바우처 제도의 이용자 선택권 강화가 돌봄노동자에게 미친 영향과 그 의미 (Enhanced User Choice of Voucher Program and its Impact and Implications on Care Worker)

  • 본 연구는 이용자 선택권 강화 맥락에 초점을 두고 바우처 도입 이후 돌봄노동자에게 나타난 변화를 분석하였다. 분석 결과, 바우처 도입 이후 돌봄노동자들의 고용, 해고, 노동강도 등에 서비스 이용자들이 미치는 영향력이 강화됨에 따라 돌봄노동자들은 노동지위의 취약화와 노동 의미와 상실, 돌봄노동자로서의 정체성 혼란 등 '노동소외'를 경험하는 것으로 나타났다. 돌봄노동자의 노동소외는 사회서비스 질 향상에 부정적인 영향을 미칠 수 있는 요인으로 양질의 사회서비스 공급을 위해서는 서비스 이용자의 권리 향상과 더불어 돌봄노동자의 인권 및 노동권 보호 논의가 함께 이루어져야 함을 제안하였다.

산업장 근로자의 치과의료이용양상 및 관련요인 (Dental care utilization patterns and its related factors of industrial workers)

  • Objectives : This purpose of this study was to examine worker's Dental care utilization patterns and its related factors, to prepare basic data for oral health promotion of workers. Methods : The study subject were 1016 industrial workers from7 working places in Gyung-Sang-Buk Do and Daegu-City. Data for this study was obtained by self-administrated questionnaire. The questionnaire consists of general characteristics, yes or no of dental care utilization before six month, purpose of visit, yes or no of dental disease form the results of dental examination screening before one years, yes or no of and patterns dental care utilization of workers with dental disease, etc. The data was utilized using frequency test and $x^2-test$. Results : Rate of dental care utilization was 40% within six month last, among the general characteristics, it were statistically significant that higher age, married state, bad oral health state, high Work period. Rate of dental disease as a result of their dental examination last was 53.1% among the general characteristics, it were statistically significant that enough economic state, bad oral health state, blue collar work type. Dental care utilization's rate of dental disease as a result of their dental examination last was 48.6% among the general characteristics, it were statistically significant that Enough economic state, higher age, married state, high Work period. Conclusions : In order to promote workers of abnormal opinions to take the medical treatment on their dental diseases later dental examination, it is necessary to educate them consistently for the importance of early medical treatment.

노인환자와 가족의 임종의료결정 권리 및 사회복지사 역할 이해도 - 장기요양 입소 시설 사회복지사를 대상으로 - (A Study of Social Workers' Understanding of Elderly Patients' and Family Caregivers' Rights to End-of-Life Care Decisions and of Their Own Roles in the Process)

  • 목적: 본 연구는 장기요양 입소시설의 사회복지사를 대상으로 임종의료결정 권리, 사회복지사의 역할 이해도를 분석하고, 이들의 상관관계를 분석하고자 한다. 방법: 본 연구는 설문지를 사용하여 수집된 자료 334부 중 분석에 포함하기 어려운 37부를 제외한 297부를 SPSS 20.0을 이용하여 T-test, 분산분석, 상관관계분석 등을 실시하였다. 결과: 사회복지사가 이해하는 노인환자와 가족의 임종의료결정 권리 이해도는 5점 만점에 $3.46{\pm}0.69$, 사회복지사 역할 이해도는 $3.48{\pm}0.84$이다. 사회복지사의 사전의료의향서 및 연명치료 결정 도움 경험, 임종의료결정 도움 업무, 근무 경력, 침상 수 등에 따라 임종의료 결정 권리 및 사회복지사 역할 이해도에 의미 있는 수준에서 집단 간 차이를 보였다. 임종의료결정 권리와 사회복지사 역할 이해도와의 상관관계는 의미 있는 수준에서 긍정적인 상관관계를 보였다(Pearson r=0.329, P<0.001). 결론: 장기요양 입소시설에서 노인환자와 가족에게 사전의료의향서 및 연명치료 결정 도움을 제공하는 사회복지사 역할의 중요성을 인식하고, 노인환자와 가족의 임종의료결정 권리를 보호하기 위하여 임종의료결정에서의 사회복지사 역할 수행을 위한 교육이나 훈련 프로그램의 개발이 필요하다.

