Optimal Scan time Analysis for Pancreatic Cancer Distinction in Dual time PET-CT Exam (이중시간 PET/CT 검사에서 췌장암 판별을 위한 최적의 Scan time 분석)

  • In this study, present the most useful delay scan time by statistical analysis of SUVm data for 30 suspected pancreatic cancer patients. Two statistical analysis and a mathematical model was applied to the theoretical formula by glucose and insulin mechanics, and a mathematical model was created. Statistical analysis was performed via Metlab p/g. Optimal delay scan time was suggested by Metlab p/g for the change of SUV value over time.In this study, for diagnosis pancreatic cancer by dual time point PET/CT, propose optimal delay scan time 131.5 minuts. The proposed delay scan time showed statistical reliability applicable to the diagnosis of pancreatic cancer (p<0.05). Delayed scanning with the suggested delay scan time of 131.5 minutes is considered to be useful for the diagnosis of pancreatic cancer compared to general PET / CT scan.hen the delayed test is performed with the proposed delay scan time 131.5 minuts, Compared with general PET/CT scans.

Impact of the Planning CT Scan Time on the Reflection of the Lung Tumor Motion (전산화단층촬영 주사시간(Scan Time)이 폐종양운동의 재현성에 미치는 영향 분석)

  • Purpose : To evaluate the reflection of tumor motion according to the planning CT scan time. Material and Methods : A model of N-shape, which moved aiong the longitudinal axis during the ventilation caused by a mechanical ventilator, was produced. The model was scanned by planning CT, while setting the relative CT scan time (T: CT scan time/ventilatory period) to 0.33, 0.50, 0.67, 0.75, 1.00, 1.337, and 1.537. In addition, three patients with non-small cell lung cancer who received stereotactic radiosurgery In the Department of Radiation Oncology, Asan Medical Center from 03/19/2002 to 05/21/2002 were scanned. Slow (10 Premier, Picker, scan time 2.0 seconds per slice) and fast CT scans (Lightspeed, GE Medical Systems, with a scan time of 0.8 second per slice) were peformed for each patient. The magnitude of reflected movement of the N-shaped model was evaluated by measuring the transverse length, which reflected the movement of the declined bar of the model at each slice. For patients' scans, all CT data sets were registered using a stereotactic body frame scale with the gross tumor volumes delineated in one CT image set. The volume and three-dimensional diameter of the gross tumor volume were measured and analyzed between the slow and fast CT scans. Results : The reflection degree of longitudinal movement of the model increased in proportion to the relative CT scan times below 1.00 7, but remained constant above 1.00 T Assuming the mean value of scanned transverse lengths with CT scan time 1.00 T to be $100\%$, CT scans with scan times of 0.33, 0.50, 0.57, and 0.75 T missed the tumor motion by 30, 27, 20, and $7.0\%$ respectively, Slow (scan time 2.0 sec) and Fast (scan time 0.8 sec) CT scans of three patients with longitudinal movement of 3, 5, and 10 mm measured by fluoroscopy revealed the increases in the diameter along the longitudinal axis Increased by 6.3, 17, and $23\%$ in the slow CT scans. Conculsion : As the relative CT scan time increased, the reflection of the respiratory tumor movement on planning CT also Increased, but remained constant with relative CT scan times above 1.00 T When setting the planning CT scan time above one respiration period (>1.00 T), only the set-up margin is needed to delineate the planning target volume. Therefore, therapeutic ratio can be increased by reducing the radiation dose delivered to normal lung tissue.

Data getting through questionnaire investigated examination comprehension and satisfaction through questionnaire after experiment Scan Positioning to 500 coming to help customers to be source CT examination for 3 months February December - 2007 year in 2006 manufacturing Voice and Visual announce media for reference. To customer who interest degree appeared, and answers preparatory audit from preparatory audit about curiosity of CT examination customer to order of examination time required(43%), contrast media side effect(26%), examination region(20%), breath(10%), etc..(1.5%) audio-visual materials in questionnaire that attain after do reclamation among examination age, sex, reception type of irrelatively in 91% of target increase of hailing degree and examination satisfaction appear. Searched that customer hailing and satisfaction are increased greatly when use of audio-visual materials in satisfaction result that use CT Positioning delay time. In experiment process, It took lacking part by method that use hearing in case of do not use sight as is unavoidable in subject position or old age. Through this, audio-visual materials could analogize that it is more useful method that use sight and hearing at the same time.

