As currently conducted, standard rodent bioassays do not provide sufficient information to assess carcinogenic risk to humans at doses thousands of times below the maximum tolerated dose. Recent analyses indicate that measures of carcinogenic potency from these tests are restricted to a narrow range about the maximum tolerated dose and that information on shape of the dose-response is limited in experiments with only two doses and a control. Extrapolation from high to low doses should be based on an understanding of the mechanisms of carcinogenesis. We have postulated that administration of the maximum tolerated dose can increase mitogenesis which, in turn. increases rates of mutagenesis and, thus, carcinogenesis. The animal data are consistent with this mechanism, because about half of all chemicals tested are indeed rodent carcinogens, and about 40% of the positives are not detectably mutagenic. Thus, at low doses where cell killing does not occur, the hazards to humans of rodent carcinogens may be much lower than commonly assumed. In contrast, for high-dose exposures in the workplace, assessment of hazard requires comparatively little extrapolation. Nevertheless. permitted workplace exposures are sometimes close to the tumorigenic dose-rate in animal tests. Regulatory policy to prevent human cancer has primarily addressed synthetic chemicals, yet similar proportions of natural chemicals and synthetic chemicals test positive in rodent studies as expected from an understanding of toxicological defenses, and the vast proportion of human exposures are to natural chemicals. Thus, human exposures to rodent carcinogens are common. The natural chemicals are the control to evaluate regulatory strategies, and the possible hazards from synthetic chemicals should be compared to the possible hazards from natural chemicals. Qualitative extrapolation of the carcinogenic response between species has been investigated by comparing two closely related species: rats and mice. Overall predictive values provide moderate confidence in interspecies extrapolation; however, knowing that a chemical is positive at any site in one species gives only about a 50% chance that it will be positive at the same site in the other species.
Epigenetic alterations, represented by aberrant DNA methylation, are deeply involved in human cancers. In gastric cancers, tumor-suppressor genes are inactivated more frequently by promoter methylation than by mutations. We recently showed that H. pylori infection, a potent gastric carcinogenic factor, induces methylation of specific genes in the gastric mucosae. When the methylation levels were analyzed in the gastric mucosae of healthy volunteers, cases with a single gastric cancer, and cases with multiple gastric cancers, who have increasing levels of risks for gastric cancers, there was a significant increasing trend in the methylation levels among the individuals without current H. pylori infection. This finding unequivocally showed the presence of an epigenetic field for cancerization. The degree of the field defect was measured more conveniently using methylation levels of marker genes than using those of tumor-suppressor genes. The presence of an epigenetic field for cancerization has been indicated for liver, colon, Barrett's esophageal, lung, breast, and renal cancers. Since decreased transcription is involved in the specificity of methylated genes, it is likely that specific genes are methylated according to carcinogenic factors. These findings emphasize the usefulness of DNA methylation as a marker for past exposure to carcinogens and future risk of cancer development.
Side effects of tattoos can occur due to unexpected body responses. Concerns about the side effects of tattoos are growing concomitantly with the increasing number of tattooed individuals. We report a rare case of basal cell carcinoma (BCC) on a tattooed eyebrow. A 48-year-old woman with no family history or occupational risk of skin cancer had her eyebrows tattooed 6 years prior, and she noticed a black mass on her right eyebrow 1 year before presentation. Staged excision was planned due to the patient's reluctance regarding the possibility of a wide scar. After the first surgical procedure, a histopathological examination confirmed BCC. Wide excision and primary closure were performed for remnant BCC, and no recurrence was observed. The mechanism of skin neoplasms involving tattoos has not been clearly identified. Unapproved tattoo inks contain carcinogenic substances, and previous case reports indicate that skin neoplasms may differ depending on the tattoo color. Therefore, the carcinogenic effects of these unknown components of tattoo ink might cause skin neoplasms. Accurate component analysis and systematic management of tattoo ink is necessary, and medical practitioners must also pay attention to this possibility because it is easy to overlook tattoos as a causal factor contributing to cancer.

