약학교과과정의 개국약국실습 방향에 관한 연구 (The Study on Identifying the Components of Community Pharmacy Externship Based on Korean Community Pharmacists' Consensus)

  • The need for and components of a contemporary community pharmacy externship for pharmacy students have not been clearly identified in Korea. Mail survey was performed among 20 college of pharmacy deans and 800 community pharmacists to analyze the current status and develop a consensus regarding major focus area and criteria of community pharmacy externship to be implemented under the separation of dispensary from medical practice in year 2000. Mail survey yielded $80\%\;and\;23.5\%$ response rate for pharmacy school deans and community pharmacists, respectively. Of the 16 pharmacy schools that responded 14 said they have externship program in hospital pharmacy, and only 8 pharmacy schools responded of having externship program for community pharmacy. However, these community pharmacy programs lacked criteria and standard guideline for the externship. The results of survey revealed that community pharmacy externship program for students should be organized and directed toward developing expert knowledge and skills in pharmacy practice activities, clinical services, communications, pharmacy management, and professionalism. Pharmacy practice components should include competencies and skills in computer application, prescription processing, dispensing, pharmaceutical compounding, Narcotics Control Law application, maintenance and provision of drug information, and laws and regulations. Clinical service components should include the ability to identify patient's drug-related problems, provide long-term patient care and appreciate drug therapy services. Communication skills should be taught to effectively express his/her professional opinion, deduce the needs of others, utilize appropriate techniques and media to communicate ideas and conduct a patient interview and to obtain patient drug history. Pharmacy management skills should be taught to be efficient in medical insurance and drug control process. It was found that professionalism, morality, pharmacy practice experience, ability to provide clinical services, collect and provide drug information and regality are important criteria of preceptors. Externship sites should possess the ability to stock various drugs, access and provide diverse pharmacy services and should have private patient counseling area. Most pharmacists agreed that top 200 drugs' generic and brand name, indications, dosage, side effects, and contraindication should be instructed during the externship. It was also found that student and preceptor should be evaluated for their performances during the externship. This information will be incorporated into teaming objectives for students and to develop Academic Extemship Program Guidelines.

퇴원한 정신분열병 환자와 가족에 대한 지역사회복지관에서의 단기가족교육 프로그램의 효과 (The Effectiveness of Brief Family Education Programs for Schizophrenic Outpatients and their Families in a Community Social Welfare Center.)

  • 본 연구에서는 퇴원한 정신분열병 환자와 가족에 대한 지역사회복지관에서의 단기가족교육 프로그램의 효과를 알아보기 위하여 전형적인 실험연구인 전-후 시험 통제집단설계를 이용하여 그 개입효과를 검증하였다. 본 연구의 결과, 지역사회복지관에서의 단기가족교육 프로그램이 정신분열병 환자 가족의 정신분열병에 대한 병적 지식, 가족의 정신분열병 환자에 대한 거부감과 불안 그리고 정신분열병 환자가 인지하는 가족의 정서적 지지에서 긍정적인 개입효과가 있는 것으로 검증되었다. 이러한 연구결과들을 볼 때, 지역사회복지관에서의 단기가족교육 프로그램이 정신분열병 환자의 치료와 재활 그리고 환자 가족의 안정에 긍정적인 영향을 미치는 지역사회 자원으로서 적극 활용될 수 있을 것이다. 이에 본 연구에서는 전문 인력의 확보, 보다 다양한 프로그램 내용의 개발, 프로그램 종료 후의 가족모임에 대한 지원 등을 지속적인 연구와 함께 제안하였다.

