여닫이형 방사선 치료의 검증: 잔여 움직임의 선량적 영향 (Verification of Gated Radiation Therapy: Dosimetric Impact of Residual Motion)

  • 여닫이형 방사선 치료에서, 잔여 움직임으로 인하여 방사선은 실직적인 질병의 부위 뿐만이 아니라 주변 정상조직까지 투여 되도록 되어 있다. 비록 표적이 방사선 투여 중 움직이지만, 방사선이 최소한도로 실질적인 부위 (임상적 표적 체적)에 조사되기를 원한다. 본 연구의 목적은 여닫이형 치료에 있어서 방사선이 실질적인 표적에 투여되는지를 검증하고, 여닫이 범위, 움직임의 정도 및 임상적 표적체적의 크기의 변화에 따라, 표적및 주변 조직에 투여되는 방사선의 경향을 연구하는 데 있다. 이 목적을 달성하기 위하여, 실험 및 이론적인 연구를 고안하여 수행하였다. 직육각형 및 피라미드형의 표적 체적을 내포하는 팬텀을 만들어 움직이며 4차원 영상을 얻었다. 여러 여닫이 범위를 얻어진 영상에 적용하여 치료계획용 내부표적(표적체적 및 내부 움직임범위포함)을 만들었다. 직육각형 표적에는 전통적인 치료계획을 그리고 피라미드형 표적에는 세기 변조형 치료계획을 세웠다. 평판형 다이오드에 치료계획에서 얻어진 여닫이형 방사선을 수직으로 조사하여 실험적으로 선량평가를 수행하였고 또한 움직이는 상황에서 선량투여를 전산적으로 모사하였다. 본 연구는 두 표적에 대한 반음영 영역의 확장 및 움직임으로 인하여 방해 받았으나 확실하게 수행된 표적 선량투여 그리고 주변 조직에 투여된 상당량의 선량등을 수반하는 잔여움직임의 영향을 정량적으로 그리고 해석적으로 분석하였다. 선량-체적 히스토그램 분석에 따르면, 내부표적에는 여닫이 범위 또는 움직임 정도가 감소함에 따라 또한 표적체적이 증가함에 따라 선량이 증가함을 보였고, 내부 움직임 범위에 해당하는 체적에 대하여는 여닫이 범위 또는 움직임 정도가 감소함에 따라 선량이 증가하였고, 마지막으로 주변 정상조직에 대하여는, 내부 움직임 범위와는 반대의 경향을 보였다. 본 연구는 잔여움직임의 영향에 대하여 확실한 이해를 주었고 호흡행태가 재생되는한 불연속적인 투여과 표적의 움직임에도 불구하고 여당이형 방사선 치료는 안전함을 입증하였다. 본연구에서 수반된 절차와 전산적 모델은 여닫이형 치료의 시작점 검증, 주기적인 품질관리 및 환자별 검증에 사용될 수 있다. 환자별 영상에 선량을 재구성하는 방향으로 추후 연구가 필요하다.

세기변조방사선치료의 품질관리를 위한 이온전리함 매트릭스의 유용성 고찰 (Feasibility of MatriXX for Intensity Modulated Radiation Therapy Quality Assurance)

  • 목 적: 세기변조방사선치료 시 선행되는 품질관리에서 선량분포와 절대선량 값을 비교하여 2차원 이온전리함의 유용성을 알아보고자 한다. 대상 및 방법: 본 실험은 선형가속기(CL 21EX, Varian, Palo Alto, USA)의 6 MV 광자선을 이용하여 비인두암 환자를 대상으로 하였다. 세기변조방사선치료를 위한 품질관리에는 2차원 이온전리함 배열(MatriXX, Wellhofer Dosimetrie, Germany)을 사용하였다. 비인두암 환자의 치료를 위해 역방향치료계획을 시행하였다. 고체팬톰에 삽입된 MatriXX를 전산화단층 촬영하여 환자 치료계획과 동일한 플루언스로 하이브리드(갠트리 각도 0$^\circ$) 치료계획을 실시하였다. 실험적 선량분포의 측정은 하이브리드 치료계획과 동일한 기하학적 조건으로 MatriXX와 고체팬톰을 이용하여 측정하였다. 선량분포에서 고선량 저변위(High Dose Low Gradient: HDLG) 점을 선정하여 절대선량을 비교하였으며, 선량분포 일치성분석을 위해 감마 인덱스 히스토그람($\gamma$-index, Dose difference: 3%, Distance to agreement: 3 mm)을 이용하여 정량화하였다. 결 과: 감마 인덱스 히스토그람 분석결과 인정허용범위(${\gamma}{\leq}$1) 비율을 모든 조사면에서 각각 확인하였다. 갠트리 각도 0$^\circ$에서 95.08%, 55$^\circ$에서 97.52%, 110$^\circ$에서 96.28%, 140$^\circ$에서 98.2%, 220$^\circ$에서 97.78%, 250$^\circ$에서 96.64%, 305$^\circ$에서 92.7%로 나타났다. HDLG에서 절대선량은 $\pm$3% 이내의 일치도를 보였다. 결 론: MatriXX를 이용한 세기변조방사선치료의 품질관리는 필름이나 단일 이온전리함을 이용한 일반적인 세기변조방사선치료 품질관리에 비해 시간, 인력을 최소화하면서 보다 효율적인 접근이 가능하였으며, 선량분포일치성 분석 및 절대선량확인에 매우 유용한 장치임을 알 수 있었다.

