Cor Triatriatum (A case report) (삼중방심 치험 1례)

  • Cor triatriatum is a rare congenital malformation of the heart in which a diaphragm stretches in a transverse plane through the left pulmonary venous chamber, thus creates two subchambers. The proximal chamber connects with the pulmonary veins, and the distal one has left auricle and the mitral valve. A 3 year old boy who had Cor triatriatum underwent surgical excision of the abnormal diaphragm in March, 1989 in Chungnam University Hospital. An obliquely oriented fibromuscular diaphragm divided the left atrium into a proximal chamber which was connected to the pulmonary veins and a distal chamber which had the atrial appendage and the mitral valve. The opening in the diaphragm was 5 mm in diameter. There were no associated abnormalities. The abnormal diaphragm was completely excised. The postoperative result was excellent.

A case of Angiomatosis on the Chest Wall -1 Case Report- (흉벽에 발생한 혈관종증 -1례 보고-)

  • Angiomatosis [ or Diffuse Hemangima] is a rare condition in which large segments of the body are involved by proliferating vessels. By now, some authers say that this lesion begins during early intrauterine life when the limb buds form, grow proportionately with the fetus, and consequently affect large areas of the trunk or extremity. The majority of angiomatoses present during childhood or infancy as swelling, induration, or discoloration of the affected area. The patient was 13 year old male and had no specific signs and symptoms except palpable mass on the left lateral chest wall from childhood and a painful tender mass on the posterior chest wall for 5 days before admission. The chest PA showed no abnormalities.The operation was done with the resection of left 10th rib and 9th and 10th intercostal muscles including masses and the ligation of the both intercostal and feeding vessels of the masses. The pathologic result was angiomatosis involving intercostal muscles and rib.

Infantile Lobar Emphysema -A Case Report- (유아 엽성 폐기종 -1례 보고-)

  • Infantile lobar emphysema is a pulmonary hyperinflation state that has the clinical features of an air block syndrome characterized by bronchial cartilaginous abnormalities or unknown origin. Left upper lobe was affected in most of the reported infantile lobar emphysema. Infantile lobar emphyema is divided into two categories. e.g., congenital and acquired. We have experienced a case of left lower lobe involved infantile lobar emphysema which had undergone left pneumonectomy. She had progressive signs of tension accompanied by mediastinal displacement, ventilatory and circulatory failure in infant period. Because of the combined left upper lobe hypoplasia, left pneumonectomy was performed. And there was no cartiliginous abnormality in pathologic finding. This is the first domestic case which was affected in the lower lobe and successful surgical repaired.

Surgical treatment of Ebstein`s Anomaly-Report of 6 cases- (Ebstein 기형의 외과적 치료-6례 보고-)

  • Increased clinical interest, stimulated by newer diagnostic techniques, has made it apparent that Ebstein`s anomaly represents a spectrum of right heart abnormalities. In the usual case, the septal and posterior leaflets of the tricuspid valve are not attached to the true annulus, but are displaced distally into the ventricle. Also the degree of malattachment and the structural deformities of the cusps may vary greatly. Recently we experienced 6 cases of Ebstein`s anomaly who underwent corrective operation in our hospital. Five patients were operated by plication and annuloplasty techniques based on the construction of a monocusp valve by the use of the anterior leaflet. Remained one was operated on by tricuspid valve replacement using No 31 mm C-E tissue valve. There was no operative mortality and the postoperative courses were relatively good with marked symptom improvement.

A Clinical Study of the Funnel Chest: Report of 14 Cases (누두흉의 임상적 고찰-14례 보고-)

  • Fourteen cases of funnel deformity, 11 were male and the others female, treated over a eleven-year period, are presented. The overall results with the methods described by the authors, Wada, Shannon, Adkins and Ravitch appear to be excellent. The symmetric depression was more common In children under the age of 12 years and asymmetric one Increased after the age of 12. Six cases of abnormal cardiac auscultatory findings revealed no consistent hemodynamic abnormalities. An 21-year-old female was associated with left lung agenesis and dextroversion of the heart, and a 6-year-old boy with congenital bronchogenic cyst who underwent right upper lobectomy. EKG changes associated with the anomaly were observed in all. Pulmonary function test showed the range from normal to moderate restriction which did not Improve in only one patient after operation, but the patient did not complain any restriction In activity. Some transient complications were developed which resulted in improvement.

