거골하 관절 제동술을 이용한 소아 유연성 편평족의 치료 결과 (The Results of Subtalar Arthroereisis for Flexible Flatfoot of Children)

  • Purpose: We evaluate the results of subtalar arthroereisis with $Kalix^{(R)}$ implant (Newdeal, Lyon, France) that were performed in painful flatfoot deformity. Materials and Methods: We performed the subtalar arthroereisis on 16 feet of children symptomatic flexible flat feet after more than 6 months of conservative treatment. Average age was 11 years (8-14 years) old. We checked the functional status with AOFAS functional score in pre-operatively and at final follow-up. Radiologically, we took weight bearing anterior to posterior and lateral view of the feet, and measured the talo-$1^{st}$ metatarsal angle, calcaneal pitch angle, cuboid-surface height. Finally, we asked to patient's parents for satisfaction of the surgery. Results: Mean follow up period was 34.1 months. Average AOFAS score improved from preoperatively 71.9 to postoperatively 91.3. Only one patient has subtalar pain. Average lateral $1^{st}$ metatarsal angle reduced from $-18.2^{\circ}$ preoperatively to $-4.6^{\circ}$ at final follow-up. Average anterior to posterior $1^{st}$ metatarsal angle was reduced from $18.9^{\circ}$ preoperatively to $6.5^{\circ}$ at final follow-up. Average calcaneal pitch angle was increased from $8.6^{\circ}$ preoperatively to $12.6^{\circ}$ at final follow-up. Average cuoboid-surface height was improved from 12.1 mm preoperatively to 16.0 mm at final follow-up. All patients had excellent or good satisfaction. Conclusions: Subtalar arthroereisis with $Kalix^{(R)}$ implant is a viable surgical alternative for painful flatfoot deformity of children.

뒤꿈치 통증이 있는 족부의 방사선학적 분석 (A Radiographic Analysis of the Feet in Heel Pain)

  • Purpose: This study is to compare the medial longitudinal arch between heel pain group and normal painless group. Materials and Methods: Heel pain group 242 feet and normal group 140 feet were evaluated through the radiographic images of standing foot-ankle lateral view. Four radiographic indices, talo-1stmetatarsal angle (TMA), calcaneal-1st metatarsal angle (CMA), calcaneal pitch angle (CPA), and arch ratio (AR), were used as well as BMI. Results: There was no difference between heel pain group and normal group in the TMA, CMA, CPA, and AR. But in the BMI, heel pain group showed $1.7\;kg/m^2$ (p=0.0002) higher than normal group. To eliminate the sexual error, male and female were evaluated separately. Male heel pain group showed 2.9 degrees more dorsiflexion (p=0.001) in the TMA, 3.1 degrees greater (p=0.007) in the CMA, 0.01 lower (p=0.028) in the AR, and $1.0\;kg/m^2$ greater (p=0.033) in the BMI than normal male group. There were no difference in the CPA. Female heel pain group showed 3.6 degree greater (p=0.035) in the CMA, and $1.9\;kg/m^2$ greater (p=0.002) in the BMI than normal female group. But other indices demonstrated no differences. Conclusion: talo-$1^{st}$ metatarsal angle, calcaneal-$1^{st}$ metatarsal angle and arch ratio were radiographic indices related with heel pain.

유연성 편평족에 대한 내측 전위 종골 절골술의 단기 추시 결과 (Short-term Results of Medial Displacement Calcaneal Osteotomy for Flexible Flatfoot)

  • Purpose: To evaluate the short-term results of medial displacement calcaneal osteotomy without flexor digitorum longus transfer for flexible flatfoot deformity. Materials and Methods: Twenty four patients (25 feet) who had undergone medial displacement calcaneal osteotomy without flexor digitorum longus transfer for flexible flatfoot between July 2004 and May 2007 were included. The mean age was 43.6 years (16~78 years). The mean follow-up was 26 months (14~50 months). Clinical outcomes were assessed using American Orthopaedic Foot and Ankle Society (AOFAS) score and visual analogue scale (VAS). Six radiographic parameters were measured from weightbearing radiographs to assess the difference between preoperative and postoperative radiographs. Results: The mean AOFAS score improved from 57.9 points preoperatively to 89.2 points at latest follow-up (p=0.000). The mean VAS improved from 62 points preoperatively to 23 points at latest follow-up (p=0.000). The mean talonavicular coverage angle on anteroposterior (AP) view changed from 20.2 degrees to 16.0 degrees (p=0.002). The mean calcaneal pitch angle on lateral view changed from 12.6 degrees preoperatively to 14.5 degrees at latest follow-up (p=0.001). Regarding these radiographic parameters, the difference between interobserver measurements was larger than that between pre- and post-operative measurements. The calcaneus was transferred medially by average 11.8 mm (p=0.003), which was 27.9% of the width of calcaneal tuberosity (p=0.000). The mean talo-first metatarsal angle on AP and lateral views, and navicular height on lateral view were not significantly changed. Conclusion: Medial displacement calcaneal osteotomy without flexor digitorum longus transfer for flexible flatfoot could lead to improve the clinical outcomes, although the restoration of medial longitudinal arch was not clinically significant.