Due to its excellent diagnostic performance, CT is the mainstay of diagnostic test in adults with suspected acute appendicitis in many countries. Although debatable, extensive epidemiological studies have suggested that CT radiation is carcinogenic, at least in children and adolescents. Setting aside the debate over the carcinogenic risk of CT radiation, the value of judicious use of CT radiation cannot be overstated for the diagnosis of appendicitis, considering that appendicitis is a very common disease, and that the vast majority of patients with suspected acute appendicitis are adolescents and young adults with average life expectancies. Given the accumulated evidence justifying the use of low-dose CT (LDCT) of only 2 mSv, there is no reasonable basis to insist on using radiation dose of multi-purpose abdominal CT for the diagnosis of appendicitis, particularly in adolescents and young adults. Published data strongly suggest that LDCT is comparable to conventional dose CT in terms of clinical outcomes and diagnostic performance. In this narrative review, we will discuss such evidence for reducing CT radiation in adolescents and young adults with suspected appendicitis.
In Korea, talc that has been widely used for a lot of consumer products as well as industrial usage until recently was found to be contaminated with asbestos. It becomes a major social issue. Critical health risk about both talc and talc contaminated with asbestos was summarized through literature review. It has been confirmed that talc can pose ovarian cancer when talc powder is used in the genital area. International Agency for Research on Cancer (IARC) already concluded that the perineal use of cosmetic talc can cause possibly carcinogenic to humane(Group 2B), although there was study reporting the lack of a consistent an established correlation between perineal dusting frequency and ovarian tissue talc concentrations and the lack of a consistent dose-response relationship with ovarian cancer risk. The association between talc exposure and ovarian cancer is as strong as in recent studies. The epidemiological studies to date provided inadequate evidence for the carcinogenicity of either inhaled or ingested talc that does not contain asbestos or asbestosiform fibers. Future studies should focus on seeking evidence in talc-exposed populations, collecting reliable information on age at initial used of body powder, exposure assessments related to talc use and dose response relationship in order to identify possible risk of talc ingested or inhaled.

Aflatoxins are metabolites of fungi Aspergillus spp. that world-widely contaminate diverse foodstuffs including com and peanuts. It is well known that aflatoxins are highly mutagenic, carcinogenic and possibly teratogenic. Although aflatoxins have received the great attention among the various mycotoxins due to their potent hepatocarcinogenicity in certain species, it is extremely crucial to elucidate the relative toxicity and carcinogenicity of the types (B$_1$, B$_2$, G$_1$ and G$_2$) of aflatoxins for the risk assessment.(omitted)
Polychlorinated hydrocarbons are continuously released into the environment from various industrial processes. Trichloroethylene (TCE) and tetrachloroethylene (perchloroethylene, PCE) are of primary concern because of their large-scale production, wide industrial application, poor biodegradability, and tendency to circulate in the air and water. The common routes of human exposure to these compounds include inhalation, ingestion, and dermal adsorption. Additionally, they have been detected in various plant foods. Prolonged exposure to these contaminants is associated with certain risks. They are carcinogenic and have other toxic effects, including gastrointestinal, developmental, neurological, and hematological toxicity. To analyze these contaminants, they are generally extracted from various matrices, followed by instrumental analysis. Gas chromatography, often in combination with different detectors, is the most widely used analytical method. This review covers the toxicity, analytical methods, occurrence in foods, and risk assessment of these contaminants.
