Image-guided surgery (IGS) system has become well known in the field of neurosurgery and spine surgery. A patient's anatomy is first registered to preoperatively acquired CT/ MRI data using the point matching algorithm. A magnetic field digitizer was used to measure the physical space data and the system was based on Workstation of Unix system. To evaluate the spatial accuracy of interactive IGS system, the phantom consisting of rods varied height and known location was used. The RMS error value between CT/MR images and real location was 3-4mm. For the more convenience of the surgery, we provide various image display modules.
kV cone-beam CT (CBCT)가 결합된 선형가속기의 등장으로 더욱 정확한 영상유도 방사선치료(Image-guided radiation therapy, IGRT)가 가능해졌다. IGRT는 영상장비를 이용해 표적의 이동을 보정할 수 있는 방사선치료기술로, CBCT를 이용한 IGRT의 경우 내부 장기의 정보를 바탕으로 병변의 이동을 정확히 알 수 있다. 내부 장기의 정보를 얻기 위해서는 방사선이 조사되기 바로 전에 환자의 CBCT 영상을 획득하여 치료계획 시 사용한 모의치료 CT영상과 정합을 수행하게 된다. 본 연구에서는 CBCT 영상의 재구성 품질에 따른 정합결과를 비교해 보았다. 6명의 환자로부터 총 56개의 CBCT 투과 정보를 획득하여 분석한 결과, 평행이동벡터의 차가 1 mm를 초과하는 경우는 단 3개에 불과하였다. 회전이동의 경우, x, y, z, 세 축을 모두 고려하였으며, 그 결과 총 168 ($56{\times}3$)개에서 단 3개만이 $1^{\circ}$ 이상의 차이를 나타냈다.
In image guided surgery, a patient registration process is a critical process for the successful operation, which is required to use pre-operative images such as CT and MRI during operation. Though several patient registration methods have been studied, we concentrate on one method that utilizes 3D surface measurement data in this paper. First, a hand-held 3D surface measurement device measures the surface of the patient, and secondly this data is matched with CT or MRI data using optimization algorithms. However, generally used ICP algorithm is very slow without a proper initial location and also suffers from local minimum problem. Usually, this problem is solved by manually providing the proper initial location before performing ICP. But, it has a disadvantage that an experience user has to perform the method and also takes a long time. In this paper, we propose a method that can accurately find the proper initial location automatically. The proposed method finds the proper initial location for ICP by converting 3D data to 2D curvature images and performing image matching. Curvature features are robust to the rotation, translation, and even some deformation. Also, the proposed method is faster than traditional methods because it performs 2D image matching instead of 3D point cloud matching.

Background: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a standard procedure to evaluate suspicious lymph node involvement of lung cancer because computed tomography (CT) and $^{18}F$-fluorodeoxyglucose positron emission tomography-CT (PET-CT) have limitations in their sensitivity and specificity. There are a number of benign causes of false positive lymph node such as anthracosis or anthracofibrosis, pneumoconiosis, old or active tuberculosis, interstitial lung disease, and other infectious conditions including pneumonia. The purpose of this study was to evaluate possible causes of false positive lymph node detected in chest CT or PET-CT. Methods: Two hundred forty-seven patients who were initially diagnosed with lung cancer between May 2009 and December 2012, and underwent EBUS-TBNA to confirm suspicious lymph node involvement by chest CT or PET-CT were analyzed for the study. Results: Of 247 cases, EBUS-TBNA confirmed malignancy in at least one lymph node in 189. The remaining 58 patients whose EBUS-TBNA results were negative were analyzed. Age ${\geq}65$, squamous cell carcinoma as the histologic type, and pneumoconiosis were related with false-positive lymph node involvement on imaging studies such as chest CT and PET-CT. Conclusion: These findings suggest that lung cancer staging should be done more carefully when a patient has clinically benign lymph node characteristics including older age, squamous cell carcinoma, and benign lung conditions.
Posterior mediastinal goiter extending to carotid sheath posteriorly is rare repoted case. Recently we experienced huge posterior mediastinal goiter with compressin of trachea in 57-year old male, The patient that complained of dyspnea referred to our hospital for further evaluation of mediasitnal tumor. We confirmed huge secondary posterior mediastinal intrathoracic goiter with diagnostic methods following by chest X-ray, thyroid scan, chest CT, and CT guided fine needle aspiration biopsy in this patient, and performed operation for excision Exision of posterior mediasitnal goiter performed through initial transeverse cervical incision and additional median sternotomy, and the mass removed completely without any complications. The postoperative course were uneventful.

Background: Silk cocoon is composed of multiple layers. The natural silk cocoon containing all layers was cut as a rectangular shape as defined as total group. The inner and outermost layers were removed from the total group and the remained mat was defined as the middle group. The objectives of this study was to compare the total group with the middle group as a barrier membrane for the guided bone regeneration. Methods: The effects of these materials on the cellular proliferation and alkaline phosphatase (ALP) expression of MG63 cells were explored. For comparing bone regeneration ability, bilateral bone defects were created in calvarial areas in ten adult New Zealand white rabbits. The defects were covered with silk membranes of the middle group, with silk membrane of the total group used as the control on the contralateral side. The defects were allowed to heal for 4 and 8 weeks. Micro-computerized tomography (${\mu}CT$) and histological examination were performed. Results: The middle group exhibited a higher MTT value 48 and 72 h after treatment compared to the total group. ALP expression was also higher in the middle group. The results of ${\mu}CT$ and histologic examination showed that new bone formation was significantly higher in the middle group compared to the total group 8 weeks postoperatively (P < 0.05). Conclusions: In conclusion, the middle layer of the silk cocoon supports guided bone regeneration better than unprocessed silk cocoon.

