CalTOX 모델을 이용한 벤젠 종합위해성평가의 불확실성 분석과 민감도 분석 (Uncertainty and Sensitivity Analyses of Human Aggregate Risk Assessment of Benzene using the CalTOX Model)

  • Objectives: The purpose of this study was to perform an aggregate human risk assessment for benzene in an industrial complex using the CalTOX model and to improve the reliability and predictability of the model by analyzing the uncertainty and sensitivity of the predicted assessment results. Methods: The CalTOXTM 4.0 beta model was used to evaluate a selected region, and @Risk 7.6 software was used to analyze uncertainty and sensitivity. Results: As a result of performing the aggregate risk assessment on the assumption that 6.45E+04 g/d of benzene would be emitted into the atmosphere over two decades, 3% of the daily source term to air remained in the selected region, and 97% (6.26E+04 g/d) moved out of the region. As for exposure by breathing, the predicted LADDinhalation was 2.14E-04 mg/kg-d, and that was assessed as making a 99.99% contribution to the LADDtotal. Regarding human Riskcancer assessment, the predicted human cancer risk was 5.19E-06 (95% CI; 4.07E-06-6.81E-06) (in the 95th percentile corresponding to the highest exposure level, a confidence interval of 90%). As a result of analyzing sensitivity, 'source term to air' was identified as the most influential variable, followed by 'exposure time, active indoors (h/day)', and 'exposure duration (years)'. Conclusions: As for the results of the human cancer risk assessment for the selected region, the predicted human cancer risk was 5.19E-06 (95% CI; 4.07E-06-6.81E-06) (in the 95th percentile, corresponding to the highest exposure level, a confidence interval of 90%). As a result of analyzing sensitivity, 'source term to air' was found to be most influential.

  • Recent criticisms of the conduct and use of risk assessment by regulatory agencies have led to a wide range of proposed remedies, including changes in regulatory statutes and the development of new methods for assessing risk. The mandate to this Committee was more limited. Our objective was to examine whether alterations in institutional arrangements or procedures, particularly the organizational separation of risk assessment from regulatory decision-making and the use of uniform guidelines for inferring risk from available scientific information, can improve federal risk assessment activities. Before undertaking to determine whether organizational and procedural reforms could improve the performance and use of risk assessment in the federal government, the Committee examined the state of risk assessment and the regulatory environment in which it is performed. In this chapter, we define risk assessment and differentiate it from other elements in the regulatory process, analyze the types of judgments made in risk assessment, and examine its current government context. Because one chronic health hazard, cancer, was highlighted in the Committee's congressional mandate and has dominated public concern about public health risks in recent years, most of our report focuses on it. Furthermore, because activities in four agencies--the Environmental Protection Agency (EPA), the Food and Drug Administration (FDA), the Occupational Safety and Health Administration (OSHA), and the Consumer Product Safety Commission (CPSC)--have given rise to many of the proposals for changes in risk assessment practices, our review focuses on these four agencies. The conclusions of this report, although directed primarily at risk assessment of potential carcinogens as performed by these four agencies, may be applicable to other federal programs to reduce health risks.

다중이용시설에서의 먹는물 위해성 평가 (Risk Assessment of Drinking Water Pollutants)

  • As expanding municipal and industrial areas since started the economic development plan in earnest in 1970's, the water resource, mainly river surface water has been seriously polluted. Nevertheless, being upgraded in their treatment technologies for the drinking water, the safety has been issued one of crucially social problem in Korea. The water authorities has tried to improve the quality such as amending the drinking water quality standard to be monitored; hazardous chemicals and microorganis have been added and now 55 items, before 47 items, since in 2002. The Water Authorities of Seoul, the capital city of Korea, planned to assess the safty of drinking water quality after amended the standard. This study was conducted to assess the risk due to polluted chemicals including 21 heavy metals, VOCs, pesticides, PAHs, DBPs and organic chemicals among the regulated items. The risk assessment were undertaken hazard identification, exposure assessment, dose-response assessment and risk characterization. For the exposoure assessment, tap water, bottled water and purified water were sampled and analyzed in February, 2004. Risk characterization of detected chemicals was categorized into carcinogenecity and noncarcinogenecity, and estimated the excess of carcinogens and compapared with the reference dose (RfD) of noncarcinogenns. The excess risk of carcinogens from samples were considered comparatively in the acceptable levels; $10^{-6}$ for cancer risk and hazard quotient (HQ) 1.0 for noncancer risk. The deteced levels were estimated in $10^{-5}{\sim}10^{-6}$ of cancer risk and below 1.0 of HQ of noncancer risk. While three kinds of water were determined within the acceptal levels, DBPs were detected in tap water and purified were and some undesireable chemicals such as more fluoride detected rather than the quality stanadard. For the drinking water safety, it shoud be continuously monitored, assessed and managed as well risk communiction between the authoritis and public.

