The Degree of Safety Education Requirement for Younger Children in Child Daycare Center Teachers (보육시설 아동에 대한 교사의 안전교육 요구도)

  • Purpose: The purpose of this study was to identify the degree of safety education requirement in child daycare center teachers. Method: The data using questionnaire were collected from eight child daycare centers located in Seoul, Gyounggi-Do, and Kwangwon-Do. Final participants were 133 child daycare center teachers. Results: The degree of safety education requirement were as follows: education need of safety knowledge ($4.26{\pm}.50$), safety education need of daycare center ($3.29{\pm}.60$), recognition about safety education training ($3.94{\pm}.58$), and confidence of safety care practice ($2.75{\pm}.43$). There is significant correlation between education need of safety knowledge and safety education need of daycare center (r=.495, p<.001), education need of safety knowledge and recognition about safety education training (r=.555, p<.001), education need of safety knowledge and confidence of safety care practice (r=.201, p=.020), safety education need of daycare center and recognition about safety education training (r=.464, p<.001), and recognition about safety education training and confidence of safety care practice (r=.187, p=.032). Conclusion: On this study, child daycare center teachers' safety education requirement was relatively high. It is recommended that safety education for them should be done for children's healthy life.

Analysis of the Performance and Need of Professional Nursing Care in Nurses: Focusing on the Department of Neurosurgery in a Tertiary Hospital in Gangwon Province (임상간호사의 전문 간호업무 수행도와 필요도 분석 - 강원도 상급병원 신경외과 영역을 중심으로)

  • Purpose: This study was conducted to identify the performance and need of professional nursing care among nurses in a neurosurgery department at a tertiary hospital. Methods: This study used a descriptive design using a questionnaire with 77-item. The participants were 58 nurses with more than 3 years of clinical experience in both the neurosurgery intensive care unit and ward at a tertiary hospital in Gangwon province. Data collection was conducted from January to February of 2021. The performance and need for each workforce were analyzed with the mean and standard deviation, and the effectiveness of the difference between the performance and the need were analyzed with a paired t-test using SAS 9.4. Results: The recognition for work performance level was in order of direct nursing activities (3.20±.54), consultation and coordination (3.18±1.15), education (2.55±.85), administration (1.92±.86), and research (1.57±.89). The recognition for need level was in order of consultation and coordination (3.95±.83), education (3.91±.87), direct nursing practice (3.83±.55), research (3.49±.87), and administration (3.45±.54). The results showed a statistically significant differences between work performance and need (p<.001). Conclusion: It is required to hire professional nursing personnel who can bridge the gap between the performance and need of professional nursing workforce in a neurosurgery department at a tertiary hospital. A professional nurse may be a good alternative.

The National Hospice Care Service Development in Korea (한국형 호스피스 케어 개발을 위한 기초 조사 연구)

  • The urgent needs to establish hospice care systems in Korea arise from the following reasons: 0) a drastic increase in chronically ill patients with the increase of aged population: (2) rapid changes in living environment from the traditional habitation (e. g., Many Koreans living in apartment complexes, which is the most popular form of modern residence in recent years, prefer to die in the hospital.): the overall increase in patients with advanced cancer: (4) recent trends in early discharge of terminally ill patients from the limited hospital facilities to accomodate other medical insurance beneficiaries; (5) easy acceptance of euthanasia owing to the recent social atmosphere that belittles the dignity of human life; (6) medical and nursing care of AIDS patient in terminal stage; (7) and the problem associated with inhumane medical care system, overtreatment, and groundless fears against narcotics. Terminally ill patients were used to be treated in the hospital in the past. In these days, however, they are forced to have home cares with little assistance from the qualified medical personnel because of insufficient hospital facilities, which are even short for the need of emergency patients and provide priority cares to medical insurance beneficiaries with other acute problems. And yet, neither are there any administrative organizations nor systematic medical studies that deal with the level of terminally ill patient's need, their family's problems and resources of hospice care systems in Korea. Thus, most patients are not able to get appropriate medical care at the terminal stage of their lives. The objective of this study is to make comprehensive database for various hospice care organization currently in operation, link them through medical information system, and develop an easily accessible hospice care model that meets the need of most Korean people. Our survey results may be summarized as follows: Nationally there are 40 organizations that provide partial or full hospice care. However, these organizations are not linked to any formal medical service network. Furthermore, the objective of hospice care, care principles, personnel with appropriate training, educational programs, standard for care, costs, consulting service to patients' family members, the extent of medical care from professional staff members, status of hospice facility, and management of those institutions are neither clearly defined nor organized compared to the international hospice care standards. The surveys on patients of terminal stage. grouped in hospice and non-hospice care patients. reveal what they want visiting nursing care to help their pain control. psychological. social and spiritual demands. While the more than 90% of hospice care patients want to reduce their pains. the non-hospice care patients. in addition to their desire for pain control. demanded more psychological. social and spiritual helps as well. The results of this research could be utilized to 0) define the standard of hospice care. (2) provide the guidance for hospice medical care costs. (3) establish the database of hospice care systems. (4) develop softwares. (5) build communication network through Medinet. and (6) provide an organized visiting home nursing care system. These information should be a valuable resource to many medical staffs who are involved in cancer therapy. nursing care. and social welfare programs.

