Early Outcomes of Sutureless Aortic Valves

  • Background: In elderly high-risk surgical patients, sutureless aortic valve replacement (AVR) should be an alternative to standard AVR. The potential advantages of sutureless aortic prostheses include reducing cross-clamping and cardiopulmonary bypass (CPB) time and facilitating minimally invasive surgery and complex cardiac interventions, while maintaining satisfactory hemodynamic outcomes and low rates of paravalvular leakage. The current study reports our single-center experience regarding the early outcomes of sutureless aortic valve implantation. Methods: Between October 2012 and June 2015, 65 patients scheduled for surgical valve replacement with symptomatic aortic valve disease and New York Heart Association function of class II or higher were included to this study. Perceval S (Sorin Biomedica Cardio Srl, Sallugia, Italy) and Edwards Intuity (Edwards Lifesciences, Irvine, CA, USA) valves were used. Results: The mean age of the patients was $71.15{\pm}8.60years$. Forty-four patients (67.7%) were female. The average preoperative left ventricular ejection fraction was $56.9{\pm}9.93$. The CPB time was $96.51{\pm}41.27minutes$ and the cross-clamping time was $60.85{\pm}27.08minutes$. The intubation time was $8.95{\pm}4.19hours$, and the intensive care unit and hospital stays were $2.89{\pm}1.42days$ and $7.86{\pm}1.42days$, respectively. The mean quantity of drainage from chest tubes was $407.69{\pm}149.28mL$. The hospital mortality rate was 3.1%. A total of five patients (7.69%) died during follow-up. The mean follow-up time was $687.24{\pm}24.76days$. The one-year survival rate was over 90%. Conclusion: In the last few years, several models of valvular sutureless bioprostheses have been developed. The present study evaluating the single-center early outcomes of sutureless aortic valve implantation presents the results of an innovative surgical technique, finding that it resulted in appropriate hemodynamic conditions with acceptable ischemic time.

Analysis of factors affecting crestal bone loss around the implants

  • Purpose : To determine whether peri-implant crestal bone loss could be affected by systemic disease, primary ISQ value, implantation method (submerged vs. non-submerged), surface treatment, and bone density Materials and methods : Patients who underwent fixture installation from June 24, 2005 to October 23, 2008 at Seoul National University Bundang Hospital were evaluated. A total of 157 patients (male: 52, female: 85) had 346 fixtures installed. Among them, 49 patients had periapical radiographs taken 1 year after prostheses were first set. A total of 97 fixtures were implanted. In particular, 30 fixtures were installed in patients with systemic diseases such as diabetes mellitus, cardiovascular disease, hypertension, and liver disease. The immediate stability of implants was measured with $Osstell^{tm}$. Implant surface treatment was classified into two groups (RBM, Cellnest (Anodized)), and bone density, into four groups (D1~D4). The bone resorption on the mesial and distal areas of fixtures was measured with periapical radiographs using the paralleling technique, and the mean value was calculated. The length determination program in IMPAX (AGFA, Belgium) was used. Results : At least 332 out of 346 (96%) installed GS II implants were successfully osseointegrated 1 year after prostheses were first set. The mean value of the bone resorption of the installed GS II implants was 0.44mm. The minimum value was 0mm, and the maximum value, 2.85mm. There was a statistically significant difference between the implantation methods (submerged, non-submerged) with regard to the amount of alveolar bone loss 1 year after prostheses were first set (p<0.05). Non-submerged implants showed less crestal bone loss. Note, however, that other variables had no correlation with crestal bone loss (p>0.05). Conclusion : There was a statistically significant difference between the 1-stage method and 2-stage method with regard to the amount of alveolar bone loss 1 year after prostheses were first set. Systemic disease, primary ISQ value, surface treatment, and bone density were not associated with alveolar bone loss. Other variables were assumed to have a correlation with alveolar bone loss.