노인전문병원 근무자들의 직무만족도 결정요인 (Determinants of Job Satisfaction among Workers at Elderly Care Hospitals)

  • This study purports to investigate the determinant of job satisfaction among workers working at Long-term care hospitals. The independent variables contain three groups of determinants: organizational characteristics variables(job autonomy, job variety, distributive justice, role conflict, supervisor support, job suitability, job significance, job security, organizational support, job growth, promotional opportunity), environmental variables(job opportunity), and psychological variables (met expectation, job efficacy, positive affectivity, and negative affectivity). The sample used in this study consisted of 250 workers from 4 Long-term care hospitals nationwide. Data were collected with self-administered questionnaires and analyzed using multiple regression analysis. The results of the study are as follows: 1) the following variables, listed in order of size, have significant effects on job satisfaction: negative affectivity(-), job significance(+), job growth(+), age(+), positive affectivity(+), organizational support(+), job opportunity(-). 2) the variance of job satisfaction explained by the variables used in the study are 53.8%. When demographic variables added to Model I, job satisfaction explained by variables are 55.4%. 3) the results of this study indicate that three variables of negative affectivity, job significance, job growth are especially important for improving the level of job satisfaction among workers at Long-term care hospitals.

코로나19 대유행 시 일 노인요양시설에 종사하는 요양보호사의 직무경험에 대한 연구 (A Phenomenological Study on the Experiences of Long Term Care Facility Workers during the COVID-19 Pandemic)

  • Purpose: The purpose of this study was to understand the experiences of those who take care of the elderly at a long-term care facility during the COVID-19 pandemic. Methods: Colaizzi's phenomenological research method was applied to describe the meaning and nature of the experiences of eight workers in an elderly care facility during the COVID-19 crisis. Results: Studies have shown that these workers experienced 'fear from infectious disease' and had the opportunity to 'reflect upon life changed by infectious disease.' They also worked together to overcome the crisis by 'protect nursing home with the nurses at COVID-19 sites' and felt 'safety from the nursing home,' and experienced 'a strong sense of calling for the job' through the crisis. Conclusion: This study is the first of its kind conducted in Korea on workers in an elderly care facility. The results indicate that it is necessary to supports these workers with practical useful education, counseling, and emotional support during the epidemic.

근로자의 고용상태가 미충족 의료에 미치는 영향 (The Effect of Employment Status on the Unmet Needs of Medical Utilization in Workers)

  • Objectives: This study was designed to evaluate whether employment status is associated with the experience of unmet care needs. Methods: This study utilized the Korea Health Panel data from 2012 to 2016. A total of 4,083 workers were selected as baseline subjects in 2012 and were followed for four years. This study used the GLIMMIX procedure under the marginal model while adjusting for covariates. Results: A total of 12.4% of 4,083 people said they had failed at least once to have a treatment or checkup despite the needs. Those more likely to experience unmet care needs were women and people of lower income level, with worse health conditions and chronic disease. Precarious workers, the self-employed, and the unemployed were more likely to experience unmet care needs caused by economic burden than permanent workers (Odds Ratio: 2.14, 2.07, 2.74, respectively). Conclusions: This disparity means that precarious workers and the unemployed are more likely to face barriers in obtaining needed health services. Given their insecure employment status, meeting their needs for health care is an important consideration.

재가요양보호사의 직무스트레스와 이직의도의 관계에서 직업의식 매개효과 검증 (The Mediating Effort of Vacational Consciousness on the Relationship between Job Stress and Turnover Intention in Long-Term Care Worker)

  • 본 연구는 재가요양보호사의 직무스트레스와 이직의도와의 관계에서 직업의식 매개효과를 검증하고, 그 결과를 기초하여 재가요양보호사의 이직의도를 감소시키려는 목적으로 수행하게 되었다. 연구대상은 서울·경기지역에서 일하고 있는 재가요양보호사 447명을 대상으로 하였다. 연구목적을 위해 자료분석은 SPSS 통계패키지를 활용하여 기술분석과 상관관계 분석 및 다중공선성 확인 후 매개효과를 검증하였다. 분석결과 첫째, 재가요양보호사가 연령이 낮고, 월가구소득이 높을수록 이직의도가 높을 것으로 나타났다. 둘째, 재가요양보호사가 직무스트레스를 높게 인식할수록 이직의도가 높아지는 것으로 나타났다. 셋째, 직업의식이 높을수록 이직의도는 낮게 나타났다. 넷째, 재가요양보호사의 직무스트레스는 직업의식을 부분 매개하여 이직의도에 통계적으로 유의미한 영향을 주는 것으로 나타났다. 이상의 결과를 토대로 재가요양보호사의 이직을 예방하고 노인돌봄 서비스의 질을 향상시키기 위한 임상적·정책적 실천에 대한 제언을 하였다.