세기조절방사선치료(IMRT)의 Commissioning 및 정도관리: AAPM TG119 적용 (Intensity Modulated Radiation Therapy Commissioning and Quality Assurance: Implementation of AAPM TG119)

  • 본 연구에서는 미국의학물리학회(AAPM)에서 제안된 세기조절방사선치료(IMRT)의 commissioning 및 정도관리를 위해 제안된 일련의 시험을 통해 본원의 세기조절방사선치료의 정확도를 평가하고 정도관리 절차를 수립하고자 하였다. TG119에 제시된 5가지 세기조절방사선치료의 표적체적: multi-target, 두경부, 전립선, 및 두 가지의 C-shape (easy&hard)을 대상으로 CT 스캔된 팬텀을 이용하여 치료계획시스템에서 IMRT 치료계획을 수행하였다. 얻어진 치료계획을 dynamic sliding window 전달방식으로 팬텀에 조사하였다. 절대 선량은 표적 및 위험장기 부위에 이온 전리함(CC13, IBA)을 이용하여 측정하고, 2차원 상대 선량분포는 EBT2 필름을 사용하여 측정하였다. 측정된 값들은 해당하는 치료계획시스템 계산값과 비교하여 오차의 평균 및 표준편차를 구하였다. 이를 바탕으로 TG119에 제시된 신뢰구간을 계산하여 제시된 값과 비교하였다. 이온 전리함을 이용한 측정 결과에서 표적 및 위험장기에 대한 계산값과 측정값 사이의 평균오차는 각각 $1.2{\pm}1.1%$$1.2{\pm}0.7%$이었고, 95% 신뢰구간은 각각 3.4%와 2.6%으로 TG119에서 제시하고 있는 4.5%와 4.7% 이내에 있었다. 필름을 이용한 측정 결과에서는 허용기준 3%/3 mm를 통과하는 감마인덱스는 평균$ 97.7{\pm}0.8%$로 95% 신뢰구간은 3.9%로 TG119에서 제시하고 있는 7.0% 이내에 있었다. 따라서 TG119에서 제시된 일련의 표준화된 시험을 통해 본원의 세기조절방사선치료의 commissioning 및 정도관리를 평가할 수 있었다. 이온 전리함을 이용한 절대 선량에 대한 action level은 표적과 위험장기에서 각각 ${\pm}4%$${\pm}3%$로, EBT2 필름을 이용한 2차원 상대선량분포는 허용기준 3%/3 mm 기준으로 감마인덱스 1이하를 통과하는 비율이 96%로 정할 수 있었다. 이를 통해 TG119에서 제시된 일련의 표준화된 시험을 통해 각 기관의 IMRT 정확도 평가에 대한 객관적인 지표로 이용될 수 있을 것으로 기대된다.

방사광 X-선을 이용한 고해상도 영상획득과 응용 (Acquisition of High Resolution Images and its Application using Synchrotron Radiation Imaging System)

