Future of Autofluorescence Bronchoscopy (형광기관지경의 미래)

  • Lung cancer could be developed through a series of morphological changes from dysplasia to carcinoma in situ and then invasive cancer. However, precancerous lesions are generally a few cell layers thick and are detected only by chance. Autofluorescence bronchoscopy(AFB) is one of the newly developed diagnostic tools to detect the pre-cancerous lesions m the bronchial tissue. Several studies have shown that AFB improved the rate of detection of cancer and dysplastic lesions of the airway, especially those in intraepithelial stage. However, there were high rates of false positive with AFB, and it is also important to develop non-biopsy methods because of lack of accurate information of variable course of preneoplastic lesions regarding progression. So, many other technologies were developed, such as narrow band imaging(NBI), endobronchoscopic ultrasound, optical coherence tomography, and confocal fluorescence microendoscopy. Among the new machines, NBI is a new optical technology that can clearly visualize the microvascular structure m the mucosal layer. NBI seems to increase specificity without compromising sensitivity. In the future such techniques would make it possible to precisely study in detail the natural history of the premalignant epithelium.

Determination of HER2 Gene Amplification in Breast Cancer using Dual-color Silver Enhanced in situ Hybridization (dc-SISH) and Comparison with Fluorescence ISH (FISH)

  • Background: The two basic methods that are currently accepted to identify the HER2 status are immunohistochemistry and flyorescence in situ hybridization (FISH). The aim of this study was to perform the dual-color silver in situ hybridization (dc-SISH) technique as an alternative to FISH. Materials and Methods: A total of 40 invasive breast carcinoma cases were assessed for HER2 gene amplification by FISH and dual-color SISH. Results: Significant correlation was found in the HER2 expression results obtained with the two approaches (p=0.001, p<0.05). The concordance rate was 92.3%. Conclusions: Foutine practical use of the dc-SISH method, which is much easier to apply, score, and evaluate, has many advantages. HER2 and CEN17 status can be evaluated simultaneously with the newly developed "Dual-Color Probe". All these specifications and the reliable results obtained support the widespread use of SISH technique in clinical practice.

  • The most common forms of nonmelanocytic skin cancer (NMSC) are basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). The growing incidence of skin cancer in the Republic of Korea has sparked increasing scientific interest in these types of tumors. In the case described herein, multiple NMSCs occurred asynchronously in various areas of the body in a single patient. A 67-year-old man presented with an ulcerative lesion on the right retro-auricle and multiple keratinized masses on the back and face. The right retro-auricular lesion was diagnosed as BCC, and the keratinized masses on the patient's back and face were diagnosed as SCC. He subsequently presented with numerous pigmented skin lesions on the forehead, temple, pre-auricle, neck, right forearm, right hand, and both thighs. One lesion on the neck was diagnosed as BCC, and five lesions on the right hand and forearm were diagnosed as SCC. The patient was also diagnosed with supraglottic SCC and external auditory canal SCC. An otolaryngologist performed radical excision of the primary SCC. Suspected skin cancer lesions observed on the face and both ears were diagnosed as SCC. Patients with multiple NMSCs are at an elevated risk for additional skin cancers, making periodic follow-up important; furthermore, all suspicious lesions should be biopsied.

Comparison of the Imaging Features of Lobular Carcinoma In Situ and Invasive Lobular Carcinoma of the Breast (유방의 소엽상피내암과 침윤성 소엽암의 영상의학적 소견 비교)

  • Purpose To investigate the usefulness of imaging features for differentiating between small lobular carcinoma in situ (LCIS) and invasive lobular carcinoma (ILC). Materials and Methods It included 52 female with LCISs (median 45 years, range 32-67 years) and 180 female with ILCs (median 49 years, range 36-75 years), with the longest diameter of ≤ 2 cm, who were evaluated between January 2012 and December 2016. All the female underwent mammography and ultrasonography. Twenty female with LCIS and 150 female with ILC underwent MRI. The clinical and imaging features were compared, and multivariate analysis was performed to identify the independent predictors of LCIS. Female with LCIS were also sub-grouped by lesion size and compared with the female with ILC. Results Multivariate analysis showed that younger age (odds ratio [OR] = 1.100), smaller lesion size (OR = 1.103), oval or round shape (OR = 4.098), parallel orientation (OR = 5.464), and isoechotexture (OR = 3.360) were significant independent factors predictive of LCIS. The area under the receiver operating characteristic curve for distinguishing LCIS from ILC was 0.904 (95% confidence interval, 0.857-0.951). Subgroup analysis showed that benign features were more prevalent in female with smaller LCISs (≤ 1 cm) than in those with ILC. Conclusion Small LCISs tend to demonstrate more benign features than small ILCs. Several imaging features are independently predictive of LCIS.