치과 치료를 위한 외래 정주진정 법에 대한 다기관 후향적 임상연구 (A MULTICENTER RETROSPECTIVE STUDY OF OUTPATIENT INTRAVENOUS SEDATION FOR DENTAL TREATMENTS)

  • Purpose : On outpatient facilities, our sedation protocol focuses on the intermittent bolus injections of midazolam intravenously, according to patient's and operator's needs during the dental treatment. This multicenter retrospective study was aimed to prove the efficiency and safety of our sedation protocol. Patients and Methods : In three centers using the same outpatient sedation protocol for dental treatment (Division of Oral and Maxillofacial Surgery/Department of Dentistry in Hanyang University Medical Center, S-plant Dental Hospital. and Grand Oral and Maxillofacial Surgery), total 937 patients had various dental treatments under intravenous conscious sedations with independent patient monitoring from March 2006 to March 2009. By reviewing charts, we analyzed the results of sedation and dental treatment, retrospectively. Results : Our sedation protocol had no severe postoperative complications requiring admission. while showing good compatibility with almost all dental treatments, with acceptable satisfaction of both patients and operators. Conclusion : We assure that our sedation protocol can be used efficiently and safely on routine outpatient basis. We also hope that this study will provide the concrete concepts to common dental practitioners, who desire to perform sedation for dental treatment.

전국 간호전문대학 남학생의 제특성에 관한 조사연구 (Characteristics of Male Diploma Nursing Students in Korea)

  • This study was performed to investigated of characteristics of male diploma nursing students. Data were collected by means of a mailed questionnaire. The sample included 29 respondents from 3 diploma nursing colleges during the period of Nov. 1st-15th, 1978. Major findings included. 1 ) Motive by which the objects of this investigation have chosen the science of nursing. Twenty-one (72.4%) responded, “As nursing occupation is a public welfare work, ”the highest rate, eighteen (62.3%) chose on advices of their parents and acquaintances. Seventeen (58.6%) reflected as a means of life with an occupation in hope of employment abroad. 2) Appreciation of nursing occupation. Twenty-two (75.9%) of opinions that the nursing job is called for by society was pre-dominent. While eighteen (62.1%) replied, “It is the job fit for the male sex, too.”“It is admitted as specialized occupation.”, or“It needs various human relation.”3) Degree of satisfaction wilt the science of nursing. Fifteen (51.7%) responded neither satisfied nor dissatisfied, while eight (27.6%) indicated as “satisfied”and four (13.8%) as“dissatisfied.”4) Degree of satisfaction with the faculty. Sixteen (55.2%) replied, “common, ”the highest, while ten (34.5%) indicated as“dissatisfied, ”two (6.9%) as“satisfied.”The reason for dissatisfaction with the faculty; The responses regarding dissatisfaction was twenty-three (79.3%) as insufficiency of the faculty. Thirteen (44.8%) indicated“the lack of personal cultivation of the faculty.”, And eleven (37.9%) indicated as“the quantitive shortage of the faculty, ”or“the vagueness of learning estimation.”5) Degree of satisfaction with the clinical, training. Eight (27.6%) responded as“common, ”or “dissatisfied, ”while seven (24.1%) indicated as“satisfied.”Reason for dissatisfaction with the training ; Twenty (69.0%) indicated“deficiency of personal treatment to the students of the men of business in the hospital”with respect to the reason, eighteen (62.1%) was indicated as gap between theory and practice, while eleven (37.9%) indicated“insufficiency of the equipment and materials of the hospital.”6) Interest in employment after graduation. Twenty-five (86.2%) indicated“going abroad”while fifteen (51.7%) indicated “education of nursing, ”which were the highest responses. Thirteen (44.8%) chose“Community Health Nursing (Health Center, Industrial Health).”7) Interest an employment during clinical nursing. Sixteen (55.2%) was interested in an operating room or the department of anesthesia, while fifteen (51.7%) was indicated “psychiatry, ”Eight (21.6%) chose a intensive care unit or a emergency room.

