Physical properties of cyclosporin A-loaded nanostructured lipid carriers (Cyclosporin A가 봉입된 nanostructured lipid carriers의 물리적 특성연구)

  • Cyclosporin A (CyA), a potent immunosuppressive drug used in allogeneic transplants and autoimmune disease, is a typical water-insoluble drug. Recently, nanoparticle carriers were investigated to improve the intestinal absorption of drugs. In this study, CyA-loaded nanostructured lipid carriers (NLCs) were prepared from a hot o/w emulsion using the high pressure homogenization method. The NLCs were consisted of cationic lipids, solid lipids, liquid lipids (oils), surfactant and stabilizer. Encapsulation efficiency of CyA in NLCs was approximately 71%. The average particle size and zeta potential of NLCs were below 250 nm and above +40 mV, respectively. The morphology of NLCs was confirmed by transmission electron microscopy (TEM) analysis. Compared to the CyA powder, higher in vitro release of CyA from NLCs was observed after burst release within 30 min. Thus, CyA-loaded NLCs could be applied not only for parenteral route but also for gastrointestinal administration, which needs further investigation.

Studies on Suppositories of $Phenytoin-{\beta}-Cyclodextrin$ Inclusion Complexes (페니토인-${\beta}$-시클로덱스트린 포접 복합체의 좌제에 관한 연구)

  • An inclusion complex of phenytoin (PT) with ${\beta}-cyclodextrin\;({\beta}-CyD)$ in molar ratio of 1 : 1 was prepared, and the interaction between host and guest molecules was confirmed by infrared spectrometry, differential scanning calorimetry and X-ray diffractometry. Suppositories were prepared by the fusion method. PT and $PT-{\beta}-CyD$ complex were added to PEG 1540 and Witepsol H-15 under the vigorous stirring at $40^{\circ}C$. Content uniformity was tested for different formulations of the PT suppositories. The release rates were dependent on the K.P. V dissolution apparatus and the dialyzing tubing method. Then, the release rates were increased in the following order: $PT-{\beta}-CyD$ complex in PEG 1540>PT in PEG 1540>$PT-{\beta}-CyD$ complex in Witepsol H-15>PT in Witepsol H-15. The area under the curve and maximum blood concentration after rectal administration were increased in the following order: $PT-{\beta}-CyD$ complex in PEG 1540>PT in PEG 1540>$PT-{\beta}-CyD$ complex in Witepsol H-15>PT in Witepsol-15.

Studies of Cyclosporin A Biosynthesis under the Conditions of Limited Dissolved Oxygen or Carbon Source in Fed-batch Culture (용존산소 제한 또는 탄소원 제한 조건의 유가식배양에서의 Cyclosporin A 생합성 연구)

  • We investigated the effects of dissolved oxygen (D.O.) and fructose (C-source) on cell growth and biosynthesis of cyclosporin A (CyA) produced as a secondary metabolite by a wild-type filamentous fungus, Tolypocladium inflatum. This was performed by controlling the level of D.O. and the residual C-source, as required, through adjustment of medium flow rate, medium concentration and agitation rate in fed-batch cultures. CyA production was furned out to be maximal, when D.O. level was controlled around 10% saturated D.O. and concentration of the C-source was maintained sufficiently low (below 2 g/L) not to cause carbon catabolite repression. Under this culture condition, we obtained the highest values of CyA concentration (507.14 mg/L), Qp (2.11 mg CyA/L/hr), $Y_x/s$ (0.49 g DCW/g fructose), $Y_p/s$<(22.56 mg CyA/g fructose), and YTEX>$_p/x$ (48.31 mg CyA/g DCW), but relatively lower values of cell concentration (11.98 g DCW/L) and cell productivity (0.043 g DCW/L/hr), in comparison with other parallel fed-batch fermentation conditions. These results implied that, in the carbon-limited culture with 10% saturated D.O. level, the producer microorganism utilized the C-source more efficiently for secondary metabolism.

