The Effect of Health Behavior Practice and Jab Characteristics on Periodontal Disease of Employees (근로자의 건강행위실천과 직업특성이 치주질환에 미치는 영향)

  • The purpose of this study was to reveal association between health behavior practice and jab characteristics on periodontal disease of employees. The analysis was conducted on 1,737 of above the age of nineteen in employees by using raw data from the 2015 National Health and Nutrition Survey. The data were analyzed using SPSS for windows version 24.0. Smoking, BMI and aerobic exercise in health behavior practice had statistically significant difference. Occupation, work status and regular work in jab characteristics had statistically significant difference. In conclusion, business owners need active management to health behavior practice their employees and the government needs to resolve the polarization of medical services through the development of oral health care policies for low-income people.

Developing Geriatric Nursing Competency Using Information and Communication Technology: Focusing on a Case of Depression Assessment (ICT 기반 노인간호 역량 개발: 우울증 측정 사례를 중심으로)

  • Purpose: The aim of this study was to describe problems and solutions for older adults living alone and using wearable devices to report depressive moods in daily life. In addition, gerontology nurses' competencies and strategies were discussed based on lessons learned from the observational study. Methods: A hybrid method was used, combining literature review and content analyses based on interviews with 53 participants and observations done by the research team. Results: Main issues and relevant strategies were identified in terms of (1) applicability of information and communications technology (ICT) devices for older adults, (2) feasibility of older adults' use of ICT devices, (3) integration of ICT devices into daily life, and (4) relevant issues when older adults use ICT. Important roles and competencies were discussed regarding general and gerontology aspects of ICT. Conclusion: Findings indicate that a wearable device is feasible to measure daily depressive moods in older adults living alone in community settings. To extend our research protocol to practice, gerontology nurses must adequately prepare to meet emerging health care trends and end users' needs for ICT-based nursing. It is necessary to develop diverse educational programs and active involvement in research and design of relevant devices and programs.

Production Regimes, Family Policy and Gender Wage Gap (생산레짐과 일가정양립정책이 성별 임금격차에 미치는 영향연구)

  • Female plays an important role in new welfare policies as emerging new social risks including care needs resulted from increasing female employment participation and changes in family structures. Whereas the effects of work and life reconciliation policies on female employment are well established, less is known for the role of production regime as an important institution on gender wage gap. This study examines the questions in what way and to what extent production regimes and work and family reconciliation policies influence gender wage gap in advanced capitalism countries using the Luxembourg Income Study (LIS). The coordinated market economies (CMEs), presented as higher firm-specific skills, are associated with lower income rank for female workers than male workers, hence larger degree of gender wage gap. Longer parental leave weeks and higher childcare expenditures are associated with less degree of gender wage gap. This research highlights the importance of production regimes in understanding gender wage gap and potential interaction between production regimes and work and life reconciliation policies on gender wage gap.

A Study on the Utilization of Closed School in Library (도서관의 폐교 활용방안에 관한 연구)

  • Prior to the concrete and practical use of closed schools, this study was designed to identify trends in the use of closed schools, and the possibility and direction of the library's use of closed schools based on the status of closed schools and use cases. As a result of the case analysis, the building of libraries using closed schools can be reborn as libraries with different characteristics from general public libraries, such as providing complex cultural spaces, eco-friendly spaces, and local community places. In addition, based on the results of the study, future direction of library closure is suggested as follows: Provide local economic contribution and local revitalization opportunities, life-friendly and friendly space for local residents, recycling space linked with elderly welfare, care service, urban to rural Immigrants, creation of added value and creation of local business using closed schools, environment-friendly space, recycling as a complex community center, recycled into a space that meets and complements local needs.

Assessment of Korean Hospitals Management Using Dupont Analysis (듀퐁 분석을 통한 한국 병원계의 경영 현황 분석)

  • Purposes: The hospitals needs to generate a minimum profit, in order to perform its own role such as providing high-quality medical services. The demand for hospital management is increasing, as the social demands are diversified and the financial transparency is emphasized. The purpose of this study is to compare hospitals management based on Dupont Identity, by various hospital classification. Methodology: This study is based on '2016 Statistics for Hospital Management' provided by the Korea Health Industry Development Institute. The hospitals were classified according to the scope of care, the type of establishment, the location, and the number of beds. We analyzed the general and financial characteristics of over 337 hospitals using the method of Dupont Identity. Findings: Net profit margin (PM) has the biggest impact on return of equity (ROE). By the number of beds, general hospital with 160-299 beds have the highest return on equity (ROE). By location, hospitals in local municipalities have higher return on equity than hospitals in urban municipalities. According to the type of establishment, public hospitals have lower business performance, and although they invest more than private hospitals. Practical Implications: This study can inspire interest and provide understanding in hospital management and financial structure, by analyzing through an intuitive indicator named Dupont identity. It is possible to provide basic data for hospital management methods for each financial elements, in order to increase the profitability of hospitals.

The Calculation of Geographic Practice Cost Index and the Feasibility of Using It in Korean Payment System (진료비용 지역보정지수의 산출 및 국내 적용의 타당성)

  • The fee-for-service system is used as the main payment system for health care providers in Korea. It has been argued that it can't reflect differences in the medical practice costs across regions because the fee schedule is calculated based on the average cost. So, some researchers and providers have disputed that there is need for adopting geographic practice cost index (GPCI) used in the United States for the Medicare program for the elderly to the fee-for-service payment system. This study performed to identify whether the difference in the practice costs among regions exists or not and to examine the feasibility of applying GPCI to Korea payment system. For this purpose, we calculated modified-GPCI and examined considerations to introduce GPCI in Korea. First we identified available data to calculate GPCI. Second, we made applicable GPCI equations to Korea payment system and computed it based on four types of regions (metropolitan, urban, suburban, and rural). We also categorize the regions based on the availability of the medical resources and the capability of utilizing them. As a result, we found that there wasn't any significant difference in the GPCI by regional types in general, but the indices of rural areas (0.91-0.98) was relatively low compared to the indices of other regions (0.96-1.07). Considering the need to use GPCI floor, the pros and cons of using GPCI, and the concern of the regional imbalance of resources, the introduction of GPCI needs to be carefully considered.

