Korean Guideline for the Prevention and Treatment of Glucocorticoid-induced Osteoporosis

  • Background: To develop guidelines and recommendations to prevent and treat glucocorticoid (GC)-induced osteoporosis (GIOP) in Korea. Methods: The Korean Society for Bone and Mineral Research and the Korean College of Rheumatology have developed this guideline based on Guidance for the Development of Clinical Practice Guidelines ver. 1.0 established by the National Evidence-Based Healthcare Collaborating Agency. This guideline was developed by adapting previously published guidelines, and a systematic review and quality assessment were performed. Results: This guideline applies to adults aged ${\geq}19years$ who are using or plan to use GCs. It does not include children and adolescents. An initial assessment of fracture risk should be performed within 6 months of initial GC use. Fracture risk should be estimated using the fracture-risk assessment tool (FRAX) after adjustments for GC dose, history of osteoporotic fractures, and bone mineral density (BMD) results. All patients administered with prednisolone or an equivalent medication at a dose ${\geq}2.5mg/day$ for ${\geq}3months$ are recommended to use adequate calcium and vitamin D during treatment. Patients showing a moderate-to-high fracture risk should be treated with additional medication for osteoporosis. All patients continuing GC therapy should undergo annual BMD testing, vertebral X-ray, and fracture risk assessment using FRAX. When treatment failure is suspected, switching to another drug should be considered. Conclusions: This guideline is intended to guide clinicians in the prevention and treatment of GIOP.

Fluoride content of bottled water available in South Korea (국내 시판 생수의 불소 이온농도 측정)

  • Objectives: The market for bottled water is increasing steadily in South Korea. Bottled water contains several naturally occuring minerals, such as calcium, magnesium, sodium, and fluoride. Fluoride is proven to be effective in preventing dental caries. In South Korea, the maximum permissible concentration of fluoride is 2 ppm for bottled water and 1.5 ppm for tap water. The aim of this study was to investigate the fluoride content of different commercially available brands of bottled water in South Korea, and compare the measured fluoride concentration to the concentration written on the label of each brand of bottled water. Methods: Twenty-seven of the 59 different brands of bottled water produced in South Korea were investigated in this study. Three bottles of each brand were purchased from supermarkets, marts, and convenience stores in each region of Korea in August 2016. For each bottled water brand, the fluoride content was measured three times using a fluoride-ion selective electrode (Orion ionplus Fluoride Electrode 9609, Orion Research, USA). The calibration curve was generated using 0.2 and 2 ppm standard solutions, and confirmed using a 1 ppm standard solution. Results: The mean fluoride content of the 27 brands of bottled water was $0.374{\pm}0.332mg/L$ (range=0.040 to 1.172 mg/L). The fluoride content was labeled by the manufacturer, on each of the tested brands of bottled water. In eight brands, the labeled fluoride content differed from the experimental data. The minimum to maximum fluoride content measured from 10 brands showed a variation of 0.3 mg/L or more when compared to the labeled fluoride content. Conclusions: This study investigated the fluoride content of various brands of bottled water produced in South Korea and compared the measured fluoride levels with fluoride information on the bottle labels. To ensure that consumers are suitably informed regarding their exposure to fluoride, correct labelling of fluoride content in bottled water is important.

Comparative Efficacy of Synthetic and Natural Tenderizers on Quality Characteristics of Restructured Spent Hen Meat Slices (RSHS)

  • In the present study, comparative efficacy of natural as well as synthetic tenderizers on the quality characteristics of restructured spent hen meat slices (RSHS) was studied. Four different batches of RSHS viz. Control (without any tenderizer), T1 (1.25% calcium chloride replacing salt in formulation), T2 and T3 (1.5% each of pineapple rind and fig powder, replacing binder in the formulation) were developed in pre-standardized formulation. Vacuum tumbling was performed for 2.5 h and cooked product (RSHS) was assayed for quality attributes. Samples were packaged in aerobic conditions, stored for 21 days under refrigeration ($4{\pm}1^{\circ}C$) and were evaluated for pH, oxidative and microbial quality parameters at regular interval of 7 days. Water holding capacity of T2 was recorded the highest and significantly higher (p<0.05) than all other samples. The textural attributes of T2 were comparable to T1 but significantly higher (p<0.05) than C and T3. The colour attributes ($L^*$, $a^*$, and $b^*$ value) of T2 and T3 were improved due to use of natural tenderizers. During sensory evaluation, tenderness scores for T2 samples were recorded the highest. Throughout storage period, thiobarbituric acid reactive substances (TBARS), free fatty acids (FFA) and peroxide value (PV) followed an increasing trend for control as well as treated products; however, T2 showed a significantly (p<0.05) lower value than control and other treated samples. It can be concluded that good quality RSHS with better storage stability could be prepared by utilizing 1.5% pineapple rind powder as natural tenderizer.