족부측방촬영을 통한 정상 아동의 족근골 및 중족골간 각도 계측 결과 (Measurements on Radiographs of the Foot in Normal Children)

  • 정상 아동 124명에서 족부 단순방사선촬영 소견에서 편평족과 관련하여 방사선학적 계측치를 나이에 따라 비교한 결과 나이가 어릴수록 대부분이 편평족 소견을 보였고 나이가 많아짐에 따가 차츰 발아치가 형성되어 13세 이후에는 대부분의 예에서 성인과 비슷한 소견을 보였다. 따라서 아동에서는 방사선학적 계측에서 편평족 소견을 보이더라도 이를 치료할 필요가 있을지 결정하는데는 임상 소견이 더 중요할 것으로 사료된다. 본 연구는 정상아동에 대한 횡적 조사연구이므로 향후 종적 조사를 시행하면 나이에 따른 발아치의 변화를 좀 더 정확하게 파악할 수 있을 것으로 사료된다.

Effect of Taping Therapy and Inner Arch Support on Plantar Lower Body Alignment and Gait

  • Objective: The purpose of this study was to identify the effects of taping therapy and inner arch support on pes planus lower extremity alignment and gait. Method: The study was conducted on 13 women in their 20s who had pes planus and no gait problems. Independent variables were the condition of wearing basic socks (S1) and the condition of wearing socks with taping therapy and inner arch support (S2). The dependent variables were resting calcaneal stance position (RCSP), plantar pressure distribution during gait, and underlying and medial longitudinal arch angle measured using radiography. Statistical analysis was performed using the Wilcoxon test with SPSS 23.0 for comparison of S1 and S2. Results: In the RCSP measurement, the angle range of S2 changed to normal. Meary's angle appeared to be less than the angle of S1, indicating alleviation of the degree of pes planus. The calcaneal pitch angle increased at S2 from that at S1. The plantar pressure distribution was divided into four areas (toe, forefoot, midfoot, and hindfoot). At S2, the maximum pressure increased in the toe and midfoot. The maximum force increased significantly in the toe and midfoot but decreased significantly in the forefoot and hindfoot. In addition, the contact area increased overall especially at the midfoot and hindfoot. Contact time decreased in the toe and forefoot, but increased in the midfoot and hindfoot. Conclusion: Taping therapy and inner arch support showed structural improvement of the pes planus. In addition, the force and pressure applied to the foot during walking are distributed evenly in the area of the sole, thus positively affecting walking.

영상학적 및 관절경적 소견으로 평가한 족근동 증후군의 다양한 병적 상태 (Various Pathologic Conditions of Sinus Tarsi Syndrome Assessed by Imaging and Arthroscopic Findings)