A study on the relationship between socioeconomic level and oral health: analysis of data from the Sixth Korean National Health and Nutritional Examination Survey (사회경제적 수준과 구강건강과의 관련성 연구: 제6기 국민건강영양조사 자료를 이용하여)

  • Objectives: In this study, both subjective and objective levels of oral health were used to identify the relationship between oral health inequalities. Methods: Korean National Health and Nutritional Examination Survey data from 2013 to 2015 were combined to create an analysis plan. Oral health questions categorized as subjective oral health conditions and oral health-related diseases used dental tissue disease status as data measured by the Community Periodical Index(CPI) and decayed, missing, filled teeth(DMFT) experience. Other data on oral health behaviors such as toothache experience, the frequency of toothbrush use, chewing problems, oral examination status, and unmet dental care needs were classified and analyzed according to the socioeconomic level. Data were analyzed using frequency and cross analyses, and the statistical significance level was set at 0.05. Results: It was found that higher the economic and educational level, better was the subjective oral health, lower the CPI, lower the experience of toothache, higher the frequency of toothbrush use, lower the number of people having chewing problems, and higher the frequency of oral checkups. Conclusions: Oral health inequality exists among social classes. It is suggested that continuous research and efforts be carried out to promote oral health while considering socioeconomic and educational levels. Further, active government efforts will be needed to address polarization by social class.

Perceptual differences in internet oral health information use between dental hygienists and patients (인터넷 구강건강정보 이용에 대한 환자와 치과위생사의 인식비교)

  • Objectives: To analyze whether there are perceptual differences in internet oral health information use between ordinary people who underwent dental treatments and dental hygienists. Second, the study aimed to analyze related issues, and, third, to find a developmental direction to provide fundamental information for developing patient-customized websites to improve dental services to suit contemporary needs. Methods: From September 1 to September 30, 2018, a questionnaire survey was conducted, with 367 participants (209 dental hygienists and 149 patients) in order to analyze their objective agreement, subjective congruence, and accuracy. As a study tool, the questionnaire consisted of 11 items about the perception of internet oral health information use and 11 on the estimated perceptions of both groups in order to compare their perceptions and estimates of each other. Results: Objective agreement was analyzed and, as a result, dental hygienists and patients were found to demonstrate similar results in terms of their perceptions of internet oral health information use. With regard to subjective agreement, the study subjects either underestimated or overestimated internet oral health information use more than they perceived. In terms of accuracy, dental hygienists and patients had different perceptions regarding internet oral health information use. Therefore, accuracy was somewhat low. Conclusions: In order to improve individuals' self-care ability and increase therapeutic instructional adherence for oral healthcare, it is necessary to apply internet oral health information properly to each patient's individual oral state.

DENTAL CHARACTERISTICS OF A PATIENT WITH INCONTINENTIA PIGMENTI : A CASE REPORT (색소실조증 환자의 치과적 특성 : 증례보고)

  • Incontinentia pigmenti(IP), or Block-Sulzberger syndrome is a rare X-linked dominant genodermatosis that affects almost in female infant and is usually lethal for males in utero. IP is characterized by four cutaneous stages and is frequently associated with dental, ocular, central nervous system and structural anomalies. Dental problems are congenital missing of teeth, delayed eruption, abnormal crown shape. We reported a case of 5 year-old female with IP. She had congenital missing of multiple primary and permanent teeth, accessory cusp and cone-shaped crowns. Systemically, she had a problem of retina and hyperpigmented macules on her trunk and extremities as typical character of IP. As the syndrome involves many different medical problems and needs comprehensive consideration. Dental care should be performed in a multidisciplinary consultation system.

A Study on Current Status of Acupuncture and Chiropractic Health Insurance in the United States (미국에서의 침술과 카이로프랙틱 건강보험 급여 현황)

  • Backgrounds : The market of Complementary Alternative Medicine(CAM) in the United State(U.S.) accounts for a large proportion of the global CAM market and has a high growth rate. The recent introduction of Obama Care has brought the change in the health insurance system for CAM, and we need to analyze it for its implication to Korean system. Objectives : The purpose of this study is to investigate the current status of acupuncture and chiropractic health insurance in the U.S., and to draw implications for expanding the health insurance coverage for Korean traditional medicine through the comparison between the U.S. and Korean health insurance systems. Methods : We examined the data through the literature search and from the websites of both U.S. government departments and related organizations for the health insurance policy. Based on the collected data, we analyzed its CAM health insurance system in Korea. Results : The acupuncture covered by public health insurance in the U.S. has a limit in the number of treatments and a range of applied diseases compared with Korea. In addition, the practice of acupuncture is not subdivided. However, the chiropractic in the U.S. which also has a limited number of coverage and only three categories of practices are similar to that of Korea. Conclusions : Although the use of CAM by public health insurance is not active in the U.S., but the organizations such as Veterans Health Administration in Vermont is already discussing the use of acupuncture to solve the problem of opioid overuse. Thus Korea also needs to discuss to promote the expansion of the insurance system for CAM.
