Recurrent Glottic Cancer Treated with Carbon Dioxide Laser: A Case Report

  • A 38-year-old man visited the hospital for a voice change that began two months previously. There were no other accompanying symptoms; the patient had a history of 20 years of smoking. Carbon dioxide (CO2) laser cordectomy was performed on the left side of the vocal cord, and the biopsy results showed squamous cell carcinoma. Radiation therapy was not performed, and CO2 laser cordectomy was performed again because of a recurrence of left vocal cord mass three years after a progress observation.

  • Mess removal, electrocoagulation, cryosurgery are conventional methods in the treatment of various oral mucosal diseases. However, there are several problems or complication during or after surgery using conventional tools. Recently, LASER gradually become useful tool in the surgery of oral mucosal diseases. Of the LASER, carbon dioxide-mediated LASER is widely used one. Carbon dioxide LASER has many advantages such as good bleeding control, decreased damage to adjacent tissue, decreased pain and swelling, reduced scar formation, even bacteriocidal effects. In this reports, the author describe pros and cons of LASER, especially focused on carbon dioxide, and shed light on the field of LASER application in treatment of various oral mucosal diseases.

  • 만성 후두및 기관협착증의 빈도는 점차 증가하고 있는 추세에 있으며 이에 대한 치료로서 후두 및 기관확장술이나 성형술을 비롯하여 단단문합술등, 여러종류의 방법들이 각각의 적응증에 따라 시행되어 왔지만 아직도 만족스러운 결과를 얻기 어려운 경우가 적지 않다. 1972년 Strong과 Jako가 후두질환에 $CO_2$ Laser 사용을 시도한 것을 비롯하여 이비인후과영역에서 $CO_2$ Laser는 새로운 수술방법으로 각광을 받게 되었고 그 뒤를 이어 Mihashi(1976), Lyons(1980)등에 의하여 성공적으로 후두협착증의 치료에 이용된 바가 있다. 저자들은 1982년 4월부터 1983년 3월까지 1년간 후두기관성형술 및 확장술로는 성공적으로 기도를 유지하기 어려웠던 4례와 후두암으로 반측후두절제술 및 성문성형술 (neoglottis)을 받은 후 기도협착을 초래하였던 1례에서 수술현미경하에 $CO_2$ Laser를 사용함으로써 만족할 만한 결과를 얻었기에 문헌고찰과 함께 보고하는 바이다. 1) 후두 및 기관협착의 치료에 Laser를 이용한 술식은 기도유지에 있어 매우 효과적인 새로운 방법이다. 2) Laser수술 후 육아조직형성은 다른 방법에 비하여 비교적 적었다.

Post Treatment Application of Jaungo after a Combined Therapy of Carbon Dioxide Laser and Trichloroacetic Acid in a Case of Vulvar Syringoma

  • Syringoma is a benign eccrine sweat gland tumor that predominantly appears in females during puberty with multiple smooth papules measuring 1-2 mm in diameter. Common locations are on lower eyelids and cheeks. Vulvar syringoma is quite a rare, occurring condition with only a few cases reported. Here, we are reporting a case of 31-year-old female with vulvar syringoma associated with pruritus. The lesion was treated with carbon dioxide (CO2) laser ablation and 50% trichloroacetic acid (TCA) chemical peeling. Jaungo was used for wound care after laser abrasion. The combination treatment was effective for removing syringoma lesions. Post laser management with fusidic acid cream and jaungo cream was sufficient to prevent infection and promote wound healing.

임플란트 주위염 치료시 이산화탄소 레이저의 효과 (The Effect on Treatment of Peri-implantitis with Carbon Dioxide Laser: An Experimental Study in Dogs)

  • The purpose of this experimental study was to examine the effect of the decontamination of carbon dioxide ($CO_2$) laser in treatment of ligature-induced peri-implantitis in dogs. A total 24 implants with a sandblasted with large-grit and acid-etched (SLA) surface were inserted in six mongrel dogs. After a 3-month healing period, experimental peri-implantitis characterized by a bone loss of about 3mm was established by inducing with wires. And then wires were removed and plaque control was implemented. Surgical treatment involving flap procedure + debridement of implants surface with chlorhexidine and saline (group 1), flap procedure + GBR with absorbable collagen membrane (Bio-Gide) and mineralized bone graft (Bio-Oss) (group 2), and flap procedure + $CO_2$ laser application + GBR (group 3) was performed. The animals were killed 8 weeks and 16 weeks after treatment, respectively. A histomorphometric analysis confirmed statistically considerable new bone formation within the limit of the 5 most coronal threads in group 3 compared with group 1 at 16 weeks (P<0.05). And intragroup analysis showed considerable increase of new bone formation in group 3 at 16 weeks compared with 8 weeks (P<0.05). The present study demonstrates considerable new bone formation after treatment of experimental peri-implantitis with flap procedure, $CO_2$ laser application and GBR.