Changes in the Prosthesis Types Used for Aortic Valve Replacement after the Introduction of Sutureless and Rapid Deployment Valves in Korea: A Nationwide Population-Based Cohort Study

  • Background: Sutureless and rapid deployment valves for aortic valve replacement (AVR) were introduced in Korea in December 2016. This study evaluated changing trends in the prosthetic valves used for AVR in Korea after the introduction of sutureless and rapid deployment valves. Methods: From December 2016 to December 2018, 4,899 patients underwent AVR in Korea. After applying the exclusion criteria, 4,872 patients were analyzed to determine changes in the type of prosthetic valve used for AVR. The study period was divided into 5 groups corresponding to 5-month intervals. Results: The total number of AVR cases was 194.88±28.78 per month during the study period. Mechanical valves were used in approximately 27% to 33% of cases, and the proportion of mechanical valve use showed a tendency to decrease, with marginal significance overall (p=0.078) and significant decreases in patients less than 60 years of age and in men (p=0.013 and p=0.023, respectively). The use of sutureless valves increased from 13.4% to 25.8% of cases (p<0.001), especially in elderly patients (>70 years) and those requiring concomitant surgery. In a comparison between sutureless and rapid deployment valves, the use of Perceval S valves (a type of sutureless valve), gradually increased (p<0.001). Conclusion: After the introduction of sutureless and rapid deployment valves in Korea, the rate of use of these new valves remarkably increased, especially in elderly patients and those requiring concomitant surgery. Further studies should investigate the clinical outcomes of these new prostheses.

Mechanical versus Tissue Aortic Prosthesis in Sexagenarians: Comparison of Hemodynamic and Clinical Outcomes

  • Background: The question of which type of prosthetic aortic valve leads to the best outcomes in patients in their 60s remains controversial. We examined the hemodynamic and clinical outcomes of aortic valve replacement in sexagenarians according to the type of prosthesis. Methods: We retrospectively reviewed 270 patients in their 60s who underwent first-time aortic valve replacement from 1995 to 2011. Early and late mortality, major adverse valve-related events, anticoagulation-related events, and hemodynamic outcomes were assessed. The mean follow-up duration was $58.7{\pm}44.0$ months. Results: Of the 270 patients, 93 had a mechanical prosthesis (mechanical group), and 177 had a bioprosthesis (tissue group). The tissue group had a higher mean age and prevalence of preoperative stroke than the mechanical group. The groups had no differences in the aortic valve mean pressure gradient (AVMPG) or the left ventricular mass index (LVMI) at 5 years after surgery. In a sub-analysis limited to prostheses in the supra-annular position, the AVMPG was higher in the tissue group, but the LVMI was still not significantly different. There was no early mortality. The 10-year survival rate was 83% in the mechanical group and 90% in the tissue group. The type of aortic prosthesis did not influence overall mortality, cardiac mortality, or major adverse valve-related events. Anticoagulation-related events were more common in the mechanical group than in the tissue group (p=0.034; hazard ratio, 4.100; 95% confidence interval, 1.111-15.132). Conclusion: The type of aortic prosthesis was not associated with hemodynamic or clinical outcomes, except for anticoagulation-related events.

기계판막치환후의 판막음에 대한 연구 (Physical Analysis of the Prosthetic Valve Sound)

  • The frequency spectrum of the metallic closing sound and its loudness were measured by non invasive techniques in 66 patients. They had examined a total of 7 Carbomedics valve, 10 Duromedics valve, 11 St. Jude heart valve in mitral position and 8 Carbomedics, 10 Duromedics, 20 St. Jude heart valve prostheses functioning normally in aortic position. Statistical comparison of the loudness from sound produced by the three valves in each position, the following; The Carbomedics valve has the lowest average loudness, followed by the St. Jude medical valve, and finally the Edward Duromedics valve. And we analysis the changing factor of the loudness of valve sound, only the velocity of the flow through the valve influenced to the valve sound.

