  • Obturation is a important procedure of root canal treatment. Canal filling should be both provide a hermetic seal for the root canal system and eliminate leakage channel from the oral cavity. Gutta-percha have been the standard material of choice for root canal obturation. Canal filling has been aimed at maximizing the amount of gutta-percha and minimizing the amount of sealer. However recently, single cone technique has been introduced that include calcium silicate-based sealer and single gutta-percha cone. It is important to select an obturation technique that offers consistency and is easy to use. From the standpoint of appropriate technology, the single cone technique is thought to be useful for general dentist.

  • The present report describes the endodontic treatment of an Oehlers type II dens invaginatus in a maxillary lateral incisor with 5 root canals, an extremely rare condition. Apical periodontitis and related symptoms were noted. Cone-beam computed tomography was used to aid the diagnosis, reveal tooth morphology, and assist in canal location. The pulp chamber was carefully accessed, and the root canals were explored under magnification. All root canals were prepared with an R25 Reciproc Blue system and sodium hypochlorite (NaOCl) irrigation. After initial preparation, a self-adjusting file (SAF) with NaOCl and ethylenediaminetetraacetic acid was used to complement the disinfection. Additionally, calcium hydroxide medication was applied. Vertical compaction was used to fill the canals with a calcium silicate-based endodontic sealer and gutta-percha. After 12 months, the patient exhibited healing of the periapical region, absence of symptoms, and normal dental function. In conclusion, this nonsurgical treatment protocol was successful in promoting the cure of apical periodontitis. Both complementary disinfection with an SAF and use of calcium hydroxide medication should be considered when choosing the best treatment approach for dens invaginatus with very complex anatomy.

  • Purpose: Capseal I and Capseal II are calcium silicate and calcium phosphate based experimental root canal sealers. This study sought to evaluate the biocompatibility and mineralization potential of Capseal I and Capseal II. Materials and Methods: The biocompatibility and mineralization related gene expression (alkaline phosphatase [ALP], bone sialoprotein [BSP], and osteocalcin) of Capseal I and Capseal II were compared using methylthiazol tetrazolium assay and reverse transcription-polymerization chain reaction analysis, respectively. The results were analyzed by Kruskal-Wallis test. A P-value of <0.05 was considered significant. Result: Both Capseal I and Capseal II were favorable in terms of biocompatibility, influencing the messenger RNA expression of ALP and BSP. Conclusion: Within the limitation of this study, Capseal is biocompatible, with mineralization promoting potential; thus, it could be a promising root canal sealer.

  • Objectives: This study evaluated the effect of ultrasonic agitation of mineral trioxide aggregate (MTA), calcium silicate-based cement (CSC), and Sealer 26 (S26) on adaptation at the cement/dentin interface and push-out bond strength. Materials and Methods: Sixty maxillary canines were divided into 6 groups (n = 10): MTA, S26, and CSC, with or without ultrasonic activation (US). After obturation, the apical portions of the teeth were sectioned, and retrograde cavities were prepared and filled with cement by hand condensation. In the US groups, the cement was activated for 60 seconds: 30 seconds in the mesio-distal direction and 30 seconds in the buccal-lingual direction, using a mini Irrisonic insert coupled with the ultrasound transducer. After the materials set, 1.5-mm thick sections were obtained from the apexes. The presence of gaps and the bond between cement and dentin were analyzed using low-vacuum scanning electron microscopy. Push-out bond strength was measured using a universal testing machine. Results: Ultrasonic agitation increased the interfacial adaptation of the cements. The S26 US group showed a higher adaptation value than MTA (p < 0.05). US improved the push-out bond strength for all the cements (p < 0.05). Conclusions: The US of retrograde filling cements enhanced the bond to the dentin wall of the root-end filling materials tested.
