Comparison of survival rates between patients treated with conventional radiotherapy and helical tomotherapy for head and neck cancer

  • Purpose: Compared to conventional radiotherapy (RT), intensity-modulated radiotherapy (IMRT) significantly reduces the rate of treatment-induced late toxicities in head and neck cancer. However, a clear survival benefit of IMRT over conventional RT has not yet been shown. This study is among the first comparative study to compare the survival rates between conventional RT and helical tomotherapy in head and neck cancer. Materials and Methods: From January 2008 to November 2011, 37 patients received conventional RT and 30 patients received helical tomotherapy for management of head and neck cancer. We retrospectively compared the survival rates between patients treated with conventional RT and helical tomotherapy, and analyzed the prognostic factors for survival. Results: The 1- and 2-year locoregional recurrence-free survival rates were 61.2% and 58.1% for the conventional RT group, 89.3% and 80.3% for the helical tomotherapy group, respectively. The locoregional recurrence-free survival rates of the helical tomotherapy group were significantly higher than conventional RT group (p = 0.029). There were no significant differences in the overall and distant metastasis-free survival between the two groups. RT technique, tumor stage, and RT duration were significant prognostic factors for locoregional recurrence-free survival. Conclusion: This study showed the locoregional recurrence-free survival benefits of helical tomotherapy in the treatment of head and neck cancers.

비인강악성종양에서 유도화학요법과 방사선요법을 병행치료한 결과 (The Result of Combined Treatment with Induction Chemotherpy and Radiotherapy in Nasopharyngeal Cancer)

  • The malignant tumor of nasopharynx occurs in china and other oriental contries as high incidence and its prognosis is relatively poor because of frequent intracranial extension and early metastasis. Traditional therapeutic modality of nasopharyngeal cancer was definite radiotherapy, but recently some cancer institute had tried combined modality with induction chemotherapy and reported it may be valuable. We report the clinical evaluation and therapeutic result about 28 nasopharyngeal cancer patients which were treated with 2 courses of induction chemotherapy(Cisplatin+5-FU) and radiotherapy. The results were as follows: 1) The 3 years and 5 years survival rate were 76% and 47% in total patients. 2) The 3 years and 5 years survival rate were 92% and 63% in T1, T2, T3 group, and 25% and 0% in T4 group. 3) The 3 year and 5 year survival rate were 100% and 60% in neck node negative group, and 60% and 40% in neck node positive group. 4) The 3 year and 5 years survival rate were 100% and 50% in stage I II group, and 71% and 44% in stage III, IV group.

Newly Developed Weakness of Lower Extremities Despite Improved Brain Metastasis of Lung Cancer after Radiotherapy

  • An intramedullary spinal cord metastasis (ISCM) rarely develops in systemic cancer but is indicative of a poor prognosis. A 56-year-old man was admitted due to weakness of the lower extremities. He had received radiotherapy 3 months prior for a brain metastasis that had developed 1 year after achieving a complete response from chemotherapy for extended stage small cell lung cancer. Although the brain lesion had improved partially, ISCM from the cervical to lumbar-sacral spinal cords, which was accompanied by a leptomeningeal dissemination, was diagnosed based on magnetic resonance imaging of the spine and cerebrospinal fluid cytology. Finally, he died of sudden cardiac arrest during treatment. This is the first case of ISCM involving the whole spinal segments. Physicians should be aware of the subsequent development of ISCM in lung cancer patients with a previously known brain metastasis who present with new neurological symptoms.

Statistical Modelling and Forecasting of Cervix Cancer Cases in Radiation Oncology Treatment: A Hospital Based Study from Western Nepal

  • Background: To estimate the numbers and trends in cervix cancer cases visiting the Radiotherapy Department at Manipal Teaching Hospital, Pokhara, Nepal, statistical modelling from retrospective data was applied. Materials and Methods: A retrospective study was carried out on data for a total of 159 patients treated for cervix cancer at Manipal Teaching Hospital, Pokhara, Nepal, between $28^{th}$ September 2000 and $31^{st}$ December 2008. Theoretical statistics were used for statistical modelling and forecasting. Results: Using curve fitting method, Linear, Logarithmic, Inverse, Quadratic, Cubic, Compound, Power and Exponential growth models were validated. Including the constant term, none of the models fit the data well. Excluding the constant term, the cubic model demonstrated the best fit, with $R^2$=0.871 (p=0.004). In 2008, the observed and estimated numbers of cases were same (12). According to our model, 273 patients with cervical cancer are expected to visit the hospital in 2015. Conclusions: Our data predict a significant increase in cervical cancer cases in this region in the near future. This observation suggests the need for more focus and resource allocation on cervical cancer screening and treatment.