  • 구강 내 연조직 소수술 시, 수술도, 열 요법, 전기요법, 한냉요법 등은 현재까지 사용되고 있는 고전적인 수술법이다. 하지만 이들 통상적인 수술적 방법의 문제로 멸균 상태의 유지, 공간의 제한, 출혈, 창상 치유의 문제, 흉터, 전신질환에 의한 수술의 제한 등이 있다. 최근 레이저가 또 하나의 수술법으로 널리 사용되고 있고, 통상적인 외과적 수술법에 비하여 레이저의 장점으로는 멸균상태 유지, 출혈감소, 통증 감소, 창상의 치유 촉진, 반흔 생성 억제 등이 있다. 특히 탄산가스 레이저는 조직 내수분에 최대한으로 흡수되어 $100{\mu}m$ 정도의 낮은 침투도를 가지며, 주변부 모세 혈관을 응고시켜 구강 내 소수술에 적용 시우수한 지혈 효과 및 수술 시야의 확보를 얻을 수 있다.

후두질환에서 경구강 $CO_2$ 레이저 미세수술 (Transoral $CO_2$ Laser Microsurgery for Laryngeal Lesions)

  • Transoral $CO_2$ laser microsurgery has been widely used for various laryngeal diseases. Laser cordectomy for the management of early laryngeal carcinoma has advantages with regards to oncologic results, preservation of laryngeal functions, morbidity and cost in comparison to those of open surgery or radiation therapy. $CO_2$ laser is also applicable to benign vocal fold lesions such as nodules, polyps, cysts, reinke's edema, granulomas, papillomas, hemangiomas and glottis webs. $CO_2$ laser is suitable for microsurgery because it has small focus size and hemostatic effect. We should also recognize the risks of $CO_2$ laser for safe use.

레이저 후두미세수술의 원칙 (Principles of Laser Laryngeal Microsurgery)

  • Laser laryngeal microsurgery is currently the primary method of treatment of various laryngeal diseases. The development of laryngeal microsurgery came from the introduction of a small spot $CO_2$ laser micromanipulator and more precise microlaryngeal instruments. $CO_2$ laser laryngeal microsurgery has enabled very precise surgery because it has small focus size and hemostatic effect. There are some limitations to the use of the $CO_2$ Laser such as adjacent tissue damage and vocal fold scarring. These problems can be minimized through understanding the mechanisms by which lasers function and correctly manipulating the parameters under a surgeon's control. We should also recognize the safety of $CO_2$ laser for the surgeon to precisely perform the procedure.

Endpoint Depth When Removing Xanthelasma Using CO2 Laser Ablation: A Case Report

  • The carbon dioxide (CO2) laser is one of the treatment options used for xanthelasma palpebrarum. However, even if the full dermal layer is removed, the lesion can recur due to the residual lipid deposits. A 44-year-old male patient with xanthelasma on both upper eyelids was treated with a pulsed dye CO2 laser. On the right upper eyelid, we carried out a CO2 laser treatment until the yellowish plaque was almost invisible and the full thickness of the dermal layer was removed. On the left upper eyelid, the dermal layer was partially removed and an additional squeezing out of yellowish particles was done. The lesion treated by the squeezing out of lipid droplets showed better long-term results than the lesion treated up to the deeper dermal layer.

  • Objectives : Carbon dioxide ($CO_2$) laser, a high power laser has been used for pain management, dermatology, and surgery and laser acupuncture and moxibustion as well, since it had been oscillated in 1964 at Bell Telephone Laboratories in the US. The purpose of this study is to investigate the history of $CO_2$ laser acupuncture through reviewing studies published in early stage of laser medicine. Methods : To investigate the early history of $CO_2$ laser acupuncture, studies were searched in the electronic databases, including PubMed, ScienceDirect, CNKI, Wanfang, J-STAGE, CiNii, KTNP, and OASIS, since 1964. Articles in English, Chinese, Japanese and Korean were included, and there were no limitations in literature types such as reviews, essays, clinical trials, animal experiments and veterinary research. Results : We found that the first $CO_2$ laser application to acupoints was done by a research team in Shanghai in 1976. They used $CO_2$ laser for acupuncture treatment and it was also the first laser acupuncture treatment in China. Since the first case report of $CO_2$ laser for leukopenia, it has been applied to various diseases in China, Korea, Japan and other western countries. It has been widely applied in the fields of dentistry and veterinary medicine, as well as clinical applications. Conclusions : Not only $CO_2$ laser can be used as laser acupuncture by stimulating the acupoints and meridians, but also can produce moxibustion effect by using heat stimulus. Therefore, it is expected that it will be used in various clinical fields in the future.