  • Tetralogy of Fallot has shown its familial aggregation in several familial studies. This reported case is another example which occurred in a family in two brothers. They revealed no cytogenetic abnormalities, but the anatomical cardiac malformation of them showed much similarity, total conal defect in ventricular septum and .one had patent foramen ovale, the other atrial septal defect. The familial recurrence tendency of Tetralogy of Fallot as well as other congenital heart diseases could be explained on multifactorial inheritance as shown in many reports. In spite that we couldn`t find out any environmental trigger or teratogens, our case may be accepted on the base of multifactorial mechanism.

Aorto-coronary Bypass for Unstable Angina - one case report - (불안정형 협심증의 관상동맥 우회수술치험 1)

  • Unstable angina pectoris is often premonitory to an acute myocardial infarction. Surgical revascularization in this syndrome is of great potential benefit and relatively low mortality. A patient with unstable angina pectoris is reported. A 65-year-old man complained of dyspnea and pain in the left anterior chest. The pain was brought on by mild exercise, occurred at rest and sleeping time. The pain worsened over a month period and more aggravated in intensity and duration. Physical examination showed no abnormalities except hypertension and laboratory data were within normal limits. His anginal pain was not relived by nitroglycerin ingestion. Preoperative coronary angiograms revealed significant obstruction [>90%] of left anterior descending coronary artery. Aorto-left anterior descending coronary bypass with autogenous saphenous vein used as conduit was performed. The postoperative course was uneventful and he was discharged on 16th postoperative day in a healthy condition.

  • In "Hwangjenaegyeong(黃帝內經)", the name of blood vessels is closely related to images of the river, and the word for blood vessel contains the following meaning, such as blood vessel, blood wind as well as blood stasis and so on. In a physiological perspective, the unobstructed blood vessel and normal operation blood are important symbols of health, for blood is the basis for human survival, physical activity and mental activity. In pathology, the external cold, excessive seven emotions and a biased diet can cause many kinds of abnormalities of the blood vessels, which can in turn become causes for a variety of diseases. The theory of blood vessels in "Hwangjenaegyeong" can provide guidance for diagnosis and treatment of cardiovascular diseases in practice.

Clinical characteristics in Taiwanese women with polycystic ovary syndrome

  • Polycystic ovary syndrome (PCOS) is one of the most common hormonal endocrine disorders in women of reproductive age. It consists of a heterogeneous collection of signs and symptoms that together form a disorder spectrum. The diagnosis of PCOS is principally based on clinical and physical findings. The extent of metabolic abnormalities in women with PCOS varies with phenotype, body weight, age, and ethnicity. For general population, the prevalence of hyperandrogenism and oligomenorrhea decreases with age, while complications such as insulin resistance and other metabolic disturbances increase with age. Obese women with PCOS have a higher risk of developing oligomenorrhea, amenorrhea, hyperandrogenemia, insulin resistance, and lower luteinizing hormone (LH) to follicle stimulation hormone (FSH) ratios than non-obese women with PCOS. The LH to FSH ratio is a valuable diagnostic tool in evaluating Taiwanese women with PCOS, especially in the diagnosis of oligomenorrhea. Overweight/obesity is the major determinant of cardiovascular and metabolic disturbances in women of reproductive age.

Surgical Treatment of Spontaneous Pneumothorax by Thoracoscopic Wedge Resection with Fibrin Glue (비디오흉강경을 이용한 자연기흉의 치료시 국소적 Fibrin Glue 도포)

  • Background ; To evaluate the efficacy of Fibrin glue to decrease recurrence in video-assisted thoracoscopic surgery(VATS) for a treatment of spontaneous penumothorax. Material and Method : All medical records of 17 patients who underwent a thoracoscopic wedge resections of bullae with stapling device with Fibrin glue in our institute between May 1998 and December 1999 were reviewed. variables analyzed include affected sites primary indication of VATS. duration from admission to discharge duration of postoperative stay duration of chest tube drainage recurrence and complication. There were 16 men and 1 woman. Result : There was no evidence of hemodynamic instability or arterial blood gas abnormalities encountered during the procedure. Mean age at the time of the VATS was 26.9 years (range 15 to 61 years) The mean duration from admission to discharge was 7.8 days and mean postoperative stay was 5.1days mean chest tube indwelling period was 4..0 days. There was no recurrence of pneumothorx. Conclusion : Thoracoscopic wedge resections with introduction of fibrin glue are safe and effective and requires only a short hospital stay. We believe that this thoracoscopic technique will further simplify the surgical treatment of pneumothorax.
