A Case Report on Cervical Spinal Stenosis with Ossification of posterior Longitudinal Ligament Applied Chuna Treatment (추나요법을 적용한 후종인대골화증을 동반한 경추척추관협착증 환자의 경과관찰 1례)

  • Objects : This study was to report a clinical effect of Korean Treatment for a patient diagnosed by Computed Tomography as Cervical Spinal Stenosis with Ossification of posterior Longitudinal Ligament. Methods : In order to alleviate left shoulder pain and neck stiffness, the patient was treated by acupuncture therapy, herbal medicine, distilled herbal injection, cervical traction technique of Chuna treatment and conservative managements. To evaluate the effect of the treatment, Recovery rate of Hirabayashi, VAS(Visual Anlaogue Scale), NDI and ROM(Range of Motion) were used. Lhermitte's sign, Valsalva test, Distraction test, Compression test and Spurling test were carried out. Results : VAS, NDI and ROM were improved and Recovery rate was 100%. Conclusions : Korean Treatment can be effectively used for a patient with Cervical Spinal Stenosis with OPLL. Further clinical studies are needed to verify the findings.

  • Objects: This study was to report a clinical effect of Chuna Treatment for a patient diagnosed by Magnetic Resonance Imaging(MRI) as Cervical Myelopathy. Methods: In order to alleviate shoulder pain and neck stiffness, the patient was treated by acupuncture therapy, herbal medicine, pharmacopucture, cervical traction technique of Chuna treatment and conservative managements. To evaluate the effect of the treatment, Recovery rate of Hirabayashi, VAS(Visual Analogue Scale), NDI and ROM(Range of Motion) were used.   • Objectives This study aimed to report the effects of giant cell tumor treatment to a patient who had cervical and lumbar pain and hypoesthesia of the limbs after total spondylectomy with Korean medicine. Methods A 67-year-old female patient with cervical and lumbar pain and hypoesthesia of the limbs after total spondylectomy was treated with herbal medicine (Cheongpa-jeon, Yukkongbaro-hwa), acupuncture, pharmacopuncture, and physical therapy for 36 days. The effects were evaluated using a numerical rating scale (NRS), Neck Disability Index (NDI), Oswestry Disability Index (ODI), and EuroQoL-5D (EQ-5D). Results After treatment, changes were observed in the patient's pain and hypoesthesia. NRS, NDI, ODI, and EQ-5D scores decreased. Conclusions This case study suggests that Korean medicine treatment may be effective for symptomatic treatment in patients with giant cell tumor following total spondylectomy.

Facial Chuna Manual Therapy and Acupuncture Treatment for the Sequelae of Peripheral Facial Nerve Palsy: Two Clinical Cases

  • Two patients with sequelae of peripheral facial nerve palsy were treated with Facial Chuna Manual Therapy (FCMT) and acupuncture over 6 months. The House-Brackmann (HB) scale, facial nerve grading system 2.0 (FNGS), the scale of Peitersen, the scale of Murata, and the Numeric Rating Scale (NRS) were used to assess the effects of treatment. The HB scale, FNGS and NRS scores showed improvement for both patients (Case 1: HB scale 5 to 3, FNGS 4 to 2, NRS 10 to 5; Case 2: HB scale 5 to 3, FNGS 4 to 3, NRS 10 to 2.5) following 6 months of treatment. The scores for the Peitersen and Murata scales showed improvement over 6 months in Case 1 (Peitersen 2 to 1, Murata 10 to 7), but there was no change in Case 2 over the test period (4 months). FCMT and acupuncture may help patients with sequelae of facial palsy.

Research Trends Using Manual Therapy in Obstetrics and Gynecological Diseases (한방부인과 영역에 활용되는 수기요법의 국내외 연구 동향)

  • Objectives: The purpose of this study is to analyze the research trends using manual therapy in obstetrics and gynecological diseases. Methods: We searched for studies that performed manual therapy on diseases related to obstetrics and gynecology. All relevant papers were selected and extracted to be analyzed. Results: 12 Randomized controlled trials, 8 cases, 3 pilot studies, 3 non-comparative studies, and 1 before-after comparative study were collected. The study subjects, results, techniques of manual therapy used were analyzed. There were studies related to pregnancy-related symptoms, postpartum pain, dysmenorrhea, chronic pelvic pain, abnormal uterine bleeding, sexual dysfunction, and childbirth. Conclusions: It has been found that various manual therapies can be used for obstetrics and gynecological diseases.

Analysis of Osteopathic Manipulation and Study on Relationship with Chuna Manual Therapy for Meridian Sinew System (정골의학적(Osteopathic) 수기요법 분석 및 경근추나와의 관련성 연구)

  • Objectives : The aim of this study is to analyze the actual investigation and classification of osteopathic manipulation by investigation of the various literature of the inside and outside of the country, and to present the application plan of osteopathy in chuna manual therapy for meridian sinew system. Methods : I referred to the domestic and foreign books about osteopathy and chiropractic. In order to investigate domestic dissertations, I searched 4 Korean medical databases and 4 Korean medical journals of osteopathy. Search terms used were osteopathy, osteopathic, fascia, proprioreceptor, mechanoreceptor, muscle spindle, golgi tendon organ, osteopathic manipulation technics. And I classified all the searched studies into principle and region and etc. In order to investigate foreign dissertations, I search 'NCBI pubmed'. Search terms used were osteopathy, osteopathic technique, osteopathic manipulative technique. Results : 1. Osteopathy do not regard the systems which compose the human body in individual territory, but regard whole. It is diagnosis, prevention and medicine which treats 2. Osteopathic manipulation techniques are classified into direct techniques, indirect techniques, and compound techniques. 3. Osteopathic manipulation techniques are classified into fascia, muscle, ligament-joint in applied region. 4. I could search clinical cases in domestic and foreign study. I found cases about myofascial release technique(MFR), postisometric relaxation(PIR), proprioceptive neuromuscular fascilitation(PNF), muscle energy technique(MET), joint mobilization in domestic studies, and strain-counterstrain technique(SCS), MET, AK in foreign studies. Conclusions : Osteopathic manipulation techniques can be used in diagnosis and treatment of meridian muscle theory, because osteopathy and the oriental medicine have many similarities in theoretical background. So osteopathic manipulation technique can be useful in oriental medicine treatment techniques.