A Study on Sexual Desire Outburst of the Male Elderly Residents Experienced by the Workers in Living Facilities (생활시설 종사자가 경험한 남성노인의 성적욕구 표출에 관한 연구)

  • This study has been performed to explore verbal and behavioral expression of sexual desires among male elderly residents who have been living in long tenn care facilities. There are three topics covered in this study; first, in what situations and how seriously do care workers encounter expression of sexual desires of the elderly residents? Second, what kind of negative consequences do they believe those sexual behaviors will lead to? Third, how can we implement defensive measures against the sexual behaviors? In this study, twenty three care workers working full time in five retirement and care facilities were asked about their experience and perception of the above study agendas, and answers of the qualitative interview were drawn as follow; first, the elderly residents apparently show a variety of sexual harassment and provoking behaviors such as sticking to specific women, physically touching and attacking, and induce obscenely activities against female residents, care workers, and volunteer visitors. Second, their sexual behaviors are often influenced by their isolated and abandoned emotionality as well as living situation in rural areas. Third, their sexual behaviors often critically affect care work plan and facility managements by severely discouraging female care givers and community supporters. Therefore in this study, suggestions and defensive measures were made as follows: first, education and counselling programs toward female workers and volunteers need to be developed, and the programs should cover psychological and behavioral mechanism of sexuality in later life. Second, self control plans need to be empowered toward the elderly residents; in the plans, the elderly residents shall be encouraged to evaluate primary cause and proper solutions of sexual behaviors of their peering residents. Third, combination of healthy housing and care facilities for frail elderly need to be integrated in a neighboring location, so that when residents and workers encounter extreme episodes of sexuality of healthy residents in a housing facility, the problematic sexual residents are partially transferred into a neighboring care facility and thereafter other residents and cafe givers are relieved from stressful contacts with the extremely sexual residents.

  • National long-term care insurance started in July, 2008. We try to make up for weak points and develop a long-term care insurance system. Especially, it is important to upgrade the rating model of the category of need for long-term care continually. We improve the rating model using the data after enforcement of the system to reflect the rapidly changing long-term care marketplace. A decision tree model was adpoted to upgrade the rating model that makes it easy to compare with the current system. This model is based on the first assumption that, a person with worse functional conditions needs more long-term care services than others. Second, the volume of long-term care services are de ned as a service time. This study was conducted to reflect the changing circumstances. Rating models have to be continually improved to reflect changing circumstances, like the infrastructure of the system or the characteristics of the insurance beneficiary.

A Study on Nursing Needs of Patients in the Recovery Room (회복실 환자의 간호요구도에 관한 연구 - 일 종합병원을 중심으로 -)

  • Purpose: The purpose of the study was done to identify the nursing care needs of patients in the recovery room. Method: The subjects in this study were 127 patients in a recovery room between 6/9/2001 and 24/9/2001. The instrument used for this study was the descriptive questionnaire developed by Shin Hyun-Jin (1999). The data was analysed by frequency, percentage, mean, standard deviation, t-test, ANOVA, and factor analysis using the SPSS program. Result: 1) Kaiser - Meyer -O1kin sample appropriateness was 799 and Bartlett's test of sphericity significant probability was .000. 2) The mean score for nursing care need of patients in the recovery room was $4.17{\pm}.51$ of a total possible score of 5. The score of nursing need for different parameters was as follows : Educational need ($4.31{\pm}.49$), physical need ($4.27{\pm}.47$), emotional need ($4.11{\pm}.52$), environmental need ($3.99{\pm}.56$). 3) Differences in the needs for nursing care according to the demographics were significant for gender, marital status, operation experience, and departments consulted. General characteristic variables significantly related to nursing need were as follows: Physical need significantly related to the departments consulted (F=2.23, p=.036). Educational need significantly related to the marital status (F=2.55. p=.012), departments consulted (F=2.30, p= 031). Emotional need significantly related to the marital status (F=2.22, p=.028). Environmental need significantly related to the gender (t=-2.44, p= .016), marital status (F=2.01, p= .046). operation experience (t=-1.99. p= .048). Conclusion: Nursing care needs of patients in the recovery room are significantly related to educational need, physical need, emotional need and environmental need. Intervention plans and program need to be developed to improve strategies to meet nursing needs of patients in the recovery room.

