Acute heart failure is associated with high mortality and frequent rehospitalization, resulting in enormous healthcare costs and declining physical function, activities of daily living, and quality of life. Cardiac rehabilitation has been recommended as one of the non-pharmacologic treatments for patients with heart failure. However, much of the evidence for cardiac rehabilitation interventions reported to date has been limited to chronic heart failure. In recent years, the effectiveness of rehabilitation intervention in patients with acute heart failure has been reported, led by the Rehabilitation Therapy in Older Acute Heart Failure Patients (REHAB-HF) trial. This review overviews the recent evidence of rehabilitation in patients with acute heart failure.
The purpose of this study was to determine the effects of an rehabilitation educational program on anxiety, health compliance and quality of life of the patient with cardiac valve replacement. The 80 Subjects for the study were surveyed at the outpatient clinic in S medical center in Buchon City in Korea. Among them, 40 patients had been taught about the cardiac rehabilitation education program and 40 patients had not been taught while they were in the hospital. Those 2 groups of patients were surveyed with the instruments of Spilberger's anxiety scale, and the questionnaire of health compliance and quality of life. The data were analyzed by using $X^2$-test, t-test, Pearson's correlation. The results of this study were as follows: 1. Anxiety in educated group was significantly decreased (t=-2.60, p= .011) comparing to u n edu cat ed group. 2. The health compliance in educated group was significantly in creased (t=2.09, p= .040) comparing to uneducated group. 3. The quality of life of the two groups of pa tients was not different significantly (t=1.83, p= .072). 4. The correlation between anxiety and health compliance was significantly reversed (r=- .461, p= .000). The correlation between anxiety and quality of life was significantly reversed (r=- .654, p= .000). The relationship between health compliance and quality of life revealed significant (r=.438, p= .000). Considering above results, the cardiac rehabilitation educational program for the patients with the cardiac valve replacement was strongly recommended to be used.

Purpose: The purpose of this study was to investigate the knowledge of cardiac rehabilitation, educational needs, and self care in inpatients with coronary artery disease in China and to provide basic data for development of cardiac rehabilitation programs. Method: From Jan. 16th to Feb. 26th, 2009, 104 patients with coronary artery disease were hospitalized in general hospitals in K and H city in China. They were interviewed with a structured questionnaire. The data was analyzed using descriptive statistics, t-test, ANOVA, and Pearson's correlation analysis. Results: In inpatients with coronary artery disease in China, the score of knowledge of cardiac rehabilitation was average ($66{\pm}.18$), and the score of educational needs was high ($4.27{\pm}.49$). The score of self care was average ($3.12{\pm}.58$). The regular exercise group (p= .005) had a high knowledge of cardiac rehabilitation, the nonsmoking group (p= .022) and regular exercise group (p= .037) did well by themselves. Educational needs also affected the overall knowledge of inpatients with coronary artery disease (r= .288, p= .003). Conclusion: This study suggests that knowledge of cardiac rehabilitation and educational needs should be considered in enhancing cardiac rehabilitation programs designed for inpatients with coronary artery disease in China.
Purpose: This study was to analyze research reports published in Korea on cardiac rehabilitation for patients with cardiovascular disease. Methods: Based on inclusion criteria, 19 research reports were included in this review. Published year of selected articles was between 1996 and 2008. Nineteen studies were analyzed by guidelines of the cardiac rehabilitation programs done by the American Heart Association(AHA) and Scottish Intercollegiate Guidelines Network(SIGN). Results: The characteristics of patients were mostly of ischemic heart disease, 50-59 yr old, and male. Educational sessions were administered twice, and each lesson lasted less than 30 min. Exercise was done 3 times per week for 6 weeks. Most educational content were about risk factors, but there was no information, such as coping with feelings. Most exercise was performed as ROM, treadmill, and cycle ergometer. A large percentage of outcome indicators were health behavior, hemodynamic changes, and exercise capacity. There was more 'no effect' than 'positive effect' in trait anxiety and depression, whereas similar in physiologic domain. Conclusion: Various types of cardiac rehabilitation in Korea were performed by researchers. Therefore, we need to develop the standard protocol, to add psychosocial intervention, and to study cost effectiveness of cardiac rehabilitation.