환경중 담배연기와 폐암유발 위험성의 관련 여부 (Scientific Data for an Assessment of the Potential Risk for Lung Cancer Associated with the Exposure to Environmental Tobacco Smoke in Korea)

  • 비흡연자가 환경중의 담배연기를 흡입하게 되면 폐암유발 위험성을 증대시킨다는 주장이 미국, 유럽, 일본 그리고 몇몇 아시아 국가에서 제기되어 왔다. 본 논문에서는 한국에서 이러한 잠재위험성의 사실여부를 판단하는데 이용될 수 있는 현재까지의 과학적 근거를 제시하고 그에 대하여 토의하고자 한다. 잠재위험성의 성격, 환경중의 담배연기량에 대한 반응과 관련된 자료, 그리고 환경중 담배연기에 대한 실제적인 노출형태를 구분할 수 있는 자료 등이 토의된다. 잠재위험성에 대한 최종적인 판단은 한국의 공중보건정책의 기초로서 잠재위험성의 평가임무와 관련된 자격을 갖춘 사람들에게 맏겨질 것이다.

Semi-Quantitative Exposure Assessment of Occupational Exposure to Wood Dust and Nasopharyngeal Cancer Risk

  • Occupational exposure to wood dust is one cause of nasopharyngeal cancer (NPC); however, assessing this exposure remains problematic. Therefore, the objective of this study was to develop a semi-quantitative exposure assessment method and then utilize it to evaluate the association between occupational exposure to wood dust and the development of NPC. In addition, variations in risk by histology were examined. A case-control study was conducted with 327 newly diagnosed cases of NPC at the National Cancer Institute and regional cancer centers in Thailand with 1:1 controls matched for age, gender and geographical residence. Occupational information was obtained through personal interviews. The potential probability, frequency and intensity of exposure to wood dust were assessed on a job-by-job basis by experienced experts. Analysis was performed by conditional logistic regression and presented in odds ratio (ORs) estimates and 95% confidence intervals (CI). Overall, a non significant relationship between occupational wood dust exposure and NPC risk for all subjects was observed (ORs=1.61, 95%CI 0.99-2.59); however, the risk became significant when analyses focused on types 2 and 3 of NPC (ORs=1.62, 95%CI 1.03-2.74). The significant association was stronger for those exposed to wood dust for > 10 year (ORs=2.26, 95%CI 1.10-4.63), for those with first-time exposure at age > 25 year (ORs=2.07, 95%CI 1.08-3.94), and for those who had a high cumulative exposure (ORs=2.17, 95%CI 1.03-4.58) when compared with those considered unexposed. In conclusion, wood dust is likely to be associated with an increased risk of type 2 or 3 NPC in the Thai population. The results of this study show that semi-quantitative exposure assessment is suitable for occupational exposure assessment in a case control study and complements the information from self-reporting.

수도수중 염소 소독부산물로 인한 건강위해성 평가에 관한 연구 - 서울시 수도수중 Trihalomethanes 및 Haloaceticnitriles을 중심으로 - (Health Risk Assessment of Disinfection By-products by Chlorination in Tap Water Ingestion)

  • Public concerns about hazardous health effect from the exposure to organic by-products of the chlorination have been increased. There are numerous studies reporting that chlorination of drinking water produces numerous chlorinated organic by-products including THMs, HAAs, HANs. Some of these products are known to be animal carcinogens. The purpose of this study was to estimate health risk of DBPs by chlorinated drinking water ingestion in Seoul based on methodologies that have been developed for conducting risk assessment of complex-chemical-mixture. The drinking water sample was collected seperately at six water treatment plant in Seoul at March, April, 1996. In tap water of households in Seoul, DBPs were measured wilfh the mean value of 36.6 $\mu$g/L. Risk assessment processes,. which include processes for the estimation of human cancer potency using animal bioassay data and calculation of human exposure, entail uncertainties. In the exposure assessment process, exposure scenarios with various assumptions could affect the exposure amount and excess cancer risk. The reference dose of haloacetonitriles was estimated to be 0.0023 mg/kg/day by applying dibromoacetonitrile NOAEL and uncertainty factor to the mean concentration. In the first case, human excess cancer risk was estimated by the US EPA method used to set the MCL (maximum contaminant level). In the second and third case, the risk was estimated for multi-route exposure with and without adopting Monte-Carlo simulation, respectively. In the second case, exposure input parameters and cancer potencies used probability distributions, and in the third case, those values used point estimates (mean, and maximum or 95% upper-bound value). As a result, while the excess cancer risk estimated by US EPA method considering only direct ingestion tended to be underestimated, the risk which was estimated by considering multi-route exposure without Monte-Carlo simulation and then using the maximum or 95% upper-bound value as input parameters tended to be overestimated. In risk assessment for Trihalomethanes, considering multi-route exposure with adopting Monte-Carlo analysis seems to provide the most reasonable estimations.