Controlled Release of Cyclosporin A from Liposomes-in-Microspheres as an Oral Delivery System

  • The aim of this study was to prepare cyclosporin A-loaded liposome (CyA-Lip) as an oral delivery carrier, with their encapsulation into microspheres based on alginate or extracellular polysaccharide (EPS) p-m10356. The main advantage of liposomes in the microspheres (LIMs) is to improve the restricted drug release property from liposomes and their stability in the stomach environment. Alginate microspheres containing CyA-Lip were prepared with a spray nozzle; CyA-Liploaded EPS microspheres were also prepared using a w/o emulsion method. The shape of the LIMs was spherical and uniform, and the particle size of the alginate-LIMs ranged from 5 to $10\;{\mu}m$, and that of the EPS-LIMs was about $100\;{\mu}m$. In a release test, release rate of CyA in simulated intestinal fluid (SIF) from the LIMs was significantly enhanced compared to that in simulated gastric fluid (SGF). In addition, the CyA release rates were slower from formulations containing the liposomes compared to the microspheres without the liposome. Therefore, alginate-and EPS-LIMs have the potential for the controlled release of CyA and as an oral delivery system.

Effect of Incubation Period, Temperature and pH on Mycelial Growth of Cylindrocarpon destructans (Zinssm.) Scholten Causing Root-rot of Ginseng (배양기간, 온도, pH가 인삼 근부병균 Cylindrocarpon destructans (Zinssm.) Scholten의 균사생육에 미치는 영향)

  • Cylindvocarpon destmtalns isolate CY-92-01, pathogen of root-rot of Panax ginseng showed t the maximum mycelial growth on the Czapek solution agar among the thirteen kinds of media. Five isolates (Isolate CY-92-01, CY-92-03, CY-92-07, CY-94-01, CY-94-02) of C. destructan from various growth stages of p. ginseng recovered from several geographical sites also showed maximum growth in the Czapek-Dox broth compared with potato dextrose broth and V-8 juice broth. Rapid growth rate was maintained until 12 days after inoculation on the Czapek-Dox broth and mycelial weight was somewhat constant until 20 days. After 30 days of incubation, the mycelial weight began to decrease. The fungal growth occurred from 5$^{\circ}C$ to $25^{\circ}C$ and optimum temperature for growth was 2$0^{\circ}C$. Mycelial weight orderly decreased at 15, 25, 10, and 5$^{\circ}C$. Quantitative measurement was impossible at 5$^{\circ}C$. No fungal growth was occurred at the temperature higher than 3$0^{\circ}C$. Growth was observed at all tested pH ranges from 2.8 to 8.0. Optimum pH for growth was 4.0~5.0 followed by pH 3.3~3.5 and 5.4~6.0. The least growth occurred at pH 2.8.

Exploring the Relationship Between International Classification of Functioning, Disability, and Health Items Linked to Clinical Assessments in Children With Cerebral Palsy

  • Background: The International Classification of Functioning, Disability, and Health-Child and Youth version (ICF-CY) is designed to record the characteristics of developing children and examine the influence of a child's environment on their health. Objects: This study was designed to determine the relationship between the clinically extracted ICF-CY items and The Pediatric Evaluation of Disability Inventory (PEDI) and Gross Motor Function Measure (GMFM) items. Methods: Thirty patients (17 males and 13 females) who were hospitalized in a pediatric and youth patient unit of a rehabilitation hospital were included in the study. Four health professionals (two physical therapists and two occupational therapists) working independently linked the PEDI and GMFM-66 items to the activity and participation domains of the ICF-CY. Results: There were strong negative correlations between the ICF-CY subdomains and the PEDI subdomains (r = 0.76-0.95; p < 0.05). There were positive strong correlations between the ICF-CY subdomains and the GMFM-66 (r = 0.76-0.95; p < 0.05). Conclusion: The extracted ICF codes were a valid tool for evaluating the mobility and selfcare conditions of cerebral palsy in the pediatric rehabilitation area.