Physiochemical characteristics and fermentation ability of milk from Czech Fleckvieh cows are related to genetic polymorphisms of β-casein, κ-casein, and β-lactoglobulin

  • Objective: The aim of the study was to find a possible association between the ${\beta}-$ and ${\kappa}-casein$ and ${\beta}-lactoglobulin$ genotypes and important milk physiochemical and technological characteristics such as acidity, alcohol stability, the contents of some minerals and the parameters of acid fermentation ability (FEA) in Czech Fleckvieh Cattle. Methods: Milk and blood samples were collected from 338 primiparous Czech Fleckvieh cows at the same stage of lactation. The genotypes of individual cows for ${\kappa}-casein$ (alleles A, B, and E) and ${\beta}-lactoglobulin$ (alleles A and B) were ascertained by polymerase chain reaction-restriction fragment length polymorphism, while their ${\beta}-casein$ (alleles $A^1$, $A^2$, $A^3$, and B) genotype was determined using melting curve genotyping analysis. The data collected were i) milk traits including active acidity (pH), titratable acidity (TA), alcohol stability (AS); calcium (Ca), phosphorus (P), sodium (Na), magnesium (Mg), and potassium (K) contents; and ii) yoghurt traits including active acidity (Y-pH), titratable acidity (Y-TA), and the counts of both Lactobacilli and Streptococci in 1 mL of yoghurt. A linear model was assumed with fixed effects of herd, year, and season of calving, an effect of the age of the cow at first calving and effects of the casein and lactoglobulin genotypes of ${\beta}-CN$ (${\beta}-casein$, CSN2), ${\kappa}-CN$ (${\kappa}-casein$, CSN3), and ${\beta}-LG$ (${\beta}-lactoglobulin$, LGB), or the three-way interaction between those genes. Results: The genetic polymorphisms were related to the milk TA, AS, content of P and Ca, Y-pH and Lactobacilli number in the fresh yoghurt. The CSN3 genotype was significantly associated with milk AS (p<0.05). The effect of the composite CSN2-CSN3-LGB genotype on the investigated traits mostly reflected the effects of the individual genes. It significantly influenced TA (p<0.01), Y-pH (p<0.05) and the log of the Lactobacilli count (p<0.05). Conclusion: Our findings indicate that the yoghurt fermentation test together with milk proteins genotyping could contribute to milk quality control and highlight new perspectives in dairy cattle selection.

  • The mitochondria is the major cellular organelle of energy metabolism for the supply of cellular energy; it also plays an important role in controlling calcium regulation, reactive oxygen species (ROS) production, and apoptosis. Mitochondrial dysfunction causes various diseases, such as neurodegenerative diseases, Lou Gehrig's disease, cardiovascular disease, mental disorders, diabetes, and cancer. Most of the diseases are age-related diseases. In this review, we focus on the roles of mitochondrial dysfunction in cancer. Mitochondrial dysfunction induces carcinogenesis and is found in many cancers. The factors that cause mitochondrial dysfunction differ depending on the types of carcinoma, and those factors could cause cancer malignancy, such as resistance to therapy and metastasis. Mitochondrial dysfunction is caused by a lack of mitochondria, an inability to provide key substances, or a dysfunction in the ATP synthesis machinery. The main factor associated with cancer malignancy is mtDNA depletion. Mitochondrial dysfunction would leads to malignancy through changes in molecular activity or expression, but it is not known in detail which changes lead to cancer malignancy. In order to explore the relationship between mitochondrial dysfunction and cancer malignancy in detail, mitochondria dysfunctional cell lines are constructed using chemical methods such as EtBr treatment or gene editing methods, including shRNA and CRISPR/Cas9. Those mitochondria dysfunctional cell lines are used in the study of various diseases caused by mitochondrial dysfunction, including cancer.