  • Despite of the development of medical equipments and technology. Pancreatic cancer has maked high false positive rate and low survival rate compared to other cancers. Therefore, early catch of pancreatic cancer is the only way to enhance the viability. It is important to find the exact location of the pancreas cancer in early stage. The method of optimum scan for early detection of pancreatic cancer on PET/CT exam is proposed. Examined the anatomical region that potentially can be missing from the supine position of ordinary pet/ct exam. The characteristics and usefulness of angle variation ($0^{\circ}$, $30^{\circ}$, $45^{\circ}$, $60^{\circ}$, $90^{\circ}$) of patients scan position is analyzed. The proposed scan method (named JJ-projection) is bringing advantage of anatomical discrimination by separating stomach, liver, gallbladder duodenum and pancreas. ROC curve analysis is shows to advantage of the JJ-Projection method. The sensitivity has increased 4.6% than the supine delay scan method, the results sensitivity has increased from 91% to 95.2%. The specificity has increased from 75.1% to 84%. Compared with the results observed in cancer by biological biopsy, The accuracy has increased from 86.8% to 94.1%.

Measurement of Radiation Dose of HR CT and Low Dose CT by using Anthropomorphic Chest Phantom and Glass Dosimetry (인체등가형 흉부팬텀과 유리선량계를 이용한 고해상력 및 저선량 CT의 선량측정)

  • The purpose of this study is to provide basic clinical data by evaluating images, measuring absorbed dose and effective dose by using high resolution CT and low dose CT by using anthropomorphic chest phantom and glass dosimeter. Tissue dose was measured by inserting a glass dosimeter into the anthropomorphic chest phantom. A 64-slice CT system (SOMATOM Sensation 64, Siemens AG, Forchheim, Germany) and CARE Dose 4D were used, and the parameters of the high resolution CT were 120 kVp, Eff. Scan parameters of mAs 104, scan time 7.93 s, slice 1.0 mm (Acq. 64 × 0.6 mm), convolution kernel (B60f sharp) were used, and low dose CT was 120 kVp, Eff. mAs 15, scan time 7.41 s, slice 3.0 mm (Acq. 64 × 0.6 mm), scan of convolution kernel B50f medium sharp. CTDIvol was measured at 8.01 mGy for high resolution CT and 1.18 mGy for low dose CT. Low dose CT scans showed 85.49% less absorbed dose than high resolution CT scans.

Effect of Work Improvement for Promotion of Outpatient Satisfaction on CT scan (CT 외래환자의 만족도 향상을 위한 업무개선 연구)

  • Nowadays, most of the hospital serves "one stop service" for CT scan. The patients could be taken the CT scan in the day they register for scan. On the contrary to the time convenience, patients are not satisfied with long waiting time and unkindness of staff. The objective of this study is to improve the patient's satisfaction for the CT scan, by analyzing inconvenience factors and improving the service qualities. From April 1 to August 30 in 2011, we investigated the satisfaction of patients who did examined abdomen CT scan with contrast media. We analyzed the 89 questionnaires before and after the service improvements from them. The worker's kindness, the environment of CT room and understanding about CT scan were answered by questionnaire and the waiting time of a day CT scan was drawn by medical information statistics. Also, the period before improvement was from April to June and the period after improvement was from July to September. And these questionnaire was analyzed through SPSS V. 15.0. In this study, kindness of staff, environment of CT room, intelligibility for CT scan and waiting time was explored and analyzed by SPSS V.15.0. The score of kindness was improved by 32%, satisfaction level of the environment was improved by 52.54%. The understanding level about CT scan was improved by 52.36% and the wating time of a day CT was shortened by 21% through our service enhancement programs. Consequentially, it is considered that these efforts would contribute to increase the revenue of hospital.

A Study on the Mitigation of the Exposure Dose Applying Bolus Tracking in Brain Perfusion CT Scan (뇌 관류 CT검사에서 BolusTracking기법을 적용한 피폭선량 저감화에 관한 연구)

  • This study was conducted to analyze the patient's exposed dose targeting the patients who had acute ischemic stroke symptoms and CT brain perfusion scan, by comparing fixed time technique and bolus tracking technique which was provided by the manufacturer and to identify the Time graph to implement the usability of contrast medium's tracking technique the best contrast enhancement intervals. $CTDI_{VOL}$ of PCT in patient appeared to be 431.72mGy in fixed scan delay protocol, whereas 323.61mGy in Bolus tracking technique. The value of DLP appeared to be $1243.47mGy{\cdot}cm$ in fixed scan delay protocol, whereas $932mGy{\cdot}cm$ in Bolus tracking technique. Time graph appeared to be various in fixed scan delay protocol, whereas the optimal time graph could be obtained in Bolus tracking. The exposure dose could be reduced by 25% applying Bolus tracking technique when taking brain perfusion CT scan.