서울시내 일부 노인들의 소외정도에 관한 조사연구 (A Study on The Extent of Alienation Revealed among A Group of Aged People in Seoul City)

  • A steady increase in the aged population poses a great challenge to nurses with their diversities of health care needs, especially of psychological nature, such as an alienation problems among the aged. The purpose of this study is to examine the nature of alienation problems among the aged in urban areas to provide basic informations for a better understanding of aged people. In this study, the investigator has tried to identify causes of alienation in the aged, and to determine the extent of alienation in relation to sex, family structure, religion, economic status and participation in social activities. The survey was done during the period from the last part of August to the end of September 1973. After a day's training, 10 interviewers have interviewed 210 stratified random samples of people over the age of 65 residing in Seoul with the free-ended questionnaire prepared by the investigator. The result of the study was tested by Critical Ratio. Following Hypothesis have guides formulation of the study: I. The extent of alienation in old man would be higher than in old woman, II. The extent of alienation in the aged who live alone would be higher than those living with their family. III. The extent of alienation in the aged who have no spouse would be higher than who live with their spouse. IV. The extent of alienation in the people without religious beliefs would be higher than the people with religious biles. V. The extent of alienation in the aged would be higher in proportion to decreased level of their economic productivity. Ⅵ. The extent of alienation in the aged who do not participate in social activities would be higher than that of the aged who actively participate. The analytic results of the study are as fellows: 1. The extent of alienation in woman was revealed higher than that of man by showing significant difference at P〈0.01 level. (CR=3.66) Accordingly hypothesis I was denied. 2. The extent of alienation in the aged wile live alone was revealed higher than that of the aged who live with their family by showing a significant difference at P<0.01 level. (CR=7.31) So hypothesis II was supported. 3. The extent of alienation of the aged who have no spouse was revealed higher than that of the aged who live with their spouse at the significant level of P <0.01. CR=4.65) Accordingly hypothesis III was supported, 4. There was no significant difference in the extent of alienation between the people with and-without religion. Thus hypothesis IV was rejected. 5. The extent of alienation in the aged was found to be higher in proportion to decreased level of their economic ability. a. The aged with greatest income showed least alienation b. The aged with no income had a greater extent of alienation than the aged with minimal income by showing significant difference at P <0.01 level . (CR=4.82) c. The difference between the greatest income group and the minimal income group was. found to be less significant than the difference between minimal income group and the-people without income. Thus hypothesis V was positively supported. 6. The extent of alienation in the aged who do not participate in social activities was higher than that of the aged who actively participate. (P <0.01, CR=6.24) According1y hypothesis Ⅵ was supported.

산업간호사를 대상으로 한 구강보건교육의 효과 평가 (Effects of Oral Health Education for Occupational Health Nurses)

  • 이번 연구에서는 근로자들의 구강건강증진 및 구강질환 예방을 도모하고, 구강건강에 대한 관심을 증가시키며, 근로자 구강보건사업의 지지 및 참여를 높이고자 산업간호사를 대상으로 한 구강보건교육 프로그램을 개발 및 수행하여 교육 전 후의 구강보건지식 및 태도수준을 비교하여 평가함으로써 근로자 구강보건교육 프로그램 개발을 위한 기초 자료로 활용하고자 하였다. 산업간호사를 대상으로 구강보건교육을 실시한 후, 교육 전 후 자가인지 구강건강수준 및 구강건강관리법에 대한 인지 및 권장수준의 변화를 비교 분석한 결과는 다음과 같다. 산업간호사를 대상으로 한 구강보건교육의 시행 후, 자가인지 구강건강수준과 구강건강관심도는 교육 시행 전에 비해 모두 유의하게 증가하였다. 치실 혹은 치간솔 사용, 법정 구강검진을 포함한 정기검진, 정기적인 치면세마를 포함한 치주병 예방법에 대한 인지수준이 교육 전에 비해 모두 유의하게 향상되었고, 구강질병 예방 및 구강건강증진을 위한 구강건강관리법에 대한 권장수준도 교육 전에 비해 매우 유의하게 향상되었다. 이와 연관된 요인을 확인해 본 결과, 구강건강관심도가 낮을수록 치주병 예방법별 인지수준과 구강건강관리법별 권장수준의 향상 정도가 모든 항목에서 크게 나타났다. 산업간호사의 근로자 구강건강증진 프로그램에 대한 의견을 듣고자 설문조사를 실시한 결과, 근로자를 위한 구강건강증진 프로그램을 수행하는 데 필요한 항목으로는 구강보건교육 전후 모두 '산업간호사에 대한 구강보건교육'이 1순위로 나타났다. 그러므로, 근로자들의 구강건강증진을 도모하기 위하여 산업구강보건의 중요한 협력자인 산업간호사를 대상으로 하는 지속적인 구강보건교육 시행이 필요하고, 이러한 교육을 통해 산업간호사의 구강보건지식 및 태도가 향상된다면, 앞으로 근로자 구강보건사업을 계획하고 수행하는 데 있어 산업간호사의 지지와 참여를 효율적으로 이끌어낼 수 있을 것이라 생각된다.

