The leading paradigm of dentistry had been focused on the rehabilitation treatment that identifies active caries, manages them surgically, and restores their original functions. However, changes in the external environment including the current disease prevalence require dentistry to have a paradigm shift. The new paradigm suggests the detection of caries in their earlier stages over the visual diagnosis of cavities, and the reversal of the incipient caries by non-surgical approach. For this to be achieved, a high-technology detection device recognizing changes in the earlier stages which can not he visually observed is needed. Development of early caries detection device has recently become a major issue in preventive dentistry on account of this need, and QLF(Quantitified Light induced Fluorescence) conspicuously stands out among the newly released devices. In this study, the fundamental concept of QLF(Quantitified Light induced Fluorescence) and the possible clinical applications of the earlier intraoral camera model as well as the recently designed digital camera model will be discussed.
최근 2세에서 5세 사이 소아의 우식 발생율이 상당히 증가하고 있다. 유아기우식증(Early Childhood Caries)은 향후 영구치의 우식 발생 가능성에 대한 예측인자이며 사회 저소득층과 다문화 가정 자녀에서 특히 발생빈도가 높은데, 이는 충분히 예방과 관리가 가능하다. 만1세때부터 정기적으로 치과를 방문하고 이 때 우식위험도를 체계적으로 평가하여 환자들에게 맞춤형 예방 전략을 제시할 수 있기 때문이다. 유아기우식증의 예방을 위한 CAMBRA (Caries Management by Risk Assessment)에 대해 가장 최신의 개념과 사용법을 소개하고자 한다. 소아치과의사는 CAMBRA를 통해 개개인의 위험인자를 분석하여 연령에 따른 적절한 예방법을 수립하고 향후 우식 발생시 치료방향의 지침을 정하게 된다.
연구목적은 중증 유아기 우식증에 대한 두 개의 정의를 진단 기준으로 하였을 때 각 기준에 따른 유병률이 일치하는 정도를 알기 위함이었다. 연구대상은 익산시 어린이집 중에서 무작위로 추출한 7개 어린이집의 36개월 이상 71개월 이하 어린이 401명이었다. 치경과 탐침을 사용한 구강검사를 통하여 치아별 우식경험유치면(dmfs)의 수를 조사하였으며, 중증 유아기 우식증의 진단 기준은 (1) 상악 전치에 1개 이상의, 와동이 형성된 우식, 상실(우식증에 의한), 충전 평활면이 있거나 (2) 우식 상실, 충전 치면의 수가 3세는 4개 이상, 4세는 5개 이상, 5세는 6개 이상인 경우로 하였다. 상악 전치부 우식증에 의한 우식 유병률과 우식경험유치면수(dmfs)에 의한 우식유병률은 3세에서 각각 26.7%, 28.9%, 4세에서 각각 32.4%, 30.4%, 5세에서 각각 39.4%, 41.3%, 전체에서 각각 35.4%, 35.9%으로서, 연령별로는 약간의 차이가 있었으나 전체에서는 차이가 0.5%로서 거의 일치하였다. 따라서 중증 유아기 우식증의 진단은 두 가지 기준 중에서 어느 하나를 사용하더라도 비슷한 결과가 나올 것이라는 것을 확인할 수 있었다.
Purpose: The aim of this study was to evaluate the effects of oral health care programs in 3 school-based oral health care center among primary schoolchildren. Methods: School-based oral health care programs included fluoride mouth rinsing, pit and fissure sealing for permanent premolars and molars, fluoride gel application and chewing of xylitol candy. All of the programs were carried out by one dental hygienist among 'D' primary schoolchildren in Daegu city under the supervision of a dentist. Baseline dental examinations were completed and preventive care was implemented for 544 children during one year. All of the children visited a school-based oral health care center every three months for a regular check-up. The final oral examination was conducted from March 15 to April 1, 2004. The data analysis data was made on the basis of SAS 8.01. Mean differences between 2003 and 2004 data were compared by paired t-test. Corresponding p-values were considered significant at values less than 0.05. Results: The DMF rate and DFT index were reduced to 8.0% and 8.4% during one year respectively, but there were no statistically significant differences. The DMF rate was significantly reduced (16.3%) after a one year program of school-based oral health care practice. The DMFT(Decay Missing Filling Tooth) index was also reduced compared to 2003 throughout the entire grade. Conclusion: School-based oral health care programs can reduce the prevalence of dental caries prevalence among schoolchildren during one year. This program also improved the oral health capacity of schoolchildren. It is recommend that the school-based oral health care program should be extended to every primary school in Korea.
Objectives: This study aims to correlate caries-causing microorganism load, lactic acid estimation, and blood groups to high caries risk in diabetic and non-diabetic individuals and low caries risk in healthy individuals. Materials and Methods: This study includes 30 participants divided into 3 groups: Group A, High-risk caries diabetic individuals; Group B, High-risk caries non-diabetic individuals; and Group C, Low-risk caries individuals. The medical condition, oral hygiene, and caries risk assessment (American Dental Association classification and International Caries Detection and Assessment System scoring) were documented. Each individual's 3 mL of saliva was analyzed for microbial load and lactic acid as follows: Part I: 2 mL for microbial quantity estimation using nutrient agar and blood agar medium, biochemical investigation, and carbohydrate fermentation tests; Part II: 0.5 mL for lactic acid estimation using spectrophotometric analysis. Among the selected individuals, blood group correlation was assessed. The χ2 test, Kruskal-Wallis test, and post hoc analysis were done using Dunn's test (p < 0.05). Results: Group A had the highest microbial load and lactic acid concentration, followed by Groups B and C. The predominant bacteria were Lactobacilli (63.00 ± 15.49) and Streptococcus mutans (76.00 ± 13.90) in saliva. Blood Group B is prevalent in diabetic and non-diabetic high-risk caries patients but statistically insignificant. Conclusions: Diabetic individuals are more susceptible to dental caries due to high microbial loads and increased lactic acid production. These factors also lower the executing tendency of neutrophils, which accelerates microbial accumulation and increases the risk of caries in diabetic individuals.
