Malignant Neoplasm Burden in Nepal - Data from the Seven Major Cancer Service Hospitals for 2012

  • In Nepal, while no population based cancer registry program exists to assess the incidence, prevalence, morbidity and mortality of cancer, at the national level a number of hospital based cancer registries are cooperating to provide relevant data. Seven major cancer diagnosis and treatment hospitals are involved, including the BP Koirala Memorial Cancer hospital, supported by WHO-Nepal since 2003. The present retrospective analysis of cancer patients of all age groups was conducted to assess the frequencies of different types of cancer presenting from January 1st to December 31st 2012. A total of 7,212 cancer cases were registered, the mean age of the patients being 51.9 years. The most prevalent age group in males was 60-64 yrs (13.6%), while in females it was 50-54 yrs (12.8%). The commonest forms of cancer in males were bronchus and lung (17.6%) followed by stomach (7.3%), larynx (5.2%) and non Hodgkins lymphoma (4.5%). In females, cervix uteri (19.1%) and breast (16.3%), were the top ranking cancer sites followed by bronchus and lung (10.2%), ovary (6.1%) and stomach (3.8%). The present data provide an update of the cancer burden in Nepal and highlight the relatively young age of breast and cervical cancer patients.

The Development of the Korean Lung Cancer Registry (KALC-R)

  • Lung cancer is the most commonly diagnosed cancer and the leading cause of cancer-related deaths worldwide. Globally, there were an estimated 1.8 million new cases and 1.59 million deaths in 2012. In Korea, the incidence of lung cancer is increasing and 24,267 (47.6/100,000) patients with lung cancer were registered at the Korea Central Cancer Registry in 2015. Previous nationwide surveys of lung cancer were performed in 1998 by the Korean Academy of Tuberculosis and Respiratory Diseases and in 2007 by the Korean Association for Lung Cancer (KALC), but the studies faced difficulties in maintaining lung cancer registry because of limitations regarding the Private Information Protection Act. To produce unbiased and reliable epidemiological data, the KALC and Korean Central Cancer Registry developed a detailed lung cancer registry (KALC-R) data structure. Following a pilot survey of 489 lung cancer cases in 2013, about 10% of the sampled lung cancer cases from the Korean Central Cancer Registry are surveyed each year. With the analysis of detailed data from the KALC-R, an important epidemiological background for scientific research or policy development is expected to be generated.

Descriptive Epidemiology of Primary Brain and CNS Tumors in Delhi, 2003-2007

  • The Delhi Population Based Cancer Registry data during the period 2003-2007 were used to describe the epidemiology of primary malignant brain and central nervous system tumors in Delhi. A total of 1989 brain and CNS tumors cases in 1291 males and 698 females were registered during the period 1st January 2003 to 31st December 2007. The age adjusted (world population) incidence rates were 3.9 per 100,000 for males and 2.4 per 100,000 for females. Gliomas were the most frequently reported histology both in males (26.6%) and females (23.2%). A male predominance in incidence was observed for all histological classifications. The rates in Delhi are low compared to the incidences reported from developed countries.

Coverage, Density and Completeness of Sources used in Tehran Metropolitan Area Cancer Registry: According to the Data of Esophageal Cancer, 2003-2007

  • Background: The completeness of cancer registration is a major validity index of any reported cancer incidence. The present study aimed to evaluate the esophageal cancer incidence registered in the Tehran Metropolitan Area Cancer Registry. Materials and methods: The data on esophageal cancer abstracted from three sources of 1) pathology departments, 2) medical records, and 3) death certificates during 2003 till 2007 were utilized. The completeness of the data sources were evaluated using coverage (defined as the proportion of a community population with esophageal cancer identified by the source) and density (defined as the proportion of non-empty fields of the data by source). Results: A total 1,404 cases of esophageal cancer were reported for the duration of the study. Pathology provided 771, medical records 432, and death certificates 609. The coverage was 0.55 for pathology, 0.31 for medical records, and 0.43 for death certificates. The respective density values were 0.82, 0.96 and 0.98, respectively. Pathology (0.45) was the most complete source followed by medical records (0.42), and death certificates (0.29). Discussion: A low degree of completeness dictates putting more effort into case finding plus abstracting data more thoroughly.