  • 물질의 X-선 흡수도에 의해 영상을 얻는 일반 X-선 시스템과 달리 방사광 X-선은 위상이 일치하고 평행하며 진동방향이 일치하는 특성들을 이용하면 고 분해능, 고 대조도의 투사영상을 얻을 수 있다. 국내에서는 포항 방사광 가속기 연구소에 최근 건설된 5C1 빔라인에 미세구조 X-선 영상 획득을 위한 영상시스템을 구축하여 여러 기초 생물, 의학연구분야의 고 분해능 영상획득이 가능하게 되었다. 방사광 X-선을 이용하여 얻은 고 해상도 투사영상들 및 단층 재구성 영상들을 일반 X-선을 사용하는 유방찰영시스템, 치아 X-선 찰영시스템, 전신측정용 CT 시스템에서 각각 획득한 동일한 대상의 영상들과 비교하였다. 실험에 사용한 방사광 X-선은 6 ~30 keV 사이의 연속적인 에너지 분포를 가지며, 실험의 대상에 따라서 실리콘웨이퍼 필터들을 사용하여 빔의 세기와 에너지 스펙트럼 분포를 조절하여 사용하였다. 실험 대상 물체를 통과한 방사광 X-선의 투사영상은 형광판 (CdWO$_4$ scintillator)과 반응하여 가시광선으로 바뀐 후, 금도금된 거울을 통해 90$^{\circ}$ 반사되어 CCD 카메라로 획득하며, 이러한 디지털 영상정보는 PC로 전송되어 저장된다. 방사광 X-선의 공간 분해능 특성은 X-선 시험패턴(25 $\mu$m)과 초 고해상도 패턴 (13.5 $\mu$m)을 방사광 X-선 영상획득시스템 과 일반 X-선을 사용하는 유방촬영시스템에서 획득하여 분석하였다. 영상획득 실험대상으로는 일반 구조물로 커패시터, 생체조직으로 성인치아, 유아치아, 생쥐 척추뼈 및 유방암조직을 대상으로 실험하였다. 단층영상은 각각의 샘플을 0.72$^{\circ}$ 간격으로 180$^{\circ}$ 회전시켜 250개의 투과영상들로부터 재구성한 후 컴퓨터 단층촬영기에서 얻은 영상과 비교하였다. 포항 방사광가속기연구소 5C1 빔라인에 간단하고, 경제적인 방사광 X-선 영상획득시스템을 성공적으로 구축할 수 있었고, 획득한 투사 영상과 재구성한 단층영상을 기존 X-선을 사용한 시스템으로 획득한 단층영상들과 분해능, 대조도의 특성들을 비교, 분석하였다. 방사광 X-선을 사용하여 획득한 영상들은 일반 시스템에서 얻은 영상보다 고 해상도의 영상 질을 보여주었고, 기초 의학영상 연구 측면에서 많은 정보를 제공해 주었다. 방사광 X-선을 이용한 영상획득시스템은 고 분해능과 고 대조도로 미세구조의 상세한 의학영상을 얻기 위한 유용한 방법으로 활용될 수 있을 것으로 기대된다. 향후 해부학적, 병리학적 및 임상학적 의료영상 분야에 효과적으로 응용하기 위하여 X-선 선량 정량 분석과 수치적 영상 해석연구가 계속되어야 할 것으로 사료된다.

치료 장비 별 표면 선량 비교평가 (Evaluation of surface dose comparison by treatment equipment)

  • 목 적: 본 연구는 6MV-FFF(Flattening Filter Free) 에너지를 사용하는 Tomotherapy, Halcyon, True-Beam 장비를 이용하여 가상의 표적 용적에 대한 표면 선량 값을 측정, 비교평가하고자 한다. 대상 및 방법: IMRT 팬텀(IBA, Germany)을 이용하였으며 표면 선량 측정의 정확도를 높이기 위해 without bolus, 0.5 cm bolus, 1 cm bolus 총 세 가지 조건으로 전산화 단층촬영을 시행하였다. 가상의 표적 깊이에 치료 계획 용적(Planning Target Volume, PTV)을 설정하고 1회, 200 cGy로 치료 계획을 각각 수립하였다. 표면 선량 측정은 Gafchromic EBT3 필름을 전산화 치료 계획과 동일한 구간에 위치시켜 10회 반복 측정한 뒤 분석하였다. 결 과: 각 장비 별 실험 결과 without bolus, 0.5 cm bolus, 1 cm bolus 순서로 표면 선량 측정값의 평균은 Tomotherapy에서 115.2±2.0 cGy, 194.4±3.3 cGy, 200.7±2.9 cGy로 확인되었고, Halcyon에서 104.7±3.0 cGy, 180.1±10.8 cGy, 187.0±10.1 cGy로 확인되었으며, TrueBeam에서 92.4±3.2 cGy, 148.6±5.7 cGy, 155.8±6.1 cGy로 확인되었다. 세 가지 조건에서 모두 치료 계획 시스템과 동일하게 Tomotherapy, Halcyon, TrueBeam 순으로 높게 측정되었다. 결 론: Tomotherapy와 Halcyon에서 TrueBeam 장비에 비해 표면 선량이 높게 측정되었다. 치료 부위와 치료 목적에 맞춰 장비 별 특성이 고려된다면 치료 효율성의 증가뿐 아니라 환자의 치료 만족도도 높아질 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology. 30609 T00401030609 ^x To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed a composite that is consisted of calcium phosphate and collagen. To use as the structural matrix of the composite, collagen was purified from human umbilical cord.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