Purpose: This study aimed to develop a surface-guided radiosurgery system customized for a neurosurgery clinic that could be used as an auxiliary system for improving the accuracy, monitoring the movements of patients while performing hypofractionated radiosurgery, and minimizing the geometric misses. Methods: RGB-D cameras were installed in the treatment room and a monitoring system was constructed to perform a three-dimensional (3D) scan of the body surface of the patient and to express it as a point cloud. This could be used to confirm the exact position of the body of the patient and monitor their movements during radiosurgery. The image from the system was matched with the computed tomography (CT) image, and the positional accuracy was compared and analyzed in relation to the existing system to evaluate the accuracy of the setup. Results: The user interface was configured to register the patient and display the setup image to position the setup location by matching the 3D points on the body of the patient with the CT image. The error rate for the position difference was within 1-mm distance (min, -0.21 mm; max, 0.63 mm). Compared with the existing system, the differences were found to be as follows: x=0.08 mm, y=0.13 mm, and z=0.26 mm. Conclusions: We developed a surface-guided repositioning and monitoring system that can be customized and applied in a radiation surgery environment with an existing linear accelerator. It was confirmed that this system could be easily applied for accurate patient repositioning and inter-treatment motion monitoring.
마늘(Allium sativum)의 주요 생리활성 성분들은 다양한 종류의 암에 대해 항암효과가 보고되고 있다. 본 연구에서는 활성추적분리방법(activity-guided purification)을 이용하여 마늘의 항암성분을 발굴하고자 하였다. 마늘 에탄올 추출물을 칼럼크로마토그래피로 얻은 각각의 분획물에 대해서 AGS세포의 증식 억제율을 검증하여 가장 효과가 좋은 분획물로부터 물질을 순수분리하여 구조를 동정한 결과 N-benzyl-N-methyldecan-1-amine (NBNMA)로 밝혀졌다. NBNMA의 암생장 억제효능을 검증하기 위해서 CT-26, AGS, HepG2, HCT-116, MCF7, B16F10 및 Sarcoma-180 세포에 대한 in vitro 효과와 CT-26 결장암 세포를 마우스에 이식한 다음 in vivo 효과를 조사하였다. NBNMA는 Bcl-2의 down-regulation과 Bad의 up-regulation을 유도하여 CT-26 세포의 세포사멸 촉진시켰다. 또한, NBNMA는 세포사멸의 외적 및 내적 경로에서 caspases 억제자인 caspase 3과 caspase 9의 활성을 약간 증가시켰다. CT-26세포를 이식한 쥐에 $19.13{\mu}M/kg$의 NBNMA를 21일 동안 경구투여한 결과 암종의 크기가 43% 감소하였다. NBNMA는 in vitro 및 in vivo에서 항암 효과를 나타내었는데, 이러한 결과는 마늘로부터 순수분리한 NBNMA가 대장암치료를 위한 항암제 후보물질로서 활용 가능성이 있을 것으로 기대된다.
목 적 : 영상유도방사선치료 시 사용하는 콘빔 CT는 치료자세 오차를 확인하는 중요한 수단이지만 피폭선량을 증가 시키는 단점이 있다. 이에 본 연구는 콘빔 CT의 시행주기를 격일로 하는 시나리오를 만들어 유용성을 평가하고자 한다. 대상 및 방법 : 콘빔 CT를 이용하여 세기변조방사선치료를 받은 전립선암 환자 9명을 대상으로 실제 치료 시 매일 콘빔 CT로 치료자세 오차를 분석하여 보정한 값을 바탕으로 격일로 콘빔 CT를 시행하는 시나리오를 만들었다. 시나리오에서 콘빔 CT를 시행하지 않은 날의 치료자세 오차 값을 실제 치료 시의 치료중심점에서 이동하여 치료계획시스템(Pinnacle 9.2, Philips, USA)에 적용한 후 실제 치료와 동일한 조건으로 재 치료계획을 수립하였으며, 이를 바탕으로 PTV(Planning Target Volume)와 정상장기의 선량분포를 비교 분석하였다. 결 과 : 매일 콘빔 CT를 시행하였을 때의 치료자세 오차 값을 기준으로 격일로 콘빔 CT를 시행하는 시나리오에서는 X, Y, Z축으로 각각 $0.2{\pm}0.73mm$, $0.1{\pm}0.58mm$, $-1.3{\pm}1.17mm$ 차이가 나타났다. 이를 치료계획에 적용하여 재 치료계획을 수립하여 선량분포를 평가한 결과는 매일 콘빔 CT를 시행한 결과와 비교하여 PTV의 Dmean : -0.17 Gy, $D_{99%}$ : -0.71 Gy, 차이가 나타났다. 정상 장기는 직장 벽의 $V_{66}$ : 1.55%, 방광의 $V_{66}$ : -0.76% 차이가 나타났다. 결 론 : 격일로 콘빔 CT를 시행하였을 경우 콘빔 CT에 의한 피폭선량을 감소시키고 촬영으로 인한 추가적인 치료시간을 줄여 줄 수 있다. 또한 PTV, 정상장기의 선량분포의 차이가 크지 않으므로 환자의 상태의 따라 격일 콘빔 CT의 적용을 고려할 수 있을 것으로 사료된다.