Audit of Cancer Patients from Eastern Uttar Pradesh (UP), India: A University Hospital Based Two Year Retrospective Analysis

  • Background: A retrospective analysis of all cancer patients attending the radiotherapy outpatient department (OPD) of a single unit during the period of January 2005 till December 2006 was conducted to know the geographical distribution and incidence of the most common cancers, their stage of presentation, treatment compliance among the patients and follow-up. Materials and Methods: A total of 4,484 patients were registered in the Institute of Medical Sciences, Banaras Hindu University during the period of January 2005-December 2006; of which 1,975 registered in an individual unit were included for the retrospective analysis. Results: Most of the patients hailed from the various districts of UP and Bihar. Females outnumbered males with a ratio of 1.33:1. Females mostly belonged to the age group of 40-59 years; whilst males were a decade older. Major cancer sites in females were cervix and breast followed by head and neck. Leading cancer sites in males were head and neck, brain, bone, soft tissue and lung. Most of the cases presented in advanced stage of disease (74%). Squamous cell carcinoma was the most common histopathology (56%). A significant proportion of patients defaulted after undergoing preliminary investigations (16%). Only 53.9% of females and 58.5% of males took treatment out of which 68% and 63% completed the prescribed treatment. Compliance with follow-up was poor. Conclusions: The outcome of this study will significantly help us to define region specific strategies needed for cancer management in eastern Uttar Pradesh.

Notch Inhibitor: a Promising Carcinoma Radiosensitizer

  • Radiotherapy is an important part of modern cancer management for many malignancies, and enhancing the radiosensitivity of tumor cells is critical for effective cancer therapies. The Notch signaling pathway plays a key role in regulation of numerous fundamental cellular processes. Further, there is accumulating evidence that dysregulated Notch activity is involved in the genesis of many human cancers. As such, Notch inhibitors are attractive therapeutic agents, although as for other anticancer agents, they exhibit significant and potential side effects. Thus, Notch inhibitors may be best used in combination with other agents or therapy. Herein, we describe evidence supporting the use of Notch inhibitors as novel and potent radiosensitizers in cancer therapy.

Prognostic Factors in Stage III Non-Small-Cell Lung Cancer Patients

  • Aim: The objective of this study is to investigate prognostic factors affecting survival of patients undergoing concurrent or sequential chemoradiotherapy (CRT) for stage III non-small-cell lung cancer (NSCL). Methods and materials: We retrospectively reviewed the clinical records of 148 patients with advanced, inoperable stage III NSCLC, who were treated between 2007 and 2015. Results: The median survival was found to be 19 months and 3-year overall survival was 27%. Age (<65 vs ${\geq}65years$, p=0.026), stage (IIIA vs IIIB, p=0.033), dose of radiotherapy (RT) (<60 vs ${\geq}60Gy$, p=0.024) and treatment method (sequential chemotherapy+RT vs concurrent CRT, p=0.023) were found to be factors affecting survival in univariate analyses. Gender, histological subtype, weight loss during CRT, performance status, induction/consolidation chemotherapy and presence of comorbidities did not affect survival (p>0.050). Conclusion: Young age, stage IIIA, radiotherapy dose and concurrent chemoradiotherapy may positively affect survival in stage III NSCL cases.

Meta Analysis of Treatment for Stage IE~IIE Extranodal Natural Killer /T Cell Lymphomas in China

  • Objective: To evaluate early treatment for extranodal natural killer/T cell lymphoma (ENK/TCL) in China and provide reference for clinical treatment of these patients. Methods: Computer-based retrieval was performed in PubMed, CNKI, CBM, VIP and WanFang Data to search for randomized controlled trials (RCTs) of treatment for early ENK/TCL, and a meta-analysis was conducted with RevMan 5.0 software. Results: A total of 11 RCTs, including 871 patients, were selected, of which the first radiotherapy had a higher complete response (CR) than the first chemotherapy [OR=14.16, 95%CI (8.68, 23.10), P<0.00001] and CR was not different between combined treatment group and radiotherapy group [OR=1.86, 95%CI (0.47, 3.58), P=0.61], but long-term survival rate was higher with combined treatment[OR=1.88, 95%CI (1.09, 3.19), P=0.02]. No difference in survival rate was observed between radio-chemotherapy and chemo-radiotherapy groups [OR=1.11, 95%CI (0.73, 1.69), P=0.63]. Conclusions: Radiotherapy is of great significance in the treatment of early ENK/TCL, but combined therapy could further enhance long-term survival rate of patients. This conclusion still requires further confirmation using RCTs with high quality and large sample size.