Ulmus macrocarpa Hance Water Extract Improved Splenocytes Survival and NK Cell Cytotoxicity (왕느릅나무 추출물에 의한 비장세포 생존률 및 NK cell 세포독성 증가)

  • Ulmi cortex is the elm bark or root bark of Ulmus macrocarpa Hance and has been used as an ingredient of traditional medicine for anti-inflammatory, analgesic, anti-cancer and wound healing on both the East and the West. This study investigated whether the Ulmus macrocarpa Hance Water extract (UMWE) has the in vivo and in vitro immune activating effect. Animals were orally administrated for 14 days as follows: no treat group with distilled water, cyclophosphamide (CY) group with 120 mg/kg of CY, UMWE 100+CY group with 100 mg/kg of UMWE and 120 mg/kg of CY, UMWE 200+CY group with 200 mg/kg of UMWE and 120 mg/kg of CY, UMWE 100 group with 100 mg/kg of UMWE and UMWE 200 group with 200 mg/kg of UMWE. The immunosuppressive drug CY was intraperitoneally injected to induce immune suppression. Spleen indices showed small changes in CY injected groups but splenocyte indices showed greater decrease in the same groups. However, UMWE appeared to relieve CY’s immunosuppression. UMWE also delayed in vitro splenocyte death increasing its longevity. These data obtained by MTT assay and 7-amino-actinomycin D which stains preferentially dead than live cells. UMWE alone did not show cytotoxicity based on its apoptototic effect on splenocytes in vitro and in vivo. Splenic NK cell activity was maintained by UMWE under the presence of CY in vitro. The data indicated that UMWE protects splenocytes from the immunosuppressive drug CY under in vitro and in vivo conditions.

Distinctive Characteristics of an Autonomous Replication Sequence of Cephalosporium acremoniurn in Yeast (Cephalosporium acremonium의 자율복제 기점의 특성)

  • An autonomous replication sequence (ARS) derived from Cephalosporium acremonium ATCC 20339 was cloned in Sarchuromyces cerevisiae SHY 3 using YIp5 as a cloning vector. A new recombinant plasmid, designated pCY-2, which contained a 3.7 kb BamHI fragment of C. acrenzonium DNA showed the highest stability among the 40 recombinant plasmids composed of the YIp5 2nd ARS of C. ucremoniztm. Also, Southern hybridization and transformation of E, cull with DNA purified from yeast transformants verified that pCY-2 autonomously replicates in yeasts. Transformation efficiency and plasmid stability of pCY-2 in yeast were higher than those ol YRp 7 containing ARS which originated from yeast. Detailed studies by subcloning revealed that two ARSs existed within 2.6 kb of the insert, which is a novel discovery. However, it was concluded that these two ARSs were ligated during the gene manipulation in vitro.

Peritoneal Washing Cytology Positivity in Gastric Cancer: Role of Lymph Node Metastasis as a Risk Factor

  • Purpose: Peritoneal washing cytology (PWC) is a widely used diagnostic tool for detecting peritoneal metastasis of advanced gastric cancer. However, the prognosis of patients with positive PWC remains poor even after gastrectomy, and treatments vary among institutions and eras. In this study, we identified the clinical factors that can help predict cytology-positive (CY(+)) gastric cancer. Materials and Methods: We retrospectively reviewed the national data of patients with gastric cancer from 2019, as provided by the Information Committee of the Korean Gastric Cancer Association. Of the 13,447 patients with gastric cancer, 3,672 underwent PWC. Based on cytology results, we analyzed the clinicopathological characteristics and assessed the possibility of CY(+) outcomes in relation to T and N stages. Results: Of the 3,270 patients who underwent PWC without preoperative chemotherapy, 325 were CY(+), whereas 2,945 were negative. CY(+) was more commonly observed in patients with Borrmann type IV gastric cancer, an undifferentiated histological type, and advanced pathological stages. Multivariate analysis revealed Borrmann type IV (odds ratio [OR], 1.821), tumor invasion to T3-4 (OR, 2.041), and lymph node metastasis (OR, 3.155) as independent predictors of CY(+). Furthermore, for circular tumor location, the N stage emerged as a significant risk factor for CY(+), particularly when the tumor was located on the posterior wall (PW) side. Conclusions: Lymph node metastasis significantly affects CY(+) outcomes, particularly when the tumor is located on the PW side. Therefore, PWC should be considered not only in suspected serosal exposure cases but also in cases of lymph node metastasis.
