Objectives: Understanding the reason for an unsuccessful non-surgical endodontic treatment outcome, as well as the complex anatomy of the root canal system, is very important. This study examined the cross-sectional root canal structure of mandibular first molars confirmed to have failed non-surgical root canal treatment using digital images obtained during intentional replantation surgery, as well as the causative factors of the failed conventional endodontic treatments. Materials and Methods: This study evaluated 115 mandibular first molars. Digital photographic images of the resected surface were taken at the apical 3 mm level and examined. The discolored dentin area around the root canal was investigated by measuring the total surface area, the treated areas as determined by the endodontic filling material, and the discolored dentin area. Results: Forty 2-rooted teeth showed discolored root dentin in both the mesial and distal roots. Compared to the original filled area, significant expansion of root dentin discoloration was observed. Moreover, the mesial roots were significantly more discolored than the distal roots. Of the 115 molars, 92 had 2 roots. Among the mesial roots of the 2-rooted teeth, 95.7% of the roots had 2 canals and 79.4% had partial/complete isthmuses and/or accessory canals. Conclusions: Dentin discoloration that was not visible on periapical radiographs and cone-beam computed tomography was frequently found in mandibular first molars that failed endodontic treatment. The complex anatomy of the mesial roots of the mandibular first molars is another reason for the failure of conventional endodontic treatment.
Objectives: This micro-computed tomographic (CT) study aimed to compare the shaping abilities of ProTaper Next (PTN), One Shape (OS), and One Curve (OC) files in 3-dimensionally (3D)-printed mandibular molars. Materials and Methods: In order to ensure standardization, 3D-printed mandibular molars with a consistent mesiobuccal canal curvature (45°) were used in the present study (n = 18). Specimens were instrumented with the OC, OS, or PTN files. The teeth were scanned pre- and post-instrumentation using micro-CT to detect changes of the canal volume and surface area, as well as to quantify transportation of the canals after instrumentation. Two-way analysis of variance was used for statistical comparisons. Results: No statistically significant differences were found between the OC and OS groups in the changes of the canal volume and surface area before and after instrumentation (p > 0.05). The OC files showed significantly less transportation than the OS or PTN systems for the apical section (p < 0.05). In a comparison of the systems, similar values were found at the coronal and middle levels, without any significant differences (p > 0.05). Conclusions: These 3 instrumentation systems showed similar shaping abilities, although the OC file achieved a lesser extent of transportation in the apical zone than the OS and PTN files. All 3 file systems were confirmed to be safe for use in mandibular mesial canals.
The prenatal development of thoracic spinal cord was studied by electron microscope in human embryos and fetuses ranging from 9mm to 260mm crown-rump length (5-30 weeks of gestational age). Ependymal cells in all fetal ages had conspicuous junctional complexes close to the lumen of the central canal into which microvilli and cilia projected. The ependymal cells contained numerous longitudinally arranged mitochondria, flattened cisternae of endoplasmic reticulum and Golgi complex. At 20 mm embryo, the floor and roof plates were composed of ependymoglial cells and undifferentiated neuroepithelial cells. The neuroepithelia of the sacral spinal cord were delineated from central medullary cord. By 100 mm fetus few undifferentiated neuroepithelial cells remained in the floor and roof plates. At 150 mm fetus, the whole central canal was formed by ciliated columnar epithelial cells containing cilia with basal bodies. The microvilli became tangled and club-shaped and formed a matted surface. The canal was filled with areas of dark and pale amorphous materials bounded by membrane-like structure. These two types of material were found throughout the whole central canal from 100 mm fetus onwards. By 260 mm fetus, microfibrils were first observed in the ependymal cells. In conclusion, it seems that early development and differentiation of central canal ependyma are simlar to that in other part of the brain ventricular system although ependymoglial cells are more prominent.
