as the orifice of the vein by using the titanium hemostatic clip. Subsequently, the common carotid artery was connected to the vein through anastomosis. The same method was also applied to the distal anastomosis site. There after, the vessels were connected through the anastomosis, and a circulation was restored by releasing a vascular clamp. Then, a titanium hemostatic clip-applied redundant portion on the outside of carotid artery was sutured by the blanket edge suture method, using 6-0 Prolene. Results: We have experienced this method in two patients with recurrent squamous cell carcinoma and neuroblostoma, respectively. The interposition saphenous vein graft of these patients was found to maintain good patency on the follow up angiography after one year, and they had no specific vascular complication, such as atherosclerosis. Conclusion: This method made it possible to simply perform the interposition saphenous vein graft (ISVG) within a short time and, therefore, was very useful for shortening the duration to block circulation.