Purpose: This study was done to evaluate the effectiveness of cardiac rehabilitation education individualized to PCI patients in order to improve their knowledge of these diseases and to lessen their anxiety. Method: A Quasi experimental design with non-equivalent control group non-synchronized design was used. The experimental group had the PCI operation for ischemic heart disease and individualized cardiac rehabilitation education and counseling twice for 25 minutes each time using an educational booklet developed by the authors. The effects of the education were analyzed using a knowledge assessment tool, state anxiety inventory and anxiety visual analogue scale. Results: The experimental group who received the individualized cardiac rehabilitation education showed a high level of knowledge about diseases compared to the control group and particularly showed a significant difference in knowledge about the drugs used for treatment. However, no significant difference was observed between the two groups in the level of state anxiety and anxiety visual analogue scale. Conclusion: Individualized cardiac rehabilitation education did not reduce anxiety but it was effective in enhancing the participants' knowledge about the diseases. Thus, it can be utilized effectively in addressing risk factors in ischemic heart diseases by providing education individualized according to patients' demands and knowledge levels.
Purpose: The purpose of this study was to examine the effects of the cardiac rehabilitation program(Phase III) on cardiovascular and cardiorespiratory function in the elderly (age, ${\geq}$ 60) who experienced acute myocardial infarction. Methods: The design of research was nonequivalent control group pretest-posttest design. Subjects were 10 people in the experimental group and 10 in the control group. The intervention program was the cardiac rehabilitation program(Phase III), and was performed for 50 ~ 60 minutes, twice a week for 8 weeks. The dependent variables were the cardiovascular and cardiorespiratory function. Results: Significant differences were shown in RHR, RSBP, SRPP, SBS, BMI, PT of the experimental group after the intervention. As for the differences in the amount of changes, there were differences among groups in RHR, RSBP, SRPP, SBS, PMET, and PT of the experimental group. Conclusion: The cardiac rehabilitation program(Phase III) may be effective in improving cardiovascular function and enhancing cardiorespiratory function in elderly patients with acute myocardial infarction.
Purpose: The purpose of this study was to investigate the effects of a cardiac rehabilitation program for patients with myocardial infarction. Method: The subjects were 45 myocardial infarction patients(22 for experimental group and 23 for the control group). Data were collected through questionnaire survey, blood test, and the treadmill test, between October 1, 1999 and December 31, 2000. As for data analyses, paired and unpaired t-test and $\chi^2$ test were adopted using the SAS program. Result: 1. The post-test revealed that increase in compliance score of health behavior was significantly higher in the experimental group than in the control group. 2. The increase in HDL cholesterol was significantly higher in the experimental group than in the control group, in the post-test. 3. The experimental group showed significantly higher duration of exercise time and maximal METs than the control group after the program. Conclusion: The above findings indicate that a cardiac rehabilitation program was effective in increasing compliance of health behaviors, serum HDL cholesterol level, duration of exercise time, and maximal METs in patients with myocardial infarction. Accordingly, we can adopt the individualized cardiac rehabilitation programs as a nursing intervention.
Purpose: The purpose of this study was to identify the quality of recovery and influential factors on the quality of recovery after cardiac surgery. Methods: 198 patients undergone cardiac surgery were asked to fill in a self-reported questionnaire about the quality of recovery, anxiety, depression including social support at discharge. The collected data were analyzed with mean, standard deviation, correlation and stepwised multiple regression. Results: The mean scores of quality of recovery at discharge after cardiac surgery was 2.04 on a 3 point scale. Influential factors on the quality of recovery after cardiac surgery were depression(p=.001) and anxiety(p=.027), which disclosed 44.2% of explanation. Depression was the most influential factor. Conclusion: The influential factors on the quality of recovery at discharge after cardiac surgery were depression and anxiety. More studies will be required to reduce depression and anxiety in patients undergone cardiac surgery.

The aim of this study was to evaluate the effect of exercise-based cardiac rehabilitation on pro- and anti-inflammatory markers in patients with acute coronary syndrome (ACS). ACS patients who underwent percutaneous coronary intervention (PCI) and took medicine during phase II of rehabilitation were recruited for study. Subjects were divided into two groups; exercise group (EX, n=21) and a non-exercise group (non-EX, n=13). Supervised exercise program in hospital consisted of treadmill and bicycle exercise was performed three times per week for 6 weeks. Patients of EX received individual counseling, including knowledge of heart disease, risk factor modification, and physical training. Cardiopulmonary fitness, body composition, and biochemical blood factors were analyzed before and after experiment. There was no significant difference in serum levels of hs-CRP and TGF-${\beta}1$ between groups, and between time intervals. But there was a significant decrease in serum levels of IL-18 (P<.001). And there was a significant increase in ratio of IL-18 to IL-10 (P<.01) and serum levels of IL-10 (P<.001). After cardiac rehabilitation, there was significant increase in exercise duration (P<.001), maximal oxygen uptake ($VO_{2peak}$; P<.001) and decrease in submaximal rate-pressure product (sRPP; P<.05) in EX. In conclusion, exercise-based cardiac rehabilitation during phase II in patients with ACS after PCI decreased serum IL-18 (pro-inflammatory) content and ratio of IL-18 to IL-10 in serum (highly related with disease recurrence), and increased serum IL-10 (anti-inflammatory) content. In addition, it led to improved cardiopulmonary fitness.
This study was designed to determine the outcomes of the team-based cardiac rehabilitation programs for patients with Ischemic Heart Disease(IHD) on their level of health behavior performance, anxiety, and quality of life. A total of 19 in-patients with IHD who have had PTCA treatment at G.Hospital in Inchon were selected as subject to data collection from 1 May 1998 to 30 Oct. 1998. The experimental group, consisting of 11 patients participated in the Hospital's cardiac rehabilitation program was compared with the non-rehabilitation control group of 8 patients. The experimental group has been trained on the cardiac rehabliltation programfor 120-150minute during hospitalization by a team consisted of four specialists, those are cardiologist, head nurse, dietian, and the author. Also individual follow-up training and interview were performed for the members of experimental group at their every visit to the Hospital after discharge. For all subjects, scores on health behavior performance, anxiety, and quality of life were collected two days and 12 weeks after PTCA treatment by questionnaire. The collected data were analyzed with the SPSS. The results of this study were summarized as follows : 1) The experimental group shows significant increase in health behavior performance score compared to the control group(-2.27, p=.022). 2) There was no significant difference between two group's anxiety scores (-.24, p= .803) and quality of life scores(-1.86, p= .061). 3) The correlation between health behavior performance and anxiety ( .07, p= .771), and that between health behavior performance and quality of life( .12, p= .621) respectively were not significant statistically. But the correlation between anxiety and quality of life were significant reversely ( .49, p= .032).