Childhood Cancer Incidence and Survival 1985-2009, Khon Kaen, Thailand

  • Background: The Khon Kaen Cancer Registry (KKCR) was established in 1984. Previous population-based incidences and survivals of childhood cancer in Thailand were determined using a short cancer registration period. Materials and Methods: Data were retrieved of all children residing in Khon Kaen, between 0-15 years, diagnosed as having cancer and registered in the KKCR (1985-2009). The follow-up censored date was December 31, 2012. The childhood cancers were classified into 12 diagnostic groups, according to the International Classification of Childhood Cancer. The incidence was calculated by the standard method. Survival of childhood cancer was investigated using the KKCR population-based registration data and overall survival calculated using the Kaplan Meier method. Results: In the study period, 912 newly diagnosed cases of childhood cancer were registered. The respective mean and median age was 6.4 (SD=4.6) and 6 (0-14) years. The age-peak for incidence was 0-4 years. The age-standardized rate (ASR) was 83 per million. Leukemia was the most common cancer (N=360, ASR 33.8) followed by neoplasms of the central nervous system (CNS, N=150, ASR 12.8) and lymphoma (N=79, ASR 7.0). The follow-up duration totaled 101,250 months. The death rate was 1.11 per 100 person-months (95%CI: 1.02 -1.20). The 5-year overall survival was 52% (95%CI: 53-56.9) for all cancers. The respective 5-year overall survival for (1) acute lymphoblastic leukemia (ALL), (2) acute non-lymphoblastic leukemia (ANLL), (3) lymphoma, (4) germ cell tumors, (5) renal tumors, (6) retinoblastoma, (7) soft tissue tumors, (8) CNS tumors, (9) bone tumors, (10) liver tumors, and (11) neuroblastoma was (1) 51%, (2) 37%, (3) 63%, (4) 74%, (5) 67%, (6) 55%, (7) 46%, (8) 44%, (9) 36%, (10) 34%, and (11) 25%. Conclusions: The incidence of childhood cancer is lower than those of western countries. Respective overall survival for ALL, lymphoma, renal tumors, liver tumors, retinoblastoma, soft tissue tumors is lower than that reported in developed countries while survival for CNS tumors, neuroblastoma and germ cell tumors is comparable.

  • The Box-Cox power transformation is generally used for variance stabilization. Recently, Shin and Kang (2001) showed, under the Box-Cox transformation, invariant properties to the original model under the large mean and relatively small variance assumptions in time series analysis. In this paper we obtain some invariant properties in spatial statistics. Spatial statistics, Invariant Property, Variogram, Box-Cox power Transformation.

Epidemiology of Breast Cancer among Females in Basrah

  • Breast cancer is the most frequent cancer in females. Its incidence is higher in developed countries than in developing ones partly due to variation in risk exposure and partly due to better detection methods. Scattered evidence in Basrah, Iraq, suggests that breast cancer has been increasing at a significant pace in recent years. This study aimed to measure the current level of risk of breast cancer among females in Basrah and to describe the time trend over almost a decade of years. Data on breast cancer cases from all sources of cancer registration in Basrah governorate were compiled for the years 2005-2012. The data for each year were first checked separately for duplicate reporting of cases among various sources. Then the eight files were pooled together and checked again for any duplicate cases among years of registration. The final set of data contained 2,284 cases of breast cancer (2,213 female cases and 71 male cases). All patients were inhabitants of Basrah governorate at the time of diagnosis. Figures on the Basrah population were obtained from various sources including the Ministry of Health, Ministry of Planning and Developmental Collaboration and local household surveys. It was possible to have total population estimates for each year and by age and sex. The data were imported into SPSS (version 17) software. Age specific and year specific incidence rates were calculated. The age standardized incidence rate was also calculated using world population as the standard population to be 34.9 per 100,000 females. Age-wise, no case was reported among children aged less than 15 years and the incidence increased with advancing age reaching a peak of 123.8/100,000 females at the age range of 50-54 years. The time trend of the crude incidence rate showed only modest increased risk with passage of years and no age shift could be documented in this study. Breast cancer in females in Basrah is a significant health problem. The current incidence rate (crude, 23.7/100,000, age-standardized, 34.9/100,000) is high and justifies intensive efforts to improve early detection of cases, provide better treatment amenities and introduce long term preventive measures. Using the age standardized incidence rate as reported in this paper, it is possible to put the risk in Basrah within a regional and international context.