A study on the eating behaviors and food intake of diabetic patients in Daegu·Gyeongbuk area (대구·경북지역 당뇨 환자의 식행동 및 식품 섭취에 관한 조사 연구)

  • Rapid economic development has changed the dietary habits and patterns. Especially, western style diet has increased the risk of type 2 diabetes (T2DM) in Korea. To provide more specific and appropriate diet guideline for the prevention and for the treatment of T2DM, the investigation on the characteristics of diebetic patients related to the diet needs to be done. In this study, we therefore analyzed eating behaviors, dietary attitudes, and intake of food and nutrients of normal subjects (control, n=26) and diabetic patients (case, n=18) diagnosed T2DM within one year in Daegu Gyeongbuk area. Body mass index of the patients were significantly higher than the control (p<0.05). Overeating, high fried food and low whole grain intake were revealed as risk factors for T2DM. From the food frequency questionnaire analysis, salty foods such as fermented soybean paste (Doenjang) and watery Kimch intake were associated with T2DM. Intake of vegetable lipid, ${\beta}$-carotene, calcium, copper, and vitamin K were also shown to be associated with T2DM. Taken together, these findings suggest that maintaining ideal body weight and intake the foods with low salt, fat, and refined grain in appropriate amount may help to prevent and to manage T2DM.

Endothelium-Dependent Vasorelaxation Effects of DangGuiSu-San, SamHwangSaSim-Tang extract on Rabbit Carotid Artery (당귀수산과 삼황사심탕의 혈관이완효과)

  • This study was conducted to evaluate the vasorelaxant effect of DangGuiSu-San and SamHwangSaSim-Tang extract on contracted rabbit carotid artery. To study the effect of DangGuiSu-San and SamHwangSaSim-Tang extract on contracted rabbit carotid arterial strips, arterial strips with intact or damaged endothelium were used for experiment using organ bath. The pre-contracted arterial strips with Phenylephrine(PE) was treated with various concentrations of DangGuiSu-San and SamHwangSaSim-Tang extract(0.01, 0.03, 0.1, 0.3 and $1.0mg/m{\ell}$). To determine the mechanisms of DangGuiSu-San and SamHwangSaSim-Tang-induced vasorelaxant, DangGuiSu-San and SamHwangSaSim-Tang extract were infused into contracted arterial rings which had been pretreated by indomethacin(IM), tetraethylammonium chloride(TEA), $N{\omega}$-nitro-L-arginine ($_L-NNA$), methylene blue(MB). And calcium chloride(Ca) 1 mM was infused into precontracted arterial ring induced by PE after treatment of DangGuiSu-San and SamHwangSaSim-Tang extract in $Ca^{2+}$-free krebs solution. DangGuiSu-San and SamHwangSaSim-Tang extract revealed significant relaxation on PE-induced arterial contraction. DangGuiSu-San and SamHwangSaSim-Tang extract also had an effective relaxation to the intact endothelium arterial ring. SamHwangSaSim-Tang extract on contracted rabbit carotid artery is related with NO-cGMP pathway. Pretreatment of DangGuiSu-San and SamHwangSaSim-Tang extract inhibited the contraction by influx of extracellular $Ca^{2+}$ in contracted arterial ring induced by NE. This study indicated that the relaxation effect of SamHwangSaSim-Tang extract on contracted rabbit carotid artery is related with NO-cGMP pathway. Pretreatment of DangGuiSu-San and SamHwangSaSim-Tang extract inhibited the contraction by influx of extracellular $Ca^{2+}$ in contracted arterial ring induced by NE.