Development of the Home-Based Prenatal Care System via Information Superhighway (초고속 통신망을 이용한 재택산전간호관리 시스템 개발)

  • Due to the rapid socioeconomic development and the introduction of the national health insurance system the general population's need for health care and utilization of health care services have increased dramatically. As a result of this change. Korea is experiencing a shortage of health care facilities and health manpower, and this leads long wailing line at doctor's offices. One of the solutions of this problem could be home health care system for those who have minor health related problems. With this background, this study was conducted to look at the feasibility of a home-based prenatal care system using information superhighway and nursing informatics specialists. With the home-based prenatal care system, the pregnant woman checks her blood pressure, tests her urine for sugar and protein, and measures her body weight at home and sends the information to a hospital computer via the information networks such as public telephone line and information superhighway. Nursing informatics specialist at the hospital will go through each patient record and screen those who have abnormal values and notify them to see a doctor as soon as possible. Besides telemonitoring features, the proposed system will include tole-education capabilities for the patients so that patient can learn whatever they need to know ragarding the prenatal care via information networks. If this system develops and operates, patient can save time in terms of travel to and from the hospital and waiting time in the hospital. And the health care institute can utilize its resource more efficiently.

  • This study was designed to search for the direction to develop housing service which assists the older people to live independently in their community and provides the systematization of community care. The subjects of the research include 533 older people between the age of 60 to 79 residing in Seoul and 84 welfare officials at a dong(in)-office and 67 social workers working at comprehensive social welfare institutions. The results were as follows: Both demanders and deliverers recognized that housing service for older people was necessary for community care, and, in particular, the deliverers had higher degree of necessity as people charged in welfare service. The most needed service was community alarm service which currently is proceeded partially for low-income and living-alone olders. It needed to expand the scope of the people to be served. It appeared that the need for inside safety measures rather than for housing space or structural problems. It requires less effort and cost for those who need simple repair, such as safety grips or slippery protection, and safety checks. Among financial support services, the need for tax benefits appeared the most necessary service for the older people. It suggested that more 'direct' financial support service is needed for the elderly people.

Equity in the Delivery of Health care in the Republic of Korea (의료이용의 형평성에 관한 실증적 연구 -공.교 의료보험 피부양자를 대상으로-)

  • This study is an empirical analysis on the equity in the delivery of heatlh care under the Korean Medical Insurance Corporation System. The purposes of this study are to find out effects of income on the health care utiliztion and measure the income-related inequity in the distribution of health care. This study was carried out based on the fact that the health insurance program has been organized to achieve the equity objective, "equal treatment for equal needs". Of 41, 828 insured persons who had been diagnosed in the 1993 Health Screening Test and utilifzation data from 1, January 1993 through 31, December 1993 were derived from the Benefit Managment File. Inequity was measured by means of I) share approach, ii) standardization concentration curve approach, iii) inequity index, iv) test for inequity. The major findings were as follows : 1. The expenditure shares of the top two quintile groups exceeded their morbidity shares, whereas the opposite was true of the bottom three quintile groups, Which showed a positive HI$_{LG}$ inequity index, suggesting the presence of some inequity favoring the rich group. 2. Compared with other residential areas, the rural area showed the highest positive HI$_{LG}$ irrespective of need indicatior applied. 3. Standardized expenditure concentration indices adjusted by age, gender and need structure were also found to be positive, and therefore still indicated that there has been inequity favoring the rich after the standardization. 4. The Loglikelihood Ratio (LR) test for the statistical significance of income-related inequity of medical care utilization was carried out using the logistic regression model. The resulting loglikelihood ratio test statistic value was 176, which did exceed the 0.5 percent critical value of the chi-square distribution with 28 degrees of freedom, which is 50.993. Therefore, the null hypothesis of no income-related inequity of medical care utilization was rejected at the 99.5 percent confidence level. 5. The Regression based F-test has been carried out for analyzing the income-related inequity of medical expenditure in terms of age, gender, morbidity indicators as explanary variables. The hypothesis of the absence of income-relate inequity was rejected for all need indicators at the 95% confidence level.nce level.

Comparison of Hospital Nurses' Knowledge, Emergency Coping Ability and Educational Need according to Nursing Care Experience for Patient Applied the Home Mechanical Ventilator (가정용인공호흡기 적용환자의 간호경험 여부에 따른 병원간호사의 관련 간호지식, 응급대처능력 및 교육요구도의 비교)

  • Purpose: This study aimed to investigate the hospital nurses' knowledge, emergency coping ability and educational need about nursing care for patients applied the home mechanical ventilator (PaHMV) and to compare the variables between yes or not experienced on that. Methods: Participants were 91 hospital nurses who worked at the wards where PaHMV can be highly admitted such as respiratory internal medicine, rehabilitation medicine, neurology and emergency room. Data were collected by questionnaires. The analytic methods were $n(%)/M{\pm}SD$, t-test/ANOVA and $X^2-test/ANCOVA$. Results: Knowledge and emergency coping ability were low as $2.27{\pm}0.52$(4 points) and $78.72{\pm}8.06$(100 points) respectively. But educational need was high as $3.10{\pm}0.34$(4 points). Knowledge and emergency coping ability showed the significant differences between two groups (p<.001, p=.048), and the scores of experienced nurses were higher. But the educational need didn't show the significant difference (p=.974). Conclusion: These findings indicate that education on nursing care of PaHMV is needed for hospital nurse. So we have to develop the educational program on nursing care of PaHMV and then operate it with practice and site education.