Dental Hygienist Coordination System 개발 교육주제 요구조사 (A Survey on the Demand of Education topic for the Development of Dental Hygienist Coordination System)

  • 의료시장의 개방과 더불어 의료기관의 경쟁력확보를 위한 핵심전략으로 환자관리의 중요성이 강조되고 있다. 이 연구는 치과위생사의 업무능력향상과 전문성을 높이기 위해 수행업무인식과 교육요구주제를 조사 분석하여 Dental Hygienist Coordination System을 개발하는데 기초자료로 활용하고자 수행하였다. 비례표본추출법을 이용하여 광주광역시 85개 치과의원에 근무하는 치과위생사 156명을 대상으로 직접 방문하여 설문법을 이용하여 조사 분석한 바, 다음과 같은 결론을 얻었다. 1. 코디네이터교육경험 유무에 따른 교육과정별 우선순위 요구과목으로 서비스분야에서는 코디네이터이론과 실무(유무:상담 및 교육실무), 코디네이터를 위한 기초(유무:의학용어), 코디네이터를 위한 원무관리(유무:건강보험), 시설 및 환경관리(유:환경관리, 무:감염관리)이고, 매니저분야에서는 의료서비스마케팅(유무:의료환경과 고객특성, 의료서비스마케팅), 조직인사관리(유무:병원조직), 회계 및 재무관리(유무:병원회계), 건강보험실무(유:진료비청구관리, 무:진료비산정)으로 조사되었다(p>0.05). 2. 코디네이터교육경험 유무에 따른 교육요구과목 차이를 분석한 결과, 코디네이터이론과 실무 0.14, 건강보험실무와 의료서비스마케팅 0.13이고, 인사관리 - 0.23, 코디네이터를 위한 의료기초 -0.19로 조사되었다(p>0.05). 3. 코디네이터교육경험 유무에 따른 교육요구주제는 경험자군과 비경험자군 모두 상담 및 교육실무로 나타났으며, 교육경험자군과 비교육경험자군의 차이를 보면 고객접점관리 0.25, 고객응대기법 0.24로 조사되었다(p>0.05). 이상과 같은 결론에서 우선순위가 높은 교육과목인 코디네이터 이론과 실무, 건강보험 실무에서 진료비 청구관리를 중심으로 체계적인 교육과정을 포함한 Dental Hygienist Coordination System(D.H.C.S)개발에 필요한 근거자료로 활용될 수 있을 것으로 사료된다.

기본간호학 실습교육현황과 실기능력 평가방법에 관한 조사 (Study on the Present Status of Practicum of Fundamentals of Nursing and Test for Competency of Nursing Skills)

  • Purpose: In this study the present status of the practicum in Fundamentals of Nursing and test for student competency in nursing skills were surveyed. Method: Participants included 72 nursing schools. A structured questionnaire was used for data collection and SPSS 17.0 for data analysis. Results: Credits for Fundamentals of Nursing, credits for practicum and hours per credit were 2.93, 7.34, and 2.14 respectively. Number of students per practice unit, full time assistants and students per assistant were 28.6, 0.6, and 20.2 respectively. Utilization of CPX room and OSCE room for practice was 5.6%, and 9.7% respectively. Of the school, 32.0% included clinical practice in the practicum. 90.3% provided an open lab but only 48.4% had full-time assistants for these clinical practice areas. Testing for nursing skill competency was performed twice a semester in most schools. Utilization of standard patients and high fidelity models for testing was 8.3%, and 23.6% respectively. The test was thought to be inappropriate in 33.4% of the schools. Conclusions: The results suggest that to improve the outcomes, the practicum for Fundamentals of Nursing needs to be changed to include expansion of facilities and human resources, utilization of standard patients and high fidelity models and development of appropriate test methods.