Objectives: Perchloroethylene (PCE) is a volatile chemical widely used as a solvent in the dry-cleaning and textile processing industries. It was evaluated as Group 2 "probably carcinogenic to humans" by the Integrated Risk Information System (IRIS) of the United State Environmental Protection Agency (U.S. EPA) in 2012. In order to provide a scientific basis for establishing risk management measures for chemicals on the national priority substances list, aggregate risk assessment was conducted for PCE, included in the top-10 substances. Methods: We conducted the investigation and monitoring of PCE exposure (e.g., exposure scenario, detection levels, and exposure factors, etc.) and assessed its multi-media (e.g., outdoor air, indoor air, and ground water) exposure risk with a deterministic and probabilistic approach. Results: In human risk assessment (HRA), the level of human exposure was higher in the younger age group. The exposure level through inhalation at home was the highest among the exposure routes. Outdoor air or uptake of drinking water represented less than 1% of total contributions to PCE exposure. These findings suggested that the level of risk was negligible since the Hazard Index (HI) induced by HRA was below one among all age groups, with a maximum HI value of 0.17 when reasonable maximum exposure was applied. Conclusion: In conclusion, it was suggested that despite low exposure risk, further studies are needed considering main sources, including occupational exposure.
BACKGROUND: The objectives of this study are to investigate the contamination levels of heavy metals in soil, ground water, and agricultural product near the abandoned Boeun and Sanggok mine areas in Korea and to assess the health risk for these local residents exposed to the toxic heavy metals based on analytical data. METHODS AND RESULTS: By the results of human health risk assessment for local residents around Boeun and Sanggok, human exposure to cadmium, copper, arsenic from soil and to lead, cadmium, and arsenic from rice grain were higher in Sanggok, but human exposure to zinc and arsenic from ground water was higher in Boeun. By the results of hazard index (HI) evaluation for arsenic, cadmium, copper, lead, and zinc, HI values in both areas were higher than 1.0. This result indicated that the toxicity hazard through the continuous exposure to lead, cadmium, arsenic from rice, ground water, and soil would be likely to occur to the residents in the areas. Cancer risk assessment for arsenic, risks from the rice were exposed to one to two out of 10,000 people in Boeun and one of 1,000 people in Sanggok. These results showed that the cancer risks of arsenic in both areas were 10~100 times greater than the acceptable cancer risk range of US EPA ($1{\times}10^{-6}{\sim}1{\times}10^{-5}$). CONCLUSION(S): Therefore, if these two local residents consume continuously with arsenic contaminated soil, ground water, and rice, the adverse health effects (carcinogenic potential) would be more increased.
Background: Tobacco smoking is considered a risk factor for cervical cancer development due to the presence of tobacco based carcinogenic metabolites in cervical cells of female smokers. In this study, we investigated the role of the T3801C (MspI) polymorphism of CYP1A1, a gene encoding an enzyme necessary for the initiation of tobacco based carcinogen metabolism, on cervical cancer risk. The T to C substitution may alter CYP1A1 activities, potentially elevating cervical cancer risk. Since results of gene-disease association studies vary according to the study population, the multi-ethnic population of Malaysia provides an excellent representative cohort for identifying and comparing the cervical cancer risk among the 3 major ethnics in Southeast Asia in relation to CYP1A1 MspI polymorphism. Materials and Methods: A total of 195 Thin Prep Pap smear samples from HPV negative and cancer free females were randomly selected as controls while 106 formalin fixed paraffin embedded samples from females with invasive cervical cancer were randomly selected for the cases group. The polymorphisms were identified using restriction fragment length polymorphism (RFLP) PCR. Results: We found no significant associations between CYP1A1 MspI polymorphism and cervical cancer in the general Malaysian female population. However, upon ethnic stratification, the variant C/C genotype was significantly associated with a 4.66-fold increase in cervical cancer risk in Malay females (95% CI= 1.21-17.9; p=0.03). No significant association was observed in the Chinese and Indian females. Additionally, there were no significant associations in the dominant model and allele frequency model analysis in both the general and ethnically stratified female population of Malaysia. Conclusions: Our findings suggest that the C/C genotype of CYP1A1 MspI polymorphism is associated with the development of cervical carcinoma in the Malay females of Malaysia.