Establishment of Injection Protocol of Test Bolus for Precise Scan Timing in Canine Abdominal Multi-Phase Computed Tomography

  • This study aimed to establish an injection protocol to determine the precise CT scan timing in canine abdominal multi-phase CT using the test bolus method. Three dynamic scans with different contrast injection parameters were performed using a crossover design in eight normal beagle dogs. A contrast material was administered at a fixed dose of 200 mg iodine/kg as a test bolus for dynamic scans 1 and 2, and 600 mg iodine/kg as a main bolus for dynamic scan 3. The contrast materials were administered with 1 ml/s in dynamic scan 1, and 3 ml/s in dynamic scan 2 and 3. The mean arrival time to the appearance of aortic enhancement in dynamic scan 3 was similar to that in dynamic scan 2, and different significantly to that in dynamic scan 1. The mean arrival time to the peak aortic and pancreatic parenchymal enhancement in dynamic scan 3 was similar to that in dynamic scan 1, and different significantly to that in dynamic scan 2. In multi-phase CT scan, a test bolus should be injected with the same injection duration of a main bolus, to obtain the precise arrival times to peak of arterial or pancreatic parenchymal enhancement.

Optimization of Brain Computed Tomography Protocols to Radiation Dose Reduction (뇌전산화단층검사에서 방사선량 저감을 위한 최적화 프로토콜 연구)

  • This study is a model experimental study using a phantom to propose an optimized brain CT scan protocol that can reduce the radiation dose of a patient and remain quality of image. We investigate the CT scan parameters of brain CT in clinical medical institutions and to measure the important parameters that determine the quality of CT images. We used 52 multislice spiral CT (SOMATOM Definition AS+, Siemens Healthcare, Germany). The scan parameters were tube voltage (kVp), tube current (mAs), scan time, slice thickness, pitch, and scan field of view (SFOV) directly related to the patient's exposure dose. The CT dose indicators were CTDIvol and DLP. The CT images were obtained while increasing the imaging conditions constantly from the phantom limit value (Q1) to the maximum value (Q4) for AAPM CT performance evaluation. And statistics analyzed with Pearson's correlation coefficients. The result of tube voltage that the increase in tube voltage proportionally increases the variation range of the CT number. And similar results were obtained in the qualitative evaluation of the CT image compared to the tube voltage of 120 kVp, which was applied clinically at 100 kVp. Also, the scan conditions were appropriate in the tube current range of 250 mAs to 350 mAs when the tube voltage was 100 kVp. Therefore, by applying the proposed brain CT scanning parameters can be reduced the radiation dose of the patient while maintaining quality of image.

Phantom Study of the Mutual Influences Between 18F-FDG and 99mTcO4- on the Same Day (18F-FDG와 99mTcO4-를 이용한 당일 검사 시 상호 영향에 대한 Phantom 연구)

  • Purpose The nuclear medicine examination, there is a difficulty to carry out the inspection of both on the day of residual isotope due to the half-life. In this study, by studying the mutual influence and $^{18}F$-FDG of $^{99m}TcO_4{^-}$, I would like to explain the matters to be considered in the case of performing the same day. Materials and Methods With the NEMA-1994 Phantom, and experiments were performed 3 times. Create a 1: 4 Background ratio HOT and the $^{99m}TcO_4{^-}$ The first experiment: After underwent SPECT in INFINIA (GE Healthcare, MI, USA), and were injected with $^{18}F$-FDG 37 MBq in the Background area, 13 once for 60 minutes under the same conditions was time Scan. Create a 1: 4 Background ratio HOT and the $^{18}F$-FDG second is: The Scan in PET/CT Discovery 600 (GE Healthcare, MI, USA), and 148 MBq after injection $^{99m}TcO_4{^-}$ the Background area, once for 60 minutes, 6 under the same conditions was time Scan. Create a 1: 4 Background ratio HOT and the $^{18}F$-FDG experiments las, increments of 296 MBq and 148 MBq the 1 Bed Scan after $^{99m}TcO_4{^-}$, was 1 Bed Scan under the same conditions. Non BKG area and HOT, I was measured comparing the Total Counts and SNR or CNR. Results Showed a significant difference in the ratio CNR of enforcement during SPECT $^{18}F$-FDG is, (p>0.05). The $^{99m}TcO_4{^-}$ was no significant difference between the SNR ratio of PET / CT at the time of the effective date (p<0.05). I got the results $^{99m}TcO_4{^-}$ that reduce the Total Counts of PET / CT scan. Conclusion If you make a PET / CT scan, may affect the test using the $^{99m}TcO_4{^-}$ up to 12 hours, when it is performed before the $^{99m}TcO_4{^-}$, does not affect the SNR and SUV, PET / CT scan I reduced the detection efficiency. The inspection of day, we'd like to recommend a way to complement the detection efficiency to increase the inspection time of PET / CT in move forward the inspection using the $^{99m}TcO_4{^-}$.