가족구조에 따른 미성년 자녀의 의료이용 분석 (An Analysis of Family Structure on Children's Medical Utilization)

  • 본 연구는 <한국의료패널>을 활용하여 모자가정과 부자가정을 중심으로 한부모가정에 속한 미성년 자녀의 의료이용 현황을 점검하고, 의료이용의 정도가 조부모의 동거 여부, 형제자매의 수, 그리고 한부모가정을 형성하게 된 원인에 따라 차이가 있는지를 분석하였다. 연구의 주요결과는 다음과 같다. 첫째, 아동 청소년의 개인특성과 가구특성을 통제한 이후에도 부자가정에서 자란 자녀들은 양친가정에서 자란 또래 자녀들보다 연간 총 외래이용 횟수가 적은 것으로 나타났다. 둘째, 조부모의 동거 여부는 미성년 자녀들의 외래이용 횟수와 뚜렷한 연관성을 보이지 않았지만 형제자매의 수는 외래이용 횟수와 통계적으로 유의한 음의 상관성을 갖는 것으로 나타났다. 셋째, 한부모가정 형성원인을 이혼과 사별로 구분하여 외래이용 횟수를 비교한 결과 부자가정의 경우 이혼가정 자녀들이 사별가정 자녀들에 비하여 외래이용 횟수가 더 적고 조부모의 동거 여부 또한 외래이용 횟수와 양의 상관성을 갖는 것으로 나타났다. 본 연구의 결과는 한부모가정에 대한 지원방안을 모색할 때 부자가정과 모자가정의 특성을 고려한 맞춤형 지원 정책이 필요하며 한부모가정에 대한 사회적 정책적 지원이 보건의료 분야로 확대되어야 할 필요가 있음을 시사한다.

신생아의 행동반응에 대한 실무교육이 간호사와 신생아와의 유희적 상호작용에 미치는 영향 (The Effects of In-service Education of Newborn's Behavior Response on the Nurse-Newborn Play Interaction)

  • The purpose of this study was to find out the effect of in-service education on the nurse-newborn play interaction. The research design adopted the pre-experimental design applied only for one pretest-posttest group and Barnard's mother-infant interaction model was used as a conceptual framework. The subjects were 26 nurses who were selected from 2 nurseries in general hospital in Pusan, and 52 healthy newborns who were after 4 days from birth during data collection period at the same hospital. The data were collected from June 1st to October 5th in 2001, by video-taping for the interactive behaviors between the nurse and the newborn, played for 5 minutes in nurseries 2 weeks before and after in-service education. The in-service education consisted of the newborn's behavior responses focused on the newborn baby's states, behavior, cues and state modulation, 3 times per 1 week, 90 minutes per 1 time, lecture, demonstration and hand out project. The experimental tool used for this research was Ha Young-Soo's Korean translation of the Maternal-Infant Play Interaction Scale by Thompson, Jody Baird, Sara Gordman, Bryant(1982), some parts of which were adapted to be suitable for the purpose of this research. Mother and baby scale by Wolke & James-Roberts(1987) was also modified and used as the criterion of nurse's perception of a newborn baby. The results of this study were as follows: 1. This result supported the major hypothesis : After the intervention of in-service education, the nurse and newborn play interaction was promoted. 2. After intervention of in-service education, the nurse perception of newborn, especially the criteria of newborn's reaction was promoted., The results of this research confirm that in-service education on the newborn's behavior responses such as states, behavior, cues and state modulation is an effective way to improve the interaction between the nurse and the baby. It elevated the nurse's sensitivities to the baby's needs. Therefore, in-service education can change the conditions of current nursing practice mostly centered on physical care into a better one in which nurses consider the emotional, social, and intellectual development stages of babies. Accordingly, in-service education contributed to promoting the effective nurse-newborn play interaction so positively.