  • Purpose: Sinus tarsi syndrome (STS) is caused by various pathologies. However, the exact etiology of STS remains controversial. This study evaluated the imaging and arthroscopic findings of patients who underwent surgical treatment after conservative treatment for STS failed. Materials and Methods: Between December 2014 and August 2018, 20 patients (21 cases) who underwent surgical treatment for STS were included in the study. The clinical results were analyzed using the visual analog scale (VAS) and the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot functional scale. The radiographic results were analyzed using Meary's angle, calcaneal pitch angle, and hindfoot alignment angle. The pathologic conditions of sinus tarsi were confirmed by magnetic resonance imaging (MRI) and subtalar arthroscopy. Synovitis, bone edema, and accessory anterolateral talar facet (AALTF) were evaluated on MRI. Synovial thickening, cartilage damage, interosseous talocalcaneal ligament (ITCL) and cervical ligament rupture, soft tissue impingement, AALTF, and accessory talar facet impingement (ATFI) were evaluated by subtalar arthroscopy. Results: The mean duration of symptoms was 28.7 months (4~120). All patients showed significant improvement in the VAS and AOFAS ankle-hindfoot scale. Significant improvements in hindfoot alignment angle and Meary's angle postoperatively were noted in patients who underwent medial displacement calcaneal osteotomy. MRI confirmed synovitis in all patients, AALTF in 19 cases (90.5%), and ATFI with bone edema in seven cases (33.3%). In subtalar arthroscopy, pathologic conditions were observed in the following order: synovitis in 21 cases (100%), AALTF in 20 cases (95.2%), ITCL partial rupture in nine cases (42.9%), and soft tissue impingement in seven cases (33.3%). All cases had two or more pathological conditions, and 15 (71.4%) had three or more. Conclusion: In cases of STS that do not respond to conservative treatment, a comprehensive examination of the lesions of the tarsal sinus and lesions around the subtalar joint is essential.

선천성 만곡족의 장기 수술적 치료 결과 (Long-Term Results of Surgical Treatment for the Idiopathic Clubfoot)

  • 목적: 도수조작 및 석고 교정을 통해 보존적 치료를 시행하였으나 잔여 혹은 재발변형으로 수술적 치료를 받았던 특발성 만곡족 환아의 장기 치료 결과를 분석하였다. 대상 및 방법: 방사선상 골 성숙이 완료된 32명의 환자 총 51예를 대상으로 후향적인 연구를 시행하였다. 최종 추시 시 평균 연령은 18.7세였다. 수술은 선택적 혹은 광범위 연부조직 유리술과 힘줄 연장, 힘줄 이전, 다양한 절골술 등을 시행하였다. 방사선적 분석은 최종 촬영한 체중부하 전후면 사진에서 거종골 각(talocalcaneal angle), 거골-제1 중족골 각(talo-first metatarsal angle), 측면 사진에서 측정한 거종골 각, 종골 경사각(calcaneal pitch)을 측정하여 성인정상 측정값과 비교하였다. 임상적 평가는 American Orthopaedic Foot and Ankle Society (AOFAS)의 ankle-hindfoot score와 midfoot score를 이용하여 우수(>85), 양호(71-85), 보통(56-70), 불량(<56)으로 평가하였다. 결과: 최종 방사선 계측치는 전후면 거종골 각에서 각각 41.2%, 전후면 거골과 제1 중족골 간의 각에서 90.2%, 측면 거종골 각에서 84.3%, 측면 종골 경사각에서 각각 61%가 정상범위에 포함되었다. AOFAS 평균은 ankle-hindfoot score에서 88.1±10.7점 midfoot score에서 86.7±11.5점이었다. 결론: 수술적 치료를 받았던 특발성 만곡족 환아의 장기 치료 결과에서 정상범위에 속하는 방사선 지표 비율은 43%-90%로 확인되었으며 임상적으로 AOFAS 평균값은 우수에 속하였다. 따라서 비수술적 교정 방법에 실패한 경우라도 변형의 요소를 보다 더 정확히 분석한 후 선택적 수술 방법을 통할 경우 임상적으로 만족스러운 결과를 가져올 수 있다.

족저 근막염 환자의 보존적 치료시 예후 인자 연구 (A Study of Prognostic Factors of Conservative Treatment in Plantar Fasciitis)

  • Purpose: We analyzed to find out prognostic factors in the conservative treatment for the plantar fasciitis. Materials and Methods: The data were collected from 145 patients, 159 feet (M:F = 51:108) with conservative treatment and analyzed for possible prognostic factors : sex, age of onset, the duration of symptom before treatment, pain score before treatment, the duration for symptom remission, medication period, calcaneal pitch angle, and presence of calcaneal spur. Results: The duration of symptom before conservative treatment is affected to the prognosis, and the borderline of the effective period was about 6 months. Conclusion: With the conservative treatment of the plantar fasciitis, we found that (1) the duration of symptom before the conservative treatment was a prognostic factor, and (2) if the period before the conservative treatment was more than 6 months, the other treatment option such as surgery should be considered for this chronic group.