Evaluation of the Geometric Accuracy of Anatomic Landmarks as Surrogates for Intrapulmonary Tumors in Image-guided Radiotherapy

  • Objectives: The purpose of this study was to evaluate the geometric accuracy of thoracic anatomic landmarks as target surrogates of intrapulmonary tumors for manual rigid registration during image-guided radiotherapy (IGRT). Methods: Kilovolt cone-beam computed tomography (CBCT) images acquired during IGRT for 29 lung cancer patients with 33 tumors, including 16 central and 17 peripheral lesions, were analyzed. We selected the "vertebrae", "carina", and "large bronchi" as the candidate surrogates for central targets, and the "vertebrae", "carina", and "ribs" as the candidate surrogates for peripheral lesions. Three to six pairs of small identifiable markers were noted in the tumors for the planning CT and Day 1 CBCT. The accuracy of the candidate surrogates was evaluated by comparing the distances of the corresponding markers after manual rigid matching based on the "tumor" and a particular surrogate. Differences between the surrogates were assessed using 1-way analysis of variance and post hoc least-significant-difference tests. Results: For central targets, the residual errors increased in the following ascending order: "tumor", "bronchi", "carina", and "vertebrae"; there was a significant difference between "tumor" and "vertebrae" (p = 0.010). For peripheral diseases, the residual errors increased in the following ascending order: "tumor", "rib", "vertebrae", and "carina"; There was a significant difference between "tumor" and "carina" (p = 0.005). Conclusions: The "bronchi" and "carina" are the optimal surrogates for central lung targets, while "rib" and "vertebrae" are the optimal surrogates for peripheral lung targets for manual matching of online and planned tumors.

$Fraxion^{(R)}$ System을 이용한 뇌 정위적 방사선 수술 유용성 평가 (Evaluation on Usefulness of Stereotactic Radio Surgery using $Fraxion^{(R)}$ System)