Human Health Risk Assessment of Trichloroethylene from Industrial Complex A

  • This study investigated the human health risks of trichloroethylene from Industrial Complex A. The excessive carcinogenic risks for central tendency exposure were $1.40{\times}10^{-5}$ for male and female residents in the vicinity of Industrial Complex A. The excessive cancers risk for reasonable maximum exposure were $2.88{\times}10^{-5}$ and $1.97{\times}10^{-5}$ for males and females, respectively. These values indicate that there are potential cancer risks for exposure to these concentrations. The hazard index for central tendency exposure to trichloroethylene was 1.71 for male and female residents. The hazard indexes for reasonable maximum exposure were 3.27 and 2.41 for males and females, respectively. These values were over one, which is equivalent to the threshold value. This result showed that adverse cancer and non-cancer health effects may occur and that some risk management of trichloroethylene from Industrial Complex A was needed.

Detection and Type-Distribution of Human Papillomavirus in Vulva and Vaginal Abnormal Cytology Lesions and Cancer Tissues from Thai Women

  • Vulva and Vaginal cancers are rare among all gynecological cancers worldwide, including Thailand, and typically affect women in later life. Persistent high risk human papillomavirus (HR-HPV) infection is one of several important causes of cancer development. In this study, we focused on HPV investigation and specific type distribution from Thai women with abnormality lesions and cancers of the vulva and Vaginal. A total of ninety paraffin-embedded samples of vulva and Vaginal abnormalities and cancer cells with histologically confirmed were collected from Thai women, who were diagnosed in 2003-2012 at the National Cancer Institute, Thailand. HPV DNA was detected and genotyped using polymerase chain reaction and enzyme immunoassay with GP5+/bio 6+ consensus specific primers and digoxigenin-labeled specific oligoprobes, respectively. The human ${\beta}$-globin gene was used as an internal control. Overall results represented that HPV frequency was 16/34 (47.1%) and 8/20 (40.0%) samples of vulva with cancer and abnormal cytology lesions, respectively, while, 3/5 (60%) and 16/33 (51.61%) samples of Vaginal cancer and abnormal cytology lesions, respectively, were HPV DNA positive. Single HPV type and multiple HPV type infection could be observed in both type of cancers and abnormal lesion samples in the different histological categorizes. HPV16 was the most frequent type in all cancers and abnormal cytology lesions, whereas HPV 18 was less frequent and could be detected as co-infection with other high risk HPV types. In addition, low risk types such as HPV 6, 11 and 70 could be detected in Vulva cancer and abnormal cytology lesion samples, whereas, all Vaginal cancer samples exhibited only high risk HPV types; HPV 16 and 31. In conclusion, from our results in this study we suggest that women with persistent high risk HPV type infection are at risk of developing vulva and Vaginal cancers and HPV 16 was observed at the highest frequent both of these, similar to the cervical cancer cases. Although the number of samples in this study was limited and might not represent the overall incidence and prevalence in Thai women, but the baseline data are of interest and suggest further study for primary cancer screening and/or developing the efficiency of prophylactic HPV vaccines in Thailand.

교통 혼잡지역의 대기 부유분진중 유기혼합물에 의한 발암위해성 평가 (Assessment of Health Risk Posed by Orgnic Substances of Suspended Particulate Matters in a Heavy Traffic Area of Seoul)

  • Air pollution has been recognized for many years as a factor which heightens the risk of cancer. Extractable polycyclic organic matters in air particulates have been recognized as to have carcinogenic effects. This study examined the health risks posed by organic substances of air particulates in Seoul based on methodelogies that have been developed for conducting risk posed by organic substances of sir particulates in Seoul based on methodologies that have been developed for conducting risk assessment of complex -chemical-mixture. The data used in this study was obtained from air samples collected in a heavy traffic area of Seoul (Shinchon) from 1986 to 1994. The mean concentration of total supended pariculates was 158.0.mu.g/m/msup 3/, 5% of which is consisted of organic matter. The excess cancer risk from benzo(a)pyrene (BaP) was estimated to be 3.48.times.10$^{-6}$ by applying BaP unit risk estimates to the mean concentration. 8.74ng/m BaP equivalents of potency method was 1.0.times.10$^{-3}$ . The calculated risk from EOM were comparably higher than that from benzo(a)pyrene and exceeded the acceptable risk level.