소아의 유연성 편평족에서 내측 세로궁 지지대의 효과 (The Effect of Medial Arch Support for Flexible Flat Foot of Children)

  • Purpose: The author evaluated the clinical and radiological results after wearing the medial arch supports in children. Materials and Methods: 103 patients who had symptomatic flat feet were evaluated from march, 2002 to may 2009. All patients wore the medial arch supports according to the symptoms. We measured parameters at weight-bearing radiographs before and after medial arch support were worn. We also evaluated the clinical scores using the AOFAS score. Results: Mean age of patients was 97 months (11-204 months), all foot of patients involved bilaterally. Mean talo-first metatarsal angle of right foot was $17.7{\pm}9.4$ and left foot was $19.96{\pm}9.5$ degrees at AP radiograph in pre-wearing state. Mean calcaneal pitch angle of right foot was $12.0{\pm}5.3$ and left foot was $11.9{\pm}5.8$ degrees at lateral radiograph in pre-wearing state. Mean talo-first metatarsal angle of right foot was $14.4{\pm}8.05$ and left foot was $13.1{\pm}8.77$ degrees at AP radiograph in post-wearing state. Mean calcaneal pitch angle of right foot was $16.4{\pm}5.75$ left foot was $16.5{\pm}5.6$ degrees at lateral radiograph in post-wearing state. The radiographic angles between pre-wearing and post-wearing state were statistically significant (p<0.05). Mean pre-wearing AOFAS hindfoot score was $66.7{\pm}9.25$, midfoot score was $60.0{\pm}9.34$ forefoot score was $57.1{\pm}11.8$. Mean post-wearing AOFAS hindfoot score was $73.2{\pm}9.73$, midfoot score was $68.1{\pm}10.1$, forefoot score was $67.2{\pm}11.4$. The forefoot score was highest improving scores among the AOFAS scores. Conclusion: From our study, we concluded that medial arch support was effective for symptomatic flat feet of children in radiological and clinical results from our study.

요내반족 변형에 대한 재건수술의 임상적 및 방사선학적 결과 분석 (Analysis of Clinical and Radiographic Outcome of the Reconstructive Surgery for the Cavovarus Foot Deformity)

  • Purpose: Reconstructive surgeries for equinocavovarus foot deformities are quite variable, including hind-midfoot osteotomy or arthrodesis, soft tissue procedure, tendon transfers, etc. Comprehensive evaluation of the deformity and its etiology is mandatory for achievement of successful deformity correction. Few studies in this field have been reported. We report on the clinical and radiographic outcome of reconstruction for cavovarus foot deformities. Materials and Methods: The study is based on 16 feet with cavovarus foot deformities that underwent bony and soft tissue reconstructive surgery from 2004 to 2008. We evaluated the etiologies, varieties of surgical procedures performed, pain score, functional scores, and patient satisfaction and measured the radiographic parameters. Results: The average age at the time of surgery was 39.4 years old, with a male/female ratio of 9/4 and an average follow-up period of 23.9 months (range, 12~49 months). The etiologies of the cavovarus deformity were idiopathic 7 feet, residual poliomyelitis 5 feet, Charcot-Marie-Tooth disease 2 feet, and Guillain-Barre syndrome and hemiplegia due to cerebrovascular accident sequela 1 foot each. Lateral sliding calcaneal osteotomies were performed in 12 feet (75%), followed by Achilles tendon lengthening and plantar fascia release in 11 feet (69%), and first metatarsal dorsiflexion osteotomy/arthrodesis and tendon transfer in 10 feet (63%). Visual analogue scale pain score showed improvement, from an average of 4.2 to 0.5 points. American Orthopaedic Foot and Ankle Society ankle-hindfoot score showed significant improvement, from 47.8 to 90.0 points (p<0.05). All patients were satisfied. Ankle range of motion improved from $27.5^{\circ}$ to $46.7^{\circ}$. In radiographic measurements, calcaneal pitch angle improved from $19.1^{\circ}$ to $15.8^{\circ}$, Meary angle from $13.0^{\circ}$ to $9.3^{\circ}$, Hibb's angle from $44.3^{\circ}$ to $37.0^{\circ}$, and tibio-calcaneal axis angle from varus $17.5^{\circ}$ to varus $1.5^{\circ}$ Conclusion: We achieved successful correction of cavovarus foot deformities by performing appropriate comprehensive reconstructive procedures with improved functional, radiographic measures and high patient satisfaction.