Basaloid Squamous Carcinoma of Unusual Sites (생소한 위치에 발생한 기저양 편평세포암종)

  • Basaloid squamous carcinoma (BSC) is an uncommon aggressive variant of squamous cell carcinoma with a predilection for hypopharynx, tongue base, and larynx. We present 5 cases of BSC of unusual sites, each from maxillary sinus, external auditory canal, submandibular gland, tonsil, and nasopharynx. Only a few cases arising in these sites have been reported to date. Patients included 3 men and 2 women with the age range of 45-69 years (mean, 56.4 years). Microscopically, the tumors were characterized by solid lobules and nests of ovoid basaloid cells with abundant desmoplastic stroma. Comedonecrosis, peripheral palisading of tumor cells, trabecular pattern, and rosette-like arrangement were commonly observed. Tumor cells had scanty cytoplasm and their nuclei were ovoid, relatively uniform, and hyperchromatic. In two cases, concomitant squamous cell carcinoma in situ was identified. Immunohistochemical stains revealed that tumor cells were strongly positive for pancytokeratin and negative or weakly positive for p63. Being aware of BSC that can arise from unusual sites would help diagnose correctly and treat properly this rare and distinct clinicopathologic entity.

  • 본 논문은 유방질환 중에서 Duct(관)에 발생하는 유방 종양을 benign(양성종양)/DCIS (Ductal Carcinoma In Situ)/NOS(Invasive ductal carcinoma)로 자동 분류하기 위한 분류방법을 제안한다. 분류기 생성에서 가장 중요한 단계인 특징 추출단계에서는 wavelet 변환을 적용하였으며, wavelet 변환의 각 depth에 따라 분류기를 생성하여, depth와 생성된 분류기의 분류 정확도와의 상관관계를 비교.분석하였다. 현미경 100배 배율과 400배 배율의 유방 질환 영상을 1, 2, 3, 4단계(depth)의 wavelet 변환을 적용한 후, 분할된 서브밴드에서 GLCM을 이용하여 질감 특징(Entropy, Energy, Contrast, Homogeneity)을 추출하여, 이 특징값들을 조합하여 판별분석에 의해 분류기(classifier)를 생성한 후, 분류 정확도를 검증하였다. Benign/DCIS/NOS를 분류하려면 최소 3단계 이상의 wavelet 변환을 적용해야 하고, 400배 배율 영상보다는 100배 배율의 영상이 더 나은 결과를 보였다.

  • In this paper we propose three classification algorithms to classify breast tumors that occur in duct into Benign, DCIS(ductal carcinoma in situ) NOS(invasive ductal carcinoma) The general approach for a creating classifier is composed of 2 steps: feature extraction and classification Above all feature extraction for a good classifier is very significance, because the classification performance depends on the extracted features, Therefore in the feature extraction step, we extracted morphology features describing the size of nuclei and texture features The internal structures of the tumor are reflected from wavelet transformed images with 10$\times$ and 40$\times$ magnification. Pariticulary to find the correlation between correct classification rates and wavelet depths we applied 1, 2, 3 and 4-level wavelet transforms to the images and extracted texture feature from the transformed images The morphology features used are area, perimeter, width of X axis width of Y axis and circularity The texture features used are entropy energy contrast and homogeneity. In the classification step, we created three classifiers from each of extracted features using discriminant analysis The first classifier was made by morphology features. The second and the third classifiers were made by texture features of wavelet transformed images with 10$\times$ and 40$\times$ magnification. Finally we analyzed and compared the correct classification rate of the three classifiers. In this study, we found that the best classifier was made by texture features of 3-level wavelet transformed images.