자궁내막암 환자에서 수술 전 방사선치료와 수술 후 방사선치료의 성적 비교 분석 (Comparative Analysis between Preoperative Radiotherapy and Postoperative Radiotherapy in Clinical Stage I and II Endometrial Carcinoma)

  • 목적 : 자궁내막암 1, 2기 환자에서 수술전 방사선 치료와 수술후 방사선 치료의 성적을 후향적으로 비교 분석하여 적합한 치료 방법을 얻고자 본 연구를 시행하였다. 방법 : 1985. 1월부터 1991 12월까지 연세대학교 의과대학 방사선종양학과에 내원하여 방사선 치료를 받은 자궁내막암 환자 62명을 대상으로 하였으며,이중 19명(1기 12명, 2기 7명)은 수술전4-6주전에 방사선 치료를 받았으며(1군), 43명(1기 32명, 2기 11명)은 수술후 4-5주후에 방사선 치료를 받았다(2군). 2군중에 질내조사만 시행된 1명의 환자를 제외한 61명의 환자에서 모두 외부방사선 치료가 시행되었으며, 1관의 $70{\%}$(13/19)와 2군의 $44{\%}$(23/42)의 환자에서는 외부방사선 치료와 강내조사가 시행되었다. 외부방사선 조사는 180 cGy를 하루에 한 번씩 주 5회 시행하였으며, 총 방사선 조사선량은 39.6-55 Gy(중앙값 : 45 Gy)였다. 강내조사는 1군에서는 Point A에 20-39.6 Gy(중앙값 ; 39 Gy)가 조사되었고, 2군에서는 질 표면에서 0.5cm 깊이에 18-30 Gy(중앙값 ; 21 Gy)가 조사되었다. 결과 : 수술전 방사선 치료군과 수술후 방사선 치료군의 5년 생존율은 각각 $89{\%},\;98{\%}$로 통계적으로 유의한 차이는 없었다(p>0.1). 또한 병기.분화도,조직학적 형태에 따른 양 군간의 5년 생존율도 차이가 없었다 1군에서 수술전 방사선 치료후에 잔재암의 존재 여부에 따라서는 5년 생존율의 차이가 없었으며(p>0.1), 2군에서 림프절의 전이 여부에 따라 5년 생존율이 통계적으로 유의한 차이를 보였다(P<0.5). 치료에 따른 부작용은 1군이 더 많았다($16{\%}$ vs $5{\%}$). 결론 : 수술후 방사선 치료 방법은 수술전 방사선 치료 방법에 비하여 정확한 병의 진행 정도를 알 수 있으며, 부작용이 적음으로 자궁내막암의 치료에 좀더 적합한 치료 방법이라고 하겠다.

항문암의 방사선치료 시 방사선 조사 기법에 따른 회음부 피부 독성 (Perineal Skin Toxicity according to Irradiation Technique in Radiotherapy of Anal Cancer)

  • 목 적: 항문암의 방사선치료는 급성부작용을 많이 동반하므로 다양한 치료 기법이 적용되어왔다. 본원에서 시행된 항문암 방사선치료를 회음부 피부반응의 측면에서 검토하여 최적의 방사선치료 기법을 모색하고자 하였다. 대상 및 방법: 1990년부터 2007년까지 항문암으로 근치적 항암화학방사선치료를 마친 환자 35명을 대상으로 방사선 조사 방법 및 관련된 임상적 자료들을 이용하였다. 방사선치료는 원발병소, 영역림프절 및 서혜부림프절이 치료범위에 포함된 상태로 1.8 Gy 씩 $41.4{\sim}45\;Gy$ 조사 후 원발 병소 또는 전이성 림프절에 추가 조사하는 것을 원칙으로 하였다. 방사선 조사 기법은 조사 부위 및 조사 수에 따라 4가지로 분류하였으며 각 기법에 따른 회음부 급성피부반응 및 치료중지기간 간의 관련 여부를 알아보았다. 결 과: 방사선치료 중 28명(80.0%)의 환자에서 2등급 이상의 방사선 피부염이 발생하였고 10명(28.5%)이 3등급 이상의 방사선 피부염을 보였다. 4가지 방사선 조사 기법 중 원발병소와 영역림프절에 대한 3면 X-선 및 양측 서혜부림프절에 대한 전자선 조사 군과 electron thunderbird군에서 방사선 피부염이 동반된 환자 수가 상대적으로 적었으며 치료중지기간이 각각 $8.2{\pm}10.2$일, $5.7{\pm}5.7$일로써 다른 치료 기법보다 적었다. 방사선치료 종료 후 1개월 시점에서 27명(77.1%)의 환자에서 완전 관해를 보였으며 5년 생존율은 67.7%이었다. 결 론: 항문암의 방사선치료에 있어 방사선 조사 방법 및 범위가 치료순응도에 영향을 미칠 수 있으며, X-선의 회음부 조사 범위를 줄이는 방법이 회음부의 심각한 방사선 피부염을 감소시킴으로써 환자의 치료순응도를 높이는 데 있어 적절할 것으로 생각된다.