Objectives: The objective of this in vitro study was to evaluate and compare the cytotoxicity of four different root canal sealers i.e. Apexit Plus (Ivoclar Vivadent), Endomethasone N (Septodont), AH-26 (Dentsply) and Pulpdent Root Canal Sealer (Pulpdent), on a mouse fibroblast cell line (L929). Materials and Methods: Thirty two discs for each sealer (5 mm in diameter and 2 mm in height) were fabricated in Teflon mould. The sealer extraction was made in cell culture medium (Dulbecco's Modified Eagle's Medium, DMEM) using the ratio 1.25 $cm^2/mL$ between the surface of the sealer samples and the volume of medium in a shaker incubator. Extraction of each sealer was obtained at 24 hr, 7th day, 14th day, and one month of interval. These extracts were incubated with L929 cell line and 3-(4,5-dimethylthiazol-2yl)-2,5-diphenyltetrazolium bromide (MTT) assay was done. Two-way ANOVA for interaction effects between sealer and time and Post-hoc multiple comparison using Tukey's test across all the 16 different groups were used for statistical analysis. Results: Apexit Plus root canal sealer was significantly less toxic than other sealers (p < 0.05) and showed higher cellular growth than control. Endomethasone N showed mild cytotoxicity. AH-26 showed severe toxicity which became mild after one month while Pulpdent Root Canal Sealer showed severe to moderate toxicity. Conclusions: Apexit Plus was relatively biocompatible sealer as compared to other three sealers which were cytotoxic at their initial stages, however, they became biocompatible with time.
This study was conducted to evaluate the cleansing effect of RC-Prep (10% Urea-peroxide, 15% EDTA) on apical third of root canal. Thirty single rooted human teeth were divided into three groups, and ten canals in each group were all enlarged three sizes greater than their original diameter with K-type files and irrigated with each of three irrigants. The three used irrigants were RC-prep (Premier Co.) in combination with 3.5% Sodium hypochlorite, Normal saline, 3.5% Na OCL. In each group, one of three irrigants were used in conjunction with instrumentation as they would be during clinical condition. After final irrigation, the canals were dried with paper points and the teeth were split longitudinally. The cleanness of canal walls iii the apical region were examined with Scanning Electron Microscope. The following results were drawn. 1. The use of Rc-Prep in combination with 3.5% NaOCl showed more clean canal surface than the use of other two irrigants. 2. The canals used RC-Prep in combination with 3.5% NaOCl revealed remnants of pulp tissue and smeared layer, but the openings of dentinal tubules were relatively clean and wide. 3. There was no significant difference in the debridement effect of 3.5% NaOCl and Saline solution. 4. The use of Saline solution showed great amount of remnants of pulp tissue which couldn't find in the use of 3.5% NaOCl.

The objectives of this study were to develop a hydrologic simulation model to estimate surface drainage for irrigation districts consisting of paddy and protected cultivation, and to evaluate the applicability of the developed model. The model consists of three sub-models; agricultural supply, paddy block drainage, and protected cultivation runoff. The model simulates daily total drainage as the sum of paddy field drainage, irrigation canal drainage, and protected cultivation runoff at the outlets of the irrigation districts. The agricultural supply sub-model was formulated considering crop water requirement for growing seasons and agricultural water management loss. Agricultural supply was calculated for use as input data for the paddy block sub-model. The paddy block drainage sub-model simulates paddy field drainage based on water balance, and irrigation canal drainage as a fraction of agricultural supply. Protected cultivation runoff is calculated based on NRCS (Natural Resources Conservation Service) curve number method. The Idong reservoir irrigation district was selected for surface drainage monitoring and model verification. The parameters of model were calibrated using a trial and error technique, and validated with the measured data from the study site. The model can be a useful tool to estimate surface drainage for irrigated districts consisting of paddy and protected cultivation.