Effects of Turmeric (Curcuma longa L.) on Lipid Component and Protein Concentration in Dyslipidemic Rats (울금(Curcuma longa L.) 첨가 식이가 이상지질혈증 흰쥐의 지질성분 및 단백질 농도에 미치는 영향)

  • This study aimed to investigate the improvement effect of turmeric (Curcuma longa L.) on the lipid component, protein and electrolyte concentration in dyslipidemic rats. Sprague-Dawley rats (24 male) were divided into four groups, namely the ND (normal-nondyslipidemic diet), NT (normal-nondyslipidemic diet+5% turmeric), DD (control-dyslipidemic diet), and DT groups (dyslipidemic diet+5% turmeric). Rats were sacrificed at the end of 5 weeks after experiment diet. In this study, turmeric diet (NT, DT) groups in lipid composition as evidenced from the significantly reduction of serum total cholesterol, low density lipoprotein-cholesterol (LDL-cholesterol), atherosclerotic index (AI), cardiac risk factor (CRF), triglyceride (TG), phospholipid (PL), free cholesterol, cholesteryl ester, blood glucose and non esterified fatty acid (NEFA), and elevation of high density lipoprotein-cholesterol (HDL-cholesterol) (p<0.05). The serum globulin concentration was significantly decreased (p<0.05), and the albumin concentrations were increased in turmeric diet than dyslipidemic rats. Concentrations of sodium (Na) and chlorine (Cl) in sera were lower in the DT group than DD group. Concentrations of total calcium (T-Ca), phosphorus (Pi) and potassium (K) in sera were higher in the ND, NT and DT groups than DD group. Therefore, it was concluded that the 5% turmeric diet used in the condition of this study had a beneficial effect on dyslipidemia.

Mechanisms on Struvite Production for Nitrogen and Phosphorus Recovery (질소/인 회수를 위한 Struvite 생성 메커니즘)

  • The recovery of struvite using nitrogen and phosphorus in wastewaters is useful for prevention of eutrophication and use as fertilizer, but there are theoretical and technical issues to be resolved. Through the detailed literature review, this study discusses the possible reasonable prediction of struvite formation reaction by setting a feasible reaction equation with some theoretical considerations. In a technical aspect, the purity of struvite in solid precipitates can be promoted by excluding Ca in an effective way. As for the struvite reaction prediction issue, selection of proper equilibrium reaction as well as its reaction equilibrium coefficient is significant in the neutral and basic pH regions. The equilibrium reaction agrees well with the experimental batch test results. Considering the charge balance of the ions, the pH drop along struvite formation in a diluted solution can be predicted. Also, improvement of struvite purity through elimination of Ca can be expected by providing a highly concentrated ${NH_4}^+-N$ relative to ${HPO_4}^{2-}-P$ because ${NH_4}^+$ can enhance the thermodynamic driving force toward favorable struvite formation. Even though the phosphate reacts rapidly with Ca to form a solid precipitate, the thermodynamic driving force due to the high ${NH_4}^+$ activity can dissociate the phosphate-calcium precipitates and produce struvite.

Milk Intake Patterns with Lactose and Milk Fat in Korean Male Adults (유당 및 유지방을 중심으로 한 한국 성인 남성의 우유 섭취 행태)

  • Objectives: This study examined the milk intake patterns with lactose and milk fat in Korean male adults using the following variables: milk intake level, awareness of lactose, and milk fat, health problems, and necessity of milk intake. In addition, the factors affecting milk intake were analyzed by multiple regression analysis. Methods: The subjects were 532 males aged 20 years or older among the nationwide milk purchasing group. The subjects were 223 (41.9%) in the 20-29 year age group, 188 (35.3%) in the 30-49 year age group and 121(22.7%) in the over 50 year age group. The survey was conducted using ANOVA and multiple comparative analysis to examine the differences in age and multiple regression analysis was performed to investigate the factors affecting the intake of milk. Results: The intake of milk in the subjects was $538.14{\pm}494.23ml$ per week. There were statistically significant differences in the subjects' age according to processed milk, low fat, nonfat milk, cheese, and ice cream. The perception of milk and lactose and milk fat was recognized as a good food for skeletal health when milk was consumed. Among the milk nutrients, lactose was highly recognized at the age of 20-29, and milk fat was recognized in those over 50 years. In addition to lactose and milk fat, calcium was the most highly recognized among the milk nutrients. Health problems associated with milk were skeletal health, obesity, and lactose intolerance. The perception of lactose intolerance was related to lactose intolerance and fatness, and the dietary behavior was unaffected. Conclusions: This study examined the milk intake patterns of adult Korean males. Many variables were found to be related to the intake of milk. In this study, the milk intake was high when there was no problem with the perception and dietary behaviors of milk nutrition (lactose and milk fat). This study focused on lactose and milk fat, which are major nutrients in milk, and it is a new perspective study among milk-related research.