감염된 경정맥 심박조절기의 치료 -2례 보고- (Removal of Infected Transvenous Pacemaker Electrode -2 cases-)

  • Infection, although uncommon, can be the most lethal of all potential complication after transvenous pacemaker implantation. When infection is present, removal of the electrode is the only successful means of therapy. The entrapped electrode can be removed by continuous traction, by use of endoscopic forceps and snares, and by operation. We report 2 cases of removal of infected transvenous pacemaker successfully. In one case of staphylococcus aureus endocarditis associated with a retained pacemaker electrode, the electrode was removed by hand traction. The other case of skin infection of generator pocket, the electrode removed by orthopedic pulley system. Consequently, we recommended removal of all hardware if there is infection of the pacemaker system.

  • 유로키나제와 헤파린을 사용하여 혈전용해요법에 의해 치료된 기계판막 혈전의 3례를 경험하였다. 두 명의 환자는 완전히 회복하여서 자신의 직업과 활동적인 삶으로 되돌아갔으나, 다른 한 명은 뇌출혈과 뇌경색으로 사망하였다.

Duromedics 판막의 임상적 연구 (Clinical Study of Duromedics Bileaflet Valve)

  • From June, 15th, 1987 to June, 14th, 1989, 105 Duromedics bileaflet cardiac valve prostheses were implanted in 81 patients. Mitral valve replacement was done in 42 patients, 7 underwent aortic valve replacement, 28 underwent double valve replacement, & 4 patients underwent triple valve replacement. Concomitantly used valves were 13 cases; 11 cases were St. Jude Medical valves[M: SJM #29 X4, #27 X5, #25 X 1, T: SJM #33] & two cases were Carpentier Edwards bioprostheses[T: C - E #31X2]. The early mortality rate[within 30 days] was 3.7%[2 patients] & the late mortality rate was 7.4%[3 patients]. Follow-up was done on 72 surviving patients; mean follow-up period was 21.17$\pm$5.36 months. Anticoagulant-related hemorrhage was observed in two patients, possible prosthetic valvular endocarditis was observed in one patient and other specific valve-related complications were none. We concluded, therefore, that good clinical results & a low complication rate could be achieved with Duromedics bileaflet valve in short-term follow-up, & long-term follow-up was also necessary.

The Prognostic Significance of Patient-Prosthesis Mismatch after Aortic Valve Replacement

  • Patient-prosthesis mismatch (PPM) is a controversial issue in current clinical practice. PPM has been reported to have a negative impact on patients' prognosis after aortic valve replacement in several studies, showing increased all-cause and cardiac mortality. Moreover, a close relationship has recently been described between PPM and structural valve deterioration in biological prostheses. In patients at risk for PPM, several issues should be considered, and in the current era of cardiac surgery, preoperative planning should consider the different types of valves available and the various surgical techniques that can be used to prevent PPM. The present paper analyses the state of the art of the PPM issue.

St. Jude Medical 기계판의 임상적 연구 (Clinical Analysis of St. Jude Medical Valve Replacement)

  • Results of St. Jude Medical valve replacement are presented in 171 patients who underwent operation from January 1983 through June 1989. The patients were 79 males and 92 females with ages ranging from 12 to 71 years. Total 211 of St. Jude Medical valves were implanted: 148 in mitral position, 57 in aortic position, 6 in tricuspid position. The follow-up was from 2 to 76 months with a cumulative period of 375 patient-year. The actuarial survival at 1 year, 3 year and 5 year were 92.1 %, 87.6 % and 86.3% respectively. The linearlized incidences of valve failure, thromboembolism, thrombotic obstruction, anticoagulation related hemorrhage and all valve related complication were as follows: 0.5 %/pt-yr, 0.5 %/pt-yr, 0.5 %/pt-yr, 1.1 %/pt-yr, and 2.4 %/pt-yr, respectively. The performance of the St. Jude Medical valve compares most favorably with other artificial valves. But it remains still hazards of mechanical prostheses such as thromboembolism and thrombotic obstruction.