  • Polypoid lesions of the esophagus occur infrequently and may be benign or malignant. Pseudosarcoma, a malignant polypoid tumor of the esophagus is rare and appears to be a distinct pathological entity; the polypoid portion is composed of sarcomatous spindle cells and the base of the polyp shows in situ or invasive squamous cell carcinoma. We experienced a case of pseudosarcoma of the esophagus and report the case with the review of literature.

Distribution of Oncogenic Human Papillomavirus Genotypes at High Grade Cervical Lesions above CIN 2 Grade with Histological Diagnosis

  • High risk human papillomavirus (HR-HPV) is major risk factor for uterine cervical cancer. There are approximately 15 types of HR-HPV. Liquid based cytology samples (116 samples) with high grade cervical lesions belonging to cervical intraepithelial neoplasia (CIN) 2, CIN 3, carcinoma in situ (CIS) and squamous cell carcinoma (SCC) were used after histologic confirmation. HR-HPV genotype assay was conducted using DNA chips. The HR-HPV infection rate was 81.9% with SCC samples showing the highest HR-HPV infection rate of 31%. CIN 3, CIS and CIN 2 showed infection rates of 25%, 16.4% and 9.5%, respectively. According to age with HR HPV infection rate, the 30~39 years-old group showed the highest infection rate by 92.3%. According to distribution with HR HPV genotyping, HPV 16 showed the highest infection rate by 42.3% whereas HPV 33 and HPV 58 showed infection rates of 11.7% and 10.8%, respectively. HPV 18 which is the second most common infected HPV genotype in the world showed 3.6%. Of the three most common oncogenic HR-HPV genotypes in CIN 2, we detected HPV 16, 35, 58; CIN 3 was HPV 16, 33, 58; CIS was HPV 16, 58, 33 (35/52); and SCC was HPV 16, 33, and 18 (31/52/58). Among the HPV 18, CIN 2, CIN 3, CIS and SCC showed 0.9%, 0.9%, 0% and 1.8%, respectively. The most often used preventive vaccines for cervical cancers use HPV 16 and HPV 18 as targets. However, results derived from this study suggest that a preventive vaccine against HPV 16 and HPV 18 would not be optimal for populations in this study.

Clinical and Histopathological Studies on Carcinoma of the Uterine Cervix in Taegu (대구지방으 자궁경부암에 재한 임상 및 병리조직학적 연구)

  • Clinical and histopathological studies were made on 202 cases of malignancy of cervix, that were visited to the Department of Obstertrics and Gynecology, Yeungnam University Hospital, during 5 years from 1983 to 1987. The results were summarised as follows. 1. Malignancies of the uterine cervix were 10% of total female malignancies. 2. Among 202 cases, 195 cases(96.5%) were squamous cell carcinoma, in which 60 cases(30.0%) of carcinoma in situ, 9 cases(4%) of microinvasive, and 126 cases(62.5%) of invasive carcinoma were included. 3. The average age of the patients with squamous cell carcinoma was 49.4 years old. ; In cases of carcinoma in situ, it was 43.8, microinvasive, 40.0, invasive 52.1 years old. 4. Clinical symptoms of the patient with squamous cell carcinoma in order of frequency were as follows. ; vaginal bleeding(47.5%), abnormal cytology(15.4%), and abnormal vaginal discharge(9.4%). 5. Duration of the chief complaints was most commanly less than 6 months(73.2%), and the average duration was 3.8 months. 6. The most common age of marriage was between 19 to 22 years 0Id(46.5%). The average was 21.5 years old. 7. The gravity was 51.5% in 5~8 times, and average 6.2 times. The parity was 61.9% in 1~4 times, and average 3.9 times. 8. The subdivision of 126 cases of invasive carcinoma was made according to FIGO stage classification, stage I, 40 cases(31.8%), stage II, 54 cases(42.9%), stage III, 11 cases(8.7%), and stage IV, 8 cases(6.3%). 9. The histologic subtypes of invasive squamous cell carcinoma were distributed as follows. ; large cell keratinizing type, 25 cases(19.8%), large cell nonkeratinizing type, 101 cases(80.2%). 10. In the cytologic diagnosis, class I was 2 cases(l.9%), class II was 16 cases(15.1%), class III was 33 cases(31.1%), class IV was 31 cases(29.3%), class V was 24 cases(24.6%). 11. The frequency of lymph node metastasis was 7.5% in stage I, and 11.1% in stage II.