Eighty two of extracted bovine mandibular incisors were sectioned transversely through the cementoenamel junction(CEJ) and instrumented to a size minimum-# 110 file at the working length by 5.25 % NaOCl irrigation. The roots in group 1 were received a final irrigation with 10ml of 17% EDTA followed by 10ml of 5.25% NaOCl, group 2, 10ml of 40% citric acid followed by 10ml of 5.25% NaOCl, group 3, 20ml of 5.25% NaOCl, and control group, 20ml distilled water. Canal walls of four roots in each group and control group were examined by SEM(x3000). Calcium hydroxide was placed into all experimental roots except control group. Each root was placed in nalgen bottle containing unbuffered distilled water. The pH level of the medium surrounding tooth was recorded at 0, one hour, and daily for 1week, then 14days, 21days, and 28days using pH electrode. At 1, 3, 7, 14, 21days, and 28days, four roots from each group were split longitudinally and the ratio change was recorded using spectroiluorometer. The results were followings : 1. The smear layer was totally removed from canal walls in group 1 and 2, but was observed in group 3 and control group. 2. The hydroxyl ion diffused more rapidly through radicular dentin when smear layer was removed from canal walls. 3. The hydroxyl ion derived from calcium hydroxide began to diffuse from the root canal to the exterior surface of the root at 1day, and continued to 1-2weeks. 4. The pH level of dentin near the CEJ was not different in all experimental group regardless of presence of smear layer. It is clinically advisable to place calcium hydroxide into root canal for 1-2weeks for the purpose of expectation of diffusion of hydroxyl ion. But, after the placement of calcium hydroxide into root canal, the consequent pH level of external root surface will be futher studied with respect to it's effect on the root and periodontium.
For integrated water management, it is essential to secure basic data such as the amount of agricultural water intake. The river water intake through the intake weir is carried out through the agricultural irrigation canal, and a method for measuring the quantity of water intake is required to suit the characteristics of the measuring points. In this study, the accuracy of the calculated flow data was determined by applying a microwave water surface current meter. The microwave water surface current meter is a method of calculating surface velocity using doppler effect, which is mainly used in high-velocities situations such as flood. Surface velocity is difficult to represent the average velocity of the entire section at low dicharges or high wind speeds, it is considered to be low in continuous utilization throughout the year, and it is necessary to verify whether the measurement using an microwave water surface curren meter is appropriate in agricultural irrigation canal. The data measured with an microwave water surface curren meter were compared with the actual flow data to calculate the intake data in agricultural irrigation canal. In agricultural irrigation canal, the low-level discharge calculated using an microwave water surface current meter at a minimum velocity of about 0.3 m/s and a minimum discharge of about 1.0 m3/s or higher was found to have a high tendency and accuracy compared to the standard discharge, especially when the high discharge was high. Although effective results can be obtained in terms of quantity at low discharge, it is deemed that subsequent studies are needed to calculate the average discharge of the cross section at low discharge, given that the trend of data is unstable. Through this study, it is suggested that it is appropriate to calculate the amount of water intake through the microwave water surface current meter in artificial waterways with a certain discharge or higher, so it is expected to be widely distributed as a method for measuring river water intake.
1. The author have had a case of class V and class Ⅲ combined carious lesion extended on to root surface of upper right lateral incisor. 2. The patient was 40 years old female. 3. After routine root canal treatment of the tooth, the gingival flap was made by vertical incision on gingival tissue between distal surface of upper right lateral incisor and mesial surface of upper right canine. 4. Cavity preparation and amalgam filling on the carious lesion were performed and the flap was sutured.
The aim of this study is to compare the adaptability of thermoplasticized injectable gutta-percha technique to the canal walls in ribbon-shaped canals. Thirty resin models simulated ribbon-shape canals were instrumented to #40 using .06 taper Profile systems. Three groups of each 10 resin models were obturated by the lateral condensation technique(LC) and the two thermoplasticized injectable gutta-percha technique; Ultrafil Endoset+Obtura II(EO) and Ultrafil Firmset(UF), respectively. After resin model were kept at room temperature for 4 days, they were resected horizontally with micro-tome at 1, 2, 3, 4 and 5mm levels from apex. At each levels. image of resected surface were taken using CCD camera under a stereomicroscope at $\times$40 magnification and stored. Ratio of the area of gutta-percha was obtained by calculating area of gutta-percha cone to the total area of canal using digitized image-ana-Iyzing program. The data were collected then analyzed statistically using One-way ANOVA. The results were as follows. 1 At 1mm levels, there was no statistically significant difference in the mean ratio of gutta-percha among the groups. 2. At 2mm level, EO showed the highest mean ratio of gutta-percha (p<0.05) and there was no significant difference between LC and UF. 3. At 3, 4, 5mm levels, EO and UF had significantly greater mean ratio of gutta-percha than LC(p<0.05) and there was no significant difference between EO and UF. In conclusion, the thermoplasticized injectable gutta-percha techniques demonstrated relatively favorable adaptability to canal walls than lateral condensation technique in ribbon-shaped canals except for 1mm level.