  • 목 적 : $Fraxion^{(R)}$ system과 s-thermoplastic mask을 사용하여 뇌 전이(Brain metastasis)가 있는 환자에게 정위적 방사선 수술(Stereotactic Radiosurgery, SRS)을 시행하면서 발생 되는 환자의 셋업 오차(Setup Error)를 비교하고 오차가 선량에 미치는 영향을 측정하여 $Fraxion^{(R)}$ system의 유용성을 평가하고자 한다. 대상 및 방법 : 2014년 5월부터 2014년 10월까지 본원에 내원하여 정위적 방사선 수술을 받은 뇌 전이(Brain metastasis) 환자 6명을 대상으로 하였다. 머리를 고정하기 위해 3명은 s-thermoplastic mask와 mouthpiece를 이용한 그룹과 3명은 $Fraxion^{(R)}$ system을 이용하여 제작 한 2그룹으로 나눈 뒤 방사선 수술 당시 얻어진 3D 콘빔CT(Cone Beam Computerized Tomography, CBCT) 영상과 전산화 단층모의치료 영상의 오차 값을 기록하고 환자별 최대치(Max), 최소치(Min), 평균치(Mean), 표준편차(standard deviation, SD) 구하여 비교하였다. 또한 StereoPHAN Phantom 및 Pinpoint 3D cylindrical chamber를 이용하여 환자별 정도관리(Patient Specific Quality Assurance, PSQA)와 같은 방법으로 선량을 측정하여 선량값을 비교하였다. 결 과 : 정위적 방사선 수술 전 콘빔CT 영상과 전산화단층모의치료영상을 비교하여 얻어진 셋업 오차의 경우 $Fraxion^{(R)}$ system이 s-thermoplastic mask와 mouthpiece를 함께 사용한 경우보다 셋업 오차가 평균을 기준으로 X축 83% 감소, Y축 40% 감소, Z축 92% 감소하였으며 X, Y, Z 회전성분인 Pitch 64% 감소, Roll 88% 감소, Yaw 87% 감소로 모든 방향에서 상대적으로 이동 값이 적었으며 선량 측정을 시행한 결과는 평균을 기준으로 s-thermoplastic mask와 mouthpiece를 사용한 경우가 $Fraxion^{(R)}$ system보다 셋업 오차 보정 전은 83% 감소, 보정 후는 1.9% 감소된 선량측정을 보였다. 결 론 : $Fraxion^{(R)}$ system은 개인 치열(dentition)에 맞춘 구강고정기구(mouthpiece)와 Fraxion frame, Frontpiece, thermoplastic mask nose을 사용하여 s-thermoplastic mask와 mouthpiece를 가지고 머리를 고정하는 기존의 방식보다 높은 재현성을 보였으며 이는 기존의 치료 방법보다 1회 치료로 많은 선량이 조사되는 뇌 정위적 방사선 수술에서 안정된 고정 효과를 나타낼 것으로 사료된다.

Yonsei Cancer Center QA Set을 이용한 6DoF Couch의 이동 정확성 검증 (Verify Image-Guided Shifts for 6DoF Couch using Yonsei Cancer Center QA Set)

  • 목 적: 영상유도를 통한 6DoF Couch의 이동 정확성의 검증을 위해 QA Set을 제작하였고, 그 유용성을 평가하였다. 대상 및 방법: 6DoF Couch와 CBCT가 설치된 두 대의 선형가속기를 대상으로 하였으며, 자체 제작한 YCC QA Set을 이용하여 수평수직(Translation; TX, TY, TZ)과 회전(Rotation, Pitch; RX, Roll; RY, Yaw; RZ) 여섯방향의 Off-Set 값이 설정된 Penta-Guide Phantom의 CBCT영상을 각각 15회에 걸쳐 획득하였다. 이를 통해 기준영상(Reference Image)과 보정영상(Registration Image)을 비교하였으며, 보정된 6DoF Couch의 이동정확성을 실측하여 오차를 분석하였다. 결 과: 기준영상과 보정영상의 Air Cavity에 해당하는 Pixel들은 모두 30에서 66 사이에 포함되어 보정정확도가 높게 나타났다. 6DoF Couch의 Off-set의 보정 결과 값과 실측치의 비교에서 수평수직방향의 오차는 TX방향은 $0.25{\pm}.18mm$ TY방향은 $0.25{\pm}.25mm$ TZ방향에서 $0.36{\pm}.2mm$로 나타났다. 그리고 회전방향의 오차는 RX방향은 $0.18{\pm}.08^{\circ}$ RY방향은 $0.26{\pm}.09^{\circ}$ RZ방향에서 $0.11{\pm}.08^{\circ}$로서, 임의의 값에 대하여 정확하게 보정되었다. 결 론: YCC QA Set을 이용해 매우 간단한 방법으로 6DoF Couch의 수평수직방향뿐 아니라 회전방향의 오차에 대한 검증을 할 수 있었으며, 이는 6DoF Couch의 Daily IGRT QA를 수행함에 있어서 유용할 것이라 사료된다.