Patients' Satisfaction with Chuna Manual Therapy in the Pilot Coverage Program of National Health Insurance (건강보험 추나요법 급여 시범사업 참여 환자들의 만족도 조사)

  • Backgrounds : In 2017, National Health Insurance implemented the pilot coverage program for Chuna manual therapy(CMT). 65 Korean Medicine(KM) hospitals and clinics were selected in the program to monitor the effectiveness and patients' satisfaction of insured CMT. Objectives : This study aimed to evaluate patients' satisfaction of CMT in the pilot coverage program of National Health Insurance. Methods : Survey participants were recruited among the patients who used CMT at the designated organizations. On-line questionnaire link was sent to the smart phones of patients who agreed to participate in the survey and provide personal contact information. The questionnaire consisted of the basic charactersitics of respondents, imformation on using CMT satisfaction with CMT and willingness to recommend CMT to others. The answers that were automatically coded and saved were statistically analyzed. Results : Of 386 participants who completed the questionnaire, 92.8% satisfied or strongly satisfied with the CMT. Most frequent reason of satisfaction was 'Good effectiveness', and there was no difference in satisfaction between patients of hospital and those of clinics. Patients with the highest and the lowest level of pain satisfied more than those with other pain levels(p=0.003), but the level of copayment and reasons of CMT use did not affect the satisfaction results(p=0.405). The proportions of respondents who had willingness to recommend CMT to others and to revisit for CMT use were 97.8% and 98.8%, respectively. Conclusions : Most patients were satisfied with CMT in the pilot coverage program, and it can provide the rationale for expanding the insurance coverage of CMT to all KM hospitals and clinics.

A Case Study of Spinal Cord Stimulation Acupuncture for Lower Limb Numbness Induced by Lumbar Herniated Intervertebral Disc (하지 비증을 주소증으로 한 요추 추간판 탈출증 환자에 대한 척수 자극 침술의 임상 증례 보고)

  • Objectives : The purpose of this study was to evaluate the effects of spinal cord stimulation acupuncture therapy for lower limb numbness resulting from lumbar herniated intervertebral disc(HlVD). Methods : From 8th August, 2015 to 30th October, 2015, 1 male patient diagnosed as herniated intervertebral disc at L5/S1(Diffuse bulging disc with smooth ventral thecal sac indentation at L5-S1) was treated with spinal cord stimulation acupuncture and general Korean medicine therapy(acupuncture, herbal injection). Numerical Rating Scale(NRS) and Oswestry Disability Index(ODI) were recorded during the treatment. Results : 1. The patient's chief complaints were remarkably improved - Rt. lower limb numbness remained 15% compared before treatment, Rt. lower limb paresthesia and gait disturbance almost disappeared after 25 times of treatment during 3 months. 2. NRS score decreased from 7 to 3, ODI decreased from 16 to 7. Conclusions : This study demonstrates that spinal cord stimulation acupuncture therapy with Korean medicine treatment has notable effect in improving lower limb numbness induced by lumbar HlVD.

Case Reports on Neck Pain with Cervical Syringomyelia Patients Treated by Korean Medical Therapy (척수공동증을 동반한 경추통 환자에 대한 치험 2례 보고)

  • Objectives : The purpose of these clinical cases is to evaluate the effectiveness of Korean medical therapy on neck pain with cervical syringomyelia Methods : These are case reports on 2 patients admitted to Daejeon jaseng Hospital of Korean Medicine with neck pain and diagnosis of cervical syringomyelia by Cervical spine Magnetic Resonance Imaging, were observed during hospitalization. They were analyzed according to pain and Cervical spine Range of Motion. All patients received a combination of treatments including Acupunture, Chuna, Pharmacopunture, Herbal medication. Pain scales(Number Rating Scale, Neck Disability Index) and Cervical spine Range of Motion were used during hospitalization. Results : After 3 weeks for these patients, Cervical spine Number Rating Scales decreased from 7 to 3 and from 9 to 4 respectively. And Trapezius muscle Number Rating Scales decreased from 7 to 3 and from 2 to 1 respectively. Also Cervical spine Range of Motion improved to the normal. Conclusions : Korean medical therapy might be effective in reducing pain and improving range of motion for patients with neck pain. And we hope this study further confirmed the effectiveness of Korean medical therapy on neck pain with cervical syringomyelia.

The Case Report of Conservative Treatment on the Herniation of Intervertebral Disc Patient After the Fusion Surgery of a Lumbar Segment (요추부 한 분절 유합술 이후 발생한 추간판 탈출증환자에 대한 보존적 치료 치험 1례)

  • Objectives: The purpose of this study is the evaluation of conservative medical treatment on the herniation of intervertebral disc(HIVD) patient after the fusion surgery of a lumbar segment. Methods: We used acupuncture, herbal medication, and manipulation for this patient. And we measured of VNRS score and SLR test result to evaluate effect of conservative medical treatment. Results & Conclusions: Patient's low back and left leg pain VNRS is decreased, and the SLR test result is improved.