In order to investigate dental caries and oral health status of Kindergarten children, a study was conducted from 1st December, 2000 to 21st January 2001. The subjects were consisted of 117 toddler and pre-school children who were enrolled in 4 kindergartens in a county of Kangwondo. A structured questionnaire was used for dental health behavior and dental examination was given by dentist for detecting dental caries The result were as follows: 1. As for the oral health behavior, 71.8% of subjects brushed their tooth twice a day, 18.8% of them once a day, 8.5% of them 3 times a day and 0.9% of them do not brushed. 68.6% of children brushed their teeth in the morning and evening, 20.7% of them before go to bed, 7.4%, and 2.5% of them after meal and before brak first. 71.7% of children used combined with scrubbing and vertical method, 28.3% of them used scrubbing method. 61.5% of their mother answered they observed oral status of children regularly. 2. Prevalence rate of dental caries (dft) was 68.4%. The Study reported here was designed to analyze the prevalence of Dental Caries in the primary teeth of monozygote twins, and to ascertain and compare the intrapair differences with the intrapair differences in control pairs. The prevalence of dental caries in the primary teeth of 40 pairs of presumably monozygote twins, based on phenotypic analysis, was studied. The age range was from 3 to12 years. The control group consisted of pairs that included a twin chosen at random and an unrelated child of the same sex and age, so that for each pair of twins there was a corresponding control pair. The clinical examination was carried out by an operator using an oral mirror and sharp explorers, and with good lighting. 40 pairs of twins were examined with a total of 4,950 primary tooth surfaces (990 primary teeth) and 40 control pairs with a total of 4,935 primary tooth surfaces (987 primary teeth). Five surfaces were considered for each tooth. After the def individual index was determined, the ratio between this index and the number of examined surfaces was established in each case, the cer (Caries experience rate) index. The def and cer averages were determined for the subjects and the controls. All intrapair differences in twin and control groups were compared following the paired sampling method. The following conclusions were made: 1. Intrapair differences in the control group were significantly greater than in twins, when defs, ds, es, fs, and cer were compared. 2. The genotype appeared to be a determining factor in dental caries susceptibility or resistance, although the environment played an important role as well. 3. Intrapair differences in American children were significantly greater than in Korean when defs, es, fs, and cer were compared, but ds was equal.
This study aimed to introduce what Korea national oral health surveys are and examine recent oral health status in Korean children. Korea Government has implemented national oral health-related surveys during the past two decades. The surveys with oral health examination are Korean Children's National Oral Health Survey (KCNOHS) since 2000 in 5-year-old and 12-year-old children and Korea National Hhealth and Nutrition Examination Survey (KNHANES) since 2007 aged 1 year and more. KCNOHS provides the information about the distribution of prevalence of dental caries, gingival health, behavioral factors, and dental clinic demands etc. KNHANES has huge amount of health data for representative Korean population including oral health and general conditions. Based on KCNOHS, the prevalence of dental caries and its experience has decreased steadily, and the status of oral hygiene and preventive treatment called sealents has been also improved after the year of 2000. However, there is still a gap to reach to those of Western developed countries. Therefore, more effective oral health policy plans and strategies for Korean children and adolescents are needed to prevent and manage for dental caries in private and clinical field of dentistry with public sector.
본 연구는 125명의 6-11세 혼합치열기 아동을 대상으로 연령에 따라 초기 혼합치열기 A군(6-8세; 62명)과 후기 혼합치열기 B군(9-11세; 63명)으로 분류하고 유치의 dfs, 영구치의 DFS를 기록한 후 자극성 타액을 채취하여 TYCSB 배지에 배양해서 S. mutans와 S. sobrinus를 구분하였으며 PCR을 시행하여 확인하였다. S. mutans와 S. sobrinus의 분포도와 치아우식증과의 상관관계를 조사하여 다음과 같은 결과를 얻었다. 1. S. mutans와 S. sobrinus의 상관계수는 A군 0.70, B군 0.50로 두 군 모두에서 높은 상관관계를 보였다. 2. S. mutans와 치아우식증과의 관계는 A군(r=0.25)과 B군(r=0.34) 모두에서 약간의 관련성이 나타났다. 3. A군에서 S. sobrinus와 치아우식증간에 상관관계가 없는 것으로 나타났으며, B군(r=0.21)에서는 미약한 상관관계가 성립되었다. 4. S. mutans와 나이의 상관관계는 A군과 B군 모두에서 관련성이 없는 것으로 나타났다. 5. S. sobrinus와 나이의 상관관계는 A군(r=0.32)에서는 약간의 관련성이 나타났고, B군에서는 관련성이 없는 것으로 나타났다.
The objective of this study was to evaluate relationships between the economic status and the oral health status. Data were obtained from the National Health Insurance Corporation database. The prevalence of dental caries and tooth loss were compared among the ten-percentile according to the level of the monthly income. The prevalence of dental caries and tooth loss were lower in the high level of monthly income than low level of monthly income group. There was a significant relationships between the economic status and the oral health status.