산재보험 진폐증 장기 입원환자의 의료이용 특성 (Health Care Utilization Patterns of Workers' Compensation Pneumoconiosis Patients with a Long Length of Stay)

  • Objectives : This study investigated the health care utilization patterns of workers' compensation insurance(WCI) pneumoconiosis patients with excessively long hospital stays. Methods : The discharge summary data of 3,094 WCI pneumoconiosis patients were analyzed. The study sample was divided into 3 groups based on the length of stay(LOS). Health care utilization patterns were compared among the groups with logistic regression analysis, and the LOS determinants were identified with linear regression analysis. Results : The average LOS of the 222 long stay group patients was 1,448 days. Patients in this group tended to use private general hospitals, were admitted through the emergency room and discharged without the consent of a doctor. Conclusions : Many of the long LOS patients will maintain their inpatient status for the rest of their lives. For quality of life and efficient use of health care resources, policy makers need to establish a policy that enables patients to receive outpatient care in appropriate living conditions outside the hospital.

병원근무자 유니폼에 의한 병원 내 감염에 대한 체계적 문헌고찰 (The Association of Health Care Workers' Uniforms and Health Care-associated Infection: Systematic Review)

  • Purpose: To identify an associations between health care workers' uniforms and health care-associated infection. Methods: Electronic databases, including Ovid-Medline, the Cochrane Library, CINAHL, EMBASE, KMbase, and KoreaMed, were searched. The search terms included doctor, nurse, health care worker/staff/assistant, clothing, (white) gown, uniform, (neck)tie, and attire. Only papers published in English and Korean were included. Results: 16 studies were selected from 1,900 references screened. All of the studies were non-comparative studies except for one. Four were conducted with doctors, six with nurses, one with health care workers including physiotherapists and one for medical staff plus visitors in a neonatal intensive care unit. Doctors more frequently changed their uniforms than neckties; therefore, the degree of contamination was more serious in neckties. The cuff zone was more likely to be heavily contaminated than other areas of long-sleeve gowns. Coats become contaminated quickly once worn, and colony counts reached a similar level within the first few hours after wearing them. Wearing a plastic apron or protective clothing did not prevent the bacterial contamination of nurses' uniforms, and the best way to decrease the contamination was changing to newly laundered uniforms before starting every duty. Conclusion: Healthcare workers' uniforms are a potential source of health care-associated infection although there was no robust evidence. The government must establish standards for laundering of uniforms or a requirement for institutions to provide a laundering service for healthcare workers' uniforms.

일반병원직원의 호스피스 완화의료 교육 요구도에 관한 융합 연구 (A Convergence Study on the Necessity of General Hospital Workers Hospice Palliative Care Education)

  • 이 연구는 호스피스 완화의료 비 지정병원 직원 중 간호조무사와 일반직원을 대상으로 임종 및 호스피스 완화의료 교육 요구도를 융합적으로 파악하고자 하였다. 조사기간은 2016년 9월 22일부터 10월 21일까지로, 대전 충남지역의 10개 병원 호스피스 완화의료 비지정병원 직원 158명을 대상으로 조사하였다. 구체적인 연구 결과는 다음과 같다. 호스피스 완화의료 교육 내용의 이론과 실무분야 교육 중요도에서는 연구 대상자 모두 이론보다 실무에 더 높은 중요도를 가지고 있다고 답했다. 이상의 연구결과가 호스피스 완화의료 비지정병원직원 중 특히 간호조무사를 대상으로 업무 특성과 요구도에 맞는 맞춤식 호스피스 완화의료 교육 프로그램 개발을 위한 기초적인 자료로서 도움이 되기를 기대한다.
